  • Levels of serum soluble interleukin 2 receptor (sIL-2R) provide a reliable marker of disease activity in patients with hairy cell leukemia and adult T-cell leukemia/lymphoma.
  • The malignant cells in patients with anaplastic large cell lymphoma (ALCL) express CD30 and are usually positive for expression of CD25.
  • We measured serum sIL-2R and soluble CD30 (sCD30) levels in patients with ALCL treated with EPOCH (etoposide, prednisone, Oncovin, Cytoxan, hydroxydaunorubicin) infusional chemotherapy.
  • Serum sCD30 levels were elevated and decreased in response to therapy as previously reported.
  • Serum sIL-2R levels were elevated in 7 of 9 patients with ALCL and decreased in response to treatment.
  • Patients positive for the anaplastic lymphoma kinase (ALK) gene showed elevated sIL-2R levels, whereas those negative for ALK had normal serum sIL-2R levels and their tumors lacked CD25 expression.

  • Phase 1 testing of SGN-30, a chimeric monoclonal antibody for the treatment of CD30(+) malignancies, was conducted in a multicenter study.
  • To explore the safety profile and establish the maximum tolerated dose (MTD), 24 patients with refractory or relapsed Hodgkin lymphoma or CD30(+) non-Hodgkin lymphoma received 6 weekly doses of intravenous SGN-30 at 4 dose levels (2, 4, 8, or 12 mg/kg).
  • Adverse events were mild, with nausea, fatigue, and fever attributed to study treatment.
  • One patient with cutaneous anaplastic large cell lymphoma (8 mg/kg) achieved a complete response.
  • Six patients, of whom 4 had Hodgkin lymphoma, achieved stable disease with durations ranging from 6 to 16 months.

  • Primary systemic anaplastic lymphoma kinase (ALK)-negative anaplastic large-cell lymphoma (ALCL) has a poor prognosis.
  • This study sought to determine if high-dose therapy and ASCT results in long-term disease-free survival (DFS) in patients with recurrent, chemotherapy-sensitive ALK-negative ALCL.
  • All patients with non-Hodgkin's lymphoma (NHL) who underwent ASCT at Wake Forest University and Upstate Medical University from 1 January 1990 to 12 December 2002 were reviewed to determine if they had T-, B- or null-cell NHL that was CD30+/CD15-/ALK negative.
  • All 16 patients underwent ASCT at the time of first relapse and form the basis of this report.
  • International prognostic index scores in 12 patients at the time of relapse were: low 3, LI 6 and HI 3.
  • Of 15 patients, 13 relapsed after ASCT; one patient was lost to follow-up.
  • Of 15 patients, 10 have died; nine of recurrent disease.
  • In our experience, high-dose therapy and ASCT does not produce long-term DFS in patients with recurrent chemotherapy-sensitive ALK-negative ALCL.

  • [Title] Anaplastic lymphoma kinase-positive anaplastic large cell lymphoma presenting as a bladder neoplasm.
  • Malignant lymphoma presenting in the bladder has been classified in primary cases, as the first sign of disseminated disease and as a secondary infiltration.
  • Reported herein is the case of a 45-year-old man with an anaplastic large cell lymphoma (anaplastic lymphoma kinase (ALK) and granzyme B positive) that presented as a bladder neoplasm.
  • The morphological differential diagnosis was complex because the EMA-positive immunophenotype, CD45 and CD3 negativity and the clinical manifestation simulated a transitional cell carcinoma.
  • T-cell receptor-gamma clonal rearrangement could be also helpful in these cases.
  • Although bladder involvement by recurrent lymphoma is a sign of widely disseminated disease and it is associated with a very poor prognosis, it seems that chemotherapeutic regimens in this kind of ALK-positive lymphoma could be effective, given that the present patient had an impressive response to chemotherapy treatment.

