  • Desmoplastic melanoma (DM) is a variant of spindle cell melanoma characterized by the presence of abundant fibrous matrix.
  • It is typically found in the head and neck region on chronically sun-damaged skin of older individuals.
  • It may simulate a sclerosing melanocytic nevus and various benign and malignant nonmelanocytic lesions.
  • It may be prominent throughout the entire tumor (pure DM) or represent a portion of an otherwise nondesmoplastic melanoma (combined DM).
  • DM differs from conventional melanoma in its clinical course.

  • [Title] Plaque-type blue nevus of the oral cavity.
  • BACKGROUND: The blue nevus of the oral cavity is a rare lesion with important differential diagnoses.
  • The plaque-type blue nevus is an uncommon variant of the blue nevus.
  • Because of its particular clinical appearance, it can easily be confused with satellite metastases from malignant melanoma.
  • OBJECTIVES: To describe the clinical and histological features of a plaque-type blue nevus of the buccal mucosa in a 20-year-old white woman, to review all intraoral blue nevi and all plaque-type blue nevi reported in the literature so far and to compare the criteria of blue nevi and nevus of Ota.
  • RESULTS: An intraoral blue nevus was described for the first time in 1959.
  • Our case is the first report of a plaque-type blue nevus of the oral cavity.
  • CONCLUSIONS: The exceptional widespread intraoral blue nevus described herein can clinically be confused with an intraoral malignant melanoma, and it has a very similar clinical appearance as the intraoral part of nevus of Ota.
  • Transitional states between blue nevus and nevus of Ota may occur clinically and histologically.

  • In this article, we report the occurrence of breast cancer, melanoma, squamous cell carcinoma of the alveolar ridge, colon cancer, a desmoid tumor of the abdominal wall, and pancreatic adenocarcinoma in a 65-year-old woman.
  • She was identified as having the familial atypical multiple mole melanoma-pancreatic cancer syndrome (FAMMM-PC) with a germline p16 mutation at amino acid position 15.
  • DISCUSSION: Patients with this syndrome traditionally present with multiple nevi and melanoma, and a subset also present with other cancers, including pancreatic cancer; however, no FAMMM-PC patient has yet been described with this constellation of cancers, including squamous cell carcinoma of the alveolar ridge and a desmoid tumor.

  • We have shown that patients that have to protect themselves against solar UV radiation for medical reasons, including patients with xeroderma pigmentosum (XP), basal cell nevus syndrome (BCNS), lupus erythematodes (LE) or transplant recipients, are at risk to develop vitamin D deficiency.
  • Suboptimal vitamin D status has to be substituted (e.g. via oral treatment) to protect against serious vitamin D deficiency-related health problems without increasing the risk to develop solar UV-induced skin cancer.
  • Our finding that protection against solar UV radiation causes vitamin D deficiency underlines the need for re-defining dermatological recommendations for solar UV protection in skin cancer prevention programs.

  • We have studied eye development in a species of true mole, the Iberian mole Talpa occidentalis, and present the molecular basis of abnormal lens development.
  • RESULTS: Lens fibre differentiation is not completed in the Iberian mole.
  • FOXE3-positive lens fibres continue to proliferate within the posterior compartment of the embryonic lens, but unlike in the mouse, no proliferation was detected anywhere in the postnatal mole lens.
  • Furthermore, beta-crystallin and PROX1 expression patterns are abnormal and our data suggest that genes encoding beta-crystallins are not directly regulated by PAX6, c-MAF and PROX1 in the Iberian mole, as they are in other model vertebrates.
  • These pathways are not as robust in the mole, and lead to loss of the anterior epithelial phenotype and only partial differentiation of the lens fibres, which continue to express 'epithelial' genes.
  • Paradigms of genetic regulatory networks developed in other vertebrates appear not to hold true for the Iberian mole.

  • BACKGROUND: The aim of the current study was to investigate levels of adenosine deaminase in plasma of patients with hydatidiform mole.
  • METHODS: Plasma adenosine deaminase levels were determined in 17 women with normal pregnant course, in 17 women with hydatidiform mole, and in 17 non-pregnant healthy volunteers.
  • RESULTS: Mean adenosine deaminase activity in the hydatidiform mole group was 121.5+/-24.8 U/L, significantly higher than in the pregnant control (7.8+/-6.5 U/L; p<0.0001) and non-pregnant control groups (6.4+/-7.4 U/L; p<0.0001).
  • CONCLUSIONS: Adenosine deaminase may play a role in the development of hydatidiform mole.

