  • METHODS: We present the clinical findings, laboratory results, imaging studies, and surgical pathology report in a man with thyrotoxicosis in whom metastatic well-differentiated thyroid cancer was diagnosed incidentally on a routine TcO4- thyroid scan in the setting of a presumed diagnosis of benign toxic thyroid disease.
  • A routine TcO4- thyroid scan revealed high-grade extrathyroidal uptake in the right upper hemithorax as well as in the left side of the thorax.
  • Scintigraphy revealed sites of metastatic involvement predominantly in the bones along with a cold nodule in the left thyroid lobe, consistent with the primary tumor.

  • Computed tomography (CT) and magnetic resonance imaging (MRI) revealed subpectoral mass surrounded by the minor pectoral muscle, the subscaplar muscle, and the bony thorax, and spread to cervix passing through between the right clavicle and the bony thorax, indicating similarity of the intensity and the density to subcutaneous fatty tissue, and then the mass was guessed to be extremely rare subpectoral lipoma.
  • Subpectoral lipoma is one of the benign tumor and at the operation less invasive and cosmetically superior approach should be adopted except for inevitable cases.

  • RESULTS: In all four cases laparoscopic distal pancreatic resection was performed for tumor.
  • Histologic examination showed a neuroendocrine carcinoma, a serous-microcystic adenoma, a low differentiated ductal adenocarcinoma and an intraductal papillary-mucinous tumor of borderline type.
  • CONCLUSION: Laparoscopic resection of distal pancreas shows the common benefit of minimal invasive surgery for the early postoperative period and is an attractive alternative for treatment of benign and semimalign pancreatic tumors.

  • The patient first visited our hospital for surgical treatment of superior-posterior mediastinal tumor, which was successfully removed by thoracoscopic approach to reveal a benign ganglioneuroma.
  • A retroperitoneal tumor was incidentally found postoperatively that was removed using a laparoscopic procedure.
  • The retroperitoneal tumor was a nonfunctioning pheochromocytoma, which was different from the mediastinal neurogenic tumor.
  • This association, although rare, should be recognized for management of neurogenic tumors in the thorax.

  • There is also lack of evidence that these methods can correctly classify nodules <7 mm as malignant because lung cancer can be mimicked in appearance by benign lesions that lower specificity.
  • We have previously shown that this systemically administered complex (scL) selectively targets, and efficiently delivers its payload into, tumor cells.
  • CONCLUSIONS: Because of its tumor targeting specificity, deliver of an MRI contrast agent via this nanocomplex has potential for use as an agent that can identify small lung cancers, thus improving early detection and possibly increasing survival.

  • Lymphangioleiomyomatosis (LAM) is a multisystem disease of women, characterized by proliferation of abnormal smooth muscle-like cells (LAM cells) that can metastasize, leading to the formation of lung cysts, fluid-filled cystic structures in the axial lymphatics (e.g., lymphangioleiomyomas), and angiomyolipomas, benign tumors usually involving the kidneys, comprising LAM cells and adipocytes, intermixed with incompletely developed vascular structures.
  • LAM occurs sporadically or in association with tuberous sclerosis complex, an autosomal dominant syndrome characterized by hamartoma-like tumor growths.

  • Laboratory results revealed hypercalcemia, evidence of primary hyperparathyroidism and computer tomography of the thorax showed bilateral pulmonary metastasis.
  • After undergoing CT-guided biopsy of a metastasis, histology revealed an endocrine primary tumor with low parathyroid hormone expression.
  • In view of the history, clinical and biochemical findings we diagnosed a recently metastasized functioning parathyroid carcinoma, which eight years previously has been labeled as a benign atypical thyroid adenoma.
  • Parathyroid carcinoma is an uncommon tumor.
  • Patients with parathyroid carcinoma mostly die from uncontrollable hypercalcemia rather than from other tumor-related complications.

  • RESULTS: Seven patients initially presented with myasthenia gravis, and therefore they underwent a resection of the thymus, even though imaging techniques did not reveal a tumor in any of the cases.

  • BACKGROUND: Sera from lung cancer patients contain autoantibodies that react with tumor associated antigens (TAAs) that reflect genetic over-expression, mutation, or other anomalies of cell cycle, growth, signaling, and metabolism pathways.
  • METHODS: We performed immunoassays to detect autoantibodies to ten tumor associated antigens (TAAs) selected on the basis of previous studies showing that they had preferential specificity for certain cancers.
  • Sera examined were from lung cancer patients (22); smokers with ground-glass opacities (GGOs) (46), benign solid nodules (55), or normal CTs (35); and normal non-smokers (36).