  • PURPOSE: The successful resumption of high-dose methotrexate in a 13-year-old boy with recurrent anaplastic large-cell lymphoma (ALCL) who suffered renal dysfunction after a 24-hour infusion of high-dose methotrexate and required treatment with carboxypeptidase G(2) (CPDG(2) ) is described.
  • SUMMARY: A 13-year-old boy who had been diagnosed in 2001 with stage I ALCL was admitted to the hospital in February 2005 after he developed a smaller left axillary mass in the area of his original mass.
  • Recurrent ALCL was diagnosed, and treatments were initiated based on branch K3 of the protocol published in the non-Hodgkin's lymphoma-Berlin-Frankfurt-Münster (NHL-BFM) trial 90.
  • In the NHL-BFM 90 protocol, all AA and BB courses include high-dose methotrexate therapy, which consists of aggressive alkalinized hydration, methotrexate 5 g/m(2) given as an i.v. infusion over 24 hours, and leucovorin rescue.
  • Because the patient's elimination of methotrexate remained slow and his serum creatinine level remained above normal limits, CPDG(2) was obtained for the treatment of methotrexate toxicity.
  • He is currently in remission on maintenance therapy.
  • CONCLUSION: A 13-year-old boy with recurrent ALCL had methotrexate-induced nephrotoxicity following high-dose methotrexate.
  • The resultant delayed methotrexate clearance required the standard therapies as well as use of investigational CPDG(2).

  • [Title] Relapses of childhood anaplastic large-cell lymphoma: treatment results in a series of 41 children--a report from the French Society of Pediatric Oncology.
  • PURPOSE: To study response to chemotherapy and the outcome of children treated for a relapsed anaplastic large-cell lymphoma (ALCL) and to evaluate the role of bone marrow transplantation (BMT) in these patients.
  • First-line treatment consisted of intensive chemotherapy according to the COPAD protocol for the first series of 12 patients treated between 1975 and 1989 and to the SFOP (French Society of Pediatric Oncology) HM protocols for the 30 patients treated between 1989 and 1997.
  • Twenty-eight patients were treated with CV(B)A (CCNU, vinblastine, ara-C with or without bleomycin), and the others with miscellaneous protocols for recurrent disease.
  • CONCLUSIONS: Relapsed ALCL are highly chemosensitive but over 40% of the patients experience several relapses.
  • Prolonged conventional chemotherapy based on vinblastine might, in some cases, be as efficient as short intensive treatment with ABMT.

  • Cutaneous CD30+ lymphoid infiltrates appear cytologically atypical and occasionally may be misinterpreted as recurrent disease when they occur in patients treated for other primary hematologic malignancies.
  • One patient with B-cell lymphoma and another with myeloid leukemia developed cutaneous eruptions after chemotherapy displaying highly atypical perivascular lymphoid cells on histology that mimicked recurrent disease.
  • Because one case was initially misinterpreted as recurrent leukemia, we conclude that close clinical correlation and immunophenotypic confirmation should be done for atypical cutaneous lymphoid infiltrates in patients with primary hematologic malignancies.
  • We discuss the differential diagnosis of atypical CD30+ infiltrates in this setting, which include recurrent lymphoma or myeloid leukemia, primary cutaneous anaplastic large cell lymphoma (ALCL), lymphomatoid papulosis (LyP), carbamazepine-induced CD30+ pseudolymphoma, viral infection and an atypical eruption of lymphocyte recovery.

  • Large cell lymphomas (LCL) are relatively rare in children and sub-divided among diffuse large B-cell lymphomas (DLBCL), anaplastic large cell lymphomas (ALCL), peripheral T (PT) and other rare subtypes.
  • One hundred fifty-six (40%) had large cell lymphoma.
  • All patients received nine weeks of chemotherapy including vincristine 1.5mg/m2 weekly for seven doses; doxorubicin 40mg/m2 and cyclophosphamide 750mg/m2 on days 1, 22 and 43; and prednisone 40mg/m2 daily for 28 days during the first 4 weeks and on days 43-47.
  • Only one patient with DLBCL developed recurrent disease and died.
  • At 5 years, the projected event-free survival (EFS) is 98 % (SE 3%), and the overall survival (OS), 98 % (SE 3%).
  • Thirty-five children with ALCL (60%) had T cell markers, and the remainder had null cell markers.
  • Nine patients with ALCL (T=5; null=4) failed treatment: three failed induction, and six relapsed from complete remission, but were effectively salvaged.
  • The projected 5 year EFS for early stage ALCL is 84 % (SE 7%) (DLBCL versus ALCL, p-value 0.02); the OS, 100%.
  • CONCLUSIONS: Nine weeks of modest intensity chemotherapy are sufficient for children with early stage DLBCL.

