  • The term Gardner's syndrome is used to describe extracolonic manifestations, such as osteomas, skin cysts, congenital hypertrophy of the retinal pigmented epithelium (CHRPE), and desmoid tumours (aggressive fibromatosis), that are especially prominent in families with FAP.
  • Proofs of principle that beta-catenin could be the key mediator of the ciliary disorder also rely in the findings that overexpression of beta-catenin induces polycystic kidney disease, and CHRPE phenotypes in animal models.
  • Understanding extracolonic manifestations in the context of FAP as a ciliary disorder might add new therapeutic options for patients with Gardner's syndrome.

  • PATIENTS AND METHODS: Clinical records of 20 patients (10 males, 10 females; mean age 34; range 12 to 63 years) who underwent endoscopic tumor resection with diagnosis of sinonasal tumor between January 1998 and August 2004 were retrospectively reviewed.
  • Patients were reviewed for age, sex, pathologic diagnosis and tumor location, surgical techniques, need for additional surgery, complications, recurrence, and follow-up period.
  • RESULTS: There were five osteomas, one fibrous dysplasia, one ossifying fibroma, seven inverted papillomas, one oncocytic papilloma, three angiofibromas, one schwannoma, and one esthesioneuroblastoma.

  • The suspicion of Gardner's syndrome was raised because extirpation of an osteoma of the left temporo-occipital region was made 10 years ago.
  • We conclude that cranial osteomas often precede gastrointestinal manifestations of familial adenomatous polyposis or Gardner's syndrome and such patients should be evaluated with genetic testing followed by colonoscopy if results are positive to prevent the development of colorectal carcinoma.

  • MATERIAL AND METHODS: Retrospective study of 32 patients between 1986 and 2007 diagnosed with: non-malignant tumours (31.2 %), tumorlike lesions (3.1 %), fibrous-osseous lesions (12.5 %), congenital or acquired malformations (18.7 %) and infection disease (34.3 %).
  • RESULTS: Only 6 of 43 osteomas involved the paranasal sinuses and anterior cranial fossa (13.04 %): 3 cases have developed meningitis and 1 developed a pneumocephalus.
  • 2 cases are meningiomas: 1 was asymptomatic and the other one caused destruction at subtotal frontal bone.
  • CONCLUSIONS: In this study the diagnosis, extension and surgical management were supported in the imaging.

  • METHODS: The expressions of N-cadherin and β-catenin at protein level were detected by immunohistochemical staining in 54 cases of osteosarcoma, 11 cases of osteoid osteoma, 7 cases of osteoblastoma and 8 cases of newly formed bone in nonmalignant bone diseases.
  • RESULTS: In newly formed bone, osteoblastoma, osteoid osteoma and osteosarcoma, the positive expression rate of N-cadherin protein was 75.0%, 71.4%, 63.6% and 35.2%, respectively.
  • The positive expression rate of N-cadherin protein in osteosarcoma was significantly lower than that in osteoid osteoma, osteoblastoma and newly formed bone in nonmalignant bone diseases (P = 0.035).
  • In newly formed bone, osteoblastoma, osteoid osteoma and osteosarcoma, the aberrant expression rate of β-catenin protein was 12.5%, 28.6%, 27.3% and 66.7%, respectively.
  • The aberrant expression rate of β-catenin protein in osteosarcoma was significantly higher than that in osteoid osteoma, osteoblastoma and newly formed bone (P = 0.002).

  • Primary osteoma cutis is also associated with Albright's hereditary osteodystrophy (AHO), which can include hypocalcaemic-type pseudohypoparathyroidism (also known as pseudohypoparathyroidism type Ia) or normocalcaemic-type pseudohypoparathyroidism (also known as pseudopseudohypoparathyroidism).

  • OBJECTIVES: Osteomas are common benign tumors of the paranasal sinuses.
  • Osteomas are also seen as part of Gardner's syndrome, an autosomal dominant disease characterized by intestinal polyposis, osteomas, and cutaneous and soft tissue tumors.
  • On average, osteomas are detected 17 years before colon polyps appear.
  • METHODS: Three patients with maxillary or ethmoid osteomas and chronic sinusitis are presented.
  • CONCLUSIONS: Otolaryngologists should be aware of the possibility of Gardner's syndrome in patients with paranasal sinus osteomas.

