  • Cutaneous adnexal differentiation is well-recognized in benign mixed tumors occurring in cutaneous sites.
  • The incidence of this histologic finding in salivary gland sites is not known.
  • We sought to describe the incidence of cutaneous adnexal differentiation in benign mixed tumors of the palate, lip, and parotid gland.
  • Benign mixed tumors of the palate (n=30), lip (n=13), and parotid gland (n=37) resected between 1980 and 2009 at a single academic medical institution were reviewed.
  • All hematoxylin and eosin-stained sections containing neoplasm were reviewed by all authors including one dermatopathologist (S.H.O.).
  • After confirming the diagnosis of benign mixed tumor, we evaluated for morphologic evidence of cutaneous adnexal differentiation and metaplastic epithelial and stromal changes.
  • Cutaneous adnexal differentiation was seen in 20% of palate and 39% of lip benign mixed tumors but in no parotid tumors.
  • Sebaceous differentiation was seen in only one palate tumor.
  • Osseous metaplasia was seen in one tumor from each site.
  • When cutaneous adnexal differentiation occurs in salivary gland pleomorphic adenomas, it can present a diagnostic pitfall that must not be misinterpreted as carcinoma at biopsy, fine needle aspiration, or frozen section.

  • [Title] Fine needle aspiration cytology of oncocytic lipoadenoma of the parotid gland: report of a rare case.
  • BACKGROUND: Oncocytic lipoadenoma is an uncommon benign salivary gland tumor.
  • CASE: A 50-year-old woman presented with a slow-growing swelling in the left parotid region that was clinically interpreted as a soft tissue tumor, with a differential of neurofibroma/lipoma.
  • The adipose tissue background of the cytologic smears was ignored as material derived from the normal fat tissue; based on the oncocytic population of cells, a diagnosis of oncocytoma was considered.
  • However, histopathologic examination showed it to be an oncocytic lipoadenoma, a tumor we were unaware of at the time of cytodiagnosis.
  • CONCLUSION: Both cytopathologists and histopathologists need to be aware of oncocytic lipoadenoma of the salivary gland in order to diagnose it precisely.
  • The clinicocytopathologic correlation highlighted in our case will be useful for cytopathologists in preoperative interpretation and diagnosis.

  • OBJECTIVE: To document the pattern of salivary gland neoplasia in Maiduguri, Nigeria.
  • MATERIALS AND METHODS: Information on demographics, diagnosis and cancer management in the hospital were retrieved from biopsy reports and case notes of patients.
  • RESULTS: The palatal (71.9%) and parotid (78.3%) glands were the most common minor and major salivary glands involved, with a benign-malignant ratio of 1:1 and 1.4:1, respectively.
  • Pleomorphic adenoma (44.3%) was the most common salivary gland neoplasm recorded.
  • Mucoepidermoid carcinoma (10.1%) was the most common salivary gland malignancy, occurring in the second and sixth decades; of equal gender distribution and predominantly in the palate (50%).
  • CONCLUSION: Pleomorphic adenoma and mucoepidermoid carcinoma were the most commonly reported benign and malignant neoplasia in this series.

  • [Title] Tubular-trabecular type Basal cell adenoma of the parotid gland: a patient report.
  • Basal cell adenoma (BCA) is an uncommon benign salivary gland neoplasm that includes isomorphic basaloid cells.
  • We report on a female patient with BCA that developed in the right parotid gland in her 50s.
  • The present patient demonstrated a few tumor nests in the fibrous capsule, and her tumor was larger than usual.
  • Histopathologically, the tumor was characterized by multiple duct-like structures and tubular-trabecular masses composed of small isomorphic cells with hyperchromatic, round nuclei and an eosinophilic cytoplasm.
  • It was difficult to determine whether the ductal structures noted in the tumor capsule were invasive.
  • By immunohistochemistry, tumor cells of the tubular nests were positive for cytokeratin 7 and that the outer cells of tubular nests were positive for alpha smooth muscle actin (αSMA) and calponin.
  • Tumor cells were immuno-negative for S-100 protein and glial fibrillary acidic protein.
  • We could achieve an accurate diagnosis of BCA by immunohistochemistry with MIB-1 and other markers.

  • DNA ploidy and S-Phase fraction (SPF) of 279 salivary gland tumours were analysed using high-resolution DNA flow cytometry.
  • All 229 benign neoplasms were diploid while 12 of 50 malignant tumours showed cell populations with aneuploid DNA content.
  • The SPF values of diploid malignancies were significantly higher if compared with pleomorphic adenomas but did not differ from that of the zystadenolymphoma (Warthin tumour) group.
  • While aneuploidy represents a distinct indicator of malignancy SPF values are of minor relevance for dignity assessment in salivary gland tumours.

  • The definitive histopathological diagnosis was compared with any pre-operative FNAC diagnosis.
  • RESULTS: For the University Teaching Hospital the sensitivity in distinguishing malignant from benign disease was 79% (95% CI 61-97%) with a specificity 84% (95% CI 73-95%).
  • However, three of eight patients with a primary parotid salivary gland malignancy were reported as having benign disease on FNAC.
  • For the participating District General Hospital the sensitivity in distinguishing malignant from benign disease was 38% (95% CI 13-63%) and specificity 95% (95% CI 73-95%).
  • CONCLUSIONS: Fine needle aspiration cytology does not reliably distinguish a benign from a malignant primary salivary gland neoplasm in the participating institutions.

  • AIM: To determine whether DNA ploidy by image cytometry is a good diagnostic tool to distinguish benign and malignant salivary gland tumours.
  • METHODS: A total of 62 salivary gland tumours were studied.
  • Paraffin sections (40 microm) were micro-dissected to isolate tumour areas; cell nuclei were extracted and Feulgen-stained cytospin monolayers were analysed using a DNA image cytometry system.
  • RESULTS: Fifty-three of 62 salivary gland tumours were uniformly diploid.
  • CONCLUSIONS: The vast majority of salivary gland tumours were diploid.

