  • Smooth muscle hamartoma (SMH) is a rare benign congenital or acquired lesion sometimes associated with Becker's nevus (Becker's melanosis).
  • We report an unusual lesion with combined features of SMH and melanocytic nevus.
  • The patient is a 49-year-old male with a history of a changing 'mole' on the left upper back.
  • The melanocytic component strongly expressed melanoma antigen recognized by T-cells-1 (MART-1) and HMB-45.
  • Unlike a recently reported case of SMH combined with a melanocytic nevus and basal cell carcinoma, the current lesion did not occur in association with a Becker's nevus.

  • We present two cases of melanoma arising in a dysplastic nevus that contained intradermal sebocyte-like melanocytes characterized by a scalloped dark-staining nucleus surrounded by the pale multivacuolated cytoplasm imitating sebaceous differentiation.
  • Microscopically, both cases had the following features in common: the melanoma in situ, which was of the superficially spreading type, was associated with a dysplastic compound nevus, in which the sebocyte-like cells were identified in the intradermal nevus part of the lesion.
  • The sebocyte-like cells comprised a minor component but were immediately recognizable and appeared as cells with multivacuolated neoplasm and scalloped nuclei arranged in nests.
  • The conventional part of the nevi demonstrated the same phenotype, with the exception of the melanocytic nests located at the dermoepidermal junction that were HMB-45 positive.

  • The vast majority of melanocytic lesions with hair, such as congenital melanocytic nevi, are benign.
  • However, there is a notion that the presence of one or more hairs in a melanocytic lesion is confirmatory for the benign nature of the lesion.
  • To dispel this notion, we present 3 examples of melanocytic lesions that showed terminal hairs on clinical and dermoscopic evaluation, but in which the final diagnosis was invasive melanoma.

  • Site-specific histopathology features have been reported for acral, auricular, flexural, and genital melanocytic nevi, however, to the best of our knowledge, site- and sex-specific histology of dysplastic nevi on the lower leg (between knee and ankle) of women (DN-LW) has not been reported.
  • In this retrospective histopathology study, we compared DN-LW (N = 42) with appropriate control groups of (1) DN of the lower leg of men (N = 20; DN-LM), (2) DN from the back of women (N = 20), (3) common nevi of the lower leg of women (N = 40), and (4) levels 1-2 superficial spreading melanoma of the lower leg of women (N = 20).
  • Compared with dysplastic nevi on the back, DN-LW were smaller in diameter and exhibited a significantly higher score for pagetoid spread (P < 0.05).
  • DN-LW compared with DN-LM showed sex-specific differences with (1) pagetoid spread (P < 0.05), (2) cytologic atypia (P < 0.05), (3) presence of large melanocytes (P < 0.05), and (4) band-like pigmentation in the dermis underlying the nevus (54% in DN-LW vs. 15% in DN-LM).
  • As with other body sites, the dermatopathologist should be aware that dysplastic nevi occurring on the lower leg in women have site- and sex-specific features.
  • Knowing this profile may lower the risk of misdiagnosing DN-LW and melanoma of the lower leg of women.

  • OBJECTIVE: Actively searching for leprosy, other skin diseases and BCG vaccination scars amongst school children from Agua de Dios, the municipality having the highest prevalence of leprosy in Colombia.

  • In this study, we first quantified expression of 9 PAX genes in human nevus pigmentosus tissues, melanoma tissues and melanoma cell lines by the real-time reverse transcription-PCR method.
  • As a result, we found that the expression levels of PAX4 and PAX9 were extremely low in melanoma tissues and cell lines compared to nevus pigmentosus tissues.
  • We then established melanoma cells overexpressing PAX4 and examined roles of PAX4 in cell growth.
  • PAX4-overexpression reduced in vitro cell growth of human melanoma C8161 and MeWo cells.

  • By setting a common temporal origin ("isochronal origin") at tau(T(g)) = 100 s for each mole fraction, it emerges that the substitution of ClA molecules by those of CNA (diminution of X) gives rise to a slow down in the dynamics, despite that the molecular volume of ClA molecules are smaller than those of CNA.
  • It is also shown that the heterogeneities produced by the concentration fluctuations due to the chemical disorder are the main contribution to the non-exponential character of the alpha-relaxation peaks.

  • There is a lack of histopathological factors to sub-stratify prognosis in pT3/4 melanoma primaries.
  • In the presented pilot study, the prognostic significance of different clinical and histopathological parameters was studied in thick primary melanoma taking paratumoral epidermal hyperplasia (PTEH) into consideration.
  • Of 1632 melanoma patients in the melanoma register of the Martin Luther University Halle-Wittenberg in the years 1980 to 1987, 16 cases with tumor thickness (TT) of the primary > or = 3 mm, documented metastasis-free follow-up of 10+ years after primary therapy and available histologic sections were compared with an adequate recurrence control group (n = 62) by PTEH and standard prognostic parameters.
  • Of the standard prognostic parameters, TT, sex, location, and lack of nevus association also correlated with metastasis.
  • Occurrence of PTEH might predict a more moderate course of disease in thick melanoma.
  • Thus, it might become an easily determinable and effective tool to sub-stratify prognosis in thick primary melanoma of the skin.

