  • [Title] Lipoma in oral mucosa: two case reports.
  • Lipoma is a common tumor of soft tissue.
  • Its location on the oral mucosa is rare, representing 1% to 5% of benign oral tumors although it is the most mesenchymal tumor of the trunk and proximal por-tions of extremities.
  • Lipoma of the oral cavity may occur in any region.
  • The buccal mucosa, tongue, and floor of the mouth are among the common locations.
  • The overlying epithelium is intact, and superficial blood vessels are usually evident over the tumor.
  • Other benign connective tissue lesions such as granular cell tumor, neurofibroma, traumatic fibroma and salivary gland lesions (mucocele and mixed tumor) might be included in differential diagnosis.
  • We present two cases of oral lipoma in unusual locations: one in junction of soft and hard palate and the other in tongue.

  • [Title] Spindle cell hemangioma: report of a case presenting in the oral cavity.
  • BACKGROUND: Spindle cell hemangioma, formerly termed spindle cell hemangioendothelioma, is an uncommon benign vascular tumor.
  • Presentation in the oral cavity is rare with only two previously reported cases.
  • RESULTS: The tumor was a well-circumscribed vascular proliferation of spindled to epithelioid endothelial cells.
  • Tumor cells with intracytoplasmic vascular lumens were present in several areas.
  • The tumor was positive for CD31 and CD34.
  • CONCLUSIONS: Spindle cell hemangioma rarely presents in the oral cavity and needs to be considered in the differential diagnosis of oral cavity vascular tumors to avoid misdiagnosis as a more aggressive vascular tumor.

  • Among the biologic agents already examined in pSS, those targeting tumor necrosis factor (TNF)-alpha failed to demonstrate significant benefit.

  • BACKGROUND: Regardless of advances in diagnosis and treatment during the past 40 years, the overall 5-year survival rates for oral and oropharyngeal squamous cancers have only slightly improved and remain around 50%.
  • Thus, the early diagnosis and treatment of carcinoma by health care providers are essential in achieving a good prognosis.
  • We report a case of invasive squamous cell carcinoma that presented as a benign endodontic-periodontic lesion with a 7-mm periodontal pocket on tooth #15 in a 40-year-old, non-smoking woman.
  • METHODS: Our patient was seen for a comprehensive periodontal examination including a periodontal charting, occlusal analysis, study casts, electronic pulp test for tooth #15, and complete mouth periapical radiographs.
  • The patient remained free of tumor for 5 years after the initial presentation.
  • CONCLUSIONS: Patient education and periodic oral cancer examinations by dental professionals are necessary to reduce diagnostic delay and improve prognosis.

  • BACKGROUND: Oncogenesis in the oral cavity is believed to result from genetic alterations that cause a stepwise transformation of the mucosa to invasive carcinoma.
  • In oral squamous cell carcinoma (OSCC) multiple cytogenetic abnormalities have been reported, but their practical significance remains uncertain.
  • METHODS: Fifty-one consecutive samples of OSCC, nine lymph node biopsies showing metastatic spread from OSCC, 16 biopsies diagnosed as oral leucoplakia (OLK), 13 samples corresponding to oral lichen planus (OLP) and 14 samples from normal oral mucosa were included in the study.
  • The genetic and protein status of the CCND1, MYC, EGFR, ERBB2 oncogenes and the TP53 tumour suppressor gene were assessed by fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC).
  • Overexpression of p53, Myc, Cyclin D1, c-erbB-2 and epidermal growth factor receptor (EGFR) was more prevalent in malignant samples than benign samples (P < 0·05).
  • Evaluation of the genetic status of TP53, MYC, CCND1, ERBB2 and EGFR may be an additional diagnostic tool in distinguishing benign from malignant oral lesions in histopathologically challenging cases.

  • Granular cell tumor (GCT) is an uncommon neoplasm of controversial origin that can appear in any corporal localization, including the orofacial region.
  • Although aggressive and malignant variants of this neoplasm have been described, most of GCTs are benign.
  • In spite of the amount of research, the etiology of this neoplasm remains unclear and its histogenesis and its possible muscular, connective or neural origin has been broadly debated.
  • In this paper 8 oral cases are presented, corresponding to 5 women and 3 men, with a mean age of 36.1 years old and a mean time of evolution of the lesions of 8.3 months.
  • The main clinicopathologic and diagnostic features of this neoplasm are reviewed and discussed.

  • OBJECTIVES: Mutation of the p53 gene and overexpression of its protein are frequent in human cancer, suggesting that these events play a critical role in the onset or progression of many types of tumors.
  • Angiogenesis also has been demonstrated to be associated with tumor progression, aggressiveness, and metastases.
  • This study aimed to investigate-in normal oral epithelium, hyperplasia, dysplasia, and invasive oral squamous cell carcinoma-the prevalence of p53 protein immunoreactivity and of angiogenesis, and to determine the correlation between them.
  • Seven normal samples of oral mucosa were used as control.
  • RESULTS: This study showed that p53 protein was confined to the basal cell layer in normal oral mucosa and in the hyperplastic group.
  • The percentage of p53-positive cells was evaluated, and statistically significant differences were found between normal oral mucosa and severe dysplasia, between normal mucosa and invasive carcinoma, and between mild and severe dysplasia. p53 expression showed no significant correlation with tumor grading.
  • The number and size of blood vessels increased from normal oral epithelium through dysplastic epithelium to reach a maximum in invasive carcinoma.
  • CONCLUSION: Angiogenesis and p53 protein immunoreactivity increased with progression from normal mucosa to invasive carcinoma, and both factors were directly correlated to each other suggesting that they may play a critical role in oral carcinogenesis.