  • [Title] Superficial parotidectomy versus retrograde partial superficial parotidectomy in treating benign salivary gland tumor (pleomorphic adenoma).
  • PURPOSE: Of all benign salivary gland tumors of the parotid gland, pleomorphic adenoma (mixed tumor) is the most common.
  • It accounts for 60% to 70% of all benign tumors of the parotid gland.
  • This neoplasm arises in patients in the fourth to sixth decade of life, with a female predominance.
  • PATIENTS AND METHODS: We reviewed 48 patients who underwent excision of pleomorphic adenoma of the parotid gland between 1996 and 2005 at Rambam Medical Center: 18 were treated surgically with the classical superficial parotidectomy (SP) technique, using an anterograde approach, and 30 were treated with retrograde partial superficial parotidectomy (PSP).

  • BACKGROUND: Metastasizing pleomorphic adenoma (MPA) is a rare, incompletely understood enigmatic entity having the histologic appearance of benign pleomorphic adenoma and clinically malignant behavior.
  • We report a case of MPA of the parotid gland that metastasized to the scapular region 6 years after the appearance of the primary lesion in the left parotid gland.

  • [Title] Tumors of the salivary glands in childhood and adolescence.
  • PURPOSE: We report on 52 juvenile and adolescent patients with benign and malignant tumors of the salivary glands.
  • RESULTS: Benign tumors had their highest incidence in puberty and postpuberty.
  • Forty patients (20 girls and 20 boys; 77%) had benign growths and 12 patients (9 girls and 3 boys; 23%) suffered from malignant neoplasms.
  • After surgery for benign tumors we saw no postoperative recurrences in long-term follow-up.
  • CONCLUSIONS: Salivary gland diseases are rare in childhood and adolescence.
  • This is particularly true for the malignant parotid tumors that are more frequent in juvenile patients, a fact that has to be considered in diagnosis and therapy.

  • Two cases of sebaceous lymphadenoma are presented in which, in common with most cases reported in the literature, the correct pre-operative diagnosis was not made.
  • Sebaceous lymphadenoma rarely transforms into a malignant tumour.
  • Fine needle aspiration cytology identifies a benign process in the majority of patients who receive appropriate treatment on this basis.

  • Myofibroma is a benign mesenchymal neoplasm composed of myofibroblasts which has been described with different synonyms since the first report in 1951.

  • Microscopic analysis of both lesions revealed a spindle cell tumor with immunoreactivity for vimentin, muscle-specific actin, and specific smooth muscle isoform alpha-actin, rendering the diagnoses of myofibroma.
  • Myofibroma presents a wide range of differential diagnosis, including benign and malignant neoplasms.

  • We present the cytologic features and differential diagnoses of one such case.A 45-yr-old woman presented with an enlarged submandibular gland on the left side with ipsilateral cervical lymphadenopathy that had been persisting for 1 mo.
  • The gland and the two enlarged nodes measured 2.5 x 2.5 cm(2) each and were firm in consistency.
  • Clinically, tumor of the salivary glands was suspected.
  • Isolated stromal fragments consisting of fibrocytes were seen separate from salivary acinar clusters on repeat aspirations.
  • The diagnosis of SHML involving left submandibular gland and ipsilateral lymph nodes was returned.
  • When analyzed in the context of clinical findings (laboratory data), the cytologic features of SHML involving salivary gland could be differentiated from those of malignancies and other benign lesions, especially Kuttner's tumor of the submandibular gland, which mimics neoplasm clinically.

  • BACKGROUND: Myoepithelioma is a rare benign salivary gland tumour, localized most frequently in the parotid and in minor intraoral salivary glands.
  • The diagnosis was consistent with myoepithelioma, plasmocytoid variant.
  • Tumour cells were positive for cytokeratins, vimentin and S-100 protein.
  • Surgical resection was performed and no evidences of tumour recurrence were observed after 6 years of the treatment.
  • CONCLUSION: Myoepithelioma is a very rare tumour in children and apparently presents a good prognosis, similar to occur in adult patients.

  • AIM: The aim of the present study is to establish the relative frequency and distribution of benign and malignant epithelial neoplasms of salivary glands in the Pathology and Cytology Laboratory, STUDY DESIGN: Historic cohort.
  • The neoplasms were individualized by gender, age, race of the patients, anatomic localization of the lesions and histopathological diagnosis.
  • RESULTS: Out of 162,312 registered cases, 245 were salivary gland epithelial neoplasms and 187 (76.33%) were benign and 58 (23.67%) were malignant.
  • Pleomorphic adenoma was the most frequent benign neoplasm (89.94%) and adenoid cystic carcinoma represented the most prevalent malignant neoplasm (22.41%).
  • The benign neoplasms occurred mainly between the second and third decades of life and showed preference for female, while malignant neoplasms were diagnosed between the sixth and seventh decades of life and in women.

  • INTRODUCTION: Basal cell adenoma (BCA) is a rare benign neoplasm characterized by the basaloid appearance of the tumour cells and the lack of myxo-chondroid stromal component present in pleomorphic adenoma.
  • So the diagnosis of metastatic malignant salivary gland tumor was suspected.
  • CONCLUSION: The BCA is a benign tumor located generally in the parotid gland.
  • When the malignancy is suspected, like in our case, this tumor must be differentiated from the basal cell adenocarcinoma using histological criteria.

  • Salivary gland swelling is a common and important problem.
  • Acute and chronic sialadenitis, different benign and malignant neoplasms are the common causes which present with salivary gland swelling.
  • Imaging technique is not so helpful in pre-operative diagnosis; microscopical examination is required for diagnosis.
  • Pre-operative core needle biopsy is hazardous and may damage facial nerve, lead to fistula formation or associated with tumour seeding.
  • The study was done with 40 cases of salivary gland swelling.
  • Out of 40 cases, 25 involved the parotid gland, most common age group affected was 20 - 40 years and male: female ratio was 5: 3.
  • Among these 3 cases, 2 were adenoid cystic carcinoma which was cytologically diagnosed as benign neoplasm (monomorphic adenoma).
  • One case of Warthin's tumour was cytologically diagnosed as pleomorphic adenoma.
  • This study corroborates well with other studies including immunohistochemical findings. p53 expression was found to be related with nature of the neoplasm.