  • The purpose of this work is to understand how an increase in the mole ratio of surfactant to block copolymer from low to high influences the dynamics of ionic and neutral solute molecules.
  • The variation in the mole ratio of CTAC to P123 from low to high has resulted in a drastic increase in the average reorientation time of rhodamine 110.
  • In the low mole ratio regime, rhodamine 110 and DMDPP are located at the interface and palisade layer, respectively, of P123 micelle-CTAC complexes.
  • On the other hand, in the high mole ratio regime, both the probes are located in the Stern layer of CTAC-P123 complexes.
  • The enhancement in the average reorientation time of rhodamine 110 with an increase in the mole ratio of surfactant to block copolymer has been rationalized on the basis of formation of rhodamine 110-Cl ion pair, which in turn associates with the cationic head groups of CTAC-P123 complexes.
  • The observed decrease in the average reorientation time of DMDPP with an increase in the mole ratio of CTAC to P123 is a consequence of lower microviscosity of the Stern layer of CTAC-P123 complexes compared to the palisade layer of P123 micelle-CTAC complexes.

  • Some trisomic SAB specimens can exhibit abnormal villous morphology simulating a partial hydatidiform mole, a distinct form of hydatidiform mole characterized by diandric triploidy.
  • A SAB specimen from a 27-year-old woman, G1P0 at 8 weeks gestational age, was reviewed in consultation to address the finding of morphological features suggestive of a partial hydatidiform mole but DNA ploidy analysis yielding a diploid result.
  • Since fully developed features of a partial hydatidiform mole were lacking, additional analysis was performed.

  • [Title] Dysplastic nevi with an underlying or juxtaposed neurofibroma: does a relationship exist?
  • BACKGROUND: Neurofibromas and dysplastic nevi are both common skin disorders found in the general population.
  • OBJECTIVE: We report, for the first time, three cases of dysplastic nevi in spatial association with a neurofibroma that occurred in unrelated individuals.
  • METHODS AND RESULTS: Three cases of dysplastic nevi occurring in association with solitary neurofibromas were selected prospectively from the routine accessions from August 1, 2003, to July 31, 2005.
  • In none of the three cases, was a dysplastic nevus suspected by the clinician.
  • CONCLUSION: The finding of dysplastic nevi in a spatial relationship to a neurofibroma is not happenstance since they are both of neural crest cell origin and respond to the same growth factors.

  • In vivo reflectance confocal microscopy is a novel technique for the noninvasive study and diagnosis of the skin.
  • The aim of this study was to describe and characterize the cytological and architectural aspects of cell clusters in melanocytic lesions observed by confocal microscopy, and to correlate them with routine histopathology.
  • A total of 55 melanocytic lesions comprising 20 melanomas, 25 acquired nevi and 10 Spitz nevi were studied by means of reflectance confocal microscopy, dermoscopy and routine histopathology.
  • Dense clusters appeared characteristic for benign lesions, although present in 13 out of 20 melanomas.
  • Sparse cell clusters were more frequently observable in melanomas, but also sporadically present in one Spitz nevus.
  • Confocal microscopy allowed the in vivo characterization of aspects of melanocytic nests and their exact correlation with histopathology.

  • The objective of this study is to assess the expression of heat shock protein 105 (HSP105) in melanoma and benign melanocytic lesions.
  • The expression of HSP105 in 62 human melanoma samples--46 primary and 16 metastatic lesions--and 42 melanocytic naevi samples, was assessed by immunohistochemistry.
  • Western blotting was performed on melanoma cell lines, melanoma tissues with matched normal skin and melanocytic naevi.
  • Seventy-four per cent of the primary melanoma lesions and 88% of the metastatic lesions overexpressed HSP105 by immunohistochemistry.
  • The majority of melanocytic lesions (95%) were negative (P<0.05).
  • Western blotting detected high expression of HSP105 in melanoma cell lines and tissues.
  • Melanocytic naevi expressed HSP105 at a level that was similar to that of normal skin.
  • Our results show that high expression of HSP105 is associated with malignant melanoma especially advanced and metastatic lesions.
  • The results suggest that HSP105 analysis may be a helpful tool as a poor prognostic indicator and as a diagnostic aid in problematic lesions; in addition, melanoma can be included in the growing list of tumours overexpressing HSP105 to be targeted for potential HSP105-based therapeutic strategies.

  • As a consequence, there are two main perspectives to nevi: that of a hamartoma and that of a benign tumor.
  • In this review, dermatopathological studies on congenital and acquired nevi, including studies on age-related and location-dependent changes, are analyzed.