  • We herein describe a rare case of an intradermal nevus arising in the external auditory meatus.
  • Upon excisional biopsy, the tumor was diagnosed as an intradermal nevus.
  • Thus, this is a unique case of intradermal nevus obstructing the external ear canal with the patient presenting with progressive hearing loss.
  • The possibility of a benign nevomelanocytic nevus should not be underestimated when evaluating a lesion obstructing the ear canal, and all melanocytic nevi should be excised instead of biopsied to rule out melanoma.

  • BACKGROUND: Giant congenital nevocellular nevi (GCNN) are histologically characterized by the broad distribution of nevus cells in the epidermis and dermis.
  • RESULTS: The nevus cells in the superficial region showed the immunoreactivity of E-cadherin in a membranous pattern, but those in the deep part of dermis had little immunoreactivity.
  • Ultra-structural analysis of the superficial nevus cells revealed that E-cadherin immunodeposits in the fibrillar processes around the cell body in a spotted pattern.
  • This distribution pattern is quite different from that in the adherens junction of skin squamous epithelial cells.
  • Boyden chamber experiments were performed using primary cultures of intradermal nevus cells.
  • CONCLUSIONS: These results indicate that E-cadherin in the nevus cells may affect nevus cell motility rather than intercellular attachment.

  • [Title] Atypical nevi of the scalp in adolescents.
  • BACKGROUND: A few reports in the literature point out that in special areas of the body, nevi can have peculiar pseudomelanomatous features.
  • In our routine work, we have met few examples of atypical nevi with peculiar features on the scalp of teenagers.
  • To evaluate the frequency and the biological behaviour of these lesions, we have conducted a complete survey on melanocytic lesions on the scalp in a significant group of patients.
  • MATERIALS AND METHODS: Thirty-nine nevi of the scalp were from adolescents (12-18 years), 160 from adults, and 30 from children below the age of 12 years.
  • RESULTS: About 10% of the melanocytic nevi of the scalp of adolescents have atypical cytological and architectural aspects that are different from those seen in Clark's dysplastic nevus.
  • Such distinctive aspects are not found in nevi of the same site in adults or younger children.
  • The general pattern of these atypical nevi of the scalp of adolescents closely recalls that of the so-called atypical nevi on special sites, i.e. nevi on mammary line, genitalia and body's folds.
  • CONCLUSIONS: Despite their similarities with melanoma, the nevi with atypical features of the scalp of adolescents are probably an entirely benign entity, at least at the moment of their excision.
  • However, although benign, the relationship of this peculiar group of nevi with melanomas developed in adulthood remains entirely unknown, and the complete excision with conservative margins seems a recommendable procedure.

  • The epidermal nevus syndrome is a disease complex consisting of the association of an epidermal nevus with various developmental abnormalities of the skin, eyes, nervous, skeletal, cardiovascular, and urogenital systems.
  • The epidermal nevi are classified according to their predominant component; nevus sebaceus (sebaceous glands), nevus comedonicus (hair follicles), and nevus verrucosus (keratinocytes).
  • We report a neonate who presented with a nevus sebaceus on the scalp and face as well as a coloboma and dermoid on his left eye.
  • Within the sebaceous nevus on the scalp, circumscribed lesions of aplasia cutis congenita were detected, which is consistent with the recently proposed term in the literature didymosis aplasticosebacea.

  • BACKGROUND: Acral lentiginous melanoma (ALM) is the fourth histopathological subtype of malignant melanoma, accounting for < 10% of all melanomas in white-skinned populations.

  • Melanoma affects 15 to 20 per 100, 000 in the UK.
  • Cancers such as breast and prostate are around ten times more common than melanoma.
  • It is the most common cancer between the age of 20 and 35 years in Caucasians but the incidence of melanoma is still very low in that age group.
  • Melanoma has different clinical presentations with different outcomes and management.
  • They may grow from a pre-existing naevus but also from normal skin.
  • Melanoma can appear on the palms and soles or under the nail and on those sites may present as a vascular non-pigmented lesion which may cause diagnostic problems.
  • Subjects with fair skin and hair have a two-fold increased risk of melanoma.
  • The presence of > 100 naevi with atypical naevi can increase the risk of melanoma five-to ten-fold.
  • Any new mole in a subject over 40 should be regarded as suspicious.