  • The aim of the present study was to assess the ability of a novel bone imaging technique to diagnose accurately the cause for back pain as an isolated and presenting complaint in adolescents.
  • The interpretation of PET-CT fused images was based on increased 18F-fluoride uptake and the corresponding CT-located abnormality.
  • Ten patients had positive findings that included 4 cases of spondylolysis (3 of them active), 3 frank fractures (2 of the transverse process and 1 of the facet), 2 osteoid osteomas, 1 osteitis pubis, 1 sacroiliitis, and 2 herniated disks.
  • Treatment was tailored based on final diagnosis.
  • In 5 patients, in whom no abnormality was identified, the back pain resolved spontaneously.

  • PURPOSE: To assess temperature changes in the soft tissue surrounding bone during radiofrequency (RF) ablation of osteoid osteoma in an ex vivo animal model.
  • MATERIALS AND METHODS: Intracortical cavities were created in fresh bovine long bone specimens obtained from a slaughterhouse as models for osteoid osteoma.
  • Three groups of three specimens each were defined according to the thickness (1, 3, and 5 mm) of the cortical bone lamella separating the nidus from the periosteum.
  • Three thermocouples were applied to the soft tissue surrounding the bone in defined distances (0, 5, and 10 mm) from the periosteum.
  • Before RF ablation, the thickness of the cortical bone lamella was documented at computed tomography.
  • Significant temperature differences were shown with analysis of variance and post hoc tests for the three groups of bone lamella thickness and distance (P < .001).
  • CONCLUSION: In the model of osteoid osteoma, the surrounding temperature (soft tissue) during RF ablation was shown to depend on the thickness of the cortical bone lamella and the distance from the periosteum.

  • OBJECTIVE: To evaluate choroidal osteoma for tumor growth, tumor decalcification, related choroidal neovascularization, visual acuity loss, and poor visual acuity.
  • SETTING: Ocular Oncology Service at Wills Eye Hospital, Thomas Jefferson University, Philadelphia, Pa.
  • PARTICIPANTS: There were 74 eyes of 61 patients with choroidal osteoma evaluated between January 1, 1977, and January 1, 2003.
  • A comparison of eyes with subfoveal vs extrafoveal choroidal osteoma showed poor visual acuity in 15 (34%) of 44 eyes and 3 (10%) of 30 eyes, respectively.
  • Eyes with decalcified choroidal osteomas manifested poor visual acuity in 13 (48%) of 27 eyes whereas those with nondecalcified tumors showed poor visual acuity in 5 (11%) of 47 eyes.
  • CONCLUSIONS: Choroidal osteoma showed evidence of growth in 51% of eyes and decalcification in nearly 50% of eyes by 10 years.
  • Overall, poor visual acuity of 20/200 or worse was found in 56% of eyes by 10 years, and decalcified subfoveal choroidal osteomas displayed a particularly poor visual prognosis.

  • BACKGROUND: Studies on bone neoplasms are generally scanty globally and more so in children.
  • Primary bone tumours and tumour-like lesions in children have not been reported from Zaria.
  • OBJECTIVE: To determine the relative frequencies, sex and age distributions, and anatomical sites of occurrence of primary bone tumours and tumour-like lesions in children in Zaria.
  • MATERIALS AND METHODS: A retrospective review of histopathology reports of 40 children with bone tumours and tumour-like lesions in 11 years of age.
  • RESULTS: Benign tumours accounted for 12 (30%) of the 40 tumours reviewed (osteoma 2.5%, osterochondroma 22.5%, fibroma 5%); while malignant tumours occurred in 19 (47.5%) (osteosarcoma 5%, Burkitt's lymphoma 37.5%, diffuse lympholastic lymphoma 5%).
  • Maxilla was the most common site for malignant (30%) and benign tumours (12.5%).
  • CONCLUSION: This study has shown that, primary bone tumours are relatively uncommon in children in our setting.