  • MATERIALS AND METHODS: Histologically diagnosed complete (n = 6) and partial (n = 3) hydatidiform moles were immunohistochemical analyzed for INH-alpha, INH-betaA, INH-betaB, INH-betaC and INH-betaE subunits.
  • RESULTS: Immuno-histochemical reaction with INH-alpha, INH-betaA, INH-betaB, INH-betaC and INH-betaE subunits was demonstrated in hydatidiform moles.
  • The INH-betaA and INH-betaB expression was significantly higher in complete compared to partial moles (p < 0.05 each), while INH-alpha, INH-betaC and INH-betaE did not demonstrate any statistically significant differences.
  • CONCLUSION: We demonstrated an immunohistochemical expression of all five inhibin/activin subunits in partial and complete hydatidiform moles.
  • The expression of INH-betaA and INH-betaB determined immunohistochemically was significantly up-regulated in complete moles, suggesting the utilization of these antibodies as diagnostic differentiation markers between complete and partial moles.

  • The vast majority of melanocytic lesions with hair, such as congenital melanocytic nevi, are benign.
  • However, there is a notion that the presence of one or more hairs in a melanocytic lesion is confirmatory for the benign nature of the lesion.

  • [Title] The anatomic distribution of melanoma and relationships with childhood nevus distribution in Colorado.
  • The nature of the relationship between nevus development in childhood and later development of melanoma is unclear.
  • Data on melanoma diagnoses by histologic type and anatomic site were obtained for 2351 white, non-Hispanics in Colorado from the Colorado Central Cancer Registry between 2000 and 2004.
  • Nevus size and body site were ascertained during skin exams conducted in the summer of 2007 on 717 white, non-Hispanic children aged 8-9 years.
  • Chi-square goodness-of-fit analysis was used to assess the association between the anatomic site distributions of nevi versus melanoma.
  • Superficial spreading melanoma was the most frequent histology, followed by lentigo maligna melanoma.
  • Nodular melanoma was the least common histology.
  • For males, there was no significant difference between the distribution of medium-sized (> or =2 mm) nevi and the distribution of both superficial spreading and nodular melanomas.
  • For females, there was no significant difference between the anatomic distribution of small-sized (<2 mm) nevi and the distribution of nodular melanoma, and there was marginal evidence for a difference between the distribution of medium-sized (> or =2 mm) nevi and the distribution of nodular melanoma.
  • There was evidence for a difference between all of the nevus distributions and the distributions of superficial spreading and lentigo maligna melanoma in females.
  • The similarities between the nevus and melanoma distributions are interesting findings, but it is difficult to interpret the significance of these findings based on the current state of knowledge of melanoma etiology.

  • Hemichrome mole fractions of HbS are more than HbA because of more hydrophobic interaction of DTAB-HbS in second set of binding site relative to DTAB-HbA.
  • The number of components and mole fraction of mentioned species were determined by employing chemometric resolution techniques.
  • The results show that both number and mole fraction of components which were formed during hemoglobin (HbA and HbS) oxidation by DTAB were initial hemoglobin concentrations independent.
  • Furthermore, in average the mole fraction of hemichrome of HbS is 14.4% more than HbA.
  • On the other hand, the mole fraction of HbA ferrihemoglobin is 15.6% higher than HbS averagely.

  • [Title] [Outcome of photodynamic therapy for subretinal neovascularization secondary to choroidal nevi].
  • INTRODUCTION: Choroidal nevi are the most common benign intraocular tumors.
  • However, the occurrence of subretinal neovascularization secondary to choroidal nevus is very rare.
  • Our goal was to assess the efficacy and the safety of photodynamic therapy (PDT) for treatment of subretinal neovascularization related to choroidal nevi.
  • PATIENTS AND METHOD: We conducted a retrospective study including subretinal neovascularization secondary to choroidal nevi treated by PDT from 1999 to 2005.
  • RESULTS: Six subretinal neovascularizations secondary to choroidal nevi were included in our study.
  • All nevi were localized in the posterior choroids.
  • No growth of the nevi was observed.
  • CONCLUSIONS: Subretinal neovascularization is reassuring from the oncologist's point of view because it is considered a relative indicator of benignity of the nevus.
  • PDT appears to be a safe and effective procedure for the treatment subretinal neovascularization secondary to choroidal nevus, with results similar to those observed in ARMD.

