  • Tumours of the central nervous system (CNS) have not received much scientific attention in sub-Saharan Africa, especially in the central African zone.
  • The aim of this study was to determine the relative frequency and different histologic types of CNS tumours seen in the neurosurgery units of Cameroon, a multiethnic country of central Africa.
  • This retrospective study covers the decade from January 1996 through December 2006 in the three neurosurgery departments in Cameroon, at the Yaoundé General Hospital, the Yaoundé Central Hospital, and the Douala General Hospital.
  • INCLUSION CRITERIA: All cases undergoing surgery in these units for a histologically-confirmed CNS tumour.
  • Primary tumors were malignant in 34.2% (n=12) of the children and benign in 65.8% (n=23); among adults 22.7% (n=30) were malignant and 77.3% (n=102) benign.
  • In conclusion, CNS tumors occurred mainly before the age of 55 years and had a slight predilection for girls and women.

  • [Title] Liver grafts from donors with central nervous system tumors: a single-center perspective.
  • The aim of this study was to analyze our experience with the use of liver grafts from donors with central nervous system (CNS) tumors.
  • A retrospective review of 1173 liver transplants performed between 1992 and 2006 identified 42 donors diagnosed with a CNS tumor.
  • Thirty-two tumors were malignant, and 10 tumors were benign.
  • Twenty (47.6%) of the CNS tumors were glioblastoma multiforme (astrocytoma grade IV), 11 (26.2%) were other astrocytomas, and 1 (2.4%) was an anaplastic ependymoma.
  • Twenty (62.5%) neoplasms were grade IV tumors, 8 (25%) were grade II tumors, and 4 (12.5%) were grade III tumors.
  • The rate of recurrence for the entire group was 2.4% (all CNS tumors).
  • There was no difference in survival between recipients of grafts from donors with CNS tumors and recipients of grafts from donors without CNS tumors (1 year: 82% versus 83.3%, P = not significant; 3 years: 77.4% versus 72%, P = not significant).
  • In conclusion, in our experience, despite violation of the blood-brain barrier and high-grade CNS tumors, recurrence was uncommon.

  • Tumors of the central nervous system (CNS) represent a unique, heterogeneous population of neoplasms and include both benign and malignant tumors.
  • The present study was carried out on a total of 79 archival cases of ependymal tumors in addition to a variety of other primary CNS tumors.
  • Upon comparing with other CNS tumors (41 cases), it was found that CD99 could differentiate between ependymomas and nonependymal tumors, but intensity and pattern of staining were of no consequence in determining variant type or degree of histologic aggressiveness.
  • Thus, EMA was found to be of little value in the diagnosis of ependymoma and in the differentiation between different types and grades.
  • CD99 can hence be recommended for use as a good marker for differentiation between ependymal and other CNS tumors.
  • EMA expression and pattern of distribution, on the other hand, cannot be employed to determine the type of variant or the degree of tumor aggressiveness, and hence cannot predict the behavior of ependymal neoplasms.

  • INTRODUCTION: Ganglioneuroma is a rare, benign, neuroblastic tumor arising mainly from the central or peripheral autonomic nervous system, especially the sympathetic system.
  • A diagnosis of ganglioneuroma was suggested.
  • Core biopsy and surgical resection confirmed this diagnosis.
  • CONCLUSION: Congenital ganglioneuroma of the cervical region is an uncommon soft tissue benign neoplasm of neuroblastic origin, and it should be considered in the differential diagnosis of head and neck pediatric soft tissue tumors.

  • [Title] Hemangioblastomas of central nervous system: molecular genetic analysis and clinical management.
  • OBJECTIVE: Hemangioblastomas of the central nervous system (CNS) are benign neoplasms that may occur sporadically or in association with von Hippel-Lindau (VHL) disease.
  • We investigated the frequency of VHL germline mutation in patients with symptomatic CNS hemangioblastoma without evidence of VHL disease to define the role of molecular genetic analysis in the management of such patients and their relatives.
  • METHODS: We analyzed 14 patients (6 female and 8 male; mean age, 43.5 yr) with no family history and no other clinical manifestations of VHL disease who had been operated on for symptomatic CNS hemangioblastoma.
  • CONCLUSION: Molecular genetic analysis is a safer and more specific instrument to confirm or exclude VHL disease in patients with CNS hemangioblastoma, a negative family history, or absence of other known manifestations of the disease.
  • Nonsymptomatic family members will benefit from early VHL disease diagnosis or by being excluded as at-risk subjects, reducing the psychological and economic burden of screening and surveillance protocols.

