  • Oncocytic schneiderian papilloma (OSP), also known as cylindrical cell papilloma, is a rare benign sinonasal neoplasm.
  • To the best of our knowledge, there is no documentation of OSP associated with recurrent chronic paranasal sinusitis (CPS).
  • We hereby report an unusual case of OSP in a 28 year-old man which was first diagnosed and treated as recurrent chronic paranasal sinusitis.
  • After three surgeries, OSP was revealed in the pathological findings and the tumor was completely resected endoscopically in the fourth operation.
  • Nevertheless, sufficient alertness from both the surgeon and pathologist is the cornerstone for the proper diagnosis and appropriate treatment of this rare nasal neoplasm.

  • The secondary acquired obstructions may result from infection, inflammation, neoplasm, trauma or mechanical causes.
  • The maxillary sinus cysts usually obstruct the nasolacrimal duct mechanically.
  • These cysts are benign odontogenic cysts which are associated with the crowns of unerupted teeth.
  • The clinical documentations of mechanical nasolacrimal duct obstructions due to a dentigerous cyst in the maxillary sinus are very rare in literature.
  • In this case report, we describe a dentigerous cyst with a supernumerary tooth in the maxillary sinus in an 11-year-old male child causing an obstruction to the nasolacrimal duct.

  • Although benign, it is often locally aggressive and can erode into surrounding tissues and structures resulting in significant morbidity and mortality.
  • In this paper, we report on the total excision of a large, recurrent JNA with intracranial extension into the middle cranial fossa encroaching into the cavernous sinus, by right temporal craniotomy and extended osteoplastic maxillotomy.

  • INTRODUCTION: Inverted papillomas are benign sinonasal lesions that arise primarily from the lateral nasal wall which are characterized by their tendency to recur and propensity to be associated with malignancy.
  • The patients were studied by age, gender, site of presentation of the tumor, symptoms, radiologic studies, surgical treatment and evolution.
  • The most common symptom was unilateral nasal obstruction in 91% of the cases, followed by rhinorrhea in 46%.
  • The most frequent location was the ethmoid region in 51% of cases, followed by the maxillary sinus in 28% and the lateral nasal wall in 21% of the cases.

  • Inverted papilloma is locally aggressive tumour which typically presents as a unilateral nasal polyp.
  • Whilst it has a malignant potential it rarely transforms and in its benign form the main concern originates from its locally aggressive nature and substantial propensity to recur.
  • Bilateral disease can also be due to inverted papilloma, sometimes due to direct extension of the tumour, but it can also occur as two distinct lesions.
  • Here we report two cases of bilateral involvement, review the current literature and highlight some important issues on the management and follow-up of this well known neoplasm.

  • PURPOSE OF REVIEW: Endoscopic sinus surgery has become widely accepted as the standard of treatment for chronic inflammatory diseases of the paranasal sinuses unresponsive to medical treatment.
  • With increased skill with endoscopic surgical technique, advanced technologies such as intraoperative imaging systems, and a better understanding of the complex anatomy of the paranasal sinuses and surrounding vital structures, many otolaryngologists have increasingly applied their expertise in endoscopic sinus surgery to the resection of nasal and sinus neoplasms.
  • The following represents a review of the recent literature on the latest trends regarding endoscopic resection of nasal and paranasal sinus neoplasms.
  • RECENT FINDINGS: There has been an increasing popularity in the removal of nasal and paranasal sinus neoplasms through an endoscopic approach.
  • In the recent literature, emphasis has been on the endoscopic surgery of benign tumors, especially inverted papilloma and nasopharyngeal angiofibroma.
  • Other benign neoplasms have also been reported, but only in small case reports.
  • Especially for the more common benign neoplasms, such as inverted papilloma and early stage angiofibroma.
  • Large tumor size, intracranial or orbital extension, and extensive frontal or infratemporal fossa involvement are relative, but not absolute limitations.

