  • OBJECTIVE: To evaluate the long-term results of plication of tunica albuginea in patients with penile curvature secondary to Peyronie's disease.
  • PATIENTS AND METHODS: A total of 78 men with penile curvature secondary to Peyronie's disease underwent corporeal plication over a 10 year period.
  • The questionnaire focussed on the presence or absence of penile deformity and pain, erectile function and the ability to perform sexual intercourse.
  • The cosmetic result was good or excellent (straight or almost straight penis) in 94% by 6 months.
  • A total of 90% patients had a satisfactory cosmetic result, whereas only 71% reported a satisfactory functional result (straight or almost straight penis on erection with pain free penetration and normal sexual intercourse) in the long-term.
  • CONCLUSIONS: Corporeal plication is an effective surgical option for the correction of penile deformity in patients with Peyronie's disease, with good cosmetic results and acceptable functional success rate in the long-term.

  • [Title] Recent insights into androgen action on the anatomical and physiological substrate of penile erection.
  • A number of recent developments shed new light on testosterone treatment of erectile dysfunction (ED) in aging men. (1) A recent insight is that, in contrast to younger men, elderly men might require higher levels of testosterone for normal sexual functioning. (2) Several studies have indicated that PDE5 inhibitors are not always sufficient to restore erectile potency in men, and that testosterone improves the therapeutical response to PDE5 inhibitors considerably. (3) There is growing insight that testosterone has profound effects on tissues of the penis involved in the mechanism of erection and that testosterone deficiency impairs the anatomical and physiological substrate of erectile capacity, reversible upon androgen replacement.
  • The synthesis of PDE5 is upregulated by androgens, and the arterial inflow into the penis is improved by giving androgen.

  • PURPOSE: Penile carcinoma is a rare but mutilating malignancy.
  • The suture is performed with vicryl 4-0 in a "parachute" fashion, beginning from the ventral portion of the urethra and the "V" flap, followed by the "V" flap angles and than by the dorsal portion of the penis.
  • In conclusion, the "parachute" technique for penile reconstruction after partial amputation not only improves the appearance of the penis, but also maintains an adequate function.

  • [Title] A three-dimensional model of the penis for analysis of tissue stresses during normal and abnormal erection.
  • Because adequate mechanical interactions in the penis are necessary for functional erection it is important to analyze stresses in the erect penis.
  • Previous penis models were limited to simplified or two-dimensional geometry.
  • Here we developed a three-dimensional model for structural analysis of normal erection as well as erections of a penis with substantial asymmetry of the corporal bodies, and Peyronie's disease.
  • The tunica was found to be the most highly loaded tissue in the erect penis.
  • The angles of distortion of the penis with respect to the vertical axis were approximately 4.5 degrees and approximately 2 degrees , for the asymmetric and Peyronie's cases, respectively.
  • The model's ability to determine internal stresses in the erect penis offers a new point of view on the mechanical factors involved with erection, and enables us to relate these data with different penile pathologies.

  • The injection of foreign substances into the penis for the purpose of augmentation may result in erectile dysfunction, voiding difficulties, and severe deformity.
  • We report a case of penile paraffinoma in a 71-year-old man that developed nearly 40 years after undergoing a series of penile injections with an unknown substance.
  • Penile paraffinoma can therefore present a diagnostic dilemma given its protracted latency period, after which time the initial injections have faded into distant memory.

  • OBJECTIVE: To characterize the biomechanics of erectile function, as contrary reports have modelled the penis as an isotropic material and state that only axial buckling tests can effectively predict penile rigidity; that assumption is questioned and an alternative structure proposed and validated.
  • A finite element analysis (FEA) computer model of the penis was constructed to simulate the response of the corpora cavernosa to axial and radial loading for differing diameters and lengths of the penile shaft.
  • The stresses developed in the tunica albuginea of the corporal bodies of the penis during buckling were assessed using a mathematical analysis.
  • Patient discomfort and high operator dependency further contribute to the inconsistencies of axial loading methods to determine penile buckling.
  • Based on the constant relationship between hoop stress and axial stress in thin-walled pressurized vessels this study confirms the validity and desirability of using radial compression methods to assess penile rigidity in lieu of axial loading methods.