  • BACKGROUND: Perineurioma is a rare benign soft-tissue tumor composed of cells showing differentiation toward the perineurial cells of the nerve sheath.
  • Although mutations in the neurofibromatosis 2 (NF2) gene have been documented in this tumor, there is no known association between perineuriomas and type 1 or 2 NF.
  • RESULTS: Histopathologic examination revealed a 2.0-cm well-circumscribed, spindle-cell neoplasm with slender, elongated, bipolar, wavy cytoplasmic processes and wavy, elongated nuclei in a hyalinized stroma with focal myxoid areas.

  • [Title] [Primary benign chest wall tumors: results of surgical treatment].

  • BACKGROUND: We undertook this study to describe the clinical characteristics of primary benign chest wall tumors and to evaluate the results of surgical treatment.
  • METHODS: We included patients with primary benign chest wall tumors who underwent surgical resection at the Instituto Nacional de Cancerología from January 1990 to December 2002.
  • All patients had histological confirmation of benign tumor.
  • The most frequent tumor was chondroma in seven cases (41%), followed by aponeurotic-muscle fibromatosis in five (30%) and the remaining five patients had other tumor types.
  • All patients were subject to thoracic wall resection, including at least one rib in each patient.
  • In three patients the sternum was resected (17.6%), the clavicle in two (11.7%), thoracic vertebrae in one (5.9%) and associated structures in four patients (23.5%).
  • In eight patients (47%) the thoracic wall was reconstructed with marlex mesh, whereas the remaining patients required no reconstruction.
  • Of the 17 patients, 16 are alive without disease and one presented delayed recurrence with an aponeurotic-muscle fibromatosis requiring a new thoracic wall resection.
  • One patient presented with respiratory complications and died after 42 days due to thoracic wall instability.
  • CONCLUSIONS: Primary benign chest wall tumors are locally aggressive and must be treated with wide resection of the thoracic wall.
  • In addition, surgical reconstruction with prosthetic materials and/or myocutaneous flaps is done prior to resection, for stability of the thorax when the localization is anterior or lateral.

  • RESULTS: Chest CT of a middle-aged man revealed an enormous heterogeneous neoplasm, about 10.9 cm x 9.2 cm x 17.7 cm in size, in the left pleural cavity.
  • There was abundant calcification in the tumor, with signs of invasion into the diaphragm and the pleura.
  • Pleural effusion of the left thoracic cavity was also seen on the chest CT.
  • Osteosarcoma was confirmed by pathological study after surgical resection of the tumor.
  • The differential diagnosis includes benign and malignant diseases of the thorax.

  • The aim of this study was to evaluate the overall utility of ultrasonographic guided FNAC in the diagnosis of abdominal and thoracic lesions.
  • In 264 cases (82.5%), FNAC was diagnostic with commonest diagnosis being malignant neoplasm (70.0%).
  • In liver, Metastatic adenocarcinoma is the commonest tumor, while in lung; the commonest lesion is non-small cell carcinoma.
  • Benign neoplasm (3.1%) and non neoplastic lesion (9.4%) were also diagnosed by FNAC.
  • USG guided FNAC, in conjunction with clinico-radiological parameters are accurate and safe in diagnosing deep-seated mass lesions in the thorax and abdomen.

  • BACKGROUND: This study aimed to show that SHOX2 DNA methylation is a tumor marker in patients with suspected lung cancer by using bronchial fluid aspirated during bronchoscopy.
  • Fresh-frozen and Saccomanno-fixed samples were used to show the tumor marker performance in different sample types of clinical relevance.
  • DNA methylation of SHOX2 allowed to distinguish between malignant and benign lung disease, i.e. abscesses, infections, obstructive lung diseases, sarcoidosis, scleroderma, stenoses, at high specificity (68% sensitivity [95% CI 62-73%], 95% specificity [95% CI 91-97%]).
  • CONCLUSIONS: Hypermethylation of SHOX2 in bronchial aspirates appears to be a clinically useful tumor marker for identifying subjects with lung carcinoma, especially if histological and cytological findings after bronchoscopy are ambiguous.

  • Adrenal myelolipoma is a rare benign tumor composed of mature lipomatous and hemopoietic tissue.
  • Although it is often discovered as an incidental finding on imaging of the abdomen for some other reason or at autopsy, adrenal myelolipoma has been reported to present with symptoms such as flank pain resulting from tumor bulk, necrosis, or spontaneous retroperitoneal hemorrhage.
  • They are usually unilateral but may be bilateral and may also develop in extraadrenal sites like the retroperitoneum, thorax, and pelvis.
  • We also review the literature to evaluate the presentation and optimal management of this rare adrenal tumor that is not encountered by most general surgeons and therefore not well known to most surgeons.

