  • OBJECTIVE: To evaluate the apparent diffusion coefficient (ADC) of benign and malignant salivary gland tumors in comparison to histopathological findings.
  • MATERIALS AND METHODS: This study included 32 patients with a wide spectrum of major salivary gland tumors (17 benign, 15 malignant).
  • Comparisons were made of mean ADC values between benign and malignant tumors, and among tumors showing different degrees of extracellular components.
  • RESULTS: Mean ADC values were 1.09+/-0.34 x 10(-3) mm(2)/s in malignant salivary gland tumors and 1.40+/-0.43 x 10(-3) mm(2)/s in benign salivary gland tumors.
  • No significant difference in mean ADC values was found between benign and malignant tumors (P>0.05).
  • Mean ADC values were significantly different between mild and moderate degrees (P<0.05) of extracellular components, and between mild and conspicuous degrees (P<0.05), in both benign and malignant tumor groups.
  • CONCLUSION: In this study, ADC values alone did not allow differentiation between benign and malignant salivary gland tumors.
  • Comparison with histopathological findings suggests a correlation between the amount of extracellular components and mean ADC values in salivary gland tumors.

  • BACKGROUND AND AIMS: Salivary gland tumors are rare and specific lesions.
  • There are differences in the incidence and frequency of salivary tumors in both minor and major salivary glands in different countries.
  • This study was carried out to determine the prevalence of major and minor salivary gland tumors in Ahvaz in the south of Iran.
  • RESULTS: Of 112 cases, 84 cases were benign and 28 cases were malignant.
  • The incidence of malignant salivary gland tumors increased with age and male predominance was found in malignant tumors.
  • The majority of the tumors occurred in parotid gland.
  • CONCLUSION: It was shown that the peak incidence age of all salivary gland tumors was the third to fifth decades, and malignant tumors mostly occurred in the sixth to eighth decades.
  • Pleomorphic adenoma was the most common benign tumor.

  • Pleomorphic adenoma is the benign tumor of salivary glands, which originates from the myoepithelial cells and intercalated duct cells.
  • This tumor is more common in major salivary glands.
  • This case report describes a rare and unusual lesion in a 55-year-old female, which was diagnosed as pleomorphic adenoma of the minor salivary glands in the upper lip.
  • The tumor was a circumscribed, submucosal nodule, about 2.0 cm in diameter and was characterized by slow growth and rubbery consistency.
  • Complete excision was performed and the histopathological analysis showed an epithelial salivary gland tumor with islands of plasmacytoid cells, duct like structures, in a variable stroma with chondroid, fibrous and myxoid appearance.

  • The human kallikrein 8 protein (KLK8) is expressed in many normal tissues including esophagus, skin, testis, tonsil, kidney, breast, and salivary gland, and is found in biological fluids including breast milk, amniotic fluid, seminal fluid and serum.
  • The aim of this study was to determine whether KLK8 is expressed in salivary gland tissues and salivary gland tumors (both benign and malignant), in order to compare normal with tumor tissues.
  • Pleomorphic adenomas, adenoid cystic carcinomas, polymorphous low grade adenocarcinomas, acinic cell carcinomas, mucoepidermoid carcinomas, and adenocarcinomas NOS of both minor and major salivary glands were examined.
  • The results of this study indicate that most salivary gland tumors show high levels of expression of KLK8.

  • PURPOSE: Of all benign salivary gland tumors of the parotid gland, pleomorphic adenoma (mixed tumor) is the most common.
  • It accounts for 60% to 70% of all benign tumors of the parotid gland.
  • This neoplasm arises in patients in the fourth to sixth decade of life, with a female predominance.
  • The surgical excision of this lesion continues to be the subject of major debate.
  • PATIENTS AND METHODS: We reviewed 48 patients who underwent excision of pleomorphic adenoma of the parotid gland between 1996 and 2005 at Rambam Medical Center: 18 were treated surgically with the classical superficial parotidectomy (SP) technique, using an anterograde approach, and 30 were treated with retrograde partial superficial parotidectomy (PSP).