  • BACKGROUND: Granular cell tumor (GCT), also known as Abrikossoff tumor, is an uncommon benign neoplasm, probably of neural origin derived from Schwann cells.
  • OBJECTIVES: We aimed to analyze the clinical, histologic, and immunohistochemical characteristics associated with this tumor and to determine whether these findings correspond to those reported to date in the literature.
  • METHODS: In this retrospective study of 34 patients with histologic diagnosis of GCT, we analyzed clinical characteristics (site, age, sex, duration, and suspected diagnosis), histological findings (border, cell atypia and mitoses, involvement of adnexal structures, pseudoepitheliomatous hyperplasia, and presence of the recently described pustulo-ovoid bodies), and immunohistochemical findings (S-100 staining in 16 randomly selected cases).
  • The most common site was the oral cavity (61.76 %).
  • The most frequently suspected clinical diagnosis was fibroma (17.65 %).
  • CONCLUSIONS: Our series confirms the characteristics described previously for GCT, except for certain peculiarities, and supports the presence of pustulo-ovoid bodies as an additional histologic finding for diagnosis of this tumor.

  • BACKGROUND: The oral cavity is a rare but occasional target for metastases, which may masquerade as various benign and inflammatory lesions, and sometimes also be asymptomatic.
  • Oral metastatic lesions have been described in various cancers, particularly lung, breast and kidney carcinoma.
  • The patient underwent gastric resection but died of heart failure on the tenth postoperative day; a post-mortem examination revealed a residual bilateral ovarian infiltration by gastric carcinoma (Krukenberg's tumor).
  • CONCLUSION: An occult carcinoma of the stomach may rarely metastasise to the oral cavity even as a first and exclusive manifestation; it is important to bear this possibility in mind because such conditions may mimic a benign disease.

  • [Title] Cervical lymph nodes with or without metastases from oral squamous carcinoma: a correlation of MRI findings and histopathologic architecture.
  • OBJECTIVES: The aim of this study was to describe the magnetic resonance images (MRIs) of the architecture of metastatic lymph nodes as well as healthy lymph nodes and to correlate the images with the histopathologic architecture of the lymph nodes from oral squamous cell carcinoma.
  • STUDY DESIGN: The signal intensities of 98 cervical lymph nodes from 20 patients with oral squamous cell carcinoma were evaluated on T2-weighted and diffusion-weighted whole-body imaging with background body signal suppression (DWIBS).
  • RESULTS: On T2-weighted images, very hyperintense and/or isointense areas were specific findings for lymph nodes containing metastases from oral squamous cell carcinoma.
  • Using DWIBS images with inverted black-and-white image contrast, cystic degeneration, keratinization, fibrous tissue, tumor tissue, and lymphoid tissue were either hypointense or intermediate in intensity.
  • CONCLUSIONS: On T2-weighted images, very hyperintense and/or isointense areas were characteristic findings for lymph nodes containing metastases from oral squamous cell carcinoma.
  • Using DWIBS images, it was difficult to differentiate metastatic from benign lymph nodes.

  • METHODS: Laser scanning cytometry (LSC) was used for multiparametric analysis of cells stained for cytokeratin and DNA to determine the DNA-index (DI) of the tumor cells.
  • In the remaining 99 specimens, 1 benign lesion was misclassified as malignant, while 61 of the 75 malignant lesions were correctly identified.
  • This corresponds to predictive values of 98.4% and 62.2% for the detection of malignant and benign samples by LSC.
  • CONCLUSION: This pilot study demonstrates the validity of LSC screening for the identification of tumor malignancy in the upper aerodigestive tract from swab collected cytological material.

  • Small-breed dogs, especially Cocker Spaniels, were at increased risk for plasma cell tumors.

  • An unusual case of isolated plexiform neurofibroma arising in the oral cavity without other clinical manifestations or family history of neuro-fibromatosis-1 (NF-1) is described.
  • The tumor was histopathologically analyzed and an immunohistochemical panel comprising S-100 protein, epithelial membrane antigen (EMA), collagen IV, and CD34 was performed.
  • Subjacent to the oral epithelium, tactile-like bodies were also detected.
  • On the basis of this report, we would like to emphasize that plexiform neurofibroma can occur in the oral cavity as a benign isolated tumor in patients without other stigmata of NF-1.

  • The purpose of this study is to evaluate our experience with the use of both free flaps in the reconstruction of oral cavity defects after tumoral ablation.
  • PATIENTS AND METHODS: Over a 9-year period, 103 consecutive patients were treated in our department for the reconstruction of oral defects after tumor ablation by means of microvascularized free flaps.
  • Patients were treated for benign (n = 15) and malignant (n = 82) entities.
  • CONCLUSIONS: Our results reveal that the RFFF is a reliable method for reconstructing a wide range of oral cavity defects with an acceptably low morbidity rate.
  • It provides adequate bulk and pliability, enabling the reconstruction of a wide variety of locations within the oral cavity.

  • Oral cancer is among the 10 most common cancers worldwide, and is especially seen in disadvantaged elderly males.
  • As patient awareness regarding the danger of oral cancer increases, the demand for "screening" is expected to increase.
  • The signs and symptoms of oral cancer often resemble less serious conditions more commonly found and similarly usually presenting as a lump, red or white patch or ulcer.
  • If any such lesion does not heal within 3 weeks, a malignancy or some other serious disorder must be excluded and a biopsy may be indicated.
  • Dental health care workers have a duty to detect benign and potentially malignant oral lesions such as oral cancer and are generally the best trained health care professionals in this field.
  • Prompt referral to an appropriate specialist allows for the best management but, if this is not feasible, the dental practitioner should take the biopsy which should be sent to an oral/head and neck pathologist for histological evaluation.