  • METHODS: To assess if more frequent dosing might maximize anti-tumor activity with acceptable tolerability, a multicenter, phase 1, weekly dosing, dose-escalation study (3+3 design) was conducted in pts with refractory or recurrent HL or sALCL.
  • Pts with stable disease or better (Cheson 2007) after two 28-day cycles (6 doses) were eligible to continue SGN-35 treatment.
  • Pts received a median of 4 prior therapies; 65% received an autologous SCT.
  • Multiple CRs were observed at higher doses ( table ); observed time to response in the 1 mg/kg dose group was approximately 8 wks.
  • The 7 pts with CRs all remain on treatment.
  • Enrollment to SGN-35 monotherapy continues at 1.2 mg/kg; combination therapy will be subsequently explored.
  • Pivotal trials of this antibody-drug conjugate will initiate in early 2009.

  • We previously reported the therapeutic efficacy of 13-cis retinoic acid (13-cRA) in some subtypes of peripheral T-cell lymphoma (PTCL).
  • This study sought to clarify if the addition of interferon-alpha2a (IFN-alpha2a), an agent with synergistic cytotoxicity with 13-cRA in many types of malignant cells, may be more effective in the treatment of PTCL.
  • Eligible patients has histologically proven PTCL, which was recurrent after or refractory to anthracycline-containing systemic chemotherapy.
  • The treatment included oral administration of 13-cRA 1 mg/kg/day, divided into three doses, and intramuscular injection of IFN-alpha2a 4.5 MU/M2, three times per week.
  • The histologic diagnosis included 7 cases of unspecified PTCL, 6 cases of Ki-1 anaplastic large cell lymphoma (ALCL), 1 case of angioimmunoblastic T-cell lymphoma, and 3 cases of angiocentric nasal NK/T cell lymphoma.
  • They received a median of 1.7 months of treatment (range, 0.4-13.3 months).
  • One patient refused further treatment due to toxicity.
  • Grade III/IV hematologic and non-hematologic toxicity developed in 2 and 5 patients, respectively.
  • In conclusion, a combination of 13-cRA and IFN-alpha2a is a useful salvage treatment for selected patients with recurrent or refractory PTCL, particularly those with the Ki-1 subtype.

  • Recurrent chromosomal aberrations in hematopoietic tumors target genes involved in pathogenesis.
  • Their identification and functional characterization are therefore important for the establishment of rational therapies.
  • Here, we investigated genomic amplification at 7q22 in the T-cell lymphoma cell line SU-DHL-1 belonging to the subtype of anaplastic large-cell lymphoma (ALCL).
  • In comparison with control cell lines, SU-DHL-1 expressed considerably higher levels of CDK6.
  • Functionally, SU-DHL-1 cells exhibited reduced sensitivity to rapamycin treatment, as indicated by cell growth and cell cycle analysis.
  • Amplification of the CDK6 locus was analyzed in primary T-cell lymphoma samples and, while detected infrequently in those classified as ALCL (1%), was detected in 23% of peripheral T-cell lymphomas not otherwise specified.
  • Taken together, analysis of the 7q22 amplicon identified CDK6 as an important cell cycle regulator in T-cell lymphomas, representing a novel potential target for rational therapy.

  • BACKGROUND & OBJECTIVES: The biological behavior of peripheral T-cell lymphoma (PTCL) are different from that of B-cell non-Hodgkin's lymphoma (NHL).
  • It shows low chemosensitivity, high incidence of relapse, poor prognosis, and has no standard chemotherapy regimen.
  • According to WHO classification criteria, 21 cases of PTCL concluded 7 peripheral T-cell lymphoma unspecified (PTCL-U), 7 NK/T-cell lymphoma (NK/TCL), 5 anaplastic large cell lymphoma (ALCL), 1 Mycosis fungoides/Sezary syndrome (MF/SS), and 1 subcutaneous panniculitis-like T-cell lymphoma (SPTCL).
  • Among them there were 14 previously untreated patients, 7 pretreated and recurrent patients.
  • RESULTS: Of 21 patients, 20 were eligible to evaluate treatment efficacy.
  • Seventy cycles of chemotherapy were administered to 21 patients.
  • Other toxicities were mild, no treatment-related mortality occurred.

