  • In 23 (68%) the tumor was benign: benign teratoma (16), Leydig-cell tumor (2), epidermoid cyst (2), Sertoli-cell tumor (1), cystic dysplasia (1), intratesticular focal fibrosis (1).
  • Eleven patients (32%) had a malignant tumor: yolk-sac tumor (6), embryonal carcinoma (5).
  • Twenty out of the 26 (77%) prepubertal boys had a benign tumor in contrast to only three of the eight (38%) adolescent males (P=0.079).
  • In eight, the tumor was curatively excised and remaining testis preserved.
  • Two patients with benign teratoma had a recurrence due to incomplete primary resection.
  • In one patient who underwent orchiectomy for benign teratoma, two metachronous teratomas were detected in the contralateral testis 6 years after primary surgery.
  • CONCLUSIONS: In children, most testicular tumors are benign, especially before puberty.
  • If testis-preserving surgery is contemplated, complete excision of the tumor should be ascertained.

  • [Title] [Ovarian steroid cell tumor, not otherwise specified: a clinicopathologic study].
  • OBJECTIVE: To study the clinicopathologic features, diagnostic criteria, differential diagnosis and treatment options of ovarian steroid cell tumor, not otherwise specified (NOS).
  • METHODS: Light microscopy and immunohistochemical study was carried out in 8 cases of ovarian steroid cell tumor, NOS.
  • RESULTS: The 7 cases of benign ovarian steroid cell tumor, NOS were composed mainly of polygonal cells with granular eosinophilic cytoplasm and larger cells with vacuolated cytoplasm.
  • They resembled the architecture of normal adrenal gland, with formation of cell nests and trabeculae.
  • The single case of malignant ovarian steroid cell tumor had evidence of significant cellular pleomorphism, haemorrhage and coagulative tumor necrosis.
  • Immunohistochemical study showed that the tumor cells expressed calretinin and alpha-inhibin.
  • Differential diagnosis included oxyphilic granulosa cell tumor, thecoma, Sertoli cell tumor and clear cell carcinoma.
  • The treatment options of benign ovarian steroid cell tumor, NOS was local excision or ipsilateral salpingo-oophorectomy, while the malignant counterpart should be treated with a combination of surgery and chemotherapy, including administration of GnRH agonist.
  • CONCLUSIONS: Ovarian steroid cell tumor, NOS, is the most common type of ovarian steroid cell tumors.
  • Most of which are associated with a benign clinical outcome.
  • The treatment options of ovarian steroid cell tumor, NOS depend on its malignant potential.

  • [Title] A case report of an uncommon sex-cord stromal tumor consisted of luteal and sertoli cells in a spayed bitch.
  • We report a rare case of benign sex cord-stromal tumor consisted largely of luteoma with minor portion of Sertoli cell tumor located at the position of the left ovary excision in an 11-year-old ovariectomized bitch.
  • Granulosa cell component was lacking, and both luteal and Sertoli cell portions were entirely positive for inhibin alpha and neuron-specific enolase, whereas luteoma portion alone was positive for Wilms' tumor-1 (WT1), immunohistochemically.
  • The results suggest that this tumor is a possible complication of incomplete ovarian excision at the time of ovariectomy and consisted of uncommon hybrid of luteal and Sertoli cells to be diagnosed as an unclassified sex cord-stromal tumor if applied in human cases.
  • WT1-expression pattern suggested the signature of the difference in the phenotype of these cell types.

  • Dimension was particularly related to germ cell tumors (for dimensions between 16 and 32 mm relative risk ratio [RRR] = 13.97, p=0.0449).
  • In particular an interesting correlation was found between the dimensions of the lesion and the malignant pathology and between Leydig cell tumor and infertility.