  • [Title] Serum hyaluronan levels in oral cancer patients.
  • BACKGROUND: Hyaluronan (HA) is most likely associated with tumor invasion and metastasis.
  • Studies have shown that HA levels are often increased in serum of patients with various malignant tumors.
  • The purpose of this study was to determine the levels of serum hyaluronan in patients with oral cancer and evaluate the value of serum HA in adjuvant diagnosis, staging and monitoring treatment response in these patients.
  • METHODS: Eighty-four hospitalized patients with oral cancer, 65 patients with benign tumors in the oral and maxillofacial region and 67 healthy individuals were included in this investigation.
  • One week after therapy, venous blood was collected once again in 43 patients with oral cancer.
  • RESULTS: The serum HA concentration was significantly higher in oral cancer patients than in patients with benign tumors and in healthy controls (P<0.05).
  • After a complete treatment the HA levels in patients with oral cancer became lower than before treatment, but the difference was not significant (P>0.05).
  • CONCLUSIONS: The results of this study suggest that the determination of HA levels may provide additional information in diagnosis of oral cancer, but its usefulness as an adjunct in clinical staging and in monitoring treatment response was limited.

  • Intramuscular myxoma is an uncommon benign soft tissue neoplasm and a distinct histopathological entity within the heterogeneous group of myxomas.
  • We report on a case of a myxoma within the mimetic muscles of the nasal and mouth region in a 52-year-old man.
  • The tumor was surgically removed and the patient is free of recurrence or complications 8 months after treatment.

  • The aim of this study was to determine whether hK13 is expressed in salivary gland tissues and salivary gland tumors (both benign and malignant), in order to compare normal with tumor tissues.
  • The results of this study indicate that most salivary gland tumors show high levels of expression of hK13.
  • Ductal cells and cells lining duct-like structures showed a higher intensity of staining than non-ductal cells in most tumors.
  • Tumors which exhibited only non-ductal cells also exhibited cytoplasmic staining.
  • In conclusion, we demonstrate the high expression of hK13 in several common salivary gland tumors.

  • PURPOSE: A clinico-pathologic study of typical symptoms of intra- and extraoral angiosarcomas and clinical course under therapy is presented as well as an analysis of the immunohistochemical differential diagnosis of the tumour specific formed spaces.
  • RESULTS: While the benign appearance of the lesions resulted primarily in wrong diagnoses the histopathologic examination of the biopsies revealed the characteristic pattern of angiosarcomas.

  • [Title] Human calmodulin-like protein (CLP) expression in oral squamous mucosa and in malignant transformation.
  • PURPOSE: The purpose of this study was to test whether calmodulin-like protein (CLP) is expressed in normal human oral mucosal cells and if downregulation of CLP occurs in malignant transformation.
  • MATERIALS AND METHODS: Oral mucosal tissue was taken from three individuals in a double-blind manner.
  • Three oral mucosal tissue samples with biopsy-confirmed squamous cell carcinoma were obtained.
  • Staining patterns and intensity were noted in normal oral mucosa, comparing them to the normal breast epithelium sample.
  • Staining patterns and intensity were then observed in squamous tumor cells, comparing them to the patterns of benign squamous mucosa.
  • RESULTS: CLP coding sequences were positively identified from the normal oral mucosal tissue samples by reverse transcription and polymerase chain reaction (RT-PCR) with 100% identity to the published CLP sequence (accession #M58026).
  • In the three oral mucosa tissue samples with known squamous cell carcinoma, expression of CLP was readily detected in areas of normal oral mucosa, while a notable downregulation of CLP expression occurred in areas of malignant transformation.
  • The staining intensity was equivalent to the staining seen in the benign breast epithelium used as a control.
  • There was a sharp contrast in staining quality and clarity between benign and malignant tissue.
  • CONCLUSIONS: The RNA for human CLP is found in normal oral mucosa.
  • CLP expression is seen in normal oral mucosa with a downregulation of CLP expression in malignant transformation.

  • Acanthosis nigricans (AN) is a rare condition which may affect oral cavity.
  • There are two forms of AN: benign and malignant.
  • Benign AN may be hereditary, or may be related to systemic diseases or drugs.
  • Oral lesions occurred after the tumor had been diagnosed, i.e. after third operation for tumor recurrence.
  • The fifth operation for tumor recurrence resulted in slight improvement of oral changes.

  • [Title] GLUT-1 in oral benign vascular lesions.
  • AIM: To investigate the accuracy of histological diagnosis of oral hemangioma, oral vascular malformation and oral pyogenic granuloma according to immunohistochemical evaluation of the human erythrocyte-type glucose transporter protein (GLUT-1), and to observe the immunoexpression of this protein in oral varix.
  • MATERIALS AND METHODS: Immunohistochemistry for GLUT-1 was performed in 93 histologically diagnosed cases of oral benign vascular lesions: 17 vascular malformations, 19 hemangiomas, nine varix, and 48 pyogenic granulomas.
  • RESULTS: None of the cases of the oral benign vascular lesions evaluated were immunopositive to GLUT-1.
  • The 19 cases histologically diagnosed as oral hemangioma that showed negative staining to GLUT-1 were reclassified as oral pyogenic granuloma or oral vascular malformations.
  • The histological evaluation itself is not enough to obtain the correct diagnosis of oral HEM as none of the sample cases were true hemangioma.
  • All sample cases with initial vascular malformation or pyogenic granulomas classification were negative to GLUT-1, demonstrating the accuracy of histological diagnosis of these lesions itself.
  • Oral varix showed negative staining to GLUT-1 in blood vessels.
  • CONCLUSIONS: GLUT-1 is an useful, effective and important auxiliary marker for the diagnosis of oral benign vascular lesions.
  • CLINICAL RELEVANCE: This study showed that histological diagnosis alone is not sufficient to correct diagnoses of oral hemangioma.