  • [Title] Single-drug vinblastine as salvage treatment for refractory or relapsed anaplastic large-cell lymphoma: a report from the French Society of Pediatric Oncology.
  • PURPOSE: To evaluate the efficacy of vinblastine for relapsed/refractory anaplastic large-cell lymphoma (ALCL).
  • Fifteen patients had undergone hematopoietic stem-cell transplantation (HSCT) for a previous relapse.
  • Overall, nine of 25 patients treated with vinblastine alone have remained in CR (median, 7 years since the end of treatment), and 16 patients have relapsed.
  • With a median follow-up of 9.2 years, 12 patients have died (four as a result of toxicity after HSCT and eight as a result of disease), and 24 patients are alive (15 following treatment with single-agent vinblastine for the last event).
  • CONCLUSION: Vinblastine is highly efficient in relapsed ALCL and may produce durable remissions.
  • The optimal treatment duration still has to be assessed.
  • These results should be borne in mind when designing future phase II studies with the targeted therapies directed against anaplastic lymphoma kinase.

  • BACKGROUND/AIMS: Management of patients with heavily pretreated malignant lymphoma failing frontline treatment and salvage high-dose chemotherapy and autologous peripheral stem cell rescue, is problematic.
  • A pilot study was conducted to evaluate isolated thoracic perfusion of drugs by means of stopflow technique.
  • METHODOLOGY: Six patients were enrolled in the study; diagnoses included 4 advanced Hodgkin's disease, 1 primary mediastinal B-cell lymphoma, and 1 anaplastic large cell lymphoma.
  • They had never achieved a complete response since all had progressed from front-line treatment, and 3 had even failed salvage high-dose chemotherapy with autologous peripheral stem cell rescue.
  • Carmustine (100 mg/m2) were added to these 2 drugs and cytarabine (2000 mg/m2) in patients not previously treated by carmustine, etoposide, cytarabine, and melphalan.
  • Drugs were delivered monthly via aortic perfusion performed by means of Aigner's stop-flow technique.
  • RESULTS: Overall 13 cycles of perfusional chemotherapy were administered with a median number of 2 cycles.
  • Tolerance to therapy was excellent.
  • CONCLUSIONS: This new therapeutical approach seems very active in recurrent/refractory malignant lymphoma and may play an important role in this setting.

  • RESULTS: A 71-year-old female with long-standing MF developed lymphomatous CNS involvement 10 years after the diagnosis of tumor stage MF.
  • At this time, the patient presented with a transient episode of garbled speech followed by generalized weakness.
  • Computerized tomography scan (CT scan) and magnetic resonance imaging scan (MRI scan) of the head revealed a subcortical lesion in the left temporo-frontal lobe.
  • Cerebrospinal fluid (CSF) examination showed atypical T cells, and brain biopsy confirmed parenchymal involvement by T-cell lymphoma.
  • Meanwhile, a biopsy of a skin lesion showed large-cell transformation.
  • No lymph node or other systemic involvement was noted at this time, and the patient was treated with chemotherapy.
  • Twelve months later, the patient developed recurrent CNS lymphoma with multiple organ involvement and expired soon thereafter.