  • Indeed, its anatomic opening (ostium) is strictly related to a complex ethmoidal structure prechamber mainly composed of the frontal recess.This constitutional feature makes the endoscopic approach more complex in comparison with other major sinuses treatment.In the following work, we present a systematization of surgical approach in relation to different pathologies, analyzing differences and results throughout the comparison of 2 groups: one treated with the endoscopic approach, and the other with open surgery.From these observations, we can assess that surgical approach choice must consider several parameters such as neoplasm localization, extension, dimension, and frontal recess anatomic features.

  • [Title] Removal of large benign cephalonasal tumours by transbasal surgery combined with endonasal endoscopic sinus surgery and neuronavigation.
  • OBJECTIVE: A retrospective study has been made of the transbasal and endonasal endoscopic sinus surgery of 29 cases with cephalonasal tumours treated in this hospital since 2001 in order to evaluate their clinical outcome.
  • The tumours were located in the anterior or middle skull base, the paranasal sinus, and/or the nasal cavity.
  • All patients underwent surgical resection via the transbasal surgery technique before endonasal endoscopic sinus surgery was performed.
  • In 20 cases neuronavigation was used in addition to locate the tumour and adjoining structures.
  • RESULTS: Twenty-seven cases had a complete removal of the tumour whilst the others had only a subtotal resection.
  • CONCLUSION: Transbasal surgery combined with endonasal endoscopic sinus surgery and neuronavigation is a particularly useful technique for resecting large benign cephalonasal tumours.

  • INTRODUCTION: Papilloma of the nose and paranasal sinuses is a benign tumor originated from nose mucosa.
  • Especially inverted papilloma tumor has a significant recurrence and malignancy potential rate.
  • The aim of the study was the analysis of clinical and treatment outcomes of patients with papilloma of the nose and paranasal sinuses.
  • MATERIAL AND METHODS: The retrospective analysis was curried out on 41 patients--16 with papiloma of the nasal vestibule and 25 with inverted papilloma the nose and paranasal sinuses surgically treated in I ENT Clinic Medical University in Lodz between 1998-2004 years.
  • RESULTS: The most frequent complains was increasing unilateral nasal obstruction and rhinorhea.
  • Nasal vestibule papilloma were intranasal removed in all cases.
  • In extended tumor nose and paranasal sinuses in 14 cases intranasal procedures, in 7 sublabial approached, in 4 lateral rhynothomy were performed.
  • CONCLUSION: The choice of surgical management should be individual with respect to tumor localization and extension of neoplasmatic process.
  • The treatment result depends of radical tumor resection.

  • RESULTS: Inverted papilloma is a benign, locally aggressive neoplasm that arises in the nasal cavity and is associated with squamous cell carcinoma in approximately 5% of patients.
  • Squamous cell carcinoma may be present with inverted papilloma at the initial diagnosis or it may occur metachronously after prior treatment.

  • Tumors of paranasal sinuses present a small percent of tumors detected in the region of head and neck 3-4% and they present a fraction of a percent of tumors in general /0.16%/.
  • MATERIAL: In the years 2004 and 2005, the National Register of Neoplasms recorded 120 and 132 cases of malignant tumors of paranasal sinuses, respectively.
  • Between 2006-2007 we treated 15 patients with tumors of paranasal sinuses underwent treatment at the ENT department of the MiEdzyleski Szpital Specjalistyczny in Warsaw.
  • The following were analyzed: the advancement stage of the tumor, symptoms, the time that has elapsed from the first appearance until the admission for treatment at the department, the methods of treatment, and the histological differentiation of neoplasms.
  • THE RESULTS: We detected the following:--benign neoplasms: 2 cases of angiofibroma, 3 cases of inverted papilloma;--malignant tumors: 4 cases of Ca planoepitheliale, 1 instance of adenocarcinoma, 1 instance of Ca anaplasticum, 1--melanoma, 1--esthesioneuroblastoma, 1--lymphoma malignum, 1--metastatic tumor.
  • THE CONCLUSIONS: at the moment of the admission for treatment at the ENT department, the advancement stage of tumor T3/T4, from the first signs until the proper diagnosis was that marked with the passage of eight months.
  • Ca planoepitheliale is the most common type of malignant tumors of paranasal sinuses.
  • It is only good cooperation between the surgeon and the pathologist which makes it possible to put the proper diagnosis.