  • Petraki et al have previously described presence of hK13 in salivary gland tissue, localized to duct epithelia and some acinar cells.
  • The aim of this study was to determine whether hK13 is expressed in salivary gland tissues and salivary gland tumors (both benign and malignant), in order to compare normal with tumor tissues.
  • Pleomorphic adenomas (PA), adenoid cystic carcinomas (ACC), polymorphous low grade adenocarcinomas (PLGA), acinic cell carcinomas (ACI), mucoepidermoid carcinomas (MEC) and adenocarcinomas not otherwise specified (ANOS) of both minor and major salivary glands were examined.
  • The results of this study indicate that most salivary gland tumors show high levels of expression of hK13.
  • Overall, staining in PA was significantly less than that seen in normal salivary gland tissue.
  • PLGA, ACC and ANOS each stained significantly more than normal salivary gland tissue while MEC and ACI did not.
  • In conclusion, we demonstrate the high expression of hK13 in several common salivary gland tumors.

  • Most occurrences of this benign tumor in the submandibular gland have been treated surgically without difficulty via transcervical approach.
  • In this study, we introduced the intraoral approach for the removal of the submandibular mixed tumor.
  • In total, 12 cases of pleomorphic adenoma of the submandibular gland were treated via intraoral approach.
  • During surgery the tumor and submandibular gland are easily dissected from surrounding tissue and removed through intraoral incision.
  • All patients were followed up for the recurrence of the tumor from 20 months to 10 years.
  • One patient showed a tumor recurrence at the early stage of this approach.
  • We propose that the benign mixed tumor of the submandibular gland could be removed easily via intraoral route without an external scar or nerve injury.

  • OBJECTIVE: This retrospective study aimed to investigate the types and distribution of neoplasm of salivary glands in a Turkish population.
  • STUDY DESIGN: The histological diagnosis records of the Department of Pathology at Cumhuriyet University were reevaluated for 125 patients who were treated for salivary gland tumors from 1987 to 2008.
  • RESULTS: A total of 125 primary salivary gland neoplasms, consisting of 95 (76%) benign and 30 (24%) malignant groups were recorded.
  • The most common major and minor salivary gland sites were the parotid (61.6%) and palatal glands (9.6%), respectively.
  • Pleomorphic adenoma was the most frequent benign tumor followed by Warthin's tumor.
  • CONCLUSIONS: Although there were some discrepancies, the characteristics of salivary gland tumors of Turkish patients are in line with those of patients from other countries according to tumor type, tumor site distribution, and age and sex of patients.

  • Salivary gland tumors are frequently characterized by recurrent chromosome translocations, which have recently been shown to result in pathogenetically relevant fusion oncogenes.
  • These genes encode novel fusion proteins as well as ectopically expressed normal or truncated proteins, and are found in both benign and malignant salivary gland tumors.
  • The major targets of the translocations are DNA-binding transcription factors (PLAG1 and HMGA2) involved in growth factor signaling and cell cycle regulation, and coactivators of the Notch (MAML2) and cAMP (TORC1) signaling pathways.
  • Identification of these fusion oncogenes has contributed to our knowledge of molecular pathways leading to epithelial tumors in general, and to salivary gland tumors in particular.
  • Interestingly, the fusions in salivary gland tumors do not seem to be as tumor type specific as those in leukemias and sarcomas.

  • BACKGROUND: Epithelial salivary gland tumors are very uncommon in pediatric age.
  • These results are presented in conjunction with a literature review of salivary tumors with a view to providing an up-to-date overview of the clinical course, prognosis, and treatment options for this rare tumor.
  • PROCEDURE: Fifty-two cases of epithelial salivary tumors were reviewed and the clinical-pathological information concerning tumor characteristics, therapy, and follow-up were collected.
  • RESULTS: The major salivary glands were the main site of tumor occurrence (79% of cases arose in parotid glands); 37 patients had benign tumors (pleomorphic adenoma), 15 had malignant tumors (12 mucoepidermoid carcinoma, 9 low grade).
  • All the patients were treated by surgery; local relapses after parotidectomy were 4% and 25%, in benign and malignant tumors, respectively.
  • When tumor enucleation was performed, recurrences occurred in 50% of benign neoplasms.
  • At the time of the report, all patients with benign tumors were alive, 35(95%) without evidence of disease; only one patient with malignant tumor died of disease.
  • CONCLUSIONS: Epithelial salivary glands tumor in children had different characteristics compared with their adult counterpart with respect to the frequency of histotypes and site of occurrence, but their prognosis seems to be similar.
  • Parotidectomy (total or superficial) is the best choice for achieving good cure rates in both benign and malignant tumors.