  • AIM OF THE WORK: The authors present unusually rare case primary multiple synchronicus benign tumor of the ipsilateral parotid gland: oncocytoma and Warthin's tumor, which is rare among occasionally occurred multiple tumors with different histology in the salivary glands.
  • MATERIAL AND METHOD: A 71-year-old man presented with the tumor in the right parotid gland.
  • We performed excision of the superficial lobe of the parotid gland.
  • The preoperative ultrasonography and macroscopic analysis of the removed parotid gland tissue revealed only one tumor recognised histopathologically as adenolimphoma, but upon histopathological examination Warthin's tumor proved to coexist with oncocytoma.
  • RESULTS AND CONCLUSIONS: In the ultrasonography examination some salivary glands tumors may not be recognised.
  • The histopathological examination is the ultimate method in recognising tumors, which are not detected in the preoperative diagnosis.
  • The superficial parotidectomy protects from the recrudescence of the superficial lobe of the parotid gland tumors.
  • The coexistence of the oncocytoma and Warthin's in one parotid gland is extremely rare.

  • OBJECTIVE: Pleomorphic adenomas are the most common salivary gland tumors and are typically cured with complete surgical excision.
  • There are rare reports, however, in which these histologically benign tumors have inexplicably metastasized to distant sites.
  • The patient's presentation, workup, and final diagnosis of benign metastasizing pleomorphic adenoma will be discussed, along with a pertinent review of the literature.
  • RESULTS: A diagnosis of recurrent pleomorphic adenoma was made from a fine needle aspiration biopsy of the right parotid mass.
  • Examination of the pathology specimens confirmed a diagnosis of pleomorphic adenoma in both the parotid bed and the mediastinum.
  • No histologic characteristics of malignancy were seen in either specimen; therefore, a diagnosis of benign metastasizing mixed tumor was rendered.
  • CONCLUSION: Benign metastasizing pleomorphic adenoma is a rare and controversial but distinct clinical entity.
  • Although the definition of the term benign precludes metastatic disease, these tumors do not demonstrate any malignant features yet metastasize to distant sites.
  • It remains to be determined whether these benign metastasizing pleomorphic adenomas are really low-grade salivary malignancies.

  • [Title] Benign and malignant salivary gland diseases in children A retrospective study of 549 cases from the Salivary Gland Registry, Hamburg.
  • OBJECTIVE: Tumors of salivary glands in children are rare.
  • Basically all types of salivary gland diseases during the period of childhood are described.
  • The incidences of salivary gland tumors in children (0-14 years) differ completely from those in adults.
  • METHODS: In our study we were able to draw data from the Hamburg Salivary Gland Registry, concerning the last 25 years regarding histopathological diagnosis, age, gender and localisation of the pathologies.
  • As a reference centre for salivary gland diseases some material was sent by other institutions.
  • RESULTS: This study will give a detailed survey of salivary gland diseases and tumors in children up to the age of 14 which have undergone surgical therapy/biopsy.
  • Comparing the distribution of malignant tumors with other studies, the epithelial-myoepithelial carcinomas followed by salivary duct carcinomas represent the largest group in childhood.
  • Further many salivary gland diseases in childhood underwent surgery/biopsy although this is not supposed to be the proper treatment.
  • The study shows that the right diagnosis most often can only be reached by surgery and histology.

  • Benign metastasizing pleomorphic adenoma is a rare neoplasm, in which the benign appearing pleomorphic adenoma, without any histological evidence of malignancy, metastasizes to distant sites including lung.
  • In the absence of clinical history of a pre-existing myoepithelial neoplasm, the presence of myoepithelial cells in the lung or any other organs besides salivary glands may create diagnostic difficulty.
  • Here we present the cytologic findings of such a metastatic tumor found in the lung FNA and pleural fluid specimens from a 64-year-old woman, with a history of local recurrent salivary gland pleomorphic adenomas, who presented with multiple bilateral pulmonary nodules and pleural effusion.
  • The diagnosis of benign metastasizing pleomorphic adenoma was made based on clinical information and cytomorphology, and confirmed by immunocytochemistry.

  • The proliferative activity of the tumor cells and the expression of tumor-associated genes and sex steroid hormone receptors were investigated immunohistochemically in ten cases of carcinoma ex pleomorphic adenoma (Ca-ex-PA) of the salivary glands.
  • These were analyzed in benign and malignant components separately, and then were compared with ten cases of the other malignant tumors [adenocarcinomas, not otherwise specified (ACN) and salivary duct carcinomas (SDC)] and ten cases of pleomorphic adenomas (PA).
  • (1) malignant component of Ca-ex-PA showed a higher incidence of PCNA and Ki67 than benign component of Ca-ex-PA.
  • A significant difference between benign component of Ca-ex-PA and PA was not observed. (2) A significant difference in the incidence of p53, c-erbB-2, EGFR overexpression was observed only between malignant component of Ca-ex-PA and benign component of Ca-ex-PA. (3) The incidence of PCNA, Ki67, p53, c-erbB-2 overexpression in malignant component of Ca-ex-PA showed the highest data among the four groups.
  • These results suggest that Ca-ex-PA acquired the particular biological behavior in contrast to the other salivary neoplasms in the long-standing process while PA undergoes malignant transformation.