  • Osteoid osteoma is characterized by an intracortical nidus with a variable amount of calcification, as well as cortical thickening, sclerosis, and bone marrow edema.
  • When these findings are present, a diagnosis of osteoid osteoma is easily made.
  • However, osteoid osteoma may display imaging findings that can be misleading, and it can be difficult to differentiate osteoid osteoma from other conditions such as infection, inflammatory and noninflammatory arthritis, and other tumors.
  • In addition, stress fracture, intracortical abscess, intracortical hemangioma, chondroblastoma, osteoblastoma, and compensatory hypertrophy of the pedicle may mimic osteoid osteoma.
  • To make the correct diagnosis, it is necessary to identify the nidus, and it is important to be familiar with the radiologic findings of osteoid osteoma and its mimics.

  • BACKGROUND: The purpose of our study has been to evaluate the efficacy of single photon emission computed tomography (SPECT) over planar bone scan in identifying solitary vertebral lesions in patients with low backache and its ability to differentiate various pathologies according to the uptake pattern.
  • Benign and malignant lesions were identified according to the uptake pattern in vertebral elements, based on Gary F.
  • Final diagnosis was obtained by means of biopsy or correlation with radiograph or computed tomography (CT) or magnetic resonance imaging (MRI), and / or follow up.
  • Seven out of twenty were localized in anterior vertebral body and were diagnosed as benign ostophytes in six and osteoma in one substantiating the previous observations.
  • SPECT could localize specific lesions as source of pain in eleven patients with low back pain (78%) and identified various etiologies including benign tumors (osteoid osteoma and osteoma), facet arthritis, discitis, transverse process fractures and spondylolysis.
  • Based on SPECT pattern, malignant and benign lesions could be differentiated in the given clinical context.

  • OBJECTIVE: To report the clinical characteristics, long-term follow-up and management of choroidal osteoma.
  • METHODS: Eighteen eyes of 12 cases (5 male and 7 female) of choroidal osteoma were studied.
  • One eye showed spontaneously regression during the long-term follow-up. (2) Laser photocoagulation was effective for 2 eyes, but residual flat serous detachment around the optic disc persisted in another 2 eyes and subsided with TTT. (3) Choroidal osteoma complicated with CNV treated primarily with TTT, prominent improvement was demonstrated.
  • CONCLUSIONS: Choroidal osteoma grows gradually in the natural course and shows pseudopodial margin.

  • [Title] Radiofrequency ablation of intra-articular osteoid osteoma of the hip.
  • Diagnosis and treatment of intra-articular osteoid osteoma is challenging.
  • We present 16 patients with intra-articular osteoid osteomas of the hip treated with percutaneous radiofrequency ablation.
  • Eight osteoid osteomas were located in the femoral head, six in the femoral neck, and two in the acetabulum.
  • Three of the 16 patients had had an incorrect previous diagnosis.
  • At the 12-month follow-up, computed tomography and magnetic resonance imaging showed complete ossification and bone regeneration at the site of the lesion in three patients, partial ossification in six patients and no changes in seven patients.
  • Computed tomography-guided percutaneous radiofrequency ablation is a simple, minimally invasive, safe and effective method for the treatment of most intra-articular osteoid osteomas.

  • PURPOSE OF REVIEW: Osteoid osteoma is a benign painful bone tumor usually found in the lower extremities of children and young adults.
  • The purpose of this review is to highlight and discuss current developments in the management of osteoid osteoma.
  • RECENT FINDINGS: During the past decade, efforts were deployed to minimize bone removal, lessen the risk of pathologic fracture and the need for bone grafting, and thereby shorten the period of convalescence.
  • Improved methods for the precise localization of an osteoid osteoma with use of radioisotope scanning or computed tomography scan have made it possible to treat this lesion with more limited and effective operations, mainly in deep and non-easily accessible osteoid osteomas.
  • SUMMARY: Although they bear the criticism of lacking histological proof for diagnosis of osteoid osteoma, minimally invasive techniques, such as computed tomography-guided percutaneous radiofrequency thermal ablation and laser photocoagulation have become the methods of choice for the treatment of all localizations except those in contact with neural structures (awaiting further research and experience), provided that the diagnosis is based on a typical clinical, scintigraphic and computed tomography presentation.