  • The most frequent presenting sign of neurofibroma in females is clitoromegaly with pseudopenis, and enlarged penis is the most common sign in males.
  • One boy with asymmetric penile hypertrophy since 2 years of age and ipsilateral gluteal hypertrophy had plexiform neurofibromas that extended between the left lumbogluteal and penile regions, infiltrating the left rectum wall and bladder with compression of both structures, the left prostate, and the left half of the cavernous corpi with hypertrophy of this part and asymmetry of the penis.

  • [Title] Clinical application of scrotal flap on penis lengthening.
  • OBJECTIVE: To investigate the clinical application of the scrotal flap on penis lengthening.
  • The procedure consisted of designing a positive sign shaped incision 1.5cm above the root of the penis, dissect and release the superficial suspensory ligament and part of the deep suspensory ligament, then cover the elongated cavernosum with proper scrotal flap.
  • RESULTS: Six-month to 5-year follow-up showed that all patients were satisfied with the good contour and function of the penis.
  • CONCLUSION: The method of using scrotal flap on penis lengthening has the following advantages: simple operation, reliable blood supply of the flap, one-stage operation, and satisfactory postoperative results.
  • It is a preferable operation technique for penis lengthening.

  • Patients were categorized based on NIH prostatitis definitions of type I/II-evidence of pyuria and/or bacteriuria on urinalysis or culture, type III-presence of at least 1 of the pain or urinary symptoms in the NIH Chronic Prostatitis Symptom Index (pain in the perineum, testicles, tip of penis, pubic or bladder area, dysuria, ejaculatory pain, incomplete emptying, urinary frequency), type IV-inflammation on prostate biopsy and Other-symptoms other than those listed.
  • RESULTS: Of the 413 patients 57 were previously diagnosed with prostatitis (prevalent cases), 46 had no evidence of a prostatitis diagnosis in the medical record and 7 were treated by physicians outside of the Kaiser Permanente Northwest plan.

  • OBJECTIVE: To examine the tissue expression of DNA topoisomerase I (Topo I) and IIalpha (Topo II), to pursue the possibility of future chemotherapy regimens for squamous cell carcinoma of the penis (SCCP), as high expression of Topo I might indicate sensitivity to the camptothecins, whereas high Topo II might indicate sensitivity to etoposide.
  • Tumour stage, grade and type were available.
  • There were also strong correlations between tumour grade and Ki-67, and Topo II expression (both P < 0.001).
  • Tumour type was also strongly correlated with Topo II and Ki-67 expression, with the highest expression in basaloid carcinomas and the lowest in verrucous carcinomas.
  • However, Topo I expression was not correlated with any other tumour variable.

  • PURPOSE: To evaluate erectile function after permanent prostate brachytherapy using a validated patient-administered questionnaire and to determine the effect of multiple clinical, treatment, and dosimetric parameters on penile erectile function.
  • The clinical, treatment, and dosimetric parameters evaluated included patient age; preimplant IIEF score; clinical T stage; pretreatment prostate-specific antigen level; Gleason score; elapsed time after implantation; preimplant nocturnal erections; body mass index; presence of hypertension or diabetes mellitus; tobacco consumption; the volume of the prostate gland receiving 100%, 150%, and 200% of the prescribed dose (V(100/150/200)); the dose delivered to 90% of the prostate gland (D(90)); androgen deprivation therapy; supplemental external beam radiotherapy (EBRT); isotope; prostate volume; planning volume; and radiation dose to the proximal penis.
  • On univariate analysis, the preimplant IIEF score, patient age, presence of nocturnal erections, and dose to the proximal penis predicted for postimplant erectile function.
  • Because the proximal penis is the most significant treatment-related predictor of brachytherapy-related ED, techniques to minimize the radiation dose to the proximal penis may result in improved rates of potency preservation.