  • BACKGROUND: Sebaceous lymphadenoma of the parotid gland is a rare benign neoplasm.
  • This is the first reported case of fine needle aspiration biopsy (FNAB) findings for sebaceous lymphadenoma of the parotid gland.
  • It revealed lymphoid and salivary gland parenchymal cells.
  • The surgical specimen of the parotid mass confirmed the diagnosis of sebaceous lymphadenoma on the tissue section.
  • CONCLUSION: This report is the first to describe the FNAB findings of the unusual benign parotid neoplasm sebaceous lymphadenoma.
  • Though the definitive diagnosis of any parotid mass requires tissue, generally obtained via parotidectomy, an FNAB diagnosis can be useful in counseling a patient prior to definitive biopsy.

  • BACKGROUND: Warthin's tumor is a common benign neoplasm of the salivary gland.
  • Human Herpes Virus 8 (HHV-8) is the etiologic agent for all forms of Kaposi's sarcoma (KS), and HHV-8 DNA is present in saliva, suggesting that non-sexual transmission is associated with latent infection of the salivary gland.
  • OBJECTIVES: To provide insights into the HHV-8 cell tropism, the presence of HHV-8 was investigated in a series of Warthin's tumors of the salivary gland and corresponding adjacent normal tissue.
  • RESULTS: HHV-8 DNA was detected in 19 out of 43 (44%) salivary gland tumor samples.
  • Among the 15 cases with paired samples, 9 were HHV-8-positive for both samples, 4 were HHV-8-negative for both samples while in two cases HHV-8 was detected only in the tumor specimens.
  • CONCLUSIONS: HHV-8 is frequently detected in adenoid salivary neoplasms, suggesting a significant role of the virus in the etiopathogenesis of the disease.

  • This study on 279 tumours of the salivary glands was conducted to analyse whether the assessment of DNA ploidy by flow cytometry may assist histopathology in discriminating benign from malignant types of tumours.
  • The group of benign tumours included 164 pleomorphic adenomas, 51 Warthin's tumours, 7 basal cell adenomas, 2 lipomas as well as 5 other different tumours.
  • All of the 229 benign tumours were diploid.
  • The malignant tumours consisted of 18 adenoid cystic adenomas, 10 mucoepidermoid carcinomas, 5 acinic cell carcinomas, 5 carcinoma in pleomorphic adenoma as well as of 12 other malignancies belonging to 7 different tumour entities.
  • Twelve of 50 malignant salivary gland tumours were aneuploid.
  • Examination of consecutive slides actually revealed small assemblies of carcinoma cells that required a final diagnosis as non-invasive carcinoma in pleomorphic adenoma.
  • The most obvious value of DNA flow cytometry in salivary gland tumours is thus its contribution to assist histopathology in identifying potentially malignant lesions.

  • [Title] [Carcinoma ex pleomorphic adenoma of major salivary glands--a clinicopathologic review].
  • BACKGROUND: Pleomorphic adenoma (PA) is the most common neoplasm of the major salivary glands.
  • There are three subtypes of malignant PA: carcinoma ex pleomorphic adenoma (CXPA); carcinosarcoma (true malignant mixed tumor) and metastasizing pleomorphic adenoma.
  • For proper diagnosis of CXPA, a statement of coexistence of pleomorphic adenoma and carcinoma (or carcinoma after prior PA surgery) is needed.
  • Own material is presented because of rarity and clinicopathologic specificity of this neoplasm.
  • The following clinical factors were evaluated: age, sex, symptoms (time of lasting, evolution), tumor size, invasion of the adjacent structures and facial nerve, neck nodes, clinical stage, treatment outcome.
  • One pathologist reviewed histological material of 18 out of 19 patients who had been operated and pathological factors such as sensitivity and accuracy of fine needle aspiration biopsy, grade, histology and proportion of malignant component in tumor, lymph nodes metastases were analyzed.
  • In 15 cases, tumor was localized in the parotid and in 4 in the submandibular gland.
  • Duration of symptoms of benign PA was from 2 to 40 years (mean 17.8 years).
  • Symptoms of malignant transformation occurred in 15 patients, the most common were rapid enlargement of tumor, pain and facial nerve palsy.
  • Tumor size varied from 2 to 20 cm with a mean of 6 cm.
  • Adjacent structures were invaded by neoplasm in 5 cases.
  • Clinical picture and histopathological examination determinate the diagnosis of CXPA.
  • The best method of prevention and treatment of CXPA is early and radical removal of all major salivary glands tumors.

  • BACKGROUND: Pilomatrixoma (pilomatrixoma, calcifying epithelioma of Malherbe) is a relatively uncommon, benign neoplasm arising from the skin adnexa.
  • The tumor can cause diagnostic difficulty not only for the clinician but also for the cytologist.
  • Three preliminary differential diagnoses were offered: mucoepidermoid carcinoma of the submandibular salivary gland, squamous cell carcinomatous deposit in a submandibular lymph node and calcifying odontogenic tumor.
  • An excisional biopsy of the swelling was performed, and the histologic diagnosis of pilomatrixoma was made.
  • CONCLUSION: The cytologic presentation of pilomatrixoma of the right submandibular region can masquerade as that of a malignant tumor, in this case mucoepidermoid carcinoma, squamous cell carcinoma or odontogenic tumor.

  • OBJECTIVE: To describe the fine needle aspiration cytology (FNAC) features of various salivary gland lesions and to analyze the respective diagnostic value and pitfalls.
  • METHODS: 113 FNAC specimens of salivary gland lesions were reviewed and correlated with clinical and histopathologic findings.
  • RESULTS: The FNAC diagnostic failure (2); non-neoplastic lesions (12); benign neoplasm (82) and malignant neoplasm (17).
  • The three inaccurately diagnosed cases of FNAC are, as follows: reactive lymphoid hyperplasia of lymph node mistaken as non-Hodgkin lymphoma, mucoepidermoid carcinoma diagnosed as "scanty atypical cells present" and primary lymphoepithelial carcinoma mistaken as benign lymphoepithelial lesion.
  • On the basis of FNAC, 97.4% (110 /113) were correctly depicted as benign (95/96; 99.0%) or malignant (15/17; 88.2%).
  • Furthermore, 90.3% (102 /113) (specificity = 91.9%; 102/111) were accurately diagnosed, including 91.7% (88/96) benign lesions (specificity = 92.6% ; 88/95) and 82.4% (14/17) malignant tumors (specificity = 87.5%; 14/16).
  • CONCLUSIONS: FNAC is reliable in distinguishing benign and malignant salivary gland lesions.
  • A specific cytologic diagnosis is often possible.

