  • Neoplastic cells, both malignant and benign, local occurring and metastatic, can cause alopecia of the scalp.
  • However, the infiltration of neoplastic cells is sometimes not florid; a condition known as "scalp alopecia due to a clinically unapparent or minimally apparent neoplasm" (SACUMAN).
  • The most common neoplasm in which an uncomplicated, minimally or unapparent scalp alopecia occurs and no infiltrate of cancer is suspected is metastatic breast carcinoma.
  • Dermatologists must be aware that in rare cases a bland scalp alopecia can represent a new or recurring, local or metastatic neoplasm.

  • BACKGROUND: No study has systematically evaluated the significance of involvement of the superficial specimen margin in skin-sparing mastectomies (SSMs).
  • RESULTS: 64 SSMs (38%) had a positive superficial specimen margin but only 13 (20%) of these had residual breast carcinoma in ASMs.
  • Only 1 of 104 SSMs with a negative superficial specimen margin had residual breast carcinoma in its ASM (P < 0.05).
  • In SSMs with a positive superficial specimen margin, multivariate analysis revealed that the presence of extensive ductal carcinoma in situ (DCIS) in the SSM and a thicker ASM specimen were the only independent factors predictive of residual breast carcinoma in ASM.
  • Eighty-nine (53%) ASMs contained benign breast tissue.
  • CONCLUSIONS: Superficial specimen margins in SSMs are often microscopically positive and approximately half of ASMs contain benign breast tissue, likely reflecting the difficulty in completely removing breast tissue near the skin flaps in SSMs.

  • BACKGROUND: Primary extra-osseous osteogenic sarcomas have been reported in many tissues of the body but their occurrence in the breast is extremely rare.
  • It can arise as a result of osseous metaplasia in a pre-existing benign or malignant neoplasm of the breast or as non-phylloides sarcoma from the soft tissue of a previously normal breast.
  • CASE PRESENTATION: A 40 year-old Nigerian woman was clinically diagnosed to have carcinoma of the left breast.
  • The histology report of core-needle biopsy of the mass showed a malignant neoplasm comprising islands of chondroblastic and osteoblastic stromal cells.
  • This report changed the diagnosis from carcinoma to osteogenic sarcoma of the breast.
  • She had a left modified radical mastectomy, however there was significant post surgery skin deficit.
  • CONCLUSION: A diagnosis of osteogenic sarcoma of the breast was made based on histology report and after excluding an osteogenic sarcoma arising from underlying ribs and sternum.
  • This is the second documented case of primary osteogenic sarcoma of the breast coming from Nigeria.

  • INTRODUCTION: Myofibroblastoma of the breast is a rare benign stromal tumour seen predominantly in men.
  • We present a case of an unusually large myofibroblastoma, which mimicked a malignant breast tumour.
  • CASE PRESENTATION: A 65-year-old man presented with a rapid enlargement of the right breast over 6 weeks.
  • Examination revealed a firm 15 cm hemispherical lump occupying the whole of the right breast with peau d'orange appearance of the overlying skin and distortion of the nipple.
  • The clinical and radiological features suggested the possibility of sarcoma of the breast.
  • CONCLUSION: This unexpected presentation of an unusually large myofibroblastoma in a male breast is the largest reported to date.
  • Myofibroblastomas can mimic malignant neoplasms and the clinical significance of this entity lies primarily in its recognition as a distinctive benign neoplasm.