  • OBJECTIVE: The objective of this study is to report our initial experience treating pediatric patients with central nervous system tumors using a frameless, optically guided linear accelerator.

  • Melanomas are malignant neoplasms of melanocytes developing predominantly in the skin, but occasionally arising from eyes, mucous membranes, and the central nervous system (CNS).
  • The CNS can be affected by a spectrum of melanocytic lesions ranging from diffuse neurocutaneous melanosis, to a focal and benign neoplasm (melanocytoma), and to an overtly malignant tumor (melanoma).
  • Primary melanocytic lesions involving the CNS are typically concentrated in the perimedullary and high cervical region.
  • Primary CNS melanoma cannot be reliably distinguished from metastatic melanoma on neuroimaging and histopathological characteristics alone: its diagnosis is established only after exclusion of secondary metastatic disease from a cutaneous, mucosal or retinal primary.
  • We present two patients with primary CNS melanoma and discuss relevant issues, available treatment options, and expected outcomes.
  • Awareness of disease spectrum and clinico-biological differences may be used to guide therapeutic decision-making for a patient with a proven or suspected primary CNS melanoma.

  • OBJECTIVE: To study clinicopathologic features, diagnosis, treatment and prognosis of von Hippel-Lindau (VHL) syndrome-related and sporadic hemangioblastomas of the central nervous system (CNS-HB).
  • METHODS: Histopathological, ultrastructural, immunohistochemical (EnVision method) and clinical features of 21 VHL syndrome and 63 sporadic CNS-HB cases were studied with correlation of the available follow-up information.
  • RESULTS: Twenty-one VHL patients accompanied with a total of 87 CNS-HBs, including one patient of developing 12 HBs within 13 years.
  • In the 63 cases of sporadic CNS-HB (34 male and 29 female), the mean age was 43.0 years.
  • CONCLUSIONS: VHL syndrome is a multisystem disorder with a poor prognosis and a high rate of missed diagnosis.
  • The syndrome is characterized by development of various benign and malignant tumors.
  • The most common tumor is CNS-HB, which occurs predominantly in the cerebellum.
  • Patients with VHL syndrome tend to present at a younger age than patients with sporadic CNS-HBs, and VHL related HB occurs more predominantly in the brain stem and spinal cord.
  • Prognosis of CNS-HB patients is not correlated with the nuclear atypicality, expression for Ki-67 and involvement of the brain tissue.

  • [Title] Long-term health experience of jet engine manufacturing workers: I. Mortality from central nervous system neoplasms.
  • OBJECTIVE: In response to an unusual occurrence of glioblastoma at one jet engine manufacturing facility located in North Haven (NH), Connecticut (CT), we examined mortality rates from central nervous system (CNS) neoplasms at NH and seven other company facilities.
  • RESULTS: State comparisons revealed overall deficits in deaths from all CNS neoplasms (606 deaths, SMR = 0.84, confidence interval [CI] = 0.78 to 0.91), including all malignant (462 deaths, SMR = 0.87, CI = 0.79 to 0.95), all benign (23 deaths, SMR = 0.65, CI = 0.41 to 0.98), and all unspecified (121 deaths, SMR = 0.79, CI = 0.65 to 0.94).
  • Not statistically significant excesses in deaths from all malignant brain neoplasms were found among subjects who worked only at NH (49 deaths, SMR = 1.11, CI = 0.82 to 1.47) or partly at NH (24 deaths, SMR = 1.04, CI = 0.67 to 1.55) compared with deficits in non-NH plant groups.
  • In the combined NH plant groups, we found not statistically significant higher risks of malignant brain neoplasms for salaried workers, older hires and the most recent time period, but no association with duration of employment or time since first employment.
  • CONCLUSIONS: Total cohort mortality rates for malignant, benign or unspecified CNS neoplasms were not elevated relative to the US and CT general populations.
  • The malignant brain neoplasm excesses in certain subgroups of workers from NH may reflect external occupational factors, nonoccupational factors or workplace factors unique to NH that were not measured in the current study.
  • We will explore reasons for the NH excesses and examine specific types of brain neoplasms (eg, glioblastoma) in our companion cancer incidence, case-control and exposure assessment studies.

  • Twenty-six patients (2.15%) developed a second malignant neoplasm at a median of 7.6 years (1 to 25 y) after primary osteosarcoma treatment.
  • Second neoplasms were leukemia (10), breast (7), lung (2), kidney (2), central nervous system cancer (2), soft tissue (1), parotid (1), and colon (1).
  • The rate of second neoplasms was significantly higher in female patients, and the latent period shorter in hematologic tumors compared with solid tumors.
  • The rate of second malignancies observed in the osteosarcoma group was significantly higher than that observed in the control group of 1160 patients with benign tumors treated in the same period at our Institute (2.2% vs. 0.8%, P<0.009).
  • Our study showed that the risk of second neoplasm within 15 years increased and then leveled off and that although secondary solid tumors could be explained as unrelated cases, leukemias seem to be over represented.