  • Ceruminal glands are modified sweat glands of the skin of the external auditory meatus that may give rise to (a) benign tumors such as ceruminous adenoma, pleomorphic adenoma, and syringocystadenoma papilliferum, and (b) malignant tumors such as ceruminous adenocarcinoma, adenoid cystic carcinoma, and mucoepidermoid carcinoma.
  • An alternative theory holds that pleomorphic adenomas may well arise from ectopic salivary tissue present in the external ear canal.
  • These tumors are very rare and usually have a benign course.
  • Magnetic resonance imaging showed no intracranial extension or any association with the adjacent parotid gland.
  • The patient underwent modified radical mastoidectomy and complete resection of the tumor and the entire skin of the external auditory canal.
  • CONCLUSIONS: Pleomorphic adenoma is an extremely rare tumor arising from the ceruminal glands of the external ear canal.
  • Nonspecific presentation and difficult histologic diagnosis characterize this benign neoplasm.

  • CONTEXT: Necrotizing sialometaplasia is a benign, self-limited lesion of both major and minor salivary glands, although more commonly the latter.
  • It can represent a diagnostic dilemma and may be mistaken for a malignant neoplasm, such as mucoepidermoid carcinoma, as well as invasive squamous cell carcinoma.
  • Bulimia, an eating disorder with increasing prevalence in our society, may also be an underlying underreported cause.
  • OBJECTIVE: To discuss the potential pathogenesis, diagnostic pitfalls, and the application of immunohistochemistry as an aid in the diagnosis of necrotizing sialometaplasia.
  • CONCLUSIONS: The diagnosis of necrotizing sialometaplasia may be difficult and is reliant upon a well-oriented biopsy section and a complete clinical history.
  • Diagnosis may be further supplemented via immunohistochemistry, demonstrating focal to absent immunoreactivity for p53, low immunoreactivity for MIB1 (Ki-67), and the presence of 4A4/p63- and calponin-positive myoepithelial cells.
  • Interpreted in context collectively, these findings may be helpful adjuncts in the diagnosis of necrotizing sialometaplasia; nonetheless, to date, hematoxylin-eosin staining remains the gold standard.

  • INTRODUCTION: Salivary gland neoplasms represent the diverse group of tumours encountered by the head and neck surgeons.
  • MATERIAL AND METHODS: The study is analysis of 531 patients with parotid gland tumours in patients treated in ENT Department during 1986-2006.
  • We analysed surgical treatment and intraoperative tumour stage.
  • The results of histological diagnosis were presented.
  • RESULTS: The frequency of incidence of benign neoplasm was 80%.
  • The most frequent tumour was pleomorphic adenoma--75% and adenolymphoma--16%.
  • Malignant tumours constituted 20% of all gland tumours.
  • Mucoepidermoid (27%) and adenoid cyst (22%) carcinomas were the most common malignant tumour.
  • Tumour stages T3-T4 was in 90%.

  • HYPOTHESIS: Analysis of salivary gland lesions by FNAC and correlation with histopathology.
  • To evaluate utility of FNAC in salivary gland lesions.Back ground: Salivary gland lesions form about 2-6.5% of all head and neck neoplasms in adults.
  • They are easily accessible for FNAC (Fine Needle Aspiration Cytology) and risks of fistula formation or tumour implantation are low compared surgical biopsy.
  • Also, cytology can provide a distinction between asalivary and non salivary lesion, benign and malignant lesions so also specific and non specific inflammation.
  • RESULTS: 80% of the lesions were neoplastic (61% benign, 31% malignant) and 20% were neoplastic.
  • Pleomorphic adenoma was the most frequent benign neoplasm while mucoepidermoid carcinoma was the most frequent malignant lesion.
  • CONCLUSIONS: FNAC was found to be a useful diagnostic tool in the evaluation of salivary gland lesions because of its simplicity, excellent patient compliance and rapid diagnosis.

  • RESULTS: One hundred sixty-six PPS masses were identified: 21 (12.7%) were malignant, 145 (87.3%) were benign, 76 (45.8%) were vascular, and 69 (41.6 %) involved the skull base.
  • Expected neurologic sequelae resulted from cranial nerve involvement by tumor.
  • CONCLUSION: Careful patient assessment and surgical techniques allow the oncologically safe removal of benign, vascular, and skull base PPS tumors.

  • AIMS: Epithelial-myoepithelial carcinoma (EMC) is a rare malignant salivary gland neoplasm that most commonly occurs in the parotid gland, but can also arise in the minor salivary glands.
  • Three cases are reported of epithelial-myoepithelial carcinoma of the minor salivary glands, with the goal of better defining this entity.
  • All parts of each tumor were surrounded by a myoepithelial cell rim and there was evidence of invasion.
  • RESULTS: Immunohistochemical analysis showed the tumor cells to be weakly positive for S100, cytokeratin (CK) CK5/6, CK7, CKAE-1/AE-3 and strongly positive for epithelial membrane antigen (EMA) and p63; they were focally positive for calponin and acute lymphoblastic leukemia antigen (CD10).
  • The tumor cells were negative for vimentin, alpha-smooth muscle actin (SMA) (except one case), glial fibrillar acid protein (GFAP) and MIB1.
  • CONCLUSION: Three cases of minor salivary gland tumors are described and the differential diagnosis underlined in relation to benign myoepithelioma.
  • The characteristic morphological and immunohistochemical features aided diagnosis of these biphasic tumors.