  • BACKGROUND: Lipoma, a benign tumor of adipose tissue, is rarely seen in the oral cavity.
  • When it does occur it is usually found in the cheek or tongue, followed by the lip, gingival, and floor of the mouth.
  • A clinical diagnosis of lipoma was established, and the treatment consisted of complete excision of the mass under local anesthesia.
  • SUMMARY: Although some larger oral masses are better managed surgically at hospital facilities, the present case was treated with local anesthesia with no complications.

  • Cutaneous adnexal differentiation is well-recognized in benign mixed tumors occurring in cutaneous sites.
  • We sought to describe the incidence of cutaneous adnexal differentiation in benign mixed tumors of the palate, lip, and parotid gland.
  • Benign mixed tumors of the palate (n=30), lip (n=13), and parotid gland (n=37) resected between 1980 and 2009 at a single academic medical institution were reviewed.
  • All hematoxylin and eosin-stained sections containing neoplasm were reviewed by all authors including one dermatopathologist (S.H.O.).
  • After confirming the diagnosis of benign mixed tumor, we evaluated for morphologic evidence of cutaneous adnexal differentiation and metaplastic epithelial and stromal changes.
  • Cutaneous adnexal differentiation was seen in 20% of palate and 39% of lip benign mixed tumors but in no parotid tumors.
  • The most frequent features of cutaneous adnexal differentiation were tricholemmal differentiation (20% of palate and 39% of lip tumors), infundibulocystic structures (17% and 31%), and trichohyalin granules (13% and 31%).
  • Sebaceous differentiation was seen in only one palate tumor.
  • Varying amounts of stromal adipose were seen in 62, 37, and 22% of lip, palate, and parotid tumors.
  • Osseous metaplasia was seen in one tumor from each site.

  • An incision biopsy revealed the diagnosis of oncocytic tumor, and enucleation was performed.
  • The tumor was stained for immunohistochemical analysis using the ABC method and antibodies against cytokeratin (CK), epithelial membrane antigen (EMA), desmin, S-100 protein and muscle-specific actin, respectively.
  • The tumor was stained with uranyl acetate and lead citrate for visualization by electron microscopy.
  • RESULTS: Histopathology results revealed that the tumor consisted of oncocytic cells, characterized by eosinophilic and granular cytoplasm, and atypical nuclei.
  • Immunohistochemical staining indicated that these cells were positive for CK and EMA, and negative for desmin, muscle-specific actin and S-100 protein.
  • Electron microscopy revealed numerous dilated cytoplasmic mitochondria, and the cell contours and nucleic shapes of tumor cells were often irregular.
  • CONCLUSIONS: Because the histopathologic features of OC are similar to those of benign oncocytoma, the diagnosis of OC must be confirmed by a combination of clinical and ultrastructural characteristics.

  • In panoramic radiographs was evident a coronoid tumor localized in the union of zigomatic arch and bone.
  • Under general anaesthesia coronoidectomy was made, recovering mouth opening until 43 mm.

  • Cutaneous canine melanomas are usually benign in contrast to human malignant melanoma.
  • However, the canine oropharyngeal, uveal, and mucocutaneous neoplasms are aggressive and have metastatic potential.
  • Case no. 1 was an advanced stage III oral melanoma that was cured from malignant melanoma with 2 intratumor AdCD40L injections before cytoreductive surgery.
  • After treatment, the tumor tissue was infiltrated with T lymphocytes and B lymphocytes suggesting immune activation.
  • One hundred and twenty days after start of gene therapy and 60 days after the last injection, the tumor had regressed dramatically, and the dog had a minimal tumor mass and no signs of progression or metastasis.

  • OBJECTIVE: To detect the levels of carcinoembryonic antigen (CEA) and carcinoma associated antigen CA-50 in the patients with oral or salivary malignant tumors.
  • METHODS: The concentrations of salivary CEA and CA-50 were assayed in 80 patients of oral and salivary malignant tumors, 40 patients of benign tumors and 80 health controls.
  • In 80 patients with malignant tumors, serum CEA and CA-50 were also assayed by enzyme-linked immunoabsorbent assay and immunoradiometric analysis, respectively.
  • RESULTS: Salivary CEA and CA-50 levels were significantly higher in malignant tumors than in benign tumors and in health controls, respectively (P < 0.001).
  • Only 7 cases and 3 cases of 80 patients with malignant tumors were found having increased serum CEA and CA-50 levels, respectively.
  • This may be more useful as prognostic indicators in early diagnosis of oral and salivary malignant tumors.

  • INTRODUCTION: The aim of the study was to produce a systematic review about etiology, pathology, diagnosis, prognosis and clinical management regarding oral and cervical teratomas.
  • MATERIALS AND METHODS: A systematic review of Pubmed/Medline using the following keywords was made: epignathus, cervical teratoma, fetus, oral teratoma, prenatal diagnosis, prognosis, treatment, ultrasound.
  • (1) teratomas are rare, usually benign congenital tumors which recognized multifactorial etiology;.
  • (2) prenatal ultrasound diagnosis can be made early in pregnancy (15-16 weeks);.
  • (3) 3D ultrasound and MRI may enhance the accuracy of the antenatal diagnosis (location, extension and intracranial spread) and may aid in the selection of patients requiring treatment;.
  • (5) delivery should involve elective Cesarean section with ex utero intrapartum treatment procedure or resection of the tumor mass, which may be performed on placental support operation on placental support procedure to increase the chances of postnatal survival.