  • BACKGROUND: Tumours of the nose and paranasal sinuses in sub-Saharan Africa are generally characterised by late presentation posing management challenges to the otorhinolaryngologists in the sub-region.
  • OBJECTIVES: To appraise surgical considerations in the management of tumours of the nose and paranasal sinuses in a developing nation from the experiences of an Ear, Nose, and Throat Department of a Nigerian Teaching Hospital.
  • METHODS: This was a chart review of patients with tumours of the nose and paranasal sinuses who underwent various surgical operations over a period of four and a half years.The patients were referred by other physicians or health care workers.
  • In 46(83.6%) patients the tumours had extended beyond the nasal cavity or one sinus cavity to contiguous structures.
  • The surgical operations carried out were lateral rhinotomy in 37(57.8%), intranasal clearance and intranasal antrostomy in 16(25%), total maxillectomy in seven(11%), and anterior craniofacial resection in two (3.1%) patients.The histological examination showed that 43 (78.2%) cases were benign, while 12 (21.8%) cases were malignant.
  • CONCLUSION: Majority of the cases were characterised by late presentation, requiring surgical approaches such as lateral rhinotomy, total maxillectomy or craniofacial resection depending on the extent of the tumour.
  • Poverty, ignorance, inadequate funding and administration challenges of health resources are factors militating against proper clinical management of tumours of the nose and paranasal sinuses in developing nations.
  • Provision of free medical care and modern facilities for early diagnosis, treatment, and health education are needed to reverse the trend.

  • AIM: Craniofacial resection provides multidirectional approaches to remove nasal and paranasal tumours that involve the skull base.
  • An en bloc excision of the tumour can be accomplished.
  • MATERIAL AND METHODS: The medical records of 40 consecutive patients who had undergone craniofacial resection for tumours of the nasal cavity, paranasal sinuses, and adjacent areas were reviewed.
  • RESULTS: Lesions were malignant in 7 patients and benign in the remaining 33.

  • BACKGROUND: The use of endoscopic surgery for nasal and sinus pathologies has revolutionized our approaches towards the diseases of these areas.
  • OBJECTIVE: To report our experience with endoscopic surgery of benign tumors and tumor-like conditions of the nose and paranasal sinuses.
  • METHODS: The medical records of 105 patients with benign sinonasal tumor, who had undergone endoscopic surgery for removal of their neoplasm between 1997 and 2003, were retrospectively studied.
  • RESULTS: The studied patients included 32 with juvenile nasopharyngeal angiofibroma, 24 with inverted papilloma, 23 with benign fibroosseous lesions, 18 with pyogenic granuloma, 6 with intranasal hemangioma, and 2 with pleomorphic adenoma.
  • Eight cases of recurrent tumor--2 angiofibroma, 4 inverted papilloma, 1 fibrous dysplasia, and 1 osteoma--were noted and managed endoscopically alone or in combination with Caldwell-Luc approach.
  • CONCLUSION: Endoscopic surgery is a valuable tool for removal of benign tumors of the nose and paranasal sinuses in the hands of an experienced surgeon.

  • We conducted a retrospective case-series review to identify the various diagnoses of neoplasms of the nasal cavity and paranasal sinuses in a pediatric population.
  • All patients had been diagnosed with a tumor of the nasal cavity or paranasal sinuses between Jan.
  • Lesions included adnexal neoplasm, ameloblastic fibro-odontoma, basal cell carcinoma, benign fibrous histiocytoma, blue nevus, chondrosarcoma, compound nevus, epithelioma adenoides cysticum, esthesioneuroblastoma, Ewing sarcoma, fibrosarcoma, giant cell granuloma, granulocytic sarcoma, hemangioma, hemangiopericytoma, Langerhans cell histiocytosis, lymphangioma, lymphoma, melanoma, neuroblastoma, neurofibroma, ossifying osteofibroma, osteochondroma, osteosarcoma, port wine stain, rhabdomyosarcoma, Spitz nevus, and xanthogranuloma.
  • We believe that the large size of this study and the data on disease incidence will allow clinicians to be better informed of the differential diagnosis of neoplasms of the nasal cavity and paranasal sinuses in the pediatric population.