  • There are also a few reported cases in the literature of pseudoglandular schwannoma from central nervous system, eye, submandible, and shoulder, in which the gland-like structures were lined by Schwann cells.
  • We report here a patient with a benign cutaneous schwannoma composed of predominantly gland-like spaces that contained mucinous material and were lined by Schwann cells confirmed by immunohistochemistry and ultrastructural studies.

  • While intraoperative frozen section suggested pathology of papillary ependymoma or CPP, to our surprise, final histological examination revealed a benign choroid plexus papilloma.

  • Myxopapillary ependymomas (MPEs) have historically been thought to be benign tumors occurring most frequently in adults.
  • Four (80%) of these five patients suffered from disseminated disease of the central nervous system (CNS) at the time of presentation; this incidence is much higher than that reported in the combined adult and pediatric literature.
  • In nine cases (35%) CNS metastases occurred.
  • In those cases in which patients underwent screening for CNS tumor dissemination, however, the incidence of disseminated disease was 58% (seven of 12 patients).
  • In pediatric patients MPEs may spread throughout the CNS via cerebrospinal fluid pathways; therefore, MR imaging of the entire CNS axis is recommended at both presentation and follow-up review to detect tumor dissemination.

  • Meningioma is a common neoplasm that constitutes almost 30% of all primary central nervous system tumors and is associated with inconsistent clinical outcomes.
  • The aim of this study was to evaluate the expression of SPARC with proliferation index, p53 reactivity in WHO grade 1 (benign), grade 2 (atypical) and grade 3 (anaplastic) meningiomas and correlate with clinical features of the patients, including location of the tumor, recurrence of the tumor and survival of patients.
  • We studied 111 meningiomas, 69 being benign, 34 being atypical and eight being anaplastic meningiomas of various histological types.
  • A high immunohistochemical score (4-6) for SPARC was more frequent in atypical and in anaplastic meningiomas than in benign meningiomas (p < 0.01).

  • In a prospective study over a 16 month period, 20 of 242 patients presented with isolated CN manifestations, and were assessed for the following: primary site; CN symptoms and signs; length of time between primary diagnosis and neurological involvement; and survival following the neurological diagnosis.
  • They included meningeal carcinomatosis (10/16), brain stem metastases (3/16), primary brain astrocytomas (1/16), and metastases out of the central nervous system (2/16).
  • Although most of the isolated CN manifestations were due to systemic metastasis, in particular to the meninges, up to 20% were related to benign conditions.

  • In this study, we show that the transcription factor Sox11 is specifically expressed in the nucleus of MCL compared with other lymphomas and benign lymphoid tissue.
  • Although the role of Sox11 presently is not known in lymphocyte ontogeny, it is normally expressed in the developing central nervous system in the embryo and shows sequence homology with Sox4, a transcription factor crucial for B lymphopoiesis.
  • Our novel finding of specific overexpression of Sox11 mRNA and nuclear protein in both cyclin D1-positive and - negative MCL may be useful for the diagnosis of MCL as a complement to cyclin D1 and also suggests a functional role for Sox11 in MCL.

  • During hospitalization, multiple purpuric papules developed over the abdomen, and the disease spread to mediastinal lymph nodes, lungs and the central nervous system.
  • Based on the histologic, immunophenotypic and molecular biology findings, a diagnosis of CD8+ epidermotropic cytotoxic primary cutaneous T-cell lymphoma was made.
  • Among primary cutaneous T-cell lymphomas, the CD8+ epidermotropic cytotoxic subset comprises rare, highly aggressive forms characterized by metastatic spread to unusual sites such as the oral cavity, lungs, testis and the central nervous system but usually not to the lymph nodes.

  • Hemangioblastoma is a benign tumor that can occur sporadically, or in association with von Hippel-Lindau disease in approximately one-quarter of the cases.
  • Only exceptionally does it occur outside the central nervous system.
  • The diagnosis of hemangioblastoma was confirmed by negative immunostaining for cytokeratin, and positive staining for α-inhibin, S100, and neuron-specific enolase.
  • This benign neoplasm which can be mistaken for various malignancies such as renal cell carcinoma, epithelioid angiomyolipoma, adrenal cortical carcinoma, and paraganglioma, deserves wider recognition for its occurrence as a primary renal tumor.