  • The Cub and Sushi Multiple Domains-1 (CSMD1) is a tumor suppressor gene on 8p23.2, where allelic loss is both frequent and associated with poor prognosis in head and neck squamous cell carcinoma (HNSCC).
  • To understand the extent of CSMD1 aberrations in vivo, we characterized 184 primary tumors from the head and neck, lung, breast and skin for gene copy number and analyzed expression in our HNSCCs and lung squamous cell carcinomas (SCCs).

  • Cytology from fine needle aspiration indicated spread of a melanocytic tumor and she underwent a modified supraomohyiod neck dissection.
  • One of the lymph nodes showed an inclusion of cells in the deep layers with epitheloid and spindle cells in a pattern similar to that of the primary oral lesion.
  • It is believed that this may be an example of how a Spitz tumor, although inherently benign, can spread along lymphatics in a pseudometastatic fashion.

  • The aim of the present study is to emphasize the importance of stereolithographic models in planning tumor surgery and how it affects the treatment planning, operation time and prognosis.
  • The clinical findings were 8 mm deviation of midline to the right side, 23 mm mouth opening, unilateral posterior cross-bite on the right side, and 8 mm negative horizontal overjet.

  • [Title] Oral and maxillofacial sclerosing epithelioid fibrosarcoma: report of five cases.
  • Lesions involving the oral and maxillofacial region (OMFR) and intraosseous examples are rare.
  • The OMF Pathology Department Registry was searched for cases coded from 1990 to the present as "SEF," "fibrosarcoma not otherwise specified" or "neoplasm of uncertain histiogenesis."
  • Tumor location included the infra-temporal fossa, buccal mucosa (recurrence extending into bone), anterior mandible (intraosseous primary, focally extending into soft tissue), and left parotid and submandibular gland (with metaplastic bone) regions.
  • Tumor sizes ranged from 1.0 to 5.7 cm, median 3.5 cm.
  • Histologically, the tumors were well delineated and multinodular, separated by fibrous septae.
  • The spindled to primarily epithelioid tumor cells formed moderately cellular sheets and cords of irregularly contoured medium to large, round to oval, occasionally overlapping nuclei, indistinct nucleoli, wispy eosinophilic (retracting) cytoplasm, and distinctive cytoplasmic borders, embedded in osteoid-like stroma.
  • Three tumors were graded as 2/3 and two 1/3.
  • Immunohistochemically, the tumor cells were positive for vimentin, 1 case focally for CD34, whereas all cases were negative for S100 protein, keratins, EMA, desmin, and SMA.
  • The differential diagnosis for these tumors in this site includes sclerosing carcinoma, Ewing/PNET, osteosarcoma, osteoblastoma, and benign and malignant myoepithelial salivary gland tumors.
  • The collagen, focal spindle cell features, HPC-like vasculature, and weak focal CD34 reactivity in one case might have raised a possible relationship between OMFRSEF and low grade malignant solitary fibrous tumor, but the intraosseous propensity, epithelioid features and relative lack of CD34 make this a distinctive entity.

  • BACKGROUND: Prediction of progression from pre-malignant oral mucosal lesions to malignancy, or recurrence of an existing oral squamous cell carcinoma (OSCC), is an important clinical problem in oral medicine.
  • Samples from 54 patients with OSCC, 45 with oral lichen planus (OLP) and 45 with hyperkeratosis (clinically leukoplakia), diagnosed between 1987 and 1996, were analysed for TP53 protein expression and TP53 mutation.
  • One hyperkeratosis patient (non-mutated) developed OSCC in the same site.
  • CONCLUSION: TP53 mutations can exist in benign oral mucosal lesions for many years without progression to malignancy.

  • [Title] Transoral robotic surgery for parapharyngeal space tumors.
  • PURPOSE: To evaluate the outcomes of patients with parapharyngeal space (PPS) tumors treated with a transoral robotic surgery (TORS) approach.
  • MATERIALS AND METHODS: We prospectively enrolled well-defined benign PPS tumors accessible from the oropharynx with no carotid encasement or bone erosion.
  • We designated outcome measures that included technical feasibility of the approach, local tumor control and complication rates.
  • TORS was completed in 9 of 10 patients with acceptable operative time and blood loss and no significant complications including hemorrhage, infection, trismus or tumor spillage.
  • CONCLUSIONS: We confirm the safety and feasibility of the TORS approach for PPS tumors that achieves a high local control and low surgical complication rate.
  • TORS is a viable approach for removing benign tumors and lesions of the PPS.

  • Gene expression profiling of oral premalignant (OPM) cells and normal oral epithelial (NOR) cells showed that EMMPRIN expression was markedly upregulated in OPM cells compared to NOR cells.
  • We used an oral squamous cell carcinoma (OSCC) progression model composed of cell lines, organotypic cultures and tissue specimens to characterize EMMPRIN expression patterns by microarray analysis, qRT-PCR, Western blotting and immunohistochemistry.
  • EMMPRIN expression was limited to basal cells of normal, benign hyperkeratotic and inflammatory (lichen planus) oral mucosa.
  • These results suggest that EMMPRIN overexpression occurs at a very early stage of oral carcinogenesis and plays a contributing role in OSCC tumorigenesis.

