  • [Title] Granular cells tumour in the oral cavity: report of eleven cases treated with laser surgery.
  • AIM: The aim of this paper is to examine the clinical and histological feature of oral tumours of neural derivation and discuss their treatment.
  • METHODS: Between 1999 and 2004, 11 patients (6 females, 5 males; age range 28-66 years) were treated for tumours classified as being of neural origin; they were all myoblastomas or granular cells tumours (ex Abrikossoff tumour).
  • RESULTS: During follow-up, which ranged from 5 months to 5 years, there was no indication of tumour recurrence.
  • CONCLUSIONS: Granular cell tumour is a benign neoplasm with a tendency to relapse if not completely removed.

  • Verrucous carcinoma is a low-grade variant of squamous cell carcinoma reported to occur in all anatomic sites in the head and neck region, most commonly the oral cavity.
  • The tumor grows locally invasive but is histologically benign and metastasizes rarely.
  • To date, 22 cases of verrucous carcinoma involving the nasal cavity and/or the paranasal sinuses have been reported.
  • We present a case of verrucous carcinoma involving the paranasal sinuses, nasal cavity, cranium, and orbit.
  • This case highlights the difficulty of pathologic diagnosis and management options for a rare neoplastic lesion.

  • [Title] Colorectal granular cell tumor: a clinicopathologic study of 26 cases.
  • Granular cell tumor (GCT) is commonly located in the subcutaneous tissue and oral cavity, and uncommon in the gastrointestinal tract, in which the majority arises in the esophagus with over-representation in African Americans (AA).
  • Most neoplasms were encountered on routine colonoscopy (14/24, 64%), however 3 patients presented with hematochezia, 3 with changing bowel habits, 2 with Crohn disease, 1 with diverticular disease, and 1 with appendicitis.
  • Of the 20 cases available for histologic review, the tumors were noted to either be infiltrative (12/20, 60%) or marginated (8/20, 40%) involving either the mucosa (7/20, 35%), submucosa (10/20, 50%), or both (3/20, 15%).
  • The microscopic features were similar to those of GCTs found elsewhere, but many of the neoplasms differed by displaying nuclear pleomorphism (8/20, 40%), lymphoid cuffs (9/20, 45%), and focal calcification (7/20, 35%).
  • On immunochemistry, 18 of the neoplasms were stained for S-100 and all cases showed positive staining.
  • Although GCTs were benign tumors in this series, if incompletely excised regrowth of the lesion may occur and therefore, follow-up may be warranted.

  • BACKGROUND: Fine-needle aspiration (FNA) or touch preparation with rapid assessment is a valuable diagnostic tool with a reported accuracy of diagnosis similar to intraoperative frozen section consultation.
  • Frozen section diagnosis is impractical in this setting because of very limited resources.
  • Smears were prepared and stained with hematoxylin and eosin, and an immediate diagnosis was rendered.
  • Sites sampled included the following: thyroid (n = 40); facial/jaw area (n = 19); parotid/submandibular gland (n = 13); scalp/skull region (n = 7); cervical lymph nodes (n = 5); neck (n = 5); other (n = 4); supraclavicular lymph nodes (n = 3); and oral cavity (n = 2).
  • On cytologic analysis, there were 79 benign diagnoses, 17 malignant diagnoses, and 2 nondiagnostic samples.
  • A benign versus malignant cytologic diagnosis was accurate in 97 of 98 cases.

  • Salivary gland tumours are relatively rare and comprise a diverse range of neoplasms.
  • All entries for salivary gland tumours from 1974 to 2005 inclusive were retrieved and analysed for each diagnosis including number of specimens, male:female ratio and age range.
  • These data were then analysed for the distribution of benign and malignant salivary tumours in major and minor salivary glands.
  • There were 481 (64.9%) benign and 260 (35.1%) malignant neoplasms, with the most common tumours being pleomorphic adenoma and mucoepidermoid carcinoma, respectively.
  • Accurate diagnosis is essential as salivary lesions have diverse clinical and prognostic outcomes.
  • This study has confirmed that some tumours have a predilection for certain sites and that the risk of malignant disease is also greater at specific sites within the oral cavity.

  • The perineurioma is an infrequently encountered benign peripheral nerve sheath tumor composed of a clonal proliferation of perineurial cells.
  • Rare cases of perineurioma have been reported in the oral cavity.
  • Histopathologically, the tumor is composed of a well circumscribed nodular proliferation of spindle cells arranged in a storiform growth pattern, in some areas subtly arranged around vascular channels.
  • The tumor cells reveal positive immunostaining for epithelial membrane antigen (EMA), collagen type IV and vimentin, and negative immunostaining for S-100 protein, consistent with a perineurial origin.
  • To the best of our knowledge, this case represents the first report of an extraneural sclerosing perineurioma involving the oral cavity.