  • BACKGROUND: Pleomorphic adenoma (PA) is a benign tumour of the salivary gland with a tendency to malignancy which creates many diagnostic problems.
  • N-acetyl-beta-hexosaminidase (HEX) is a lysosomal exoglycosidase involved in degradation of oligosaccharide chains of glycoproteins, glycolipids and glycosaminoglycans, known as a potential tumour marker.
  • The aim of our study was to evaluate HEX A and HEX B activity in healthy and PA human salivary glands using colorimetric and isoelectrofocusing methods.
  • METHODS: PA (n=8) and macroscopically unchanged salivary glands (n=8), served as controls, were used for the study.
  • RESULTS: Total activity of HEX, as well as HEX A and HEX B, in PA specimens determined by a colorimetric method was significantly higher compared with normal human salivary gland specimens.
  • After isoelectrofocusing, in normal human salivary and PA glands, two sets of HEX isoforms were found corresponding to HEX A and HEX B.
  • CONCLUSIONS: The present data indicate that HEX activity and activity of its isoenzymes in tumour specimens is significantly and consistently elevated, and thus suggest the need for further studies on the degradation of glycoconjugates, both in healthy salivary glands and PA.
  • It appears that HEX may be considered as a new tumour marker in these salivary gland diseases.

  • Non-sebaceous lymphadenoma (NSL) is a rare, recently described, benign salivary gland tumor characterized by a dense lymphoid infiltrate and absence of sebaceous differentiation.
  • The patient was a 50-year-old woman presenting with a slowly growing painless mass in the right parotid gland.
  • The tumor was encapsulated and measured 3 x 2 x 2 cm.
  • Microscopically, the tumor comprised islands of epithelial cells with centrally located duct-like structures within a dense lymphoid stroma.
  • Immunohistochemically, the tumor regularly expressed CKs 7, 8/18, and 19, which are typical for columnar differentiation and CKs 17 and 5/6, which are most typically expressed in basal cells of complex epithelia.

  • Although infarction of parotid gland pleomorphic adenoma (PA) following fine-needle aspiration (FNA) has been well-documented, spontaneous infarction of PA has remained as an uncommon entity in the literature.
  • To our knowledge, we report the second case of spontaneous infarction occurring in a parotid gland PA.
  • Although appears to be rare, spontaneous infarction of PA should be considered in the differential diagnosis, since necrosis may mimic carcinoma and cause misinterpretation of necrosis as an indication of malignancy, in an otherwise benign salivary gland neoplasm.

  • [Title] Endoscopic endonasal surgery for resection of benign sinonasal tumors: experience with 105 patients.
  • OBJECTIVE: To report our experience with endoscopic surgery of benign tumors and tumor-like conditions of the nose and paranasal sinuses.
  • METHODS: The medical records of 105 patients with benign sinonasal tumor, who had undergone endoscopic surgery for removal of their neoplasm between 1997 and 2003, were retrospectively studied.
  • RESULTS: The studied patients included 32 with juvenile nasopharyngeal angiofibroma, 24 with inverted papilloma, 23 with benign fibroosseous lesions, 18 with pyogenic granuloma, 6 with intranasal hemangioma, and 2 with pleomorphic adenoma.
  • Eight cases of recurrent tumor--2 angiofibroma, 4 inverted papilloma, 1 fibrous dysplasia, and 1 osteoma--were noted and managed endoscopically alone or in combination with Caldwell-Luc approach.
  • CONCLUSION: Endoscopic surgery is a valuable tool for removal of benign tumors of the nose and paranasal sinuses in the hands of an experienced surgeon.

  • The unusual case is described of a benign parotid gland neoplasm with intermingled sebaceous and lymphoid tissue, synchronous to breast cancer.
  • In the past, the patient had undergone a simple surgical procedure for a cystic parotid gland lesion in that same gland.
  • Secondary neoplasms have only occasionally been reported, since there are few cases for corroborating the strong correlation between salivary neoplasms and other carcinomas as in Muir-Torre syndrome; the previous cystic lesion showed the origin of the neoplasm from a sebaceous inclusion in the lymph node as a postulate of Warthin tumour.

  • We herein report an unusual case of a low-grade myxoid renal epithelial neoplasm, with peculiar and previously unreported morphologic and immunohistochemical features.
  • The lesion morphologically resembled a pleomorphic adenoma of the salivary gland.
  • The tumor proved benign at follow-up.
  • A definitive classification and histogenetic interpretation of this previously unreported tumor type awaits description of further cases showing similar features which, perhaps, as it may happen, went so far unnoticed.

  • OBJECTIVE: This retrospective study aimed to investigate the types and distribution of neoplasm of salivary glands in a Turkish population.
  • STUDY DESIGN: The histological diagnosis records of the Department of Pathology at Cumhuriyet University were reevaluated for 125 patients who were treated for salivary gland tumors from 1987 to 2008.
  • The neoplasms were analyzed for histological diagnosis, age, sex, and site.
  • RESULTS: A total of 125 primary salivary gland neoplasms, consisting of 95 (76%) benign and 30 (24%) malignant groups were recorded.
  • The most common major and minor salivary gland sites were the parotid (61.6%) and palatal glands (9.6%), respectively.
  • Pleomorphic adenoma was the most frequent benign tumor followed by Warthin's tumor.
  • CONCLUSIONS: Although there were some discrepancies, the characteristics of salivary gland tumors of Turkish patients are in line with those of patients from other countries according to tumor type, tumor site distribution, and age and sex of patients.

  • A case of slow-enlarging mass of the parotid region in a 44-year-old male is presented, which proved to be a lipomatous tumour of the parotid gland.

  • This report presents a case of parapharyngeal oncocytoma of the left parotid gland in 77-year-old patient, treated in 2nd ENT Department of Medical University of Silesia in Zabrze.
  • On the basis of physical examination and preoperative radiologic studies (computed tomography scan of nasopharynx, magnetic resonance imaging, Doppler ultrasonography of carotid and vertebral arteries) the extensiveness of the tumour was determined and the surgical procedure was undertaken.
  • The removal of a whole tumour was performed through a cervical--transpharyngeal approach with midline mandibulotomy.

