  • Benign tumours account for 80% of case histories with a relationship 1:4 M/F, the most frequent being pleomorphic adenoma (57.3% of cases), followed by Warthin's tumour (32.4%), this rating not having been confirmed in case histories (8-10%) in the literature.
  • Surgical treatment of benign tumours consisted, in 50% of cases, in superficial paroditectomy and in approximately 30% of total paroditectomy.
  • When no clinically evident lymph nodes were present (NO) considering the tumour histotype, two thirds of patients underwent surgery or radiotherapy, while in the remainder the wait-and-see attitude was prefered.
  • Oncological results obtained were compared with those reported in the literature: in fact for all benign neoplasms relapse ratings are about 5%, while for malignant tumours the worst prognosis was in squamous cell carcinoma with median of 37.7 on survival and metastasis rate of 16.5%.

  • OBJECTIVE: This retrospective study aimed to investigate the types and distribution of neoplasm of salivary glands in a Turkish population.
  • STUDY DESIGN: The histological diagnosis records of the Department of Pathology at Cumhuriyet University were reevaluated for 125 patients who were treated for salivary gland tumors from 1987 to 2008.
  • RESULTS: A total of 125 primary salivary gland neoplasms, consisting of 95 (76%) benign and 30 (24%) malignant groups were recorded.
  • The most common major and minor salivary gland sites were the parotid (61.6%) and palatal glands (9.6%), respectively.
  • Pleomorphic adenoma was the most frequent benign tumor followed by Warthin's tumor.
  • CONCLUSIONS: Although there were some discrepancies, the characteristics of salivary gland tumors of Turkish patients are in line with those of patients from other countries according to tumor type, tumor site distribution, and age and sex of patients.

  • BACKGROUND: Salivary gland carcinomas are relatively uncommon heterogeneous malignancies characterized by locoregional invasion and distant metastasis.
  • Topoisomerase IIalpha (topoIIalpha), located at chromosome 17q21-22, is considered a major mediator of cell proliferation and DNA replication.
  • The purpose of this study was to evaluate the expression of topoIIalpha in various types of salivary gland tumors and its biological significance.
  • METHODS: The protein expression of topoIIalpha was evaluated immunohistochemically in formalin-fixed, paraffin-embedded tissue from 54 salivary gland carcinomas and 20 benign tumors (10 pleomorphic adenomas and 10 Warthin's tumors).
  • The primary salivary gland carcinoma specimens consisted of 17 adenoid cystic carcinomas, 7 adenocarcinomas not otherwise specified, 7 mucoepidermoid carcinomas, 6 salivary duct carcinomas, 3 acinic cell carcinomas, 3 carcinomas ex pleomorphic adenomas, 3 epithelial-myoepithelial carcinomas, 2 carcinosarcomas, 2 lymphoepithelial carcinomas, 2 myoepithelial carcinomas, 1 oncocytic carcinoma, and 1 squamous cell carcinoma.
  • RESULTS: Of the 54 primary salivary gland carcinomas, 38 (70%) showed positive expression (> or = 10%) of topoIIalpha protein, and 16 carcinomas (30%) and all benign tumors were negative (p < 0.001).
  • Expression of topoIIalpha was more frequently observed in salivary duct carcinoma, carcinoma ex pleomorphic adenoma, adenocarcinoma, and adenoid cystic carcinoma, solid type, and it was associated with advanced stage and shortened survival.
  • Furthermore, it may provide useful prognostic information and suggests the potential efficacy of topoIIalpha-targeting therapy in patients with salivary gland carcinoma.

  • Most occurrences of this benign tumor in the submandibular gland have been treated surgically without difficulty via transcervical approach.
  • In this study, we introduced the intraoral approach for the removal of the submandibular mixed tumor.
  • In total, 12 cases of pleomorphic adenoma of the submandibular gland were treated via intraoral approach.
  • During surgery the tumor and submandibular gland are easily dissected from surrounding tissue and removed through intraoral incision.
  • All patients were followed up for the recurrence of the tumor from 20 months to 10 years.
  • One patient showed a tumor recurrence at the early stage of this approach.
  • We propose that the benign mixed tumor of the submandibular gland could be removed easily via intraoral route without an external scar or nerve injury.

  • INTRODUCTION: The objective of this study is to estimate the incidence of major salivary gland tumors in a geographically well-defined area (the County of Funen, Denmark) and to analyse the demographic data and distribution of histological types.
  • MATERIALS AND METHODS: Patients with salivary gland tumors were identified through a search on pathology and diagnosis codes in the period 1984-2003, which initially resulted in a group of 951 patients.
  • RESULTS: The incidence of major salivary gland tumors for the County of Funen is calculated to 7.3 per 100,000 residents per year.
  • 90% of the tumors were found in the parotid gland - 93% benign and 7% malignant.
  • 10% were located in the submandibular gland - 73% benign and 27% malignant.
  • Only one neoplasm was located in the sublingual gland and it was malignant (100%).
  • CONCLUSION: The incidence of major salivary gland tumors in the County of Funen is calculated to 7.3 per 100,000 inhabitants per year which is equivalent to 383 new cases in Denmark per year.