  • Sinonasal teratocarcinosarcoma (SNTCS) is a rare malignant neoplasm with 63 reported cases to date.
  • Microscopic examination revealed admixed epithelial and mesenchymal elements in all 3 cases; benign squamous and glandular epithelium and neuroepithelial tissue were identified, the squamous epithelium demonstrating "fetal-like" cytoplasmic clearing.
  • Our findings suggest that 12p amplification, if it occurs at all in this setting, is exceptional and that SNTCS is a somatic-type neoplasm exhibiting divergent differentiation rather than a germ cell tumor.

  • INTRODUCTION: Juvenile nasopharyngeal angiofibroma is a benign lesion which is characterized by three distinctive features: it occurs only in one sex, one period of age and in one location in the organism.
  • All of them accomplished patomorphological and clinical criteria of this neoplasm, beside of age and sex.
  • RESULTS: Clinical and radiological investigation revealed tumor in typical location - posterior part of nasal cavity and nasopharynx in all patients, with it's extension to paranasal sinuses in 2 cases.
  • All patients were treated surgically by different approaches according to tumor's extension.
  • CONCLUSION: Our study concerning the occurrence of juvenile angiofibroma in adults, also in female, as the other observations from literature may constitute some remarks in discussion about the pathogenesis and clinics of this tumor.

  • Patients 15 years of age or under who presented with a tumour of the otolaryngological region to the department of Otolaryngology were included in this study.
  • RESULTS: Forty-three patients of above age group with a neoplasm of the otolaryngological region were managed in our department, the incidence was 0.5%.
  • The ratio of benign to malignant lesion was 7.6:1.
  • Juvenile nasopharyngeal angiofibroma was the commonest tumour (11 cases, 26%) and embryonal rhabdomyosarcoma was the commonest malignant tumour (3 cases, 7%).
  • The commonest site of neoplasm was the nose and paranasal sinuses (13 cases, 30%).
  • CONCLUSION: A tumour in the otolaryngological site in the paediatric population is rare, the incidence being 1 in 200 new cases in the age group of 15 years or under, 12% of the tumours were malignant.

  • INTRODUCTION: Sino-nasal inverted papilloma is a rare benign tumor of the nasal fossa and the paranasal sinuses for which the successful surgical treatment by an endonasal approach has been corroborated in the literature.
  • MATERIAL AND METHODS: 42 patients presenting with a sino-nasal inverted papilloma and treated surgically over a ten year period were included in this retrospective study.
  • Fourteen patients were operated using a combined approach, associating either a vestibular (9 cases), a paralateral nasal (3 cases) or a fronto-orbital (2 cases) approach with endonsal endoscopy.
  • CONCLUSIONS: Endoscopic endonasal surgery is a validated surgical approach for the treatment of sino-nasal inverted papilloma in that total tumor removal is possible.

  • In this study, our objective was to retrospectively review the results of surgical treatment of IP.Sixty-eight patients with a histopathologically proven diagnosis of IP were retrospectively evaluated.
  • Ten patients were operated on for revision surgery, and 56 eventually had a primary tumor.
  • Patients operated on for primary tumor were included in this study.
  • Nasal obstruction was the most frequent symptom observed in 50 patients (89%).
  • The origin of the tumor was most common at the maxillary sinus, the lamina papyracea, and the ethmoid sinus.
  • The type of surgery should be determined according to the tumor stage.