  • [Title] Solitary fibrous tumor of the oral cavity: the need for an extensive sampling for a correct diagnosis.
  • BACKGROUND: Solitary fibrous tumor (SFT) is an uncommon but well-characterized soft tissue tumor that was first described as a pleural lesion and now is considered ubiquitous, having been detected at many extra-pleural sites (abdominal cavity, orbit, upper respiratory tract, and oral cavity).
  • Histologically, SFT may show wide morphological variability of both its cellular and stromal components, which may lead to incorrect diagnosis especially when dealing with small incisional biopsies.
  • MATERIALS: We report on the clinical, morphological and immunohistochemical features of eight SFT occurring in the oral cavity.
  • RESULTS: Microscopically all eight tumors showed widely variable morphological features in terms of cellular density and stromal architecture, thus simulating benign fibrous histiocytoma, schwannoma, hemangiopericytoma or low-grade sarcoma in distinct areas of the same lesion.
  • Among these eight cases, five had been diagnosed as SFT, two as benign fibrous histiocytoma and one as low-grade sarcoma.

  • BACKGROUND: This study evaluated the performance of oral brush biopsies using standard morphological analysis and haematoxylin and eosin (HE) staining for detecting oral squamous cell carcinomas and their respective precursor lesions PATIENTS AND METHODS: Brush biopsies were obtained in 169 consecutive patients who underwent routine biopsies and histological examination for clinically suspicious oral lesions.
  • Cytological assessment used well-established criteria of atypia to classify the specimen as either "tumor negative" (no signs of atypia, no malignant cells) or "tumor positive" (malignant cells, any sign of atypia or doubtful cells).
  • RESULTS: Despite a sufficient number of cells, a definite cytological diagnosis could not be established in six cases.
  • According to the criteria specified above, these specimens were classified as "tumor positive."
  • The cytological analysis identified 49 out of 62 oral malignancies (sensitivity 79%).
  • Seven out of 107 benign lesions were classified as false positive (specificity 93%).
  • CONCLUSION: Oral brush biopsies will identify only about 80% of oral malignancies when the smears are processed by routine HE stains and are analysed via standard morphological criteria.
  • However, oral brush biopsy provides a versatile back-up strategy to uncover the true nature of the disease if a lesion is clinically considered benign by mistake.

  • [Title] Orofacial tumors in Libyan children and adolescents. A descriptive study of 213 cases.
  • AIM: The aim of this study was to determine the relative frequency of orofacial tumors and tumor like lesions in Libyan children and adolescents, and compare our findings with reports in the literature.
  • METHODS: A total of 213 cases of tumors and tumor like lesions were seen in patients aged 18 years and less during a period of 17 years.
  • RESULTS: There were 8 cases (3.7%) of malignant tumors in our study, 35 cases (16.4%) of benign odontogenic tumors and 170 cases (89%) of benign tumors and tumor like lesions of orofacial region were seen in this study. orofacial tumors are not uncommon among the Libyan children and adolescents.
  • CONCLUSION: A slightly lower incidence of malignant tumors was observed as a result of the less number of cases of Burkitt's lymphoma, in comparison to other reports from Africa.

  • BACKGROUND: To describe sequential changes in Merkel cells (MC), and CD10(+) and CD34(+) stromal cells (SC) during the transition from benign to malignant oral lesions and correlate with clinicopathologic parameters.
  • MATERIALS AND METHODS: Changes in cytokeratin 20-positive (CK20(+)) Merkel cells, CD10(+) and CD34(+) SC were immunohistochemically examined in specimens of 28 oral verrucous carcinomas (VC), 32 squamous cell carcinomas (SCC) and 36 benign squamous lesions (BSL).
  • However, the three tumor types had similar staining level and number of CD10(+) SC.
  • CONCLUSION: CD34(+) SC and to a lesser extent MC, but not CD10(+) SC, reveal statistically different density during the transition from benign to malignant oral lesions.
  • The correlations between the CD34(+) SC expression and squamous lesions may be associated with epithelial dysplasia and/or tumor invasion.

  • Squamous dysplasia of the oral cavity indicates increased risk of progression to squamous cell carcinoma (SCC).
  • An important advance would be the development of a minimally invasive assay for identification of oral SCC and dysplasia.
  • We have investigated the suitability in this context of immunostaining oral smears for minichromosome maintainance proteins (MCMs), sensitive and specific biomarkers of cell cycle entry.
  • Immunohistochemical examination of 66 oral tissue samples showed a greater frequency of Mcm-2 expression in surface layers of moderate/severe dysplasia and SCC compared to benign keratosis/mild dysplasia.
  • Immunocytochemistry for Mcm-2/Mcm-5 was performed on 101 oral smears.
  • Conventional smears included 23 from normal mucosa, benign proliferative disease and mild dysplasia, all of which were MCM negative.
  • Six smears from benign lesions were adequate and MCM negative.
  • We conclude that MCMs are promising markers for early detection of oral SCC and dysplasia, particularly in a liquid-based cytology platform.

  • [Title] The managements of orofacial tumors of children in Iraq.
  • These studies were carried out in the maxillofacial unit, surgical specialties hospital, medical city, Baghdad, included 55 patients with orofacial tumors; their ages ranged from 2 days to 14 years (mean, 7 years).
  • Tumors included 20 cases of benign tumors and 35 cases malignant.
  • Treatment modalities ranged from complete surgical excision, surgical shaving operations, and deep x-ray therapy (DXT; radiotherapy) for some benign tumors.
  • The management of malignant tumors was carried out by the use of chemotherapy, chemotherapy and DXT to radical surgery, or radical surgery with DXT.
  • Reconstruction of the mandible was carried out using a rib graft or a block of a corticocancellous bone graft from the iliac crest with reimplantation of the condyle after resection from the tumor and fixed by rigid fixation to the bone graft.
  • Temporary reconstruction of the mandible done by Kirschner wire for malignant tumors required a postsurgical DXT.
  • A temporalis muscle flap was used for the augmentation of the orbit with a frontoorbital flap after radical excision of a malignant tumor of the orbit, and a silastic implant (silicon rubber) was used for reconstruction of the orbital floor.
  • The aim of this study was to present a certain number of cases with an interesting pathologic tumor condition showing peculiar behaviors; the management of these cases was a challenge to our surgical experience.