  • PURPOSE: The aims and objectives of this study were to evaluate the efficacy of collagen membrane as a biodegradable wound dressing material for surgical defects of the oral mucosa.
  • This study was confined to secondary defects of the oral mucosa, which occur after excision of premalignant lesions and other conditions, such as benign lesions, reactive proliferations, and incisional biopsy wounds.
  • CONCLUSION: In this study of short duration and small sample, the nature of collagen membrane was observed and was found to be a very suitable alternative to other graft materials mentioned for the repair of defects in the mucous membrane of the oral cavity.
  • Therefore, when used judiciously in a controlled clinical situation, collagen membrane is biologically acceptable to the oral mucosa and is, from the clinical point of view, an excellent wound graft material.

  • The histology and clinical features of this aggressive odontogenic neoplasm are described.
  • Within the English-language literature, only 2 cases are reported of an odontogenic tumor with muscle differentiation: a benign odontogenic tumor (ameloblastoma) with differentiation into a rhabdomyosarcoma and an odontogenic sarcoma with smooth-muscle differentiation.
  • The general practitioner should be aware that odontogenic lesions may be malignant, even though this is extremely rare, and all tissue removed from the oral cavity should be submitted for biopsy.

  • [Title] Benign tumors of the oral mucosa: a study of 300 patients.
  • OBJECTIVES: To analyze the frequency and type of the most common benign tumors of the oral mucosa found at the Hospital Stomatology Service, and to study the clinical characteristics and possible etiological factors.
  • MATERIAL AND METHODS: This is a retrospective study of 300 patients with histologically diagnosed benign tumors of the oral mucosa.
  • Data was compiled for each case, documenting information relating to age, gender, medication, habits (smoking, oral hygiene), anamnesis (reason for consultation, symptomatology, evolution), and the characteristics of the lesion (site, color, size, surface, consistency, and base).
  • RESULTS: Of all the tumors studied, 53% were histologically diagnosed as fibroma.
  • In the study of prevalence of benign tumors of the oral mucosa, no differences were found for age; however there were differences according to gender, finding a greater prevalence of fibromas, pyogenic granulomas, and giant cell granulomas in women, at a ratio of 2:1.
  • The group of tumors studied showed a significantly asymptomatic behaviour, and self-limiting and slow growth.
  • CONCLUSIONS: Following the study of 300 patients histologically diagnosed with benign tumor of the oral mucosa, we can state that with regard to prevalence, we found significant differences with respect to gender, being more frequent in women.
  • The fibroma is the most frequent benign tumor of the oral cavity.

  • We will report two cases of newborn girls, who could not be breast fed because of a benign tumour in their oral cavity.
  • The size of neoplasm was in one case 3 x 2 cm, in the other one 2 x 1.5 cm.

  • Histological findings showed large tumour cells with abundant granular eosinophilic cytoplasm and a prominent vascularity.

  • At that time, despite the diagnosis of a benign disease, an adequate staging was performed, ruling out any extracutaneous involvement.
  • Based on the histologic, immunophenotypic and molecular biology findings, a diagnosis of CD8+ epidermotropic cytotoxic primary cutaneous T-cell lymphoma was made.
  • The patient died from complications of right femoral artery thrombosis before starting specific polychemotherapy 21 months after onset of the disease.
  • Among primary cutaneous T-cell lymphomas, the CD8+ epidermotropic cytotoxic subset comprises rare, highly aggressive forms characterized by metastatic spread to unusual sites such as the oral cavity, lungs, testis and the central nervous system but usually not to the lymph nodes.

  • INTRODUCTION: This article presents a patient with sialolipoma of the submandibular gland, a rare neoplasm, and reviews related literatures.
  • Unlike other lipomatous lesion in the oral cavity, definite male predilection was not observed in the sialolipoma.
  • The most common site for the tumor was the parotid gland (17 cases, 60.7%), followed by the palate (four cases, 14.2%).
  • Histopathologically, a tumor was a benign lesion with proliferating lipocytes including scattered foci with a normal salivary gland tissue.
  • CONCLUSION: Sialolipoma is a rare benign neoplasm in the head and neck and it shows equal sex predilection.

  • Here we describe our experience with infrahyoid flaps (IHFs) employed to cover surgical defects in the oral cavity and oropharynx in patients with benign and malignant tumors.
  • The anterior floor of the mouth was reconstructed in nine patients (64.2%), the base of tongue in three (21.4%), the lateral floor in one (7.1%), and the retromolar area (7.1%) in one.
  • The most common complication was epidermolysis, which delayed the beginning of oral ingestion.
  • Due to its thinness and malleability, its use for oral cavity and oropharynx defects provides favorable cosmetic and functional outcomes.

  • BACKGROUND: Granular cell tumor (GCT) is a benign lesion that occurs at different body sites with preponderance to the oral cavity.
  • METHODS: The patients' demographic data and the cytological and architectural features of the lesions were analyzed in a large series of oral GCTs (n = 68).
  • RESULTS: The tongue was the most common location of oral GCTs (81%).
  • Furthermore, GCTs could be regarded as lesions that reflect a local metabolic or reactive change rather than a true neoplasm.