  • Penile verrucous carcinoma is a rare disease and little is known of its aetiology or pathogenesis.
  • In this study we examined cell-cycle proteins expression and correlation with human papillomavirus infection in a series of 15 pure penile verrucous carcinomas from a single centre.
  • Of 148 penile tumours, 15 (10%) were diagnosed as pure verrucous carcinomas.
  • In conclusion, lower levels of p16(INK4A) and Ki67 expressions differentiate penile verrucous carcinoma from usual type squamous cell carcinoma.
  • The low level of p16(INK4A) expression and human papillomavirus detection suggests that penile verrucous carcinoma pathogenesis is unrelated to human papillomavirus infection and the oncogenes and tumour suppressor genes classically altered by virus infection.

  • The corpus cavernosum (CC) extracellular matrix is essential for normal penile erection and is implicated in erectile dysfunction.
  • We characterized and quantified the volumetric density (Vv) of the elastic system fibers in the corpus spongiosum (CS), CC and tunica albuginea (TA) of the rabbit penis.
  • The penile mid-shaft fragments were fixed with 4% phosphate-buffered formalin solution and/or Bouin's liquid for 24-48 h, and processed using standard histological techniques.
  • The present data should therefore provide important information for devising experiments and interpreting results when using the rabbit penis as a model for penile dysfunctions, especially when making comparisons with humans.

  • OBJECTIVE: To study the effect and complications of augmentation phalloplasty by implanting autogenous tunica vaginalis grafts into the bilateral albuginea of the penile corpus.
  • The increase in the volume of the corpora cavernosa was achieved by applying autogenous tunica vaginalis grafts to longitudinal openings made bilaterally in the albuginea along the whole length of the penile cavernous corpora.
  • The perimeter of the penile cavernous corpora was measured in the flaccid state and during erection and dynamic infusion cavernosometry (DIC) was conducted for each dog before and after the operation.
  • Histology of the penis was examined using hematoxylin and eosin stains.
  • RESULTS: The average increase in the perimeter of the penile cavernous corpora was 21.

  • AIM: To introduce and evaluate the outcome of a structured management and counseling protocol for patients with a complaint of a small-sized penis.
  • METHODS: A structured protocol for consultation and management of (physically normal) patients with a complaint of a small penis through a descriptive study comprised of a series of 250 patients.
  • MAIN OUTCOME MEASURES: Percentage of patients who elect to undergo penile augmentation surgery.
  • Two had a buried penis, two had true micropenis and five had normal penile size.
  • CONCLUSIONS: Using a structured management and counseling protocol, most men chose not to undergo penile augmentation surgery, even when offered for free.

  • OBJECTIVE: To present our experience with surgical and conservative management of penile fracture.
  • METHODS: This prospective study was carried out in the Urology and Nephrology Center, at Al-Thawra Modern General and Teaching Hospital, Sana'a, Yemen from June 2003 to September 2007, and included 30 patients presenting with penile fracture.
  • Diagnosis was made clinically in all our patients.
  • Hard manipulation of the erect penis for example during masturbation was the most frequent mechanism of fracture in 53.3% of patients.
  • All operated patients described full erection with straight penis except 3 of the 8 patients who were managed by direct longitudinal incision, in whom mild curvature during erection was observed.

  • PURPOSE: Two cases of penile angioedema associated with the use of angiotensin-converting-enzyme inhibitors and angio-tensin II receptor blockers are reported.
  • SUMMARY: The first case of penile angioe-dema involved a 68-year-old man who arrived at the emergency department (ED) with a 2-12-hour history of penile swelling occurring three days after initiation of irbesartan in addition to longstanding lisinopril therapy.
  • The patient's penis was edematous at midshaft only and was nontender with normal skin coloring.
  • In the second case, a 48-year-old man arrived at the ED complaining of penile swelling over the previous two days.
  • The patient's penis was nontender and edematous at midshaft.
  • The swelling resolved within two days of stopping enalapril, and he had no further episodes of penile swelling.
  • CONCLUSION: Penile angioedema was reported in two patients.