  • PURPOSE: Of all benign salivary gland tumors of the parotid gland, pleomorphic adenoma (mixed tumor) is the most common.
  • It accounts for 60% to 70% of all benign tumors of the parotid gland.
  • This neoplasm arises in patients in the fourth to sixth decade of life, with a female predominance.
  • The surgical excision of this lesion continues to be the subject of major debate.
  • PATIENTS AND METHODS: We reviewed 48 patients who underwent excision of pleomorphic adenoma of the parotid gland between 1996 and 2005 at Rambam Medical Center: 18 were treated surgically with the classical superficial parotidectomy (SP) technique, using an anterograde approach, and 30 were treated with retrograde partial superficial parotidectomy (PSP).

  • BACKGROUND: Epithelial salivary gland tumors are very uncommon in pediatric age.
  • These results are presented in conjunction with a literature review of salivary tumors with a view to providing an up-to-date overview of the clinical course, prognosis, and treatment options for this rare tumor.
  • PROCEDURE: Fifty-two cases of epithelial salivary tumors were reviewed and the clinical-pathological information concerning tumor characteristics, therapy, and follow-up were collected.
  • RESULTS: The major salivary glands were the main site of tumor occurrence (79% of cases arose in parotid glands); 37 patients had benign tumors (pleomorphic adenoma), 15 had malignant tumors (12 mucoepidermoid carcinoma, 9 low grade).
  • All the patients were treated by surgery; local relapses after parotidectomy were 4% and 25%, in benign and malignant tumors, respectively.
  • When tumor enucleation was performed, recurrences occurred in 50% of benign neoplasms.
  • At the time of the report, all patients with benign tumors were alive, 35(95%) without evidence of disease; only one patient with malignant tumor died of disease.
  • CONCLUSIONS: Epithelial salivary glands tumor in children had different characteristics compared with their adult counterpart with respect to the frequency of histotypes and site of occurrence, but their prognosis seems to be similar.
  • Parotidectomy (total or superficial) is the best choice for achieving good cure rates in both benign and malignant tumors.

  • OBJECTIVE: To document the pattern of salivary gland neoplasia in Maiduguri, Nigeria.
  • RESULTS: The palatal (71.9%) and parotid (78.3%) glands were the most common minor and major salivary glands involved, with a benign-malignant ratio of 1:1 and 1.4:1, respectively.
  • Pleomorphic adenoma (44.3%) was the most common salivary gland neoplasm recorded.
  • Mucoepidermoid carcinoma (10.1%) was the most common salivary gland malignancy, occurring in the second and sixth decades; of equal gender distribution and predominantly in the palate (50%).
  • The squamous cell carcinoma (10.9%) and adenoidcystic carcinoma (21.9%) were the most common malignancies in the major and minor glands respectively.
  • CONCLUSION: Pleomorphic adenoma and mucoepidermoid carcinoma were the most commonly reported benign and malignant neoplasia in this series.

  • BACKGROUND: Recently, the gene for cysteine-rich secretory protein 3 (CRISP-3) was reported to be highly upregulated in prostate cancer (PCa) compared to benign prostatic tissue.
  • RESULTS: Immunostaining for CRISP-3 in benign prostatic epithelium was generally weak or not detectable.
  • Specific and strong immunostaining was found in a major proportion of cells in high-grade prostatic-intraepithelial-neoplasia (HG-PIN,12/17 patients), in most primary tumors (111/115), and in lymph node (11/15) and bone (12/15) metastases.

  • [Title] [Carcinoma ex pleomorphic adenoma of major salivary glands--a clinicopathologic review].
  • BACKGROUND: Pleomorphic adenoma (PA) is the most common neoplasm of the major salivary glands.
  • There are three subtypes of malignant PA: carcinoma ex pleomorphic adenoma (CXPA); carcinosarcoma (true malignant mixed tumor) and metastasizing pleomorphic adenoma.
  • Own material is presented because of rarity and clinicopathologic specificity of this neoplasm.
  • The following clinical factors were evaluated: age, sex, symptoms (time of lasting, evolution), tumor size, invasion of the adjacent structures and facial nerve, neck nodes, clinical stage, treatment outcome.
  • One pathologist reviewed histological material of 18 out of 19 patients who had been operated and pathological factors such as sensitivity and accuracy of fine needle aspiration biopsy, grade, histology and proportion of malignant component in tumor, lymph nodes metastases were analyzed.
  • In 15 cases, tumor was localized in the parotid and in 4 in the submandibular gland.
  • Duration of symptoms of benign PA was from 2 to 40 years (mean 17.8 years).
  • Symptoms of malignant transformation occurred in 15 patients, the most common were rapid enlargement of tumor, pain and facial nerve palsy.
  • Tumor size varied from 2 to 20 cm with a mean of 6 cm.
  • Adjacent structures were invaded by neoplasm in 5 cases.
  • The best method of prevention and treatment of CXPA is early and radical removal of all major salivary glands tumors.