  • OBJECTIVES: In this prospective study, we evaluated the efficiency of image-guided minimal invasive surgical resection of osteoid osteomas of the long bones.
  • METHODS: Fourteen patients (11 males, 3 females; mean age 13 years; range 4 to 22 years) with osteoid osteoma of the long bones underwent image-guided minimal invasive intralesional extended curettage.
  • Preoperatively, all the patients were evaluated by plain radiographs, computed tomography (CT), bone scintigraphy, and magnetic resonance imaging (MRI).
  • Bone grafting or internal fixation were not required during operations.
  • CONCLUSION: Image-guided minimal invasive surgery is effective in the local control of osteoid osteomas affecting the long bones and causes less morbidity.

  • The commonly taught premise that pediatric back pain frequently has an underlying diagnosis has been recently challenged.
  • There have been few prospective studies, however, about the diagnosis rate and appropriate diagnostic methods for back pain in children.
  • This study prospectively examines the rate of diagnosis for pediatric back pain and the value of various diagnostic studies for this problem.
  • Inclusion criteria consisted of age younger than 18 years, no previous back surgery, no previous diagnosis given, and duration of pain longer than 3 months.
  • The algorithm incorporated commonly used diagnostic techniques including radiographs, magnetic resonance imaging, computed tomography, bone scan, and laboratory studies.
  • The end point was considered to be either (1) a definitive diagnosis or (2) no diagnosis and no symptomatic or clinical changes during a 2-year period.
  • RESULTS: Fifty-seven patients (78.1%) ended with no diagnosis.
  • Three other patients had abnormal laboratory values but no definitive diagnosis.
  • Other diagnoses included Scheuermann disease (n = 2), osteoid osteoma (n = 1), and a herniated disk (n = 1).
  • Contrary to most of the previously published data, most of our patients ended the study with no definitive diagnosis.
  • These results suggest that pediatric back pain frequently does not carry a definitive diagnosis and that exhaustive diagnostic protocols may not be necessary for this problem.

  • BACKGROUND: The endoscopic approach can be used successfully for the treatment of benign tumors such as fibroosseous and vascular lesions, pleomorphic adenoma, glioma, meningioma, and schwannoma.
  • METHODS: Thirty-three patients diagnosed with benign tumors of the nasal cavity and paranasal sinuses and treated using an endoscopic approach were reviewed retrospectively.
  • The mean follow-up was 28 months and only two recurrences (6%) were observed in the juvenile angiofibroma group and in the case of the fibrous dysplasia associated to aneurysmal bone cyst, respectively, 20 and 24 months postoperatively.
  • CONCLUSION: In selected cases, endoscopic surgery can be considered an effective treatment for the resection of benign tumors involving the sinonasal tract.

  • A presumptive diagnosis of a calcified tumour was made on the basis of clinical signs and radiographic and endoscopic findings.
  • A large frontonasal bone flap was used to expose the tumours, which were cleaved into several pieces with an osteotome and removed.
  • Histological examination of the masses identified cementomas in two cases and an osteoma in the third.
  • This case series demonstrates that, although calcified tumours of the paranasal sinuses are rare in horses, they should be considered in the differential diagnosis of purulent nasal discharge, facial swelling and ocular distortion, and are amenable to surgical treatment.

  • BACKGROUND: Osteoid osteoma (OO) is a benign osteoblastic bone lesion that causes severe local pain mainly at night, and usually responds to oral administration of NSAID.
  • Percutaneous CT-guided radiofrequency ablation (RFA) is becoming the treatment of choice for osteoid osteoma and non-aggressive osteoblastoma in the last decade.
  • METHODS: During the period 2005-2009, 38 patients were diagnosed with osteoid osteoma (n = 34), and osteoblastoma (n = 4).
  • CONCLUSIONS: Percutaneous CT-guided radiofrequency ablation of osteoid osteoma and non-aggressive osteoblastoma is simple, safe, and reliable.