  • The tumour involved the nasopharynx in all the cases (6/6), the sphenoid sinus (3/6) and the medial part of the pterygopalatine fossa (4/6).
  • One patient had a recurrence in the nasopharynx and sphenoid sinuses requiring a successful revision procedure 3 years after the primary surgery.
  • Minimal intracranial extension is not an absolute contraindication if there is no clinical or radiological involvement of the cavernous sinus.

  • MATERIALS AND METHOD: 44 surgical specimens were analyzed in two groups: A= 33 benign and B= 11 degenerated.

  • RESULTS: In the histological examination in 4 patients the benign neoplasm and in 22 patients malignant tumors were diagnosed.
  • On the basis of the CT and MRI examination as well as the description of the surgical procedure we stated that in 13 cases the primary localization of neoplasm was the maxillary sinus, in 5 cases ethmoidal cells, in 3 nasal cavity.
  • In one patient the estimation of primary tumor localization was not possible, because of the very large extension of the neoplasm.
  • The choice of the surgical procedure was depend on the tumor extension and localization.
  • In 8 patients with the infiltration of lateral nasal wall the medial maxillectomy were performed.

  • INTRODUCTION: Inverted papillomas are relatively rare, benign epithelial tumours of the nasal cavity which generate considerable interest because they are locally aggressive, have a tendency to recur and are associated with malignancy.
  • Fifty-eight patients initially underwent nasal biopsy, often with polypectomy.
  • The recurrence rates were 12.8 per cent for endoscopic procedures, 17.0 per cent for lateral rhinotomy with medial maxillectomy, and 34.2 per cent for limited resections such as nasal polypectomy and Caldwell-Luc approaches.

  • PURPOSE OF REVIEW: Inverted papillomas are uncommon benign tumors that may occur in the sinonasal tract.
  • At the same time, new information relevant to the diagnosis and surveillance of inverted papillomas has been published, regarding the role of imaging, serum tumor markers, and human papillomavirus.
  • In most tumor locations, the completeness of resection achieved by a skilled endoscopic surgeon is equivalent, if not superior, to that attained with an open approach.
  • In some instances, an endoscopic exploration with tumor resection may help define the site of tumor attachment and direct adjunctive open procedures when indicated.

  • MATERIAL AND METHODS: We analyzed the histopathological characteristics of 39 cases of IP using the following parameters: site of origin of tumor; presence of bone invasion; presence of inflammatory polyp; ratio of endophytic to exophytic lesions; ratio of neoplastic epithelium to stroma; presence of hyperkeratosis; presence of squamous epithelial hyperplasia; mitotic index; number of mucin globules; and number of eosinophils.
  • Multiple recurrences (without malignancy) were related to frontal sinus origin (p=0.042), increase in hyperkeratosis (p=0.041), the presence of squamous epithelial hyperplasia (p=0.044) and increase in the mitotic index (p=0.031).
  • Clinically benign behavior was related to the presence of inflammatory polyp (p=0.010) and the absence of hyperkeratosis (p=0.008).

  • Schwannoma is a benign neoplasm originating from schwann cells of the peripheral nerve sheath, and its occurrence in the nasal cavity and paranasal sinuses is rare.
  • We present a case of nasal schwannoma originating from the left nasal cavity, accompanied by bilateral nasal polyposis.
  • A 66-year-old man presented with complaints of progressive left nasal obstruction, hyposmia, and headache.
  • Anterior rhinoscopic and endoscopic examinations revealed a mass that almost completely filled the left nasal cavity.
  • A polypoid mass was present in the right nasal cavity, as well.
  • On computed tomography, the mass occupied the left ethmoidal cells, left maxillary sinus, left sphenoid sinus, and posterior area of the left nasal cavity.
  • Endoscopic sinus surgery was performed under general anesthesia and the mass was completely removed via the nasopharynx because of its size.
  • Endoscopic anterior ethmoidectomy was also performed in the right nasal cavity for nasal polyposis.