  • BACKGROUND: Research on melanocytic nevi predominantly utilizes formalin-fixed, paraffin-embedded tissue, largely limiting research to morphologic and immunohistochemical observations.
  • Withholding portions of fresh nevus tissue for molecular studies could result in the loss of important diagnostic material.
  • OBJECTIVE: This study prospectively evaluated melanocytic nevi for histologic homogeneity to determine if using a portion for research would have affected diagnosis.
  • Slides from nevi were examined in two phases.
  • In phase I, sections of nevi were randomized and a diagnosis was rendered for each section of nevus.
  • In phase II, the dermatopathologist reviewed all slides for each nevus as a case, similar to the original interpretation of the lesion provided to the clinician.
  • RESULTS: Case material included 51 melanocytic lesions from 31 subjects.
  • The phase I diagnosis matched the original diagnosis for 99 of 121 slides that showed a melanocytic lesion (82%).
  • LIMITATIONS: The study was limited by: a small number of specimens; the clinician could have chosen clinically homogeneous nevi for biopsy; effect of interobserver and intraobserver variability on diagnosis.
  • CONCLUSIONS: For the majority of melanocytic nevi in this study, the diagnostic information present in one section of a melanocytic nevus could be extrapolated to the remainder of the specimen without adverse consequences from a diagnostic or therapeutic perspective.

  • OBJECTIVE: The purpose of this study is to do a histopathological review of all cases ofhydatidiform moles seen in pathology department of Federal Medical Centre, Gombe between June 2000 and May 2005.
  • RESULTS: Thirty four cases of hydatidiform moles were studied and these formed 7.5% of all products of conception seen during the review period.
  • There were 18 cases of complete hydatidiform mole and 16 cases of partial hydatidiform mole.
  • No invasive or tubal mole was seen during this period.
  • The frequency of hydatidiform moles was 1 in 166 delivaries.

  • Melanoma is one of the most aggressive cancers affecting humans.
  • The mechanism of melanoma development, progression, and metastasis is largely unknown.
  • In order to uncover genes unique to melanoma cells, we used high-density DNA microarrays to examine the gene expression profiles of metastatic melanoma nodules using benign nevi as controls.
  • Over 190 genes were significantly overexpressed in metastatic melanomas compared with normal nevi by at least 2-fold.
  • One of the most abundantly expressed genes in metastatic melanoma nodules is osteopontin (OPN).
  • Immunohistochemistry staining on tissue microarrays and individual skin biopsies representing different stages of melanoma progression revealed that OPN expression is first acquired at the step of melanoma tissue invasion.
  • In addition, blocking of OPN expression by RNA interference reduced melanoma cell numbers in vitro.
  • Our observations suggest that OPN may be acquired early in melanoma development and progression, and may enhance tumor cell growth in invasive melanoma.

  • Cutaneous Malignant Melanoma (CMM) is the most malignant skin tumour in humans, the incidence of which is rising rapidly in most fair-skinned populations, without apparent decline in mortality.
  • The prognosis of patients with VGP melanoma depends on several clinical and histological parameters; the latter include thickness, mitotic activity, presence or absence of ulceration and regression, and pattern of lymphocytic host response.
  • To obtain insight in the molecular mechanisms of tumor progression in CMM, and in search of new prognostic markers, we performed global gene-expression profiling using 44K oligonucleotide micro-arrays on a unique retrospective series of 83 frozen primary MM with VGP, 9 metastases and 23 benign nevi.
  • Unsupervised analysis allowed us o identify clusters of melanoma patients with different outcome based on their gene expression profile only.
  • This signature was validated on a separate series of melanoma patients, and proved to have a predictive accuracy comparable to what can be obtained by tumour thickness and ulceration.
  • On an immunohistochemical level, we identified 8 new markers that may help in the prognostication of melanoma patients; three of these markers, i.e. the mini-chromosome maintenance (mcm) proteins mcm3, 4 and 6, hat are involved in DNA-replication, had independent prognostic value.

  • BACKGROUND: Germline mutations at the INK4a/p16 locus are implicated in several human cancer syndromes, including familial atypical multiple mole melanoma (FAMMM) syndrome, FAMMM-pancreatic cancer (FAMMM-PC) syndrome, and in familial head and neck cancer syndrome.
  • METHODS: We present an individual with a family history of melanoma and pancreatic cancer who had multiple dysplastic nevi, squamous cell carcinoma of the tongue at age 22, multiple melanomas, a second squamous cell cancer of the tongue at age 40, and ultimately a pancreatic cancer.
  • We conclude that HNSCC in young individuals should prompt clinicians to obtain a family history and consider that the patient may have a germline p16 defect that could predispose them to other cancers, including melanoma and pancreatic cancer.