  • RESULTS: Of 15 primary testicular tumors, eight (53%) were germ-cell tumors (three teratomas, two yolk sac tumors, one seminoma, one embryonic carcinoma and one choriocarcinoma), four (27%) tumor-like lesions (epidermoid cysts), two (13%) gonadal stromal tumors (a Leydig and a Sertoli cell tumor), and one (7%) gonadoblastoma with gonadal dysgenesis.
  • All boys were presented with a painless scrotal mass and four (27%) of them with elevated tumor markers.
  • At a mean 4-year follow-up no patient has presented with recurrent tumor in the residual or contralateral testicle.
  • Postoperative physical examination and scrotal ultrasound were obtained in 14 patients at a median follow-up of 48.2 months, and there was no evidence of tumor progression.
  • CONCLUSIONS: Benign teratoma and epidermoid cysts were the most common prepubertal testicular tumors.
  • Any suspicion of a testicular tumor warrants an inguinal approach to prevent scrotal violation of the tumor.
  • Our limited experience with testis-sparing procedures supports the current trends that organ-confined surgery should be performed for benign lesions such as teratoma, Leydig cell tumor and epidermoid cysts based on frozen biopsy findings.

  • Herein, we describe a rare case of intermediately differentiated Sertoli-Leydig cell tumor with mucinous adenocarcinoma as the heterologous element in a 21-year-old woman.
  • Microscopically, the tumor was composed of intermediately differentiated Sertoli-Leydig cell tumor and well-differentiated mucinous adenocarcinoma.
  • Interestingly, the bulk of the tumor (more than 90%) was composed of mucinous adenocarcinoma, whereas the SLCT component comprised less than 10% of the total tumor.

  • AIMS: To analyse both the clinicopathological and immunohistochemical findings in six cases of uterine tumours resembling ovarian sex cord tumours with a predominant epithelial retiform component resembling the rete ovarii (RUTROSCT) and to compare their immunophenotype with tissues containing retiform structures such as the normal rete ovarii, retiform Wolffian adnexal tumour (RWAT) and ovarian retiform areas of Sertoli-Leydig cell tumours (ORSLCT).
  • The average age of patients was 65 years, and all tumours behaved in a benign fashion.

  • The gonadoblastoma on both sides underwent synchronous neoplastic transformation, into a stage I germinoma in the right streak gonad and a highly differentiated Sertoli cell tumor in the left one.
  • The latter was associated with a myriad of microscopic, Sertoli cell implants on the peritoneal surface, which were considered benign as they had a high grade of differentiation, minimal proliferative activity, and an absence of invasion.

  • [Title] Sertoli cell tumors of the ovary: a clinicopathologic and immunohistochemical study of 54 cases.
  • Ovarian Sertoli cell tumors are rare, and their morphologic spectrum, behavior, and factors influencing the latter are not clearly established.
  • They may be mimicked by many different tumors, some of them more frequent than Sertoli cell tumors; immunohistochemistry may aid in this differential, but its role has not been analyzed in a large series.
  • We studied the clinicopathologic features of 54 Sertoli cell tumors, including the immunohistochemical profile of 23 of them.
  • Delicate septa were occasionally seen and were conspicuous in areas of one tumor.
  • The cells usually had pale to occasionally densely eosinophilic cytoplasm, but 6 tumors were composed of cells with prominent foamy cytoplasm, falling in the category of "lipid-rich" Sertoli cell tumor, and one had cells with clear non-foamy cytoplasm.
  • Two of the three clinically malignant stage I tumors had moderate to severe cytologic atypia and brisk mitotic activity (>5 or more mitoses/10 high power fields [HPFs]), and one of these had tumor cell necrosis.
  • Among the 10 clinically benign stage I tumors with more than 5 years of follow-up, only 3 had >5 mitoses/10 HPFs, but none had more than mild cytologic atypia and none had tumor cell necrosis.
  • Although Sertoli cell tumors usually have a distinctive tubular pattern that facilitates the diagnosis, other patterns may occasionally predominate, causing confusion with various other primary and metastatic ovarian tumors.
  • EMA, inhibin, and chromogranin represent the most helpful triad of immunomarkers serving to exclude two common mimics of Sertoli cell tumors (endometrioid carcinoma [inhibin-; EMA+; chromogranin-] and carcinoid tumor [inhibin-; EMA+; chromogranin+]).
  • Most Sertoli cell tumors are stage I, unilateral, cytologically bland, and clinically benign, but occasional examples are high stage, and about 11% of stage I tumors have worrisome histologic features that may portend an adverse outcome.