  • Occurring most often in children and young adults, CFTs are clinically benign lesions that can form over a broad anatomic distribution, including in subcutaneous and deep soft tissues, as well as in serosal and visceral locations.
  • In this article, we describe a case of CFT in a 29-year-old woman with a 7-cm mass on the right upper gingiva and hard palate, discuss the differential diagnosis with other oral spindle cell lesions, such as, desmoid fibromatosis, nodular fasciitis, inflammatory myofibroblastic tumors, solitary fibrous tumor and also review the recent literature on this rare benign entity.

  • [Title] Immunohistochemical study of GLUT-1 in oral peripheral nerve sheath tumors.
  • AIM: To investigate the immunoexpression and diagnostic applicability of human erythrocyte-type glucose transporter protein (GLUT-1) in oral peripheral nerve sheath tumors.
  • MATERIAL AND METHODS: Specimens diagnosed as oral peripheral nerve sheath tumors archived in the Oral Pathology Service of Universidade Federal de Minas Gerais from 1966 to 2006 were evaluated.
  • Thirty-four lesions were included: 15 traumatic neuromas, 11 neurofibromas, four neurilemmomas, and four malignant peripheral nerve sheath tumors (MPNST).
  • RESULTS: Benign lesions were immunopositive for GLUT-1 except in two (18.2%) cases of neurofibromas.
  • One case (25%) of MPNST presented GLUT-1 positive stain in occasional cells distributed homogeneously in all the tumor area.
  • CONCLUSION: GLUT-1 is a useful marker for perineurial cells and should be included in the oral peripheral nerve sheath tumors immunophenotyping thus aiding in the correct diagnosis of these lesions.

  • Chondrosarcoma is a malignant cartilaginous neoplasm that resembles synovial chondromatosis.
  • The present case was of an 11-year-old girl referred to an Oral Disease Center and presenting with a preauricular swelling on the right side and normal ENT evaluation.
  • Discrete pain and mild limitation of mouth opening were observed.
  • Microscopic findings showed a tumor exhibiting cartilaginous tissue proliferation with cellular pleomorphism, nuclear hyperchromasia, and mixoid changes in the matrix.
  • It is concluded that cartilaginous lesions in the jaws must be regarded with suspicion, since benign and malignant lesions may show similar clinical features.

  • The perineurioma is an infrequently encountered benign peripheral nerve sheath tumor composed of a clonal proliferation of perineurial cells.
  • Rare cases of perineurioma have been reported in the oral cavity.
  • Histopathologically, the tumor is composed of a well circumscribed nodular proliferation of spindle cells arranged in a storiform growth pattern, in some areas subtly arranged around vascular channels.
  • The tumor cells reveal positive immunostaining for epithelial membrane antigen (EMA), collagen type IV and vimentin, and negative immunostaining for S-100 protein, consistent with a perineurial origin.
  • To the best of our knowledge, this case represents the first report of an extraneural sclerosing perineurioma involving the oral cavity.

  • The knowledge of cell-cycle control has shown that the capacity of malignant growth is acquired by the stepwise accumulation of defects in specific genes regulating cell growth.
  • Histologic diagnosis might be improved by a quantitative evaluation of more specific diagnosis biomarkers, which could help to precisely identify pre-malignant and malignant oral lesions.
  • The aim of the present study is to evaluate whether computer-based quantitative assessment of p53, PCNA and Ki-67 immunohistochemical expression, could be used clinically to foresee the risk of oral malignant transformation.
  • This retrospective study was carried out in ninety-five oral biopsies, 27 were classified as fibrous inflammatory hyperplasia, 40 as leukoplakia and 28 as oral squamous cell carcinoma.
  • Sixteen out of the 40 leukoplakia were diagnosed as non-dysplastic leukoplakia, the other 24 being dysplastic leukoplakia, of which 50.0% were classified as moderate to severe dysplasia.
  • Comparison of the four groups of oral tissues showed significant rises in p53 and Ki-67 positivity index, which increased steadily in the order benign, pre-malignant, and malignant.
  • In contrast, it was not possible to relate higher PCNA levels with pre-malignant and malignant oral lesions.
  • We therefore conclude that PCNA immunohistochemistry expression is probably an inappropriate marker to identify oral carcinogenesis, whereas joint quantitative evaluation of p53 and Ki-67, appears to be useful as a tumor marker, providing a pre-diagnostic estimate of the potential for cell-cycle deregulation of the oral proliferate status.

  • [Title] Radial forearm free flap for reconstruction of the oral cavity: clinical experience in 55 cases.
  • OBJECTIVE: Since the advent of modern microvascular techniques, the radial forearm free flap (RFFF) has become a reliable method for reconstruction of defects within the oral cavity.
  • The purpose of the present study was to evaluate our experience with the use of the RFFF for the reconstruction of oral cavity defects after tumor resection.
  • STUDY DESIGN: During a 9-year period, 103 consecutive patients were treated in our department for the reconstruction of oral defects after tumoral ablation by means of microvascularized free flaps.
  • Patients were treated for benign (n = 1) and malignant (n = 54) entities.
  • RESULTS: Fifty-five patients underwent reconstruction by means of the RFFF after resection of the oral cavity.
  • CONCLUSION: Our results revealed that the RFFF is a reliable method for reconstructing a wide range of oral cavity defects with an acceptable low morbidity rate.
  • It provides adequate bulkiness and pliability, resulting in adequate reconstruction of a wide variety of defects within the oral cavity.