  • BACKGROUND: Preservation of the breast skin envelope in skin-sparing mastectomy is the key component for superior aesthetic results.
  • Breast mound disproportions in primary breast reconstruction caused by a mismatch between retained skin envelope and donor-tissue volume provokes breast shape asymmetries.
  • A skin-sparing mastectomy using a vertical pattern can address these breast mound imperfections by adjusting this mismatch in a vertical direction.
  • METHODS: A retrospective chart review was conducted over a 10-year period for patients who underwent skin-sparing mastectomy using a vertical pattern for malignant, premalignant, benign, and deformational disease of the breast.
  • RESULTS: Seventy-two patients, aged 31 to 69 years (mean, 51.5 years), underwent 106 skin-sparing mastectomies using a vertical pattern and primary reconstruction with 38 unilateral and 34 bilateral free flaps (muscle-sparing transverse rectus abdominis musculocutaneous or deep inferior epigastric perforator flaps).
  • The mean follow-up period was 42 months, without any local or systemic recurrences of the breast cancer.
  • CONCLUSIONS: Skin-sparing mastectomy using a vertical pattern improves the aesthetic outcome in primary breast reconstruction without compromising oncologic safety and demonstrates low morbidity.
  • Elimination of the disharmony between skin flap and breast volume in the vertical direction while respecting the inframammary crease produces a youthful, symmetrical conical breast shape with medial fullness.

  • To understand the extent of CSMD1 aberrations in vivo, we characterized 184 primary tumors from the head and neck, lung, breast and skin for gene copy number and analyzed expression in our HNSCCs and lung squamous cell carcinomas (SCCs).
  • We detected loss of CSMD1 in a large proportion of HNSCCs (50%), lung (46%) and breast cancers (55%), and to a lesser extent in cutaneous SCCs (29%) and basal cell carcinomas (BCCs, 17%) using array-based comparative genomic hybridization (aCGH).
  • CSMD1 expression was decreased in tumors compared to adjacent benign tissue (65%, 13/20) and was likely due to gene loss in 45% of cases (9/20).

  • In two women with aggressive course the original biopsy was falsely interpreted, once as phyllodes tumor and secondly as benign sclerosing pseudotumor.
  • It arises in the deep breast tissue and structurally resembles the microcystic adnexal carcinoma of the skin.
  • Low-grade adenosquamous carcinoma, however, has nothing to do with syringomatous adenoma of the nipple, which is a benign tumor of the skin adnexa.

  • PATIENTS AND METHODS: The hardcopy prints of 337 ductal invasive breast cancers were analyzed.
  • Tumors in an advanced T-stage showed more frequently an irregular shape ( p = 0.006), an orientation parallel to the skin ( p = 0.01), hypoechogenicity ( p < 0.0001), and less often calcifications ( p = 0.002).
  • CONCLUSION: Breast ultrasound criteria, which are used to differentiate benign from malignant breast lesions, are influenced by age, size and lymph node status.

  • PURPOSE: To investigate the feasibility of applying exclusive intraoperative radiation therapy (IORT) after conservative surgery in limited-stage breast carcinoma and to evaluate late effects and cosmetic results after this new conservative treatment.
  • PATIENTS AND METHODS: From October 2000 to November 2002, 47 consecutive patients with unifocal breast carcinoma up to a diameter of 2 cm received conservative surgery followed by IORT with electrons as the sole adjuvant local therapy.
  • During follow-up, a radiologic assessment with mammography and sonography was periodically performed and any breast-imaging alterations were reported.
  • RESULTS: After a follow-up ranging from 36 to 63 months (median, 48 months), 15 patients developed breast fibrosis (grade 2 in 14 patients, grade 3 in one patient), two patients presented with grade 3 skin changes, one patient developed a clinically relevant fat necrosis, and one patient showed breast edema and pain.
  • Two patients developed contralateral breast cancer and one distant metastases; no local relapses occurred.
  • In four patients (8%), mammographic and sonographic findings suggested malignant lesions and required a rebiopsy to confirm the benign nature of the lesion.
  • CONCLUSION: IORT in breast carcinoma is still an experimental treatment option for select patients with breast cancer and its application should be restricted to prospective trials.
  • Although preliminary data on local control are encouraging, a longer follow-up is needed to confirm the efficacy of IORT in breast cancer and to exclude that severe late complications compromise the cosmetic results or modify the radiologic breast appearance during follow-up increasing the need for additional investigations.