  • In this article, the authors review both benign and malignant ovarian masses, as the colorectal surgeon who encounters an adnexal mass at the time of surgery should be aware of the steps necessary for surgical staging and optimal tumor resection.Ovarian tumors-most of which are benign-are divided into three major categories, in order of frequency: epithelial, germ cell, and sex cord-stromal tumors.
  • Nonneoplastic conditions of the ovary that may present as adnexal masses include the following, according to World Health Organization (WHO) classification: pregnancy luteoma, hyperplasia of ovarian stroma, hyperthecosis, massive edema, solitary follicle cysts and corpus luteal cysts, multiple follicle cysts, and endometriosis.Epithelial ovarian tumors arise from the surface epithelium and can be benign or malignant.
  • Histologic types are serous, mucinous, endometrioid, clear cell, or Brenner.
  • Germ cell tumors are more likely to appear in females under 20 years, accounting for 70% of ovarian tumors in this age group.
  • Teratomas are the most common germ cell tumors.
  • The more common sex cord-stromal tumors include granulosa stromal cell tumors, Sertoli-Leydig cell tumors, and gynandroblastomas.Surgical staging and optimal tumor resection are also addressed, with a focus on epithelial malignancies, as they are the most relevant to colorectal surgeons.

  • A 73-year-old woman was found to have a 22 cm unilateral multilocular mucinous cystic tumor of the ovary.
  • Microscopic examination showed a routine appearance of the epithelial component, which ranged from benign to borderline to low-grade carcinoma.
  • The stromal component was unusual because of a striking cellular theca cell component in the stroma, which, in turn, merged with a component of adult granulosa cell tumor.
  • The "parent" neoplasm in this case and 3 other similar cases in the literature appears to be the mucinous neoplasm, in contrast with the other example of mucinous neoplasia associated with sex cord neoplasia, the Sertoli-Leydig cell tumor with heterologous elements, in which the "parent" neoplasm is likely the Sertoli-Leydig cell tumor.

  • OBJECTIVE: We report a new case of Sertoli cell testicular tumor with malignant characteristics.
  • METHODS: 77 year-old male patient, suffering a general wasting syndrome presenting with a left solid testicular mass with the diagnosis of malignant Sertoli cell tumor after orchyectomy, without local, regional or distant dissemination, and a benign outcome after 18 months of follow-up.
  • RESULTS: Sertoli cell tumor or androblastoma is classified as non-germ cell tumor derived from the stroma of the sexual cords.
  • There are three types depending on its cellular composition: calcified big cell, sclerotic cell, and the most frequent of all, the classic type.
  • CONCLUSIONS: Being the Sertoli cell testicular tumor rare, its malignant type is even rarer, accounting for not more than 10% of all.