  • BACKGROUND: Osteomas of ethmoid sinus are rare, especially when they involve anterior skull base and orbit, and lead to ophthalmologic and neurological symptoms.
  • CASE PRESENTATION: The present case describes a giant ethmoid osteoma.
  • CONCLUSION: Before management of ethmoid osteomas with intraorbital and skull base extension, a thorough neurological, ophthalmological and imaging evaluation is required, in order to define the bounders of the tumor, carefully survey the severity of symptoms and signs, and precisely plan the optimal treatment.
  • The endoscopic procedure can constitute an important part of surgery undertaken for giant ethmoidal osteomas.

  • An osteoid osteoma of the dens axis was diagnosed by computed tomography and bone scintigraphy in a 14-year-old girl with a 1.5-year history of pain.
  • No case of an osteoid osteoma of the dens axis has been published in the literature yet.
  • Regarding its clinical and radiological appearance, this osteoid osteoma was essentially similar to osteoid osteomas in other locations, i.e., it had an osteolytic nidus surrounded by sclerosis, associated with nocturnal pain and a positive aspirin test.

  • [Title] A pathologic fracture of an intracortical chondroma masking as an osteoid osteoma.
  • The differential diagnosis of a tibial intracortical diaphyseal lesion includes osteoid osteoma, periosteal chondroma, nonossifying fibroma, osteofibrous dysplasia, and adamantinoma.
  • While osteoid osteomas represent 5% of all primary bone tumors, little is understood about intracortical chondromas.
  • This article presents the first reported case of a pathologic fracture of an intracortical lesion in a child that shared radiographic and clinical features similar to those of osteoid osteoma, but on histopathologic examination revealed an intracortical chondroma.
  • Our patient exhibited radiographic features of a poorly circumscribed cortical bone sclerosis, a centralized radiolucent nidus on computed tomography, and a hot bone scan of a lesion <1 cm in size that was consistent with an osteoid osteoma.
  • An excision of the bone lesion was performed.
  • The histopathology of the lesion revealed nodules of benign hyaline cartilage in cortical bone, consistent with an intracortical chondroma.
  • Demarcated by cortical bone with mature Haversian systems rather than periosteum or cancellous bone, intracortical chondroma differs from the other 2 chondroma variants, periosteal chondroma and enchondroma, by its relationship to the surrounding bone.
  • The similarities with osteoid osteoma and intracortical chondroma in our patient make it circumspect in regards to ablating lesions (ie, needle radiofrequency ablation) without acquiring a biopsy in pediatric patients that both clinically and radiographically are presumably an osteoid osteoma.

  • Osteoma is the most common benign tumor of the paranasal sinuses.
  • Turbinate osteomas are very rare and only four middle turbinate, one superior turbinate and one inferior turbinate osteoma cases have been reported.
  • We present a rare case of osteoma of the left middle turbinate in a patient presented with unilateral nasal obstruction and epiphora that was removed endoscopically, and conduct a literature review on turbinate osteomas arising from different turbinates, their symptoms and management.

  • Osteoid osteoma is a benign tumour in young adults.
  • When the typical clinical and radiological features are present, diagnosis is not difficult.
  • Problems in the differential diagnosis may arise in connection with an unusual location.
  • We report on the clinical features, radiographic and histopathological findings, treatment, and results of four patients who were managed for an intra- or juxta-articular osteoid osteoma at our medical centre between 2000 and 2002 and in whom the initial diagnosis was erroneous and delayed from 1 to 10 years.
  • In order to remove these lesions, we performed a CT-guided en block retrograde resection under arthroscopic control for juxta-articular osteoid osteomas (knee) and an arthroscopy-assisted en block antegrade resection in two cases of intra-articular osteoid osteoma (elbow and shoulder).
  • None of the osteoid osteomas recurred in the follow-up period, and each patient got relief from pain.