  • Sarcoidosis is an idiopathic systemic inflammatory granulomatous disorder comprised of epithelioid and multinucleated giant cells with little necrosis.
  • It usually invades the lungs with fibrosis and may also involve lymph nodes, skin, liver, spleen, eyes, phalangeal bones, and parotid glands.
  • Breast involvement is extremely rare, but, when present, it could be confused with a benign or, more important, a malignant neoplasm.
  • We have reported a case of sarcoidosis of the breast in a 51 years old woman with systemic manifestations of sarcoidosis (arthralgias and uveitis) associated with a breast mass and with a clinical history of fibrocystic mastopathy.
  • The mammography and ultrasonogram showed a solitary little nodular lesion localized in the breast.

  • OBJECTIVE: To investigate the clinical accuracy of magnetic resonance imaging-guided breast vacuum-assisted biopsy (MR-VAB).
  • RESULTS: Twenty-three patients (median age 51 years) were at high risk (BRCA1 = 11, BRCA2 = 7, familial risk = 5), 23 had a suspected local recurrence of breast cancer.
  • According to histopathology results, 29 lesions were benign, 10 were high-risk (papillary = 2, radial scar = 1, atypical epithelial hyperplasia = 7) and 33 malignant (ductal carcinoma in situ = 8, invasive cancers = 25).
  • Three false negative results and 3 complications occurred (1 malaise, 1 skin defect, 1 infection).

  • This tumour which is locally invasive with high potential for recurrence if incompletely excised is considered benign and a complete excision is sufficient.
  • We report a case of syringomatous adenoma of the nipple in a postmenopausal diabetic woman who was incidentally found to have a subareolar mass in her right breast.

  • BACKGROUND: Recent evidence suggests cathelicidin LL-37 to be a growth factor for various human cancers such as lung cancer, ovarian cancer and breast cancer.
  • However, the effect of LL-37 against malignant skin cancer has not been reported.
  • OBJECTIVES: To investigate whether the human cathelicidin LL-37 is involved in the carcinogenesis of various skin tumours.
  • Immunohistochemical analysis of melanoma, nonmelanoma skin cancer and precancerous and benign skin lesions was performed.
  • CONCLUSIONS: Cathelicidin LL-37 was primarily expressed in human malignant skin cancer.
  • We report that an increase in the level of LL-37 is associated with malignant skin tumours such as malignant melanoma.
  • These results highlight the importance of LL-37 in the malignant tendency of skin tumours.

  • Primary cutaneous adenomyoepithelioma is part of the spectrum of epithelial-myoepithelial tumors that includes benign mixed tumor, myoepithelioma and myoepithelial carcinoma.
  • Like its breast counterpart, primary cutaneous adenomyoepithelioma should probably be regarded as a neoplasm of borderline malignant potential.

  • For 9 years, the patient remained disease free until complaining of a slow-growing skin nodule on the lower back that was excised under clinical suspicion of metastatic mammary carcinoma.
  • Histopathological exam revealed a benign eccrine spiradenoma/cylindroma and an intermixed carcinoma, with a differential diagnosis of either primary eccrine carcinoma or mammary carcinoma metastatic to the spiradenoma/cylindroma.
  • The mammary carcinomas showed disparate LOH at 5 of 7 (right breast) and 4 of 7 (left breast) informative genetic loci, establishing these carcinomas as separate primary neoplasms.
  • This neoplasm is referred to in the literature as carcinoma ex spiradenoma/cylindroma and spiradenocylindrocarcinoma.

  • BACKGROUND: Differential diagnosis for a nodule in the nipple or subareolar area of woman includes both primary neoplasms of breast as well as those from skin and adnexae.
  • A biopsy revealed an infiltrating adnexal neoplasm with features similar to those seen in syringomas commonly occurring in locations such as upper face and pubis.
  • Microscopic appearance of such a rare benign infiltrating neoplasm of eccrine duct origin occurring in woman's breast should not be misinterpreted as more common infiltrating primary breast carcinoma.