  • Non-lymphoid mesenchymal neoplasms of salivary gland origin are rare, accounting for 1.9-5% of major salivary gland tumors.
  • We describe the clinico-pathologic features of 18 cases of mesenchymal neoplasms of the major salivary glands experienced at Asan Medical Center, Seoul, Korea, from 1998 to 2004.
  • Mesenchymal neoplasms accounted for 3.4% of the total of 524 major salivary gland tumors.
  • The parotid gland was the preponderant site (15 cases).
  • Thirteen tumors were benign, constituting 3.5% of the total of 371 benign neoplasms.
  • Schwannomas were the most common benign tumors (six cases), followed by lipomas (three cases), plexiform neurofibroma, hemangioma, desmoid tumor, and solitary fibrous tumor (one each).
  • The malignant tumors consisted of one dermatofibrosarcoma protuberans, synovial sarcoma, leiomyosarcoma, pleomorphic liposarcoma and desmoplastic small round cell tumor each.
  • Genetic analysis for fusion transcripts was conclusive in the diagnosis of desmoplastic small round cell tumor, which is extremely rare at this location.
  • Awareness of the development of various mesenchymal tumors in the major salivary glands could increase the accuracy of preoperative and postoperative diagnosis, and therapeutic efficacy.

  • Petraki et al have previously described presence of hK13 in salivary gland tissue, localized to duct epithelia and some acinar cells.
  • The aim of this study was to determine whether hK13 is expressed in salivary gland tissues and salivary gland tumors (both benign and malignant), in order to compare normal with tumor tissues.
  • Pleomorphic adenomas (PA), adenoid cystic carcinomas (ACC), polymorphous low grade adenocarcinomas (PLGA), acinic cell carcinomas (ACI), mucoepidermoid carcinomas (MEC) and adenocarcinomas not otherwise specified (ANOS) of both minor and major salivary glands were examined.
  • The results of this study indicate that most salivary gland tumors show high levels of expression of hK13.
  • Overall, staining in PA was significantly less than that seen in normal salivary gland tissue.
  • PLGA, ACC and ANOS each stained significantly more than normal salivary gland tissue while MEC and ACI did not.
  • In conclusion, we demonstrate the high expression of hK13 in several common salivary gland tumors.

  • BACKGROUND: Metastasizing pleomorphic adenoma is an uncommon malignant tumor of the salivary gland demonstrating benign epithelial and modified myoepithelial elements intermingling with tissue of mucoid, myxoid, or chondroid appearance that inexplicably manifests local or distant metastasis.
  • OBJECTIVE: To determine the expression of hormonal receptor in the patients with metastasizing pleomorphic adenoma of the major salivary gland.
  • CONCLUSION: These are the first reported cases of metastasizing pleomorphic adenoma of the major salivary glands associated with expression of progesterone receptor in both specimens of sialoadenectomy and lymph node biopsy.
  • This finding supports the pathogenesis of benign metastasis of tumor.
  • However, the role of hormonal receptor in the pathogenesis and treatment of salivary metastasizing pleomorphic adenoma should be further investigated.

  • These tumors are mainly located in the major salivary glands and have sometimes abundant, acellular, mucoid or hyaline stroma but lack chondroid and myxochondroid foci, probably representing the one end of the spectrum of pleomorphic adenoma.
  • Lipomatosis in the form of isolated small islands or scattered single lipocytes, is quite uncommon, and a large amount of adipose tissue in a pleomorphic adenoma and myoepithelioma is a rarity and only described in major salivary glands.
  • We present the case report of a 38-year old man with a myoepithelioma of the minor palatinal salivary glands with extensive lipomatosis as an example of this rare phenomena.