  • [Title] Large intracranial intradural mucocele as a complication of frontal sinus osteoma.
  • Paranasal sinus osteomas are mostly asymptomatic; however, secondary mucocele can develop if they impede the natural sinus drainage.
  • Such a mucocele can destroy the bone and extend into the adjacent structures.
  • We report on an unusual case of frontal sinus osteoma in a 27-year-old patient, complicated by large secondary mucocele that eroded the bone and extended into the frontal lobe of the brain.
  • The mucocele was completely resected through bifrontobasal osteoplastic craniotomy, whereas osteoma was evacuated in its entirety by both drilling and mobilizing.
  • Open surgical approach remains the main treatment for complicated paranasal sinus osteoma, and radical removal of intracranial mucocele is mandatory to prevent the development of life-threatening infections.

  • AIM: To report the description of a rare benign osseous lesion affecting the ribs entitled post-traumatic fibro-osseous lesion (PTFOL).
  • A constant increased uptake of radionucleotide was noted on bone scan.
  • Lesions were characterised by a network of anastomosing bone trabeculae without osteoblast lining within a fibrous stroma.
  • A zonal maturation from woven to peripheral lamellar bone was characterised.
  • PTFOL is regularly misdiagnosed with other more common lesions of the ribs, such as fibro-osseous dysplasia, osteoma osteoid and benign fibrous histiocytoma.

  • [Title] [Diagnosis and treatment of osteoid osteoma].
  • OBJECTIVE: To study the clinical features and surgical treatment of osteoid osteoma and improve the diagnostic therapeutic level.
  • METHODS: Clinical data of 35 patients (25 males and 10 females) with osteoid osteoma diagnosed and treated between January 1997 to October 2009 were retrospectively reviewed.
  • The average interval time between onset of symptoms and diagnosis was 12 months (ranged, 1 to 24 months).
  • The most common presenting complaints for patients with osteoid osteoma was pain which usually responded to NSAIDs and was generally more severe at night.
  • CONCLUSION: According to the typical clinical presentation, radiographic findings, the diagnosis of osteoid osteoma is not difficult.
  • Once the diagnosis is confirmed, the operation should be carried out as early as possible to relieve the symptoms, improve the quality of life and prevent long-term complications.

  • [Title] Percutaneous radiofrequency thermocoagulation of osteoid osteomas: factors affecting therapeutic outcome.
  • OBJECTIVE: To examine factors which affect local recurrence of osteoid osteomas treated with percutaneous CT-guided radiofrequency thermocoagulation.
  • DESIGN AND PATIENTS: A prospective study was carried out on 45 patients with osteoid osteoma who underwent percutaneous radiofrequency thermocoagulation with a minimum follow-up of 12 months.
  • CONCLUSIONS: Osteoid osteomas in a non-diaphyseal location are statistically more likely to recur than those in a diaphyseal location when treated with CT-guided percutaneous radiofrequency thermocoagulation.

  • [Title] [Ocular and orbital symptoms in benign sinonasal neopasmas].
  • Osteoma (0.01-0.4%) and inverted papilloma (0.5-4%) are the most common benign tumours of the nose and paranasal sinuses, whereas heamangioma occurs very rare.
  • AIM OF THE STUDY: Analysis of symtoms, diagnostic procedures and treatment options in 10 patients with diagnosed orbital and ocular complications of benign tumours of the nose and paranasal sinuses.
  • MATERIAL AND METHODS: There are 4 cases of inverted papilloma, 3 cases of osteoma and 3 cases haemangioma.
  • In 8 cases there were signs of bone destruction in CT, what was intraoperatively confirmed in 4 cases.
  • CONCLUSION: Orbital and ocular symptoms in benign tumors of nose and paranasal sinuses do not always correlate with radiologic and intraoperative findings.

  • [Title] [Case of a horn-shaped osteoma].
  • A Case of osteoma with horn shape is rare.
  • A 79-years-old man, in whom the osteoma with horn shape was not demonstrated three years ago.
  • Slowly, the osteoma becomes accompanied with pain, and the slow growth of a horn.
  • Computed tomography showed a bone tumor arising from the right parietal bone, but, the patient didn't request treatment.
  • The osteoma changed its form, and an ulcer developed.
  • The tumor was detaches from the skull and the surrounding cranial bone was removed.
  • The histological diagnosis was that it was a case of a compact type osteoma.
  • We believe that the osteoma in our case must have grown slowly with time and ossification of the surrounding tissue.