  • Although lipomas are the most common form of the benign neoplasm, occurrence within the thoracic cage is uncommon, and lipomas originating from the pleura are very rare.
  • A chest wall tumor was suspected based on the findings of computed tomography (CT) of the thorax.
  • The tumor was resected by video-assisted thoracoscopic surgery and the patient remains well with no recurrence 4 years postoperatively.
  • The resected tumor was a yellowish pleural mass and the pathological diagnosis was a benign lipoma.

  • [Title] The role of endoscopic surgery for completely obstructive endobronchial benign tumor.
  • BACKGROUND: The improving techniques of therapeutic bronchoscopy have been replacing conventional surgery for resecting endobronchial benign tumor.
  • To evaluate the role of endoscopic surgery for completely obstructive endobronchial benign tumor, we retrospectively reviewed the medical records of 7 patients who underwent bronchoscopic resection due to completely obstructive benign tumor.
  • After the stalk of tumor was identified with using a 1 mm biopsy forceps as a probe, a Nd-YAG laser was used to coagulate the stalk of tumor.
  • The tumor was then removed.
  • CONCLUSIONS: Endoscopic surgery could be applied to the patients with completely obstructive endobronchial benign tumor.

  • [Title] Benign myoepithelioma of the lung - a case report and review of the literature.
  • INTRODUCTION: Benign myoepithelioma is extremely rare in the lung, to the best of our knowledge; only five cases have been reported in the literature.
  • CT of the thorax demonstrated a nodule in the left segment of the Fowler.
  • No mitotic activity or necrosis was seen in the tumor.
  • Immuhistochemically, the tumor cells positive for smooth muscle actin, vimentine, and S100 protein.
  • The diagnosis of benign myoepithelioma of the lung is so confirmed.
  • CONCLUSION: Benign myoepithelioma is a rare pulmonary neoplasm distinct from pleomorphic adenoma, which should be considered in the differential diagnosis of lung nodules.

  • [Title] A thoracic wall lipoblastoma in a 3-month-old infant: A case report and review of the literature.
  • Lipoblastoma is a rare benign tumor of adipose tissue seen almost always in infancy and early childhood.
  • Lipoblastoma is a tumor with good prognosis with no reported metastases, despite its potential for local invasion and rapid growth.
  • In the postoperative period, the thorax wall was unaffected, and after 6 years of follow-up no recurrence was observed.
  • In the English literature, 8 cases of thoracic wall lipoblastoma have been previously reported, and only 3 of diffuse form.
  • Here, we report, at our knowledge, the fourth case of thoracic diffuse lipoblastoma, in which cytogenetic analysis showed a previously undescribed karyotype aberration involving chromosomes 8, 13, and 16.

  • Benign lesions are rarely a cause for this type of dysphagia, and usually occur either as an intramural tumor or as an extrinsic compression.
  • We report two cases of dysphagia in adult patients caused by mediastinal tuberculous lymphadenitis, presenting radiologically and endoscopically as an esophageal submucosal tumor.

  • The young man underwent laparoscopic enucleation of the tumor, while the 50 year old man underwent left sided laparoscopic resection of the pancreas because the tumor was adjacent to the pancreatic duct.

  • About 20% of tumors develop in the thorax, out of which vast majority is localized on its wall.
  • Within thorax, they usually originate from pleura, most frequently the visceral one.
  • Authors present a 67- year old male in whom the tumor was diagnosed during the investigation for dyspnea.
  • During operation, solitary, well circumscribed, firm, ruber-like tumour, 11 x 10 x 9 cm in diameter, covered with serosa, arising from the visceral pleura of the base of the right lung was easily excised.
  • The classical histological and immunohistochemical examinations confirmed the diagnosis of benign fibrous tumor.

  • TREATMENT AND COURSE: One month after stent implantation open surgery was performed suspecting a malignant tumour of the bile duct.
  • A solid tumour at the hepatic bifurcation was resected completely.
  • CONCLUSION: Preoperative distinction between benign and malignant bile duct stenosis can be difficult, even impossible.
  • Thus, every resectable bile duct tumour should undergo surgery even and especially if a histological diagnosis cannot be reached preoperatively and the condition of the patient is adequate.

  • [Title] Diverse histologic appearances in pulmonary mucinous cystic neoplasia: a case report.
  • INTRODUCTION: Primary pulmonary mucinous cystic neoplasia comprises a group of tumors, from benign cystadenoma to mucinous cystadenocarcinoma.
  • A lobectomy was performed and the resection specimen revealed a multicystic mucinous tumor.
  • Microscopically, the tumor was composed of confluent mucin-filled cystic spaces lined by columnar mucin-secreting cells which ranged from cytologically bland to moderately atypical with 'bronchioloalveolar pattern' invasion into the adjacent parenchyma.
  • Immunohistochemically, tumor cells were positive diffusely for Cytokeratin 7, and focally for Cytokeratin 20 and Thyroid Transcription Factor-1.
  • CONCLUSION: This case highlights the continuous spectrum of pulmonary mucinous cystic neoplasia from benign mucinous cystadenoma to malignant mucinous cystadenocarcinoma, and the probable existence of a 'borderline' mucinous cystic tumor.
  • Although molecular data are lacking to substantiate progression from benign to malignant in these neoplasms, the importance of recognizing the morphologic continuum lies in alerting pathologists to thoroughly examine specimens to rule out invasive foci in tumors with 'borderline' morphology.