  • PURPOSE: Evaluation of diagnostic accuracy of preoperative fine needle aspiration cytology (FNAC) in salivary gland lesions.
  • PATIENTS AND METHODS: This is a retrospective study of 82 patients presented at NCI, Cairo University with salivary gland lesion who underwent preoperative FNAC diagnosis with subsequent excision and histopathologic assessment.
  • RESULTS: Our study included 82 patients who underwent preoperative FNAC of major salivary glands with subsequent surgical excision.
  • Parotid gland was involved in (68.3%), submandibular in (28%) and submental gland in (3.7%).
  • Forty cases (48.8%) were cytologically diagnosed as benign lesions, 26 (31.7%) were malignant and 10 (12.2%) were suspicious.
  • The most common benign cytologic diagnosis was pleomorphic adenoma; 16 out of 40 cases (40%), while the most common malignant tumor was carcinoma; 22 out of 26 cases (84.6%).
  • The rates of agreement of histopathologic type for benign and malignant tumors were 89.2% and 91%, respectively.

  • BACKGROUND: Pleomorphic adenoma (PA) is the most common neoplasm of the major salivary glands.
  • There are three subtypes of malignant PA: carcinoma ex pleomorphic adenoma (CXPA); carcinosarcoma (true malignant mixed tumor) and metastasizing pleomorphic adenoma.
  • Own material is presented because of rarity and clinicopathologic specificity of this neoplasm.
  • The following clinical factors were evaluated: age, sex, symptoms (time of lasting, evolution), tumor size, invasion of the adjacent structures and facial nerve, neck nodes, clinical stage, treatment outcome.
  • One pathologist reviewed histological material of 18 out of 19 patients who had been operated and pathological factors such as sensitivity and accuracy of fine needle aspiration biopsy, grade, histology and proportion of malignant component in tumor, lymph nodes metastases were analyzed.
  • In 15 cases, tumor was localized in the parotid and in 4 in the submandibular gland.
  • Duration of symptoms of benign PA was from 2 to 40 years (mean 17.8 years).
  • Symptoms of malignant transformation occurred in 15 patients, the most common were rapid enlargement of tumor, pain and facial nerve palsy.
  • Tumor size varied from 2 to 20 cm with a mean of 6 cm.
  • Adjacent structures were invaded by neoplasm in 5 cases.
  • The best method of prevention and treatment of CXPA is early and radical removal of all major salivary glands tumors.

  • OBJECTIVE: To describe the clinical and cytopathologic findings in 15 cases of salivary gland myoepithelial neoplasms and to assess the value of fine needle aspiration in the diagnosis of these tumors.
  • STUDY DESIGN: Between 2000 and 2007, 15 consecutive patients were diagnosed with benign and malignant myoepithelioma of the head and neck region.
  • A literature review of PubMed using the search terms myoepithelioma, myoepithelial carcinoma and salivary gland was performed.
  • Parotid gland consisted of 73% of all primary sites.
  • There were 10 benign and 5 malignant myoepitheliomas.
  • In the benign group, only in 1 case did fine needle aspiration (FNA) revealed benign spindle cell variant, and in others, FNA diagnosis was mixed tumor.
  • The predominant cell morphology in the benign group was plasmacytoid cells, followed by a spindle and clear cell population.
  • Scant fibrillary myxoid material was observed in both benign and malignant specimens.
  • In the literature review and by analyzing the pooled data from 24 major reported series including 601 patients, the mean age at diagnosis was 51 years, and the mean male/female ratio was 1/.01.
  • In addition, 39% of all tumors were malignant, and parotid gland consisted of 45% of all primary sites.
  • CONCLUSION: According to the findings of the present study and review of the literature, the cytologic criteria for preoperative diagnosis ofsalivary gland myoepithelial neoplasms remain unsatisfactory and need to be clarified.