  • Leiomyoma is a benign smooth muscle tumor that is very rarely located in the oral cavity.
  • It is accepted that the smooth muscle giving rise to this tumor corresponds to the tunica media of the arteries.
  • The tumor can develop at any age, with no clear gender predilection, and typically manifests as a slow-growing, asymptomatic lesion.
  • The diagnosis is based on the histopathological study, with the use of specific staining techniques, including immunohistochemical markers.
  • We present the case of a 57-year-old male with a tumor located for the previous four months in the region of the hard palate, adjacent to the left upper premolars.
  • After surgical resection with safety margins, hematoxylin-eosin staining and immunohistochemical study confirmed the diagnosis of leiomyoma.

  • Liposarcoma (LS), one of the most common malignant tumors, is considered as a rarity in the oral cavity, the tongue being the most frequent site for its occurrence.
  • LS of the tongue is a tumor of adult and old age, with a mean age of approximately 62 years and a peak incidence in the seventh and eighth decades.
  • Clinically, it is a long-standing tumor that commonly presents as a solitary nodular mass, but can also present as a multi-nodular lesion.
  • The most common histopathologic type is that of atypical lipomatous tumor (ALT)/well-differentiated LS (75%).
  • LS can be easily misdiagnosed with both benign and malignant lipomatous and non-lipomatous tumors.
  • When a lipomatous lesion is encountered in the oral cavity, and especially in the tongue, LS should certainly be among the lesions that top the list of differential diagnosis.
  • The follow-up must be on a long-term basis since this tumor can recur years after initial surgical treatment and can also undergo dedifferentiation.

  • [Title] Metastatic tumours to the oral cavity: a survival study with a special focus on gingival metastases.
  • AIMS: To describe survival from oral metastases, particularly gingival metastases, and to identify clinical prognostic variables.
  • MATERIALS AND METHODS: A series of 39 patients were studied, analysing age, gender, primary tumour site, oral metastases site and histological type.
  • Gingival metastases represented 63.6% of all oral soft tissue metastases (7/11).
  • The average time between primary tumour diagnosis and appearance of the gingival metastases was 9.7+/-13.4 months.
  • The median survival time since gingival metastases appearance was 5.2 months [95% confidence interval (CI)=0-13.6]; no statistically significant difference with other oral locations was found by the Kaplan-Meier curves (log rank: 0.29; p>0.05).
  • Oral metastases involving the gingiva were more frequently found in the maxilla (85.7%versus 14.3%), whereas intra-osseous metastatic tumours were more frequent in the mandible (77.8%versus 22.2%; p<0.05; odds ratio=21; 95% CI=2.0-210.1).
  • PRACTICAL IMPLICATIONS: The data in this paper show that 25% (and in other studies up to 37%) of oral metastases came from unknown primary tumours; thus a biopsy with histopathologic analysis is mandatory for every patient with a gingival mass.
  • Dental practitioners should suspect that gingival masses mimicking benign or inflammatory lesions may represent a sign of underlying malignant tumours.

  • PATIENTS AND METHODS: We retrospectively evaluated a total of 200 neck dissections performed in 128 patients (107 males, 21 females; mean age 56+/-11 years; range 26 to 81 years) with squamous cell carcinoma of the larynx, orohypopharynx, or oral cavity, and having complete clinical and pathologic data.
  • Along with 442 metastatic lymph nodes (139 with ECS), the greatest axial diameter of the biggest benign lymph node obtained from each patient was measured.
  • CONCLUSION: In larynx, orohypopharynx, and oral cavity squamous cell carcinomas, the clinical assessment of the size of neck lymph nodes is usually not a predictor for lymph node metastasis or ECS.

  • Its location on the oral mucosa is rare, representing 1% to 5% of benign oral tumors although it is the most mesenchymal tumor of the trunk and proximal por-tions of extremities.
  • Lipoma of the oral cavity may occur in any region.
  • The buccal mucosa, tongue, and floor of the mouth are among the common locations.

  • PURPOSE: The purpose of this article is to evaluate whether hemimandibulectomy with disarticulation predisposes the patient to the development of contralateral coronoid hyperplasia, thereby justifying a coronoidectomy procedure for these patients to improve jaw function and mouth opening and decrease the length of postoperative physical therapy.
  • PATIENTS AND METHODS: Five patients who had hemimandibular resection with disarticulation for odontogenic cysts and benign tumors followed by hemimandibular graft reconstruction with either a cadaveric hemimandible or an iliac crest bone graft, or both, were retrospectively reviewed.
  • RESULTS: All patients performed postoperative range-of-motion exercises for treatment of limited mouth opening in a range between 12 to 24 mm lasting up to 3 or 4 months.
  • CONCLUSIONS: Trismus, the clinical sign of restricted jaw movement, can result from pathology in a variety of structures around the oral cavity.
  • We recommend the Levandoski analysis for patients who had hemimandibulectomy with disarticulation who complain of prolonged limited mouth opening.