  • The P48T germ line mutation of p16 was detected in a Hungarian multiple primary melanoma patient (deceased at the age of 39) with no affected family members.
  • Genetic analysis of the patient and his family revealed that the patient was homozygous for the mutation, whereas his parents (father currently aged 69 and mother 63), who are free from any malignancies and atypical moles, are both heterozygous for the mutation.
  • Our data suggest that the P48T mutation of p16 is a strong melanoma-predisposing factor, but the fact that the heterozygous mutant parents have not yet exhibited melanoma or atypical moles indicates that the penetrance of this allele might depend on modifying factors.
  • Here, we first report the inheritance of the rare P48T mutation of CDKN2A in a Hungarian family with a homozygous multiple primary melanoma member and unaffected heterozygous family members.

  • The objective of this study was to be able to distinguish between healthy skin tissue and malignant ones, furthermore determining a unique pattern of roughness for each skin lesion by microtopographic analysis of the skin surface of Mexican patients during the period from April to October 2002.
  • The standard technique used in this study for the diagnosis of skin cancer and the comparison of the results was the haematoxylin-eosin histopathological technique.
  • Latex impressions were taken from skin lesions as well as from the healthy skin of each patient to serve as control samples.
  • On average, the melanoma present an increase of roughness of 67% compared to healthy skin, obtaining a roughness relation of 1:2.54.
  • The percentage decreases to 49% (49%, 1:60) in the case of basal cell carcinoma and to 40% in pre-malignant lesions such as melanocytic nevus (40%, 1:150).
  • In benign lesions such as the seborrhoea keratosis only a small increase in roughness was noted (4%, 1:0.72).
  • Microtopographic inspection of the skin surface can be considered as a complementary diagnostic technique for skin cancer.

  • In a mouse melanoma metastasis model it has been recently shown that ADAM15 overexpression in melanoma cells significantly reduced the number of metastatic nodules on the lung.

  • In our study, we characterized the expression of ADAM15 in tissue micro-arrays of patients with primary melanoma with melanoma metastasis.
  • ADAM15 was expressed in melanocytes and endothelial cells of benign nevi and melanoma tissue.

  • A conjunctival melanocytic nevus may on occasion be difficult to distinguish from melanoma both clinically and histopathologically.
  • We evaluated a fluorescence in situ hybridization (FISH) assay, which has previously been shown to be of value for the diagnosis of melanocytic nevi and melanomas of the skin, using probes targeting 6p25 (RREB1), 6q23 (MYB), 11q13 (CCND1) and centromere 6 (CEP6), for its potential to assist in the distinction of conjunctival melanocytic nevi from melanomas.
  • Four melanocytic nevi and eight melanomas of the conjunctiva were analyzed.
  • None of the conjunctival melanocytic nevi showed a level of chromosomal aberrations that met FISH criteria for a diagnosis of melanoma.
  • All eight conjunctival melanomas (six unequivocal and two suspicious lesions) met FISH criteria for melanoma.
  • The findings encourage further exploration of this technique as an ancillary method for the work-up of conjunctival melanocytic proliferations.

  • OBJECTIVE: The purpose of this study was to compare the efficacy of using the Q-switched 532 nm neodymium:yttrium-aluminum-garnet (Nd:YAG) laser followed by the 1,064 nm laser versus the Q-switched 1,064 nm Nd:YAG laser alone in the treatment of Hori's nevus.
  • Ten women with bilateral Hori's nevus were recruited and treated with a combination of the Q-switched 532 and 1,064 nm Nd:YAG lasers on the right cheek and the Q-switched 1,064 nm Nd:YAG laser alone on the left cheek.
  • CONCLUSIONS: Concurrent use of the Q-switched 532 nm Nd:YAG laser in combination with the 1,064 nm laser is more effective in pigment clearance than the Q-switched 1,064 nm Nd:YAG laser alone for Hori's nevi.

  • Proton magnetic resonance spectroscopy at 8.5 T ex vivo was used to investigate skin lesions for metabolic signatures to predict malignancy or indicate malignant potential.
  • Magnetic resonance spectroscopy was performed on biopsy tissue obtained from 63 skin lesions and five melanoma metastases from 55 patients.
  • Samples were grouped and compared according to five clinically significant distinctions: melanoma (n=38) or nonmelanoma (n=30), primary melanoma (n=33) or secondary melanoma (involved nodes and distant metastases, n=5), primary melanoma (n=33) or nevi (n=8), malignant (n=46) or nonmalignant (n=22), and melanocytic (n=46) or nonmelanocytic (n=22).
  • There was a higher average choline metabolite signal intensity in melanoma-containing biopsies compared with nonmelanoma biopsies.
  • Discriminant analysis based on the intensity of the choline resonance alone achieved 69% accuracy in separation of melanoma and nonmelanoma tissue.
  • Tissue heterogeneity in conventionally collected full thickness skin biopsies and possible biochemical variance within individual tissue types limit classification accuracy using the methods and magnetic field strength that were earlier reported to provide accurate discrimination in other cancer types.