  • RESULTS: Out of the 713 epithelial salivary tumors, 356 (49.9%) were classified as benign and 357 (50.1%) as malignant, indicating a benign-to-malignant ratio of 1:1.
  • The majority of both benign (29%) and malignant (39%) tumors occurred in the minor salivary glands.
  • Malignant tumors were slightly more common in minor salivary glands, indicating a benign-to-malignant ratio of 1:0.75.
  • Among the minor salivary sites, palate was the most frequent location, accounting for 51% of the tumors (246 out of 486).
  • Pleomorphic adenoma was the most frequent tumor, representing 38% of all tumors (274 out of 713), followed by mucoepidermoid carcinoma 21.6% (154 out of 713) and adenoid cystic carcinoma 13.5% (96 out of 713).
  • The commonest benign and malignant tumors were pleomorphic adenoma and mucoepidermoid carcinoma in both major and minor salivary glands.
  • CONCLUSION: According to the findings of this study, equal frequencies of benign and malignant tumors indicate higher occurrences of malignancies, especially in the minor salivary glands, in the Sri Lankan population.

  • OBJECTIVE: To evaluate the types and distribution of tumors of salivary glands in north Jordanians.
  • STUDY DESIGN: The records of the Department of Pathology at Jordan University of Science and Technology were reviewed for patients who were treated for salivary gland tumors from 1991 to 2002.
  • The tumors were analyzed for age of patient, sex of patient, tumor site, and tumor type.
  • RESULTS: One hundred two true neoplasms (70% benign and 30% malignant) were found.
  • The most frequent benign and malignant neoplasms found were pleomorphic adenoma (54%) and adenoid cystic carcinoma (13%), respectively.
  • The most common major and minor salivary gland sites were the parotid (51%) and palatal glands (20%), respectively.
  • Although most of major gland tumors were adenomas, carcinomas of the minor glands were only slightly less frequent than adenomas.
  • The most frequent minor salivary gland malignant tumors were palatal adenoid cystic carcinoma.
  • CONCLUSION: North Jordanians with salivary gland tumors were found to have similar characteristics with patients of other countries with regard to tumor type, tumor site distribution, and age and sex of patients.

  • The presence of sebaceous glands in salivary neoplasms is frequent, however, and in spite of this, salivary neoplasms constituted partially or entirely of these cells are rarely observed.

  • CASE REPORT: A 58-year old man presented with a recurrent mass in the left parotid gland with a prior diagnosis of monomorphic adenoma in the same localization which had been treated by tumor excision in July 2002.
  • Left superficial parotidectomy followed by radiotherapy into tumor bed and upper neck were carried out in September 2004.
  • CONCLUSION: For an accurate approach in the management of patients, oncocytic adenocarcinoma should be considered in the differential diagnosis of lesions of the parotid gland, most of which are benign.

  • OBJECTIVE: To report our experience with endoscopic surgery of benign tumors and tumor-like conditions of the nose and paranasal sinuses.
  • METHODS: The medical records of 105 patients with benign sinonasal tumor, who had undergone endoscopic surgery for removal of their neoplasm between 1997 and 2003, were retrospectively studied.
  • RESULTS: The studied patients included 32 with juvenile nasopharyngeal angiofibroma, 24 with inverted papilloma, 23 with benign fibroosseous lesions, 18 with pyogenic granuloma, 6 with intranasal hemangioma, and 2 with pleomorphic adenoma.
  • Eight cases of recurrent tumor--2 angiofibroma, 4 inverted papilloma, 1 fibrous dysplasia, and 1 osteoma--were noted and managed endoscopically alone or in combination with Caldwell-Luc approach.
  • The major complication encountered in this study was cerebrospinal fluid leakage (2 cases) that was sealed intra-operatively with local tissue flaps without any further sequela.
  • CONCLUSION: Endoscopic surgery is a valuable tool for removal of benign tumors of the nose and paranasal sinuses in the hands of an experienced surgeon.