  • BACKGROUND: The aim of the present study was to establish the efficacy of endonasal micro-endoscopic surgery for removal of benign and malignant neoplastic lesions of the paranasal sinuses and the anterior skull base.
  • MATERIAL AND METHODS: This retrospective study evaluated 350 patients with tumors of the paranasal sinuses and the anterior skull base (215 benign and 135 malignant tumors) that were treated surgically at the ENT-Department of the Hospital Fulda gAG between 1993 and 2003.
  • RESULTS: Most frequent entities of benign tumors were osteomas, inverted papillomas and juvenile angiofibromas.
  • 54 % of the benign (n = 118) and 41 % of the malignant tumors (n = 54) were resected exclusively via the endonasal micro-endoscopic approach.
  • In the malignoma group 34 patients died because of tumor disease (16 cases due to recurrencies, 18 cases due to metastases formation).
  • Disease-specific survival also showed differences dependent on histology, tumor site and occurrence, but was without proven significancy.
  • CONCLUSION: The advances in endonasal micro-endoscopic surgery also allow a safe and effective removal of benign and malignant anterior skull base tumors with low morbidity.
  • Thereby, indication is dependent on tumor site and size as well as histology.

  • BACKGROUND: Although sinonasal inverted papilloma (IP) is a rare benign tumor, it has a tendency to recur and is sometimes associated with squamous cell carcinoma (SCC).
  • We previously reported that serum SCC antigen might be a useful tumor marker for sinonasal IP.
  • None of the patients with recurrent tumor showed symptoms at the time of detection of their recurrent tumor, and recurrence was discovered from elevated levels of SCC antigen.
  • SCC antigen is a reliable tumor marker in the management of sinonasal IPs.

  • INTRODUCTION: Inverted papilloma is benign epidermal neoplasm of not recognized etiology.
  • Generally inverted papilloma is unilateral, arises from the lateral nasal wall and characterizes with local destruction and rapid growth.
  • The tumor can possibly undergo malignant transformation.
  • RESULTS: Most common primary localization of the tumor was combined lateral nasal wall and maxillary sinus (42%).
  • In 37% the lesion involved unilaterally the lateral nasal wall, maxillary sinus and ethmoid sinuses, rarely it was localized only in the lateral nasal wall (16.1%) and exclusively in the nasal septum (4.8%).
  • 3. Regular follow-up enables early recurrence diagnosis and treatment.

  • OBJECTIVES/HYPOTHESIS: Juvenile nasal angiogibroma (JNA) is a relatively rare, benign neoplasm generally seen in prepubertal and adolescent males.
  • Our presentation demonstrates an endoscopic approach to excision of JNAs that are limited to the nasal cavity, nasopharynx, and paranasal sinuses.
  • After medialization of the middle turbinate and anterior ethmoidectomy, the tumor could be mobilized using a Cottle elevator and microdebrider.
  • The origin and attachment of the tumor were visualized and detached from the sphenopalatine foramen.
  • The tumor was resected en bloc with minimal bleeding.
  • To date, there has been no evidence of recurrence of tumor in these individuals.

  • [Title] Tumor suppressor gene alterations in respiratory epithelial adenomatoid hamartoma (REAH): comparison to sinonasal adenocarcinoma and inflamed sinonasal mucosa.
  • Respiratory epithelial adenomatoid hamartoma (REAH) is an unusual benign sinonasal glandular proliferation.
  • REAH is not considered a neoplasm, although, no molecular evidence exists to support or refute this possibility.
  • DNA was extracted and polymerase chain reaction performed using fluorescently labeled primers flanking known tumor suppressor genes on chromosomes 9p (CDKN2/p16), 11p (H-ras), 17p (p53), and 18q (DCC/DPC4).
  • The loss of heterozygosity (LOH) ratio was calculated as the allele ratio from tumor tissue divided by the allele ratio from normal tissue.
  • Appreciable allelic loss within REAH suggests the possibility that REAH may be a benign neoplasm rather than a hamartoma.