  • OBJECTIVE: The majority of tumors of the cerebellopontine angle (CPA) are benign.
  • We discuss the differential diagnosis and prognosis of melanotic lesions at this location.
  • INTERVENTION: A translabyrinthine approach revealed a pigmented, vascular neoplasm encasing vessels and cranial nerves of the left CPA.
  • The tumor was subtotally resected, and a histopathological diagnosis of melanoma was made.
  • CONCLUSION: This case most likely represents primary melanoma of the central nervous system that mimicked a vestibular schwannoma.
  • The differential diagnosis of melanotic lesions of the CPA is discussed as are the prognostic implications of each diagnosis.

  • The perineurium represents a continuum with the pia-arachnoid from the central nervous system and extends distally with the sheath of capsular cells of peripheral sensorial organs and propioceptive receptors.
  • Perineurial invasion is considered an important prognostic factor in several malignant neoplasms.
  • Perineuriomas are true benign infrequent perineurial cell neoplasms that have been divided in 2 categories: those with intraneural localization and a more common extraneural (soft tissue) group, including sclerosing and reticular variants.
  • The histologic appearance of perineuriomas may overlap with other soft tissue spindle cell neoplasms.
  • Immunohistochemistry is imperative for the diagnosis, although in certain cases ultrastructural studies may be needed.

  • OBJECTIVE: Hemangioblastoma is classified as a benign tumor of the central nervous system.
  • Peripheral nervous system hemangioblastomas to date have been described only in a few case reports.
  • These tumors occurred in 2% of all patients with hemangioblastomas of the central nervous system, or 6% of all patients with spinal hemangioblastomas.
  • In general, the tumors are surgically more challenging, and clinically significant bleeding as well as local tumor recurrence is more common than in intradural hemangioblastomas, mostly because of the frequency of incorrect initial radiographic diagnosis.
  • We suggest that because of the surgical consequences, hemangioblastoma should always be considered to be an important radiological differential diagnosis for nerve sheath tumors.

  • Craniopharyngiomas (CP) are benign epithelial tumors of the sellar region and can be clinicopathologically distinguished into adamantinomatous (adaCP) and papillary (papCP) variants.
  • Furthermore, the outer tumor cell layer of adaCP showed a distinct expression of MAP2, a novel finding helpful in the differential diagnosis of epithelial tumors in the sellar region.
  • Our data support the hypothesis that adaCP, unlike other non-neuroepithelial tumors of the central nervous system, create a tumor-specific cellular environment at the tumor-brain junction.

  • Each of these cases was diagnosed and managed differently, illustrating the relative importance of radiographic imaging, tumor markers and histopathologic examination in the diagnosis and treatment of intracranial disease.
  • Biopsy of the intrasellar mass revealed only proteinaceous material and normal anterior pituitary, consistent with cyst content without evidence of neoplasm.
  • Without a clear unifying diagnosis it is difficult to posit an underlying pathology or genetic mechanisms in this unusual set of cases.
  • At least two of the patients had benign cysts.
  • The diagnosis of the third patient is unclear as there was no tissue biopsy.

  • Most meningiomas are benign, although atypical and malignant meningiomas also exist.
  • The presence of an associated cyst is an uncommon imaging feature that may make it difficult to distinguish the tumor from a primary intra-axial glial neoplasm.

  • INTRODUCTION: Meningiomas are frequent primary neoplasms of the central nervous system, usually benign and susceptible to healing through surgery.
  • Nevertheless, a recurrence rate close to 20 % in benign meningiomas completely diseccated arises the need of considering new prognostic factors.
  • RESULTS: Though the immunohistochemical labelling index (LI) of the PR decreased with the progression of the histological grade (means of 27.37 % for grade I, 17.89% for grade II, and 13.50% for grade III), such correlation was not statistically significant and the cut off estimated in 20% was not satisfactory to discriminate among benign meningiomas (grade I) and non benign (grades II-III) due to its poor sensitivity (56.10%) and positive predictive value (56.10 %).

  • A benign non-teratoid medulloepithelioma of the optic nerve was surgically removed.
  • This is the first case described of a benign medulloepithelioma of the optic nerve, later transforming into a malignant tumour.
  • One third of patients with medulloepithelioma of the optic nerve have died from direct intracranial spread or metastasis to the central nervous system (CNS).
  • CONCLUSIONS: Medulloepithelioma of the optic nerve should be considered malignant despite morphology with tendency to spread to the CNS.
  • Furthermore; it should be considered as a differential diagnosis when observing a tumour of the optic nerve in a child.