  • THE AIM OF THE STUDY: Tumors of the salivary glands represent about 3% of head and neck neoplasms.
  • Most of theme--up to 80%--originate from the parotid gland.
  • The aim of the study is to present current epidemiological data, most frequent complains, our surgical technique, histopathological diagnosis, complications rate and follow-up.
  • All of the patients had ultrasound examination prior to operation to estimate the spread and the type of the tumor.
  • RESULTS: The frequency of incidence of benign tumours was 90% and the most frequent histopathological diagnosis was pleomorphic adenoma (49%) and adenolymphoma (42%).
  • The most frequent complication was sensory deficit about the cheek and ear (35%), temporary facial nerve paresis (5%), hematoma (1%), salivary fistula (1%).
  • CONCLUSION: Superficial and total parotidectomy are safe procedures of treatment in case of salivary gland tumors.

  • AIM: To report the unusual endodontic presentation and radiographic features of a subgroup of pleomorphic adenoma called carcinoma ex pleomorphic adenoma and to stress the importance of maintaining a high level of suspicion in cases where primary or recurrent neoplasia is included in the differential diagnosis.
  • SUMMARY: This paper describes a case in which a patient with a previous history of malignant neoplasm presented with signs and symptoms similar to a dental infection.
  • The pathology report however confirmed recurrence of the previous tumour in a malignant fashion.
  • KEY LEARNING POINTS: *Pleomorphic adenomas, also known as benign mixed tumours, are common salivary gland tumours, which infrequently undergo malignant transformation, with potentially devastating consequences.
  • *Malignant salivary gland tumours can present as dental swelling, dental pain and sudden loss of vitality of teeth so both general practitioners and specialists have the responsibility to evaluate such patients with a broad vision.

  • Sialolipoma is a rare benign neoplasm characterized by a well-circumscribed mass composed of neoplastic mature adipose tissue and non-neoplastic salivary gland elements.
  • Microscopically, the tumor was well-circumscribed consisting of lobular proliferation of the lipomatous tissue with thin fibrous tissue septa containing clustered salivary gland elements.
  • The definitive diagnosis was sialolipoma.
  • Of these 35 cases, 16 cases were located in minor salivary glands.
  • The age distribution was from 27 to 84 years (mean, 61.6 years) and the tumor size ranged from 0.9 to 4 cm (mean, 1.7 cm).

  • INTRODUCTION: The objective of this study is to estimate the incidence of major salivary gland tumors in a geographically well-defined area (the County of Funen, Denmark) and to analyse the demographic data and distribution of histological types.
  • MATERIALS AND METHODS: Patients with salivary gland tumors were identified through a search on pathology and diagnosis codes in the period 1984-2003, which initially resulted in a group of 951 patients.
  • RESULTS: The incidence of major salivary gland tumors for the County of Funen is calculated to 7.3 per 100,000 residents per year.
  • 90% of the tumors were found in the parotid gland - 93% benign and 7% malignant.
  • 10% were located in the submandibular gland - 73% benign and 27% malignant.
  • Only one neoplasm was located in the sublingual gland and it was malignant (100%).
  • CONCLUSION: The incidence of major salivary gland tumors in the County of Funen is calculated to 7.3 per 100,000 inhabitants per year which is equivalent to 383 new cases in Denmark per year.

  • OBJECTIVE: To detect the levels of carcinoembryonic antigen (CEA) and carcinoma associated antigen CA-50 in the patients with oral or salivary malignant tumors.
  • METHODS: The concentrations of salivary CEA and CA-50 were assayed in 80 patients of oral and salivary malignant tumors, 40 patients of benign tumors and 80 health controls.
  • RESULTS: Salivary CEA and CA-50 levels were significantly higher in malignant tumors than in benign tumors and in health controls, respectively (P < 0.001).
  • This may be more useful as prognostic indicators in early diagnosis of oral and salivary malignant tumors.

  • [Title] Küttner's tumor of the submandibular glands: report of five cases with fine-needle aspiration cytology.
  • Küttner's tumor (KT) is a benign tumor-like lesion of the salivary gland that mimics neoplasm clinically because of presentation as a hard mass.
  • Recently, the histomorphological and immunohistochemical findings of this lesion have been analyzed, and differential diagnostic problems relating to salivary gland lymphoma have been discussed.
  • FNAC findings of scattered ductal structures surrounded by collagens and infiltrated by a mixed population of lymphoid cells, not specific for KT, are highly suggestive of the diagnosis with the appropriate clinical findings.
  • However, a portion of cytological specimens of KT containing relatively large numbers of lymphoid cells should be differentiated from malignant lymphoma arising from the submandibular gland.

  • BACKGROUND: CA is an uncommon benign salivary gland tumor occurring almost exclusively in the intraoral glands.
  • This tumor has often been referred to as a variant of the basal cell adenoma.
  • However, the World Health Organization's latest histological classification of salivary gland tumors recognizes it as a separate entity under the broader heading of monomorphic adenoma, which is not related to any of the subtypes of basal cell adenomas.
  • Microscopic examination confirmed the final diagnosis of CA.
  • The patient is currently free of disease 54 months after surgical excision of the tumor.
  • SUMMARY: Local excision of symptomatic nodules seems to be sufficient to manage patients with multifocal CA.10 But, unlike other benign tumors, CA may need a longer follow-up due to its tendency towards multifocal occurrence and late recurrence.

  • They appear clinically as solid mass located beneath intact surface epithelium, thus quite similar with benign neoplasm.
  • The correct aetiology of this disorder is still unknown.
  • Out of an initially incisional biopsy, only the benign part of the lesion was diagnosed.
  • Definitive histological examination of the whole tumour revealed a small malignant fraction of the specimen besides a major part of benign tissue formations (PA).