  • OBJECTIVE: To illustrate the ultrastructural fibre composition of the rat cavernosal nerve at serial levels, from its origin in the main pelvic ganglion to its termination in the corpus cavernosum of the distal penile shaft, and to develop a technique that permits repeated electrophysiological recording from the fibres that form the cavernosal nerve distinct from the axons of the dorsal nerve of the penis (DNP).
  • Nerve conduction velocity for the cavernosal nerve was assessed from a point 2 mm below the main (major) pelvic ganglion after stimulating the nerve at the crus penis; multi-unit averaging techniques were used to enhance the recording of slow-conduction activity.
  • Recordings from the DNP were obtained over the proximal shaft after stimulation at the base of the penis.
  • As it descended the penile shaft, the nerve separated into small fascicles containing only one to four axons at the level of the distal shaft.
  • These yielded accurate data about the erectile status of the penis, and the state of unmyelinated and myelinated fibres in the DNP and cavernosal nerves of the same animal.

  • OBJECTIVES: Relatively little evidence is available in the published studies on the prevention of penile cancer and premalignant conditions of the penis.
  • The present review examined the current evidence available in preventing penile cancer and pathologic subtypes of premalignant conditions and their treatment.
  • METHODS: The association of human papillomavirus subtypes and penile cancer is well-established, although the etiology, natural history, and treatment of premalignant lesions have mainly been reported in retrospective case series with short-term follow-up.
  • The exact pathologic role of chronic inflammatory conditions, such as balanitis xerotica obliterans in the etiology of penile cancer remains largely unknown.
  • RESULTS: Some of the potential strategies for the prevention of penile cancer could include circumcision, reducing the risk of transmission of penile human papillomavirus infection with male vaccination, early treatment of phimosis, smoking cessation, and hygienic measures.

  • The most common site was paratesticular (n=9, 40.9%), followed by kidney (n=8, 36.4%), prostate (n=3, 13.6%) and penis and bladder (1 case each, 4.6%).
  • According to size and grade of the tumour we also found a significant difference in survival (p=0.0006 and p=0.01, respectively).
  • CONCLUSIONS: In our representative series, 3 tumor parameters (site, size and grade) appeared to represent the most important prognostic factors in adult genitourinary sarcomas.

  • INTRODUCTION: Isolated lymphedema of the penis is extremely rare: combined involvement of the scrotum and penis is the norm.
  • We present a case of isolated penile lymphedema that responded to excision of lymphedematous tissue and reconstruction with flaps.
  • CASE PRESENTATION: A 32-year-old Arab man presented with a three-year history of a gradually increasing, painless penile swelling.
  • The swelling was preceded by at least three prior episodes of severe furunculosis at the penile root.
  • On examination there was gross penile enlargement, maximally at the mid shaft, associated with thickened skin at the sites of prior furunculosis.
  • The clinical diagnosis was lymphedema following recurrent penile furunculosis.
  • Closure of the penile shaft was accomplished by bilateral advancement of flaps from both ends of the penis.
  • CONCLUSIONS: Furunculosis at the penile root may result in lymphedema confined to the penile shaft, sparing the scrotum.

  • [Title] Treatment of penile deep dorsal venous leakage of erectile dysfunction by embedding the deep dorsal vein of the penis: a single center experience with 17 patients.
  • AIMS: To assess the outcomes of embedding the deep dorsal vein of the penis (EDDVP), a new surgical technique used in patients with penile deep dorsal venous leakage of ED.
  • METHODS: Between December 2001 and November 2007, 17 patients diagnosed with penile deep dorsal venous leakage of ED underwent embedding the deep dorsal vein of the penis.
  • MAIN OUTCOME MEASURES: All cases were available for follow up by using the abridged 5-item version of the International Index of Erectile Function (IIEF-5) scoring system and penile color Doppler ultrasound.
  • During the tumescent phase, the deep dorsal vein of the penis was compressed between the dilated sinusoidal spaces and the tunica albuginea and resulted in venous drainage blockade.
  • CONCLUSIONS: The new surgical technique of EDDVP is a simple operative procedure, which seems to be efficient in the treatment of penile deep dorsal venous leakage of ED.