  • Recent development of stratified chemotherapeutic regimens has rapidly improved the survival rate of non-Hodgkin's lymphoma (NHL) of childhood.
  • Despite these improvements, the outcome for children with recurrent or refractory NHL remains dismal.
  • We explored the use of high-dose chemotherapy followed by autologous peripheral blood stem cell transplantation (HDC/PBSCT) for children with either refractory or recurrent NHL, and we evaluated various factors influencing outcome of HDC/PBSCT.
  • All patients had refractory or recurrent NHL.
  • Sex, stage at diagnosis, histologic subtype (lymphoblastic, Burkitt's, and large-cell lymphoma), LDH level at diagnosis, disease status at transplantation, and preparative regimens for HDC/PBSCT were explored.
  • In regard to the patients, six had Burkitt's lymphoma, 13 had lymphoblastic lymphoma, and 14 had large-cell lymphoma.
  • The EFS for Burkitt's, lymphoblastic, and large-cell lymphoma was 66.7+/-27.2, 50.5+/-14.8, and 82.1+/-11.7%, respectively.
  • In comparison with lymphoblastic and non-lymphoblastic lymphoma, the relative risk for lymphoblastic lymphoma was higher than the others (P = 0.037).
  • EFS between anaplastic large-cell and diffuse large-cell lymphoma was 100 and 55.6+/-24.9%, respectively (P = 0.106).
  • HDC/PBSCT is considered applicable to recurrent or refractory pediatric NHL patients safely and it could replace conventional chemotherapy.
  • In this study, children with CR status at the time of HDC/PBSCT showed higher survival rate.
  • However, refractory or recurrent lymphoblastic lymphoma patients showed dismal results.
  • Therefore, new therapeutic modalities may be needed for this group of NHL patients.

  • [Title] A clinical, molecular and cytogenetic study of 12 cases of human herpesvirus 8 associated primary effusion lymphoma in HIV-infected patients.
  • INTRODUCTION: Primary effusion lymphoma is a rare type of B-cell lymphoproliferative disorder which is mainly observed in patients with HIV infection.
  • Lymphomatous cells bridge features of immunoblastic and anaplastic cells with a non-B non-T phenotype and are characterized by the presence of the human herpesvirus 8 genome.
  • PATIENTS AND METHODS: : Twelve HIV-infected patients with serous effusions containing large HHV8(+) lymphomatous cells were extensively evaluated to disclose associated visceral involvement.
  • RESULTS: Extraserous localizations of lymphoma were present in six cases (50%): mediastinal (n = 2), mesenteric (n = 2), pancreatic (n = 1), and bone marrow involvement (n = 1).
  • Cytogenetic analysis displayed a complex karyotype in all cases without recurrent abnormalities.
  • Three patients are in complete remission at 28, 53 and 55 months after high-dose chemotherapy (n = 1), cidofovir and alpha-interferon combination therapy (n = 1), and antiretroviral therapy alone (n = 1).
  • CONCLUSION: The clinical and molecular pattern, as well as the response to therapy suggest that primary effusion lymphoma represents an heterogenous type of virus-induced B-cell lymphoproliferative disorder, sharing pathophysiological features with that induced by the Epstein-Barr virus and occurring in immunocompromised patients.

  • We report a case of a 40-year-old man with a stage 4, anaplastic, large-cell lymphoma.
  • He had been diagnosed 13 years before as having a liposarcoma, at which point he was treated with combination chemotherapy, which included anthracycline.
  • On review of the histopathology from 13 years before, the original diagnosis of liposarcoma was revised to that of an anaplastic large-cell lymphoma.
  • A diagnosis of relapsed anaplastic large-cell lymphoma was made.
  • Our patient responded initially to combination chemotherapy, which included anthracycline, without further reduction in his ejection fraction.
  • This was followed by high-dose chemotherapy and peripheral blood stem-cell transplantation.

  • Patients with refractory or early relapsed anaplastic large cell lymphoma (ALCL) have a poor chance of survival.
  • We report 20 children and adolescents with high-risk relapsed or refractory ALCL who underwent allogeneic haematopoietic stem cell transplantation (HSCT).
  • We retrospectively analysed 20 patients who relapsed between December 1991 and April 2003 during (six patients) or soon after first-line Berlin-Frankfurt-Münster-type chemotherapy (14 patients) and underwent allogeneic HSCT.
  • Two patients relapsed after allogeneic HSCT and died.
  • There was no influence of donor type or conditioning regimen on outcome.
  • Two of six patients with progressive disease during frontline therapy survived compared with 13/14 patients with a first relapse after frontline therapy.
  • Two of three patients who were transplanted with active lymphoma and all five patients who received allogeneic HSCT for relapse following autologous HSCT survived disease-free.
  • Allogeneic HSCT is effective and has acceptable toxicity as rescue therapy for high-risk ALCL relapse.
  • It even offers cure for patients refractory to chemotherapy, suggesting a graft-versus-ALCL effect.