  • Gonadal germ cell tumors continue to be the cause of diverse, diagnostically challenging issues for the pathologist, and their correct resolution often has major important therapeutic and prognostic implications.
  • Within the teratoma group, there is strong evidence that ovarian and prepubertal testicular teratomas are derived from benign germ cells, a pathogenesis that likely applies also to the rare dermoid cysts and uncommon epidermoid cysts of the testis.
  • As expected, given the foregoing, teratomas in boys are clinically benign, whereas in postpubertal males they are malignant, independent of their degree of immaturity.
  • When uncommon somatic-type malignancies (usually squamous cell carcinoma) occur in mature cystic teratomas of the ovary, this is a de novo form of malignant transformation; similar tumors in the testis, a very rare event, represent overgrowth of teratomatous elements that originated from malignant, nonteratomatous germ cell tumors and, therefore, had previously undergone malignant transformation.
  • Germinomas may have several unusual features in each gonad; these include microcystic arrangements that suggest yolk sac tumor, tubular patterns that mimic Sertoli cell tumor, apparent increased cytological atypia that causes concern for embryonal carcinoma, and prominent syncytiotrophoblast giant cells that suggest choriocarcinoma.
  • A newly recognized aspect of this tumor is the propensity for some to be relatively monomorphic, making them apt to be mistaken for usual seminoma or embryonal carcinoma, although the characteristic polymorphic appearance in some foci, absence of intratubular germ cell neoplasia, unclassified type, and immunohistochemical stains should prevent this error.
  • Yolk sac tumor continues to be confused occasionally with clear cell carcinoma of the ovary.
  • The usually younger age of patients with yolk sac tumors helps with the differential considerations with the nongerm cell tumors, as do other clinical and microscopic features and selected immunohistochemical stains.
  • Syncytiotrophoblast cells alone, admixed with other forms of germ cell tumor, still are confused with choriocarcinoma, but this phenomenon, which is much more frequent than choriocarcinoma, lacks the plexiform arrangement of different trophoblast cell types that typifies the latter.
  • Mixed germ cell tumors (which may show almost any combination of components) are common in the testis but rare in the ovary.
  • A separately categorized, rare form of mixed germ cell tumor seen in both gonads is the polyembryoma.
  • It is perhaps the most photogenic of all gonadal germ cell tumors and is also intriguing because of its distinctive, organized arrangement of yolk sac tumor and embryonal carcinoma elements and recapitulation of very early embryonic development, even to the extent of having in its fundamental unit, the embryoid body, a miniature yolk sac, and amniotic cavity.
  • Embryoid bodies are also common as a minor component of many mixed germ cell tumors, particularly in the testis, and the diffuse embryoma is another variant that has a particular arrangement of yolk sac tumor and embryonal carcinoma elements.
  • Regression of gonadal germ cell tumors is a phenomenon restricted to the testis, for unknown reasons.
  • These so-called 'burnt-out' germ cell tumors can be recognized by a distinctive constellation of findings, including sometimes minor foci of residual recognizable germ cell neoplasia, a well-defined zone of scarring (often having residual ghost tubules), associated lymphoplasmacytic infiltrate, intratubular calcification and, in about 50%, of in situ germ cell neoplasia.

  • [Title] Expression of galectin-3 in gonads and gonadal sex cord stromal and germ cell tumors.
  • The aim of our study was to investigate galectin-3 mRNA and protein expression in normal ovaries and testes as well as in a variety of 51 gonadal sex cord stromal and germ cell tumors, and two testicular seminomatous and non-seminomatous cell lines, using either real-time PCR or immunohistochemistry.
  • In human testes, galectin-3 is specifically expressed in mature Sertoli cells and Leydig cells, and is absent from fetal and pre-pubertal testes, suggesting a hormone-dependence of this gene.
  • In human ovaries, galectin-3 is absent from granulosa cells, as well as from granulosa cell and Sertoli-Leydig cell tumors, and is not a useful marker in distinguishing granulosa cell from Sertoli-Leydig cell tumors.
  • In malignant testicular Sertoli cell tumors, the expression of galectin-3 is down-regulated while, in benign Leydig cell tumors, this expression is maintained, indicating the possible implication of this gene in the development of more aggressive testicular sex cord stromal tumors.
  • In contrast to sex cord stromal tumors, galectin-3 expression is up-regulated in testicular germ cell tumors.
  • By real-time PCR, we demonstrated a significant elevation of the galectin-3 mRNA level in non-seminomatous testicular germ cell tumors and cell line as compared to normal testes and seminomas (p=0.0432 and p=0.0247, respectively), indicating the possible role of this gene in the non-seminomatous differentiation of germ cell tumors.