  • BACKGROUND: Oncogenesis in the oral cavity is believed to result from genetic alterations that cause a stepwise transformation of the mucosa to invasive carcinoma.
  • In oral squamous cell carcinoma (OSCC) multiple cytogenetic abnormalities have been reported, but their practical significance remains uncertain.
  • METHODS: Fifty-one consecutive samples of OSCC, nine lymph node biopsies showing metastatic spread from OSCC, 16 biopsies diagnosed as oral leucoplakia (OLK), 13 samples corresponding to oral lichen planus (OLP) and 14 samples from normal oral mucosa were included in the study.
  • The genetic and protein status of the CCND1, MYC, EGFR, ERBB2 oncogenes and the TP53 tumour suppressor gene were assessed by fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC).
  • Overexpression of p53, Myc, Cyclin D1, c-erbB-2 and epidermal growth factor receptor (EGFR) was more prevalent in malignant samples than benign samples (P < 0·05).
  • Evaluation of the genetic status of TP53, MYC, CCND1, ERBB2 and EGFR may be an additional diagnostic tool in distinguishing benign from malignant oral lesions in histopathologically challenging cases.

  • BACKGROUND: Solitary fibrous tumor (SFT) is an uncommon but well-characterized soft tissue tumor that was first described as a pleural lesion and now is considered ubiquitous, having been detected at many extra-pleural sites (abdominal cavity, orbit, upper respiratory tract, and oral cavity).
  • Histologically, SFT may show wide morphological variability of both its cellular and stromal components, which may lead to incorrect diagnosis especially when dealing with small incisional biopsies.
  • MATERIALS: We report on the clinical, morphological and immunohistochemical features of eight SFT occurring in the oral cavity.
  • RESULTS: Microscopically all eight tumors showed widely variable morphological features in terms of cellular density and stromal architecture, thus simulating benign fibrous histiocytoma, schwannoma, hemangiopericytoma or low-grade sarcoma in distinct areas of the same lesion.

  • OBJECTIVES: Pleomorphic adenoma is the most common benign tumor of the major salivary glands, especially of the parotid gland.
  • It is much less common in the minor salivary glands of the oral cavity, and it rarely occurs in others sites in the head and neck.
  • Even if virus involvement in salivary glands tumor has been many times discuted, the etiology of this tumor keeps being unknown.
  • As compared to other nasal tumors, we tried to prove if Epstein-Barr virus (EBV) could be involved in the development of pleomorphic adenoma in this particular nasal localization.
  • All the patients had undergone endoscopic surgery to remove the complete tumor.
  • We used hybridation technique in the search for EBV-DNA in the three tumors.
  • RESULTS AND CONCLUSION: We had positive EBV-DNA detection in the tumor in one case, which seemed to prove relationship between pleomorphic adenoma and this virus.
  • The literature concerning the subject is reviewed in order to explain EBV involvement in the development of such tumors in this particular localization.

  • The purpose of this study was to evaluate artificial dermis with the simultaneous addition of autologous epithelial cells for oral lesion defect reconstruction.
  • Surgical wounds reconstructed with artificial dermis plus scraped epithelial cells were evaluated in 5 patients with oral benign lesions or squamous cell carcinoma.
  • Clinical observation showed that the oral mucosal texture was smoother in artificial dermis with added epithelial cells at 4 weeks postoperation compared with artificial dermis alone.
  • We showed that healthy granulation tissue and neoepithelial formation in artificial dermis with epithelial cells was beneficial for the repair of oral defects.
  • Scraping oral epithelial cells and applying them to artificial dermis assisted in the early preparation of composite grafts and minimized requirement for donor sites.
  • This technique may improve the treatment of patients with oral benign tumors and early-stage squamous cell carcinoma.

  • To supplement information on vascular leiomyoma of the oral cavity (VLOC), we present cases of VLOC describing their clinical, histological, and immunohistochemical characteristics.
  • The age average of the cases was 40.6, however 3 out of our 5 cases were < or = 40 years old at the moment of their diagnosis.

  • Myofibroma is an uncommon spindle cell neoplasm rarely found in oral cavity.
  • In the oral cavity, myofibroma occurs within the submucosal or intramuscular tissue and has a predilection by the tongue, buccal mucosa and lips.
  • Misdiagnosis included benign and malignant spindle cell lesions of nerve tissue or smooth muscle origin, such as neurofibroma, leiomyoma and sarcomas.
  • Thus, immunohistochemical staining is a useful tool to identify the nature of neoplastic cells and to reach an accurate diagnosis.

  • A fetal MRI was prescribed, after discussion with the antenatal diagnostic center, to determine the localization, size, and nature of the tumor.
  • It revealed a 3 cm long tumor with cystic like areas not communicating with the encephalus.
  • Histological analysis confirmed the diagnosis.
  • DISCUSSION: A prenatal diagnosis of congenital gingival granulomatous tumor is rare.
  • Nevertheless, prenatal diagnosis allows for a better postnatal management.