  • OBJECTIVES: To assess epidemiologic characteristics of penile cancer in Brazil.
  • Their patients' clinical and epidemiological data was analyzed (age, race, place of residence, history of sexually transmitted diseases, tobacco smoking, performance of circumcision, type of hospital service), as well as the time between the appearance of the symptoms and the diagnosis, the pathological characteristics of the tumor (histological type, degree, localization and size of lesion, stage of disease), the type of treatment performed and the present state of the patient.
  • RESULTS: 283 new cases of penile cancer in Brazil were recorded.
  • Of the 283 patients presenting penile cancer, 171 (60.42%) had phimosis with the consequent impossibility to expose the glans.
  • In 48 cases (16.96%) the tumor affected the glans, the prepuce and the corpus penis; in 28 cases (9.89%) the tumor affected the entire penis.
  • CONCLUSION: Penile cancer is a very frequent pathology in Brazil, predominantly affecting low income, white, uncircumcised patients, living in the north and northeast regions of the country.

  • The aim of the study was to investigate the expression of cyclin D1 and Ki-67 in squamous cell carcinomas (SCC) and in some premalignant lesions of the penis and to correlate it with clinicopathological parameters and patient survival.
  • A tendency for an association between cyclin D1 expression and tumour differentiation (p = 0.07) but not the level of tumour invasion (p = 0.50) was found.
  • The Ki-67 expression was notably increased with the advance of tumour grade, but the difference did not reach a statistically significant level (p = 0.46).

  • [Title] Penile self-mutilation as an unusual sign of a separation-related problem in a crossbreed dog.
  • A two-year-old, male, crossbreed dog was presented for evaluation of licking, biting and severe self-mutilation of its penis, which resulted in ulcers and haemorrhagic preputial discharge.
  • History revealed signs of separation anxiety such as active greeting behaviour, excessive vocalisation and intermittent episodes of licking of the penis when the dog was left alone.
  • During that time, the behaviour progressed to frequent episodes of licking and biting of the penis.
  • The purpose of this report is to contribute to an increased awareness within veterinary practice of unusual signs of separation anxiety such as penile self-mutilation.

  • The prevalence of specific post-operative sexual problems may be related to the site of reconstruction (penile or bulbar) and to the technique of urethroplasty employed.

  • OBJECTIVE: To investigate the clinical pathological characteristics, diagnosis and differential diagnosis of Paget's disease of the scrotum and penis.
  • METHODS: Thirteen cases of Paget's disease of the scrotum and penis were analyzed by light microscopy, alcian-blue (AB)/periodic-acid-Schiff (PAS) and immunohistochemical staining.
  • RESULTS: Paget's disease of the scrotum and penis mainly affected old individuals aged 55-84 (mean 71) years.
  • Macroscopically, typical presentations of Paget's disease of the scrotum and penis were eczematoid lesions.
  • CONCLUSION: Paget's disease of the scrotum and penis is a low-malignancy cutaneous tumor with typical clinical and pathological features.

  • PURPOSE: Severe penile curvature correction by corporeal body grafting has been successfully performed using various grafts and biomaterials.
  • We present our initial experience with buccal mucosa as a free corporeal graft to correct severe penile curvature as part of a multistage approach to posterior hypospadias repair.
  • Preoperative androgen supplement was given when penile and glanular size was significantly small for age.
  • The pre-grafting penile angle, immediate post-grafting angle and penile angle at 6 months were assessed.
  • After chordee release and urethral plate transection the penile angle was between 45 and 80 degrees.
  • All children had a straight penis at the end of the procedure and none required additive dorsal plication.
  • At the last followup at 12 months all children had a straight penis except 2 with mild curvature (less than 10 degrees).

  • Penile TB is extremely rare comprising less than 1 % of all genital TB cases in males.
  • Diagnosis of penile TB is often difficult because it can mimic numerous other diseases.
  • The case described herein involves a patient with multidrug-resistant smear-positive penile TB that was undiagnosed initially due to the lack of clinical suspicion of TB, and once diagnosed failed to respond to first line antitubercular drugs because of multiple drug resistance.

  • We report a case of penile schwannoma in a young man who was accurately diagnosed preoperatively with the help of gray-scale and color flow Doppler ultrasonography, and confirmed by magnetic resonance imaging.