  • No data are available, however, on their expression in pulmonary hamartoma, a benign biphasic tumor consisting of reactive epithelial cells and neoplastic fibromyxoid stroma, cartilage and fat, which shares some morphologic, immunophenotypic, and genotypic features to pleomorphic adenoma of major salivary glands.
  • In conclusion, sex steroid hormone receptor expression is a nonrandom event in pulmonary hamartoma, and may be related to the development and growth of this tumor.

  • OBJECTIVE/HYPOTHESIS: Fine needle aspiration (FNA) is an accurate, cost-effective tool for the study of salivary gland lesions.
  • Although complications are extremely rare, salivary gland FNA can lead to hemorrhage, facial nerve injury, and cellulitis at the needle puncture site.
  • Some studies suggest that FNA can cause infarction or metaplastic transformation of benign Warthin's tumors.
  • We review our recent experience with FNA of the parotid gland, focusing on possible complications and pitfalls.
  • STUDY DESIGN AND SETTING: The medical records of all patients who underwent FNA of the parotid gland from 2000 to 2002 in the Department of Otolaryngology-Head and Neck Surgery of a major tertiary-care referral center were reviewed.
  • RESULTS: Of the 256 lesions aspirated, 99 patients (39%), were cytologically diagnosed as benign tumors, including 31 (12%) Warthin's tumors, of which 16/17 resected and confirmed histologically.
  • Five patients with Warthin's tumor had post-FNA parotitis and were treated accordingly.
  • CONCLUSIONS: The combination of cystic spaces surrounded by oncocytic cells and a poor blood supply makes the tumor susceptible to infarction and inflammation.
  • Our findings indicate that FNA is a strong and reliable tool in the investigation of the salivary glands.
  • Nevertheless, when Warthin's tumor is clinically suspected on the basis of its location (tail of the parotid gland), cystic texture, patient sex (male) and age, one should consider parotitis as a possible complication.

  • Carcinoma ex-pleomorphic adenoma (CXPA) is an aggressive salivary gland malignancy, usually derived from a long-standing or a recurrent benign tumor, the pleomorphic adenoma (PA).
  • In the context of dynamic reciprocity, changes in the composition and structure of extracellular matrix proteins and cell surface receptors have been frequently associated with dysfunctional adhesion and invasive behavior of tumor cells.
  • In this study, different progression stages of CXPA were investigated regarding the expression of the major extracellular matrix proteins, collagen type I, and of E-cadherin and beta-catenin, the components of adherens junctions.
  • By immunohistochemical analysis, we have demonstrated that direct contact of tumor cells with fibrillar type I collagen, particularly near the invasive front and in invasive areas prevailing small nests of CXPA cells, could be associated with reduced expression of the E-cadherin and beta-catenin adhesion molecules and with invasive behavior of epithelial, but not of CXPA with myoepithelial component.

  • OBJECTIVE: Kaposi's sarcoma is a common malignancy in patients infected with HIV but is rarely seen in the major salivary glands.
  • If a patient is known to be HIV-positive, however, Kaposi's sarcoma must be considered in the differential diagnosis of salivary gland masses in addition to the benign and malignant neoplasms that occur in immunocompetent patients.
  • We present a unique case in which an otherwise healthy patient was diagnosed with HIV after resection of his enlarged parotid gland revealed Kaposi's sarcoma.
  • The patient underwent a right superficial parotidectomy for a suspected diagnosis of Warthin's tumor, given the bilaterality of the lesions.
  • CONCLUSIONS: Although Kaposi's sarcoma is common in AIDS patients, there are few case reports of this malignancy arising in the salivary glands.
  • Previously reported cases include salivary gland Kaposi's sarcoma in known HIV-positive patients and a handful of reports in patients without confirmed immunocompromise.
  • The patient presented here is unique because the diagnosis of parotid gland Kaposi's sarcoma led to a new diagnosis of HIV.
  • This interesting case reiterates the need for complete history taking and the inclusion of Kaposi's sarcoma in the differential diagnosis of salivary gland masses in the appropriate patient population.