  • [Title] Intraoral epithelioid hemangioendothelioma: an intermediate vascular tumor- a case report.
  • Vascular neoplasms, other than benign are characterized as intermediate or malignant.
  • They are often enshrouded in controversy, because the same neoplasm could show variability in biologic behavior that may not be correlated with microscopic features.
  • The intermediate grade vascular neoplasm is named as epithelioid hemangioendothelioma (EHE).
  • Epithelioid hemangioendothelioma of the oral cavity has been infrequently reported.
  • We report such a rare case in a 20 year old male, presented with a growth in lower anterior lingual gingiva since five months before the diagnosis with a history of similar swelling, twice in the same area.
  • The differential diagnosis and brief review of literature is also discussed in the current article.

  • There are 47 cases presented in article (28 women and 19 men) from who pieces of nasal cavity, oral cavity or throat were taken or illness changes have been curing.
  • Group No 1 containing 11 men and 10 women with removing changes in nasal cavity.
  • Group No 3 in which changes in oral cavity were removed where were 4 men and 8 women.
  • RESULTS: Among majority patients removed pices were proliferation of neoplasm - benign type (20 patients).
  • In 2 cases were papilloma and 4 causes were malignant neoplasm.
  • Control visits did not show the increase of illness (except malignant neoplasm).
  • CONCLUSIONS: Laser ND-YAG may be used to remove egzofitical changes and to take some pices in case of neoplasm.
  • Using laser technology high precision is possible and deep penetration of laser make the operations safer in case of neoplasm.

  • Hemangioma, vascular malformation and varix are benign vascular lesions, common in the head and neck regions.
  • Studies about the prevalence of these lesions in the oral cavity are scarce.
  • The aim of this study was to estimate the prevalence of and to obtain clinical data on oral hemangioma, vascular malformation and varix in a Brazilian population.
  • Clinical data on those lesions were retrieved from the clinical forms from the files of the Oral Diagnosis Service, School of Dentistry, Federal University of Minas Gerais, Brazil, from 1992 to 2002.
  • A total of 2,419 clinical forms in the 10-year period were evaluated, of which 154 (6.4%) cases were categorized as oral hemangioma, oral vascular malformation or oral varix.
  • Oral varix was the most frequent lesion (65.6%).
  • Females had more oral hemangioma and oral varix than males.
  • Oral vascular malformation and oral varix were more prevalent in the 7th and 6th decades, respectively.
  • Oral hemangioma and oral varix were more prevalent in the ventral surface of the tongue and oral vascular malformation, in the lips.
  • Oral hemangioma was treated with sclerotherapy (54.5%), and vascular malformation was managed with sclerotherapy and surgery (19.4% each).
  • The data of this study suggests that benign vascular lesions are unusual alterations on the oral mucosa and jaws.

  • Squamous dysplasia of the oral cavity indicates increased risk of progression to squamous cell carcinoma (SCC).
  • An important advance would be the development of a minimally invasive assay for identification of oral SCC and dysplasia.
  • We have investigated the suitability in this context of immunostaining oral smears for minichromosome maintainance proteins (MCMs), sensitive and specific biomarkers of cell cycle entry.
  • Immunohistochemical examination of 66 oral tissue samples showed a greater frequency of Mcm-2 expression in surface layers of moderate/severe dysplasia and SCC compared to benign keratosis/mild dysplasia.
  • Immunocytochemistry for Mcm-2/Mcm-5 was performed on 101 oral smears.
  • Conventional smears included 23 from normal mucosa, benign proliferative disease and mild dysplasia, all of which were MCM negative.
  • Six smears from benign lesions were adequate and MCM negative.
  • We conclude that MCMs are promising markers for early detection of oral SCC and dysplasia, particularly in a liquid-based cytology platform.

  • Malignancies of the oral cavity are conventionally diagnosed by white light endoscopy, biopsy, and histopathology.
  • However, it is often difficult to distinguish between benign and premalignant or early lesions.
  • We investigate the use of an LCE with a rigid probe for diagnostic imaging of the oral cavity.
  • Fluorescein and 5-aminolevulinic acid (ALA) were used to carry out fluorescence imaging in vivo and on resected tissue samples of the oral cavity.
  • Good structural images of the oral cavity were obtained using ALA and fluorescein, and morphological differences between normal and lesion tissue can be distinguished.
  • Our initial results show that laser confocal fluorescence endomicroscopy has potential as a noninvasive optical imaging method for the diagnosis of oral cavity malignancies.

  • Intramuscular myxoma is an uncommon benign soft tissue neoplasm and a distinct histopathological entity within the heterogeneous group of myxomas.
  • We report on a case of a myxoma within the mimetic muscles of the nasal and mouth region in a 52-year-old man.
  • The tumor was surgically removed and the patient is free of recurrence or complications 8 months after treatment.
  • To the very best of our knowledge, there is no report of this neoplasm located within the nasal and oral mimetic muscles.