  • [Title] Episcleral melanoma as a complication of ocular melanocytosis.
  • BACKGROUND: Primary episcleral melanoma is exceedingly rare.
  • We report a case of primary episcleral melanoma in an eye with ocular melanocytosis.
  • Clinical examination and ultrasound biomicroscopy showed no evidence of intraocular mass or nevus.
  • Lamellar scleroconjunctivectomy of the lesion was performed, and histopathologic analysis revealed episcleral melanoma with no evidence of intra- or extra-ocular extension.
  • CONCLUSIONS: Primary episcleral melanoma is a previously unreported complication of ocular melanocytosis, and should be considered in the differential diagnosis in patients with ocular melanocytosis and pigmented nodules on the ocular surface.

  • [Title] A search for CDKN2A/p16INK4a mutations in melanocytic nevi from patients with melanoma and spouse controls by use of laser-captured microdissection.
  • OBJECTIVE: To determine the frequency at which the CDKN2A coding region is mutated in the atypical nevi of persons with sporadic melanoma.
  • DESIGN: DNA samples, isolated by laser-captured microdissection of atypical nevi from 10 patients with newly incident cases of sporadic melanoma and their spouses as matched controls, were used as templates for nested polymerase chain reaction amplification of CDKN2A exons 1 and 2.
  • RESULTS: No point mutations in the coding region of CDKN2A were observed in any of the melanocytic nevi.
  • CONCLUSIONS: Point mutations in CDKN2A are an uncommon event in the atypical nevi of persons with melanoma.
  • As such, the data may support a hypothesis of melanocytic nevus histogenesis, in which the melanocytic nevus and malignant melanoma represent separate, pleiotropic pathways resulting from common stimuli, such as genomic damage from UV radiation.

  • She also had several asymptomatic pink-tan cutaneous papules, involving the first and second ray of the left hand and wrist, clinically resembling a linear epidermal nevus.
  • The features were remarkably similar to a novel entity described by Finn and Argenyi as congenital panfollicular nevus.
  • In our case, the congenital panfollicular nevus was associated with distal thumb polysyndactyly, which may suggest an important link between limb patterning and hair follicle development.

  • Melanocytic tumours are important in horses, especially grey horses.
  • Intradermal common melanocytic naevi, cellular blue naevi and combined cellular blue naevi are subgroups of human melanocytic tumours, which have not been reported in horses.
  • In this study, we describe 20 horses with skin tumours similar to these naevi of humans.
  • These tumours represented individual skin masses in male and female horses of different breeds.
  • Tumours resembling human intradermal common melanocytic naevi were noted in 12 horses aged between 2 and 17 years.
  • A combined cellular blue naevus-like tumour was diagnosed in a 20-year-old horse.
  • Tumours similar to intradermal common melanocytic naevi were composed of nests of round and spindeloid neoplastic cells, often embedded in myxomatous stroma.
  • The combined cellular blue naevus-like tumour resembled human cellular blue naevus with in addition, an overlying junctional common melanocytic naevus.
  • These equine skin tumours differ from the commonly recognized equine melanocytic tumours by their cytomorphological features, random location and the absence of an increased tumour frequency in grey horses.
  • The resemblance of these tumours to three distinct subgroups of human naevi expands the complexity of equine proliferative cutaneous melanocytic lesions.

  • Today, the designation "capillary malformation" is widely used as a modern name for what was formerly called a nevus flammeus or port-wine stain.
  • There are at least nine different skin disorders fulfilling the criteria of a capillary malformation.
  • Examples include nevus anemicus, cutis marmorata telangiectatica congenita, angiokeratoma circumscriptum, and several vascular lesions that, in the author's view, do not represent nevi, such as the nuchal or glabellar salmon patch and the cutaneous changes of Rendu-Osler disease.

  • Larra bicolor F. (Hymenoptera: Crabronidae) is a specialist parasitoid of Scapteriscus (Orthoptera: Gryllotalpidae) mole crickets, attacking adults and medium to large nymphs of the hosts.
  • The number of immature wasps (eggs and larvae) parasitizing mole crickets was positively related to host density but also declined with distance from a plot of S. verticillata out to 200 m, based on samples of pitfall trap-collected mole crickets.
  • The occurrence of parasitized mole crickets at a 200-m distance suggests that female wasps recruited to a plot of S. verticillata forage for hosts out to at least 200 m.
  • This in turn suggests that mole cricket populations might be diminished by planting plots of S. verticillata at least 400 m apart when L. bicolor wasps are present.