  • 1999 to March 2006, eleven cases with benign tumors in the area between the neck, thorax and axilla were collected and analysed.
  • CT scans showed that the neck, thorax and axilla were affected simultaneously more or less.
  • After the disarticulation and displacement of the clavicle, the tumor was removed with the structures of importance in the neck, thorax and axilla under direct views.
  • CONCLUSIONS: Benign tumors in the area between neck, thorax and axilla could be successfully dissected and removed with displacement of the medial portion of the clavicle.

  • Whereas removal of the tumor by enucleation by conventional thoracotomy or thoracoscopy can be performed in most cases, esophagectomy is required for giant tumors of the esophagus.

  • [Title] Benign thyroid teratomas manifest painful cystic and solid composite nodules: three case reports and a review of the literature.
  • Benign thyroid teratomas are rare in adolescents and adults.
  • We report on three cases of benign thyroid teratomas that presented as painful tumors in the neck after puberty.
  • The tumor adjacent to the thyroid in each case showed rapid enlargement with predominant cystic lesions within several months.
  • Ultrasonography and computed tomography revealed few findings suggesting the origin of the tumor.
  • The patients subsequently underwent resection of the tumor, and microscopic examination revealed various types of tissue including pancreas, adipose, cartilage, muscle, and skin, and the cystic wall was lined by gastric, intestinal, respiratory, and stratified squamous epithelium.
  • Surgical resection was curative, and subsequent histologic examination revealed mature benign teratomas of the thyroid.
  • The main characteristic of our cases presented the painful tumors due to the enlarged cystic formation lined by a variety of different types of epithelium, which agreed with previous cases of benign thyroid teratomas in adolescents and adults.

  • Although primary bronchopulmonary fibrosarcoma is a rare tumor, it may be characterized by the symptoms of acute respiratory distress occurring during the first moments of life in a newborn.
  • It is one of the leading congenital malignant neoplasms of the lung, but is considered a borderline tumor since its biological behavior is much more favorable than that of adult fibrosarcomas.
  • Histopathological diagnosis is not simple, as the microscopic characteristics may be confused with benign fibromatosis or malignant mesenchymal neoplasms.
  • In this case report, we present a case of congenital pulmonary spindle cell tumor showing the features of fibrosarcoma, and we discuss the differential diagnosis of spindle cell lesions localized within the thorax.

  • Tumors with osseous elements can be found, such as hamartoma and amyloid tumor, and reactive lesions such as osseous metaplasia.
  • A 39-year-old male patient was treated for multiple myeloma and got a bone marrow transplantation 2 years and a few months before he presented with a solitary well-circumscribed tumor in the right middle lobe.
  • The tumor presented with a fibrous capsule and consisted of mature bone trabecules.
  • Within the tumor, fatty tissue was seen.
  • This tumor might have been induced by circulating stem cells; however, due to autologous bona marrow transplantation, this cannot be proven.

  • Leiomyomas are rare esophageal disorders, although among the benign esophageal neoplasms, they are the most common.
  • On endoscopy multiple nodules covered with intact mucosa were present, the largest tumor arising from the gastro-esophageal border infiltrating the cardia.
  • Barium swallow demonstrated narrowing of the middle and lower esophagus with the upper third of the stomach filled by the tumor.
  • Thorax and abdominal CT scans revealed infiltration of almost the total aboral esophagus by the tumor with compression of left and right bronchi.
  • Due to the extended tumor growth with infiltration of the upper third of the stomach, a total esophago-gastrectomy with reconstruction by colon interposition was performed.
  • Immunohistochemically the tumor stained positive for desmin and sm-actin and negative for CD34 and c-kit.
  • If the proximal third of the stomach is infiltrated by the tumor an extended resection is necessary.

  • The aim of this study was to present the author's observations of the histological tumor types, occurrence and its surgical treatment.
  • Ultrasonography, scintigraphy, computed tomography or magnetic resonance imaging of abdominal cavity, pelvis, thorax or neck--depend on the tumor localization--were done in every individual.
  • All cases were subjected to surgical procedure with an aim to resect the tumour completely.
  • In adrenal glands we observed 10 benign and 1 malignant pheochromocytoma (one bilateral female case with Multiple Endocrine Neoplasia type 2A).