  • [Title] Serum hyaluronan levels in oral cancer patients.
  • BACKGROUND: Hyaluronan (HA) is most likely associated with tumor invasion and metastasis.
  • Studies have shown that HA levels are often increased in serum of patients with various malignant tumors.
  • The purpose of this study was to determine the levels of serum hyaluronan in patients with oral cancer and evaluate the value of serum HA in adjuvant diagnosis, staging and monitoring treatment response in these patients.
  • METHODS: Eighty-four hospitalized patients with oral cancer, 65 patients with benign tumors in the oral and maxillofacial region and 67 healthy individuals were included in this investigation.
  • One week after therapy, venous blood was collected once again in 43 patients with oral cancer.
  • RESULTS: The serum HA concentration was significantly higher in oral cancer patients than in patients with benign tumors and in healthy controls (P<0.05).
  • After a complete treatment the HA levels in patients with oral cancer became lower than before treatment, but the difference was not significant (P>0.05).
  • CONCLUSIONS: The results of this study suggest that the determination of HA levels may provide additional information in diagnosis of oral cancer, but its usefulness as an adjunct in clinical staging and in monitoring treatment response was limited.

  • Dibromochloropropane (1,2-dibromo-3-chloropropane, DBCP), a pesticide used widely for over 20 years to control nematodes on crops, turf and in nurseries, was banned by the United States Environmental Protection Agency (US EPA) in 1977 because of evidence of infertility in men and induction of a variety of tumors in laboratory animals.
  • Results from long-term cancer bioassays in rodents show a statistically significant increase in the incidence of malignant and benign mammary gland tumors in female rats treated orally with DBCP compared to controls and some evidence of increased incidence of mammary fibroadenomas in DBCP low-dose treated female rats exposed by inhalation.
  • Significantly increased incidence of tumors of the forestomach occurred in both sexes of rats and mice treated orally.
  • Rats exposed to DBCP by inhalation showed significant increases in tumors of the tunica vaginalis in males; tumors of the pharynx and adrenal gland in females; and tumors of the tongue, nasal turbinate and nasal cavity in both sexes compared to controls.
  • Male and female mice exposed to DBCP by inhalation experienced increased tumor incidence in the lungs and nasal cavity compared to controls.
  • Significant increases in tumors of the lung and forestomach have also been reported in female mice treated by a dermal route.
  • Although high mortality rates in both rat and mouse bioassays limited the ability to detect tumors late in life, the induction of a variety of tumors by multiple routes of exposure in two rodent species provides clear evidence of a DBCP tumorigenic response.
  • Given the persistent nature of DBCP contamination in areas of past use, efforts should be made to continue remediation efforts and follow previously exposed populations for development of certain human cancers, including breast, ovarian, stomach, respiratory, oral and nasal cancers, among others.

  • Congenital epulis of the newborn is a rare tumour which is usually benign.
  • Epulis is also known as a congenital gingival granular cell tumour because of its histological features.
  • The histogenesis of the tumour is unknown.
  • Recurrence of the tumour after surgery has not been reported yet.
  • In one case it was an antenatal diagnosis by ultrasonography, in the other it was associated a genital anomalies (hypospadias).

  • Basal cell adenoma (BCA) is a rare benign epithelial tumor of the salivary gland.
  • BCA is seen most frequently in the parotid gland and less commonly in the submandibular gland and minor glands of the upper lips, oral cavity, and hard palate.
  • Salivary gland tumors are observed as single tumors in one salivary gland.
  • Double or multiple tumors of the salivary gland tumors are unusual and metachronous or bilateral salivary gland tumors are more observed than synchronous or unilateral tumors.
  • The most commonly seen multiple tumor unilaterally or bilaterally is the Warthin's tumor.

  • [Title] Metastatic tumours to the oral cavity - pathogenesis and analysis of 673 cases.
  • The oral region is an uncommon site for metastatic tumour cell colonization and is usually evidence of a wide spread disease.
  • In 25% of cases, oral metastases were found to be the first sign of the metastatic spread and in 23% it was the first indication of an undiscovered malignancy at a distant site.
  • The jawbones, particularly the mandible, were more frequently affected than the oral soft tissues (2:1).
  • In the oral soft tissues, the attached gingiva was the most commonly affected site (54%).
  • The major primary sites presenting oral metastases were the lung, kidney, liver, and prostate for men, breast, female genital organs (FGO), kidney, and colo-rectum for women.
  • The primary site differs according to oral site colonization, in men the lung was the most common primary site affecting both the jawbones and oral mucosa (22% and 31.3%, respectively) followed by the prostate gland in the jawbones (11%) and kidney in the oral soft tissues (14%).
  • In women, the breast was the most common primary tumour affecting the jawbones and soft tissues (41% and 24.3%, respectively), followed by the adrenal and female genital organs (FGO) in the jawbones (7.7%) and FGO in the soft tissues (14.8%).
  • The clinical presentation of the metastatic lesions differ between the various sites in the oral region.
  • Because of its rarity, the diagnosis of a metastatic lesion in the oral region is challenging, both to the clinician and to the pathologist, in recognizing that a lesion is metastatic and in determining the site of origin.
  • The clinical presentation of a metastatic lesion in the oral cavity can be deceiving leading to a misdiagnosis of a benign process, therefore, in any case where the clinical presentation is unusual especially in patients with a known malignant disease a biopsy is mandatory.