  • [Title] Sialolipoma of the parotid gland.
  • The clinical impression was that of a benign salivary gland tumor.
  • The tumor was situated in the superficial lobe of the gland, and a superficial parotidectomy was performed, with preservation of the facial nerve.
  • Histopathologic examination results revealed a sialolipoma of the parotid gland and a lesion that consisted of both mature adipose tissue and glandular elements.
  • Sialolipomas share similar clinical features with conventional lipomas of the salivary glands.
  • Preoperative diagnosis is generally difficult, and computed tomographic scanning is useful in defining these benign parotid gland masses.
  • Superficial parotidectomy is the usual surgical treatment for parotid gland superficial lobe lipomas, with near-total absence of recurrence.

  • The OMF Pathology Department Registry was searched for cases coded from 1990 to the present as "SEF," "fibrosarcoma not otherwise specified" or "neoplasm of uncertain histiogenesis."
  • Tumor location included the infra-temporal fossa, buccal mucosa (recurrence extending into bone), anterior mandible (intraosseous primary, focally extending into soft tissue), and left parotid and submandibular gland (with metaplastic bone) regions.
  • Tumor sizes ranged from 1.0 to 5.7 cm, median 3.5 cm.
  • The spindled to primarily epithelioid tumor cells formed moderately cellular sheets and cords of irregularly contoured medium to large, round to oval, occasionally overlapping nuclei, indistinct nucleoli, wispy eosinophilic (retracting) cytoplasm, and distinctive cytoplasmic borders, embedded in osteoid-like stroma.
  • Immunohistochemically, the tumor cells were positive for vimentin, 1 case focally for CD34, whereas all cases were negative for S100 protein, keratins, EMA, desmin, and SMA.
  • The differential diagnosis for these tumors in this site includes sclerosing carcinoma, Ewing/PNET, osteosarcoma, osteoblastoma, and benign and malignant myoepithelial salivary gland tumors.
  • The collagen, focal spindle cell features, HPC-like vasculature, and weak focal CD34 reactivity in one case might have raised a possible relationship between OMFRSEF and low grade malignant solitary fibrous tumor, but the intraosseous propensity, epithelioid features and relative lack of CD34 make this a distinctive entity.

  • BACKGROUND: Recent studies have proposed that minichromosome maintenance (Mcm) proteins may be sensitive proliferation markers and may serve as novel biomarkers for prognostication and diagnosis of various pre-malignant and malignant lesions.
  • The aims of this study were to determine the expression of Mcm-2, Ki-67 and geminin in salivary gland (SG) tumours, and to evaluate their usefulness for diagnosis or for prediction of tumour behaviour.
  • LI did not correlate to tumour grade or outcome for any of the markers or tumour types.
  • CONCLUSIONS: The findings suggest that Mcm-2 may be a sensitive proliferation marker in SG tumours and may be useful for differential diagnosis between PA and CEPA, and ACC and PLGA.

  • However it can sometimes arise from minor salivary glands that are distributed throughout the upper aerodigestive tract and paranasal sinuses.

  • Myoepithelioma is a rare tumor, most frequently located in the salivary gland.
  • Case reports of extra-salivary myoepithelioma are sporadic, with only one case in the nasal cavity.
  • Histological examination revealed myoepithelial cells and myxoid stroma, which confirmed the diagnosis of myoepithelioma.
  • The behavioral pattern is similar to if not identical with that of the mixed tumor, pleomorphic adenoma, which is benign, but the potential for recurrence is always present, especially if complete resection is not achieved.

  • Intraductal papilloma is a rare benign salivary gland tumor.
  • This lesion is commonly observed in the duct of the minor salivary gland, predominantly in lip and buccal mucosa, but the case in the sublingual region is quite rare.
  • This report shows a first case of intraductal papilloma developed in minor salivary gland of sublingual region.
  • The excised specimen was histologically diagnosed as intraductal papilloma of minor salivary gland, according to the microscopic finding of the papillary growth of ductal epithelium into the ductal space with fibrovascular core.

  • Papillary cystadenoma lymphomatosum is a benign salivary gland tumor most frequently located in the parotid gland (Warthin's tumor).
  • Its presentation in other major, or in minor, salivary glands is rare.
  • Clinically, it manifests as a slow growing tumor, fluctuant on palpation due to its cystic morphology.
  • The treatment of choice is complete excision with wide tumor-free margins.
  • We present a 73-year-old female patient with an asymptomatic tumor of 8 years evolution in the right posterior area of the hard palate.
  • The lesion was re-operated, performing the excision with CO2 laser and including the periosteum to ensure complete resection of the tumor.

  • Lymphadenomas (sebaceous and nonsebaceous types) of the salivary glands are extremely uncommon benign neoplasms.
  • There are rare published reports of cytopathologic characteristics of "nonsebaceous lymphadenomas" of the parotid gland.
  • An ultrasound-guided fine-needle aspiration revealed a uniform population of cohesive basaloid-type cells associated with scant myxoid stroma and was interpreted as "epithelioid neoplasm with basaloid features."
  • The rarity of this neoplasm and its superficial resemblance to more common salivary gland neoplasms may present diagnostic issues on FNA.

  • Cystadenocarcinoma (papillary cystadenocarcinoma) of the salivary gland is a rare malignant neoplasm.
  • Major locations of this neoplasm are the parotid gland, the sublingual gland, and minor salivary glands, while occurrence in the submandibular gland is extremely rare.
  • As far as we know, only one report, written in the French language, has been published concerning cystadenocarcinoma of the submandibular gland, but no report is available in the English language.
  • In this report, we describe a case of cystadenocarcinoma arising from the submandibular gland of a 54-year-old male patient.
  • The neoplasm is a low-grade carcinoma, and pre-operative examination may not show typical characteristics of malignant neoplasms.
  • Therefore, as was true in this case, the differential diagnosis from benign lesions is sometimes difficult.
  • This is the first report on cystadenocarcinoma of the submandibular gland in the English language and the first to show a computed tomography (CT) scan and magnetic resonance imaging (MRI) of this neoplasm in the submandibular gland.