  • Tumour-invasion like wound healing is characterised by the formation of an extracellular matrix with a high tenascin-C content.
  • Analysis using antibody BC2 indicates that especially the high-molecular tenascin-C (hm tn-C) variants are typically tumour-associated, while distribution in normal tissue is restrictive.
  • This study investigated whether hm tn-C is a suitable indicator of atypical cells with invasive potential in oral brush biopsies.
  • One hundred fifty nine consecutive oral brush biopsies with histopathological diagnoses were analysed for the identification of atypical cells.
  • Conventional cytology produced four false-positives when identifying atypical cells in brush biopsies of inflammatory/benign hyperproliferative mucosa (specificity 96%), while 10 in 52 carcinomas and three of eight recurrences were not identified (sensitivity 78%).
  • Ten of these 13 non-identified tumours could be marked when adding the hm tn-C assay (increasing specificity to 99%).
  • The positive and negative predictive values were 92 and 88% for conventional cytology vs 98 and 97% for the dual assay. (1) A 95%-sensitivity proves hm tn-C assisted conventional cytology to be a suitable means of identifying atypical cells in oral brush biopsies. (2) The positive (98%) and negative (97%) predictive values obtained approximate hm tn-C assisted conventional cytology to laminin-5 (100/97%).

  • OBJECTIVES: To present our early experience with the transoral approach to the superomedial parapharyngeal space (PPS) and describe our technique for removal of these neoplasms.
  • METHODS: Eight patients with various neoplasms of the superomedial PPS were retrospectively reviewed for type of neoplasm, size, success with the transoral approach, need for conversion to another approach, length of hospitalization, and complications.
  • RESULTS: The transoral approach described herein safely allowed for en bloc resection of benign neoplasms with intraoperative control and exposure of the internal carotid artery.
  • Mean tumor size was 3.3 cm (range, 1.5 to 7 cm).
  • This technique is indicated for neoplasms with benign appearance on preoperative imaging or fine needle aspiration.

  • [Title] Clinico-pathologic correlations of myofibroblastic tumors of the oral cavity. II. Myofibroma and myofibromatosis of the oral soft tissues.
  • BACKGROUND: Myofibroma is a solitary benign tumor of myofibroblasts.
  • The clinico-pathologic correlations of myofibroma/myofibromatosis confined only to oral soft tissues were analyzed.
  • METHODS: In the English language literature, 41 myofibroma and 12 myofibromatosis cases involving the oral soft tissues were found.
  • RESULTS: Age at time of diagnosis of oral mucosa myofibroma ranged from birth to 70 years (mean 21.7 years), considerably higher than myofibroma in other parts of the body.
  • Myofibromatosis involving oral soft tissues was diagnosed at birth in nine (75%) patients, within the first year in two, and as a young adult in one.
  • Histologically, oral mucosa myofibroma/myofibromatosis appearance agreed with findings in the literature.
  • CONCLUSIONS: Myofibroma should be included in the clinical differential diagnosis of masses of the oral soft tissues, especially in the tongue and buccal mucosa of children and adolescents.

  • OBJECTIVE: To describe clinical and histological features of oral granular cell tumor (OGCT)and discuss their proliferative activity.
  • METHODS: Clinical and microscopic features were assessed in 14 cases of OGCT collected from the department of oral pathology, college of stomatology of Wuhan University between 1970 and 2003.
  • Immunohistochemical analysis disclosed that 100% of granular cells demonstrated moderated/strong staining for S-100 protein, neuron specific enolase (NSE).
  • Seven patients with benign and two patients with atypical granular cell tumor had no recurrence and metastases.
  • One patient with atypical granular cell tumor had local recurrence after 9 years and died of the disease 10 months later.
  • Most OGCTs are benign but few have malignant potential and periodic follow-up is mandatory to detect malignant transformation.

  • OBJECTIVES: Lipomas are common benign mesenchymal neoplasms that rarely occur in the oral cavity and correspond to less than 4.4% of all benign oral soft tissue tumors.
  • This study describes the clinical, radiological and histological features of cases of oral lipomas seen over a period of 10 years and compared the findings with those reported in the literature.
  • STUDY DESIGN: All cases of oral lipomas seen between 1999 and 2009 were retrieved from the archives of the Stomatology Division of the Federal University of Ceará, Brazil.
  • Age, gender, tumor location, clinical findings, duration, histological subtypes, and treatment outcome were recorded.
  • The specific sites involved were the oral mucosa (n=5), vestibule (n=3), gingiva (n=1), and retromolar region (n=1).
  • The mean size of the tumors was 1.94 cm (range 1.0 to 3.0 cm).
  • The duration of the tumors ranged from 2 to 84 months, with a mean duration of 30.4 months.
  • The findings were compared with 450 cases of oral lipomas reported in the literature.
  • CONCLUSIONS: Lipomas continue to be an uncommon neoplasm of the oral cavity.

  • Malignant mixed tumors of the salivary glands, encompassing carcinoma ex pleomorphic adenoma (ca ex PA), carcinosarcoma and metastasizing pleomorphic adenoma (mPA), are rare neoplasms.
  • When the malignant component does not breach the capsule of the parent PA, the lesion is termed intracapsular ca ex PA, a neoplasm which is thought to have no metastatic potential.
  • Metastatic deposits of ca ex PA are composed exclusively of malignant elements or mixed benign and malignant components.
  • The metastatic deposits resembled benign PA with no histological evidence of malignancy.
  • This pattern of spread is described with mPA, an entity that caused controversy in the past regarding its exact classification as a benign or malignant tumor.