  • [Title] [Lipomas of the oral cavity--a retrospective study].
  • Lipomas are the most common soft tissue mesenchymal neoplasms, with 15 to 20% of the cases involving the head and neck region and 1 to 4% affecting the oral cavity.
  • They represent 0.1 to 5% of all benign tumours of the mouth and are usually found as long-standing soft nodular asymptomatic swellings covered by normal mucosa.
  • MATERIALS AND METHODS: A retrospective review was undertaken of medical records and histological studies of 19 patients with histopathologically proven intraoral lipomas, over a 15-year period (from 1992 to 2006) at the Department of Oral and Maxillofacial Surgery, Iaşi.
  • RESULTS: Most of the cases affected adults (18 cases) without gender predilection and the main involved sites were: the floor of the mouth (6 cases), the buccal mucosa (5 cases), the lips (4 cases), the tongue (3 cases) and the buccal sulcus (1 case).
  • CONCLUSIONS: Lipomas should be considered in the differential diagnosis of soft-tissue head and neck masses.

  • Salivary gland neoplasms account for <3% of all tumors.
  • Most of them are benign and parotid gland is the commonest site.
  • As a general rule, the smaller the involved salivary gland, the higher is the possibility of the tumor being malignant.
  • The role of imaging in assessment of salivary gland tumour is to define intra-glandular vs. extra-glandular location, detect malignant features, assess local extension and invasion, detect nodal metastases and systemic involvement.
  • If deep tissue extension is suspected or malignancy confirmed on cytology, an MRI or CT is mandatory to evaluate tumour extent, local invasion and perineural spread.
  • Minor salivary gland lesions in the mucosa of oral cavity, pharynx and tracheo-bronchial tree, are also not accessible by conventional ultrasound.
  • Recent study suggests that MR spectroscopy may differentiate malignant and benign salivary gland tumours as well as distinguishing Warthin's tumor from pleomorphic adenoma.
  • Sialography is used to delineate the salivary ductal system and has limited role in assessment of tumour extent.

  • METHODS: A total of 108 sentinel lymph nodes from 30 patients with T1/T2 cN0 oral cancer were re-classified histologically to find possible ITC and to describe technical pitfalls.

  • [Title] Polypoid angiomyofibroblastoma-like tumor of the oral cavity: a hitherto unreported soft tissue tumor mimicking embryonal rhabdomyosarcoma.
  • We report on a previously unrecognized fibro-myofibroblastic tumor in the oral cavity of a 15-year-old girl.
  • Morphologically, the tumor mimicked a rhabdomyosarcoma, botryoid variant.
  • The tumor contained abundant small- to medium-sized, thin-walled blood vessels without hyalinization.
  • The presence of these collagen mats and the expression of desmin, in association with no immunoreactivity to myogenin and MyoD1, were in keeping with the fibro-myofibroblastic nature of the tumor, excluding the diagnosis of embryonal rhabdomyosarcoma.
  • Regarding fibro-myofibroblastic tumors, we believe that the present case falls within the wide spectrum of benign stromal tumors, originally described in the lower female genital tract, but potentially occurring also at extragenital sites.
  • As morphological and immunohistochemical features were reminiscent of, but not identical with, angiomyofibroblastoma, the term "polypoid angiomyofibroblastoma-like tumor" is proposed.
  • Awareness and recognition of this tumor is crucial to avoid a diagnosis of malignancy.

  • Clinically, the tumour often exhibits an exophytic growth pattern, a soft consistency, a broad base connected to the underlying tissue, and frequently has a reddish-yellowish colour.
  • Depending on the localization of the tumour, the diagnosis and the resulting therapy can turn out to be a challenge for the clinician.
  • A clear differentiation to malign neoplasias of the fat tissues and other soft tissue expansions is essential.

  • Schwannomas and neurofibromas are the most common benign tumors derived from peripheral nerves, and whereas the head and neck region is the most common location for the occurrence of benign neural sheath neoplasms, origin within the oral cavity is uncommon, and occurrence centrally in the jaws is most unusual.
  • In current report, we present the first reported case of intraosseous plexiform schwannoma of the mandible, an extremely rare benign neurogenic tumor, diagnosed by optical and immunohistochemical procedures, showing the importance of differential diagnosis of these unusual intraosseous mandibular tumors.

  • [Title] Probing local tissue changes in the oral cavity for early detection of cancer using oblique polarized reflectance spectroscopy: a pilot clinical trial.
  • We report the results of an oral cavity pilot clinical trial to detect early precancer and cancer using a fiber optic probe with obliquely oriented collection fibers that preferentially probe local tissue morphology and heterogeneity using oblique polarized reflectance spectroscopy (OPRS).
  • Without stratifying the data according to anatomical location or level of keratinization, OPRS is found to be sensitive to four diagnostic categories: normal, benign, mild dysplasia, high-grade dysplasia, and carcinoma.
  • Separation of visually indistinguishable benign lesions from high-grade dysplasia and carcinoma is achieved with good sensitivity (100%) and specificity (85%), while separation of benign from mild dysplasia gives a sensitivity of 92% and a specificity of 69%.
  • These promising results suggest that OPRS has the potential to aid screening and diagnosis of oral precancer and cancer.