  • Major locations of this neoplasm are the parotid gland, the sublingual gland, and minor salivary glands, while occurrence in the submandibular gland is extremely rare.
  • As far as we know, only one report, written in the French language, has been published concerning cystadenocarcinoma of the submandibular gland, but no report is available in the English language.
  • In this report, we describe a case of cystadenocarcinoma arising from the submandibular gland of a 54-year-old male patient.
  • The neoplasm is a low-grade carcinoma, and pre-operative examination may not show typical characteristics of malignant neoplasms.
  • Therefore, as was true in this case, the differential diagnosis from benign lesions is sometimes difficult.
  • This is the first report on cystadenocarcinoma of the submandibular gland in the English language and the first to show a computed tomography (CT) scan and magnetic resonance imaging (MRI) of this neoplasm in the submandibular gland.

  • BACKGROUND: Sebaceous adenoma of the salivary gland is an extremely rare, benign neoplasm that predominantly involves the major salivary glands.
  • The major problem in dealing with sebaceous adenoma is the recognition of this entity to avoid confusion with other more aggressive neoplasms, such as mucoepidenmoid carcinoma.
  • CONCLUSION: We report a case ofa rare and unusual tumor of the salivary glands.
  • The major problem in dealing with sebaceous adenoma is the recognition ofthis entity in cytologic specimens to avoid confusion with other more aggressive neoplasms such as low grade mucoepidermoid carcinoma and potentially to avoid radical surgery.

  • Transoral approach is the most controversial one due to cited limited exposure, risk of tumor spillage, and possibility of neurovascular injury.
  • Majority of treated tumors were pleomorphic adenomas (14 minor salivary gland and 7 deep lobe parotid gland tumors).
  • Two cases of malignant salivary gland tumors, 4 nerve sheath tumors and 1 lymphangioma were also excised.
  • Mean tumor size was 6.1 cm (range 2-11 cm).
  • Neither major complications nor disordered healing were observed.
  • It can be combined with external approach to achieve safe resection of some benign tumors which would need transmandibular approach.

  • Carcinoma ex-pleomorphic adenoma (CXPA) is an aggressive salivary gland malignancy, usually derived from a long-standing or a recurrent benign tumor, the pleomorphic adenoma (PA).
  • In the context of dynamic reciprocity, changes in the composition and structure of extracellular matrix proteins and cell surface receptors have been frequently associated with dysfunctional adhesion and invasive behavior of tumor cells.
  • In this study, different progression stages of CXPA were investigated regarding the expression of the major extracellular matrix proteins, collagen type I, and of E-cadherin and beta-catenin, the components of adherens junctions.
  • By immunohistochemical analysis, we have demonstrated that direct contact of tumor cells with fibrillar type I collagen, particularly near the invasive front and in invasive areas prevailing small nests of CXPA cells, could be associated with reduced expression of the E-cadherin and beta-catenin adhesion molecules and with invasive behavior of epithelial, but not of CXPA with myoepithelial component.

  • [Title] Endoscopic endonasal surgery for resection of benign sinonasal tumors: experience with 105 patients.
  • OBJECTIVE: To report our experience with endoscopic surgery of benign tumors and tumor-like conditions of the nose and paranasal sinuses.
  • METHODS: The medical records of 105 patients with benign sinonasal tumor, who had undergone endoscopic surgery for removal of their neoplasm between 1997 and 2003, were retrospectively studied.
  • RESULTS: The studied patients included 32 with juvenile nasopharyngeal angiofibroma, 24 with inverted papilloma, 23 with benign fibroosseous lesions, 18 with pyogenic granuloma, 6 with intranasal hemangioma, and 2 with pleomorphic adenoma.
  • Eight cases of recurrent tumor--2 angiofibroma, 4 inverted papilloma, 1 fibrous dysplasia, and 1 osteoma--were noted and managed endoscopically alone or in combination with Caldwell-Luc approach.
  • The major complication encountered in this study was cerebrospinal fluid leakage (2 cases) that was sealed intra-operatively with local tissue flaps without any further sequela.
  • CONCLUSION: Endoscopic surgery is a valuable tool for removal of benign tumors of the nose and paranasal sinuses in the hands of an experienced surgeon.