  • BACKGROUND: Metastasis of a malignant tumor to the oral cavity is rare, but it can be the first manifestation of a primary tumor.
  • Because the differentiation between carcinoma and sarcoma of spindle cell tumors was difficult, additional immunohistochemical evaluation was performed.
  • The intraoral healing after tumor removal was uneventful.
  • Finally, a biopsy of the lungs confirmed a poorly differentiated adenocarcinoma with multiple metastases, including the oral cavity.
  • CONCLUSIONS: An exophytic lesion on the gingiva can be the first sign of metastatic adenocarcinoma to the oral mucosa.
  • This case emphasizes that even apparently benign-looking gingival lesions in anamnestically healthy patients need to be examined histopathologically.

  • The purpose of this study is to evaluate our experience with the use of both free flaps in the reconstruction of oral cavity defects after tumoral ablation.
  • PATIENTS AND METHODS: Over a 9-year period, 103 consecutive patients were treated in our department for the reconstruction of oral defects after tumor ablation by means of microvascularized free flaps.
  • Patients were treated for benign (n = 15) and malignant (n = 82) entities.
  • CONCLUSIONS: Our results reveal that the RFFF is a reliable method for reconstructing a wide range of oral cavity defects with an acceptably low morbidity rate.
  • It provides adequate bulk and pliability, enabling the reconstruction of a wide variety of locations within the oral cavity.

  • Helicobacter pylori has been investigated in several other organ systems and localizations such as the oral cavity, but has not been investigated extensively in squamous cell carcinoma of the larynx, a region that could be directly exposed to the bacterium by the oral route or gastro-esophageal reflux.
  • The presence of H. pylori was also investigated histopathologically in 50 benign laryngeal biopsy specimens.
  • In our study, we demonstrated that H. pylori was not present in laryngeal squamous cell carcinoma tissue or in the benign lesions.

  • PURPOSE: Autofluorescence imaging is increasingly used to noninvasively identify neoplastic oral cavity lesions.
  • Improving the diagnostic accuracy of these techniques requires a better understanding of the biological basis for optical changes associated with neoplastic transformation in oral tissue.
  • EXPERIMENTAL DESIGN: A total of 49 oral biopsies were considered in this study.
  • The autofluorescence patterns of viable normal, benign, and neoplastic oral tissue were imaged using high-resolution confocal fluorescence microscopy.
  • RESULTS: The autofluorescence properties of oral tissue vary significantly based on anatomic site and pathologic diagnosis.
  • In normal oral tissue, most of the epithelial autofluorescence originates from the cytoplasm of cells in the basal and intermediate regions, whereas structural fibers are responsible for most of the stromal fluorescence.
  • A strongly fluorescent superficial layer was observed in tissues from the palate and the gingiva, which contrasts with the weakly fluorescent superficial layer found in other oral sites.
  • Upon UV excitation, benign inflammation shows decreased epithelial fluorescence, whereas dysplasia displays increased epithelial fluorescence compared with normal oral tissue.
  • Stromal fluorescence in both benign inflammation and dysplasia drops significantly at UV and 488 nm excitation.
  • CONCLUSION: Imaging oral lesions with optical devices/probes that sample mostly stromal fluorescence may result in a similar loss of fluorescence intensity and may fail to distinguish benign from precancerous lesions.

  • The prevalence of human papillomavirus (HPV) in the oral cavity and oropharynx has not yet been as well studied as its infection of the vaginal tract.
  • The objective of this study is to show the prevalence of HPV in the oral cavity and the oropharynx.
  • An ample bibliographic review was done showing a prevalence of HPV 6, 11 in a normal oral mucous membrane (latent infection).
  • In oral benign lesions associated with HPV, a prevalence of HPV 6 and 11 was observed in squamous cell papilloma (SCP) and condylomas acuminatum, while HPV 2 and 57 were more prevalent in verruca vulgaris lesions.
  • As for focal epithelial hyperplasia (FEH) and oral cancer, especially squamous cell carcinoma (SCC), the prevalence was of HPV 13 and 32, and HPV 16, respectively.
  • Showed also discrepancy result the prevalence of human papillomavirus (HPV) in normal oral mucous (latent infection) and in oral cancer, however evidenced confirmatory result in oral benign lesions associated with virus.

  • Hobnail hemangioma (HH) is a rare benign vascular neoplasm reported as a distinctive small benign, solitary vascular neoplasm of the superficial and mid-dermis occurring on the face, trunk, or extremities of young or middle-aged adults.
  • The oral manifestations are quite uncommon, with only three cases reported in the English language literature.
  • In the oral cavity, the differential diagnosis includes hemangioma, melanoma, and Kaposi's sarcoma, whose clinical and histological features may be confused with those of HH.
