  • In this setting, the pathologist's diagnosis plays a central role in determining whether a resection is needed, and if so, how much to resect and whether it was adequate.
  • OBJECTIVE: To review the intraoperative consultation of the gastrointestinal tract, appendix, and peritoneum.
  • DATA SOURCES: The MEDLINE database was queried for keywords, including gastrointestinal, esophagus, stomach, esophageal, gastric, small intestine, and all other names of the gastrointestinal tract, peritoneum, and appendix in combination with frozen section.
  • Close interaction with the surgeon and knowing what is intended from the frozen section, what will be done following a certain diagnosis, and what is the minimal information needed from the pathologist at the time of frozen section are essential for proper patient management.

  • [Title] Secondary torsion of vermiform appendix with mucinous cystadenoma.
  • Torsion of the vermiform appendix is a rare disorder, which causes abdominal symptoms indistinguishable from acute appendicitis.
  • We report a case (a 34-year-old male) of secondary torsion of the vermiform appendix with mucinous cystadenoma.
  • This case was characterized by mild inflammatory responses, pentazocine-resistant abdominal pain, and appendiceal tumor, which was not enhanced by the contrast medium on computed tomography presumably because of reduced blood flow by the torsion.
  • These findings may be helpful for the preoperative diagnosis of secondary appendiceal torsion.

  • OBJECTIVE: To investigate the frequency and type of diagnostic errors resulting from CT of children with suspected appendicitis when compared to surgical and pathological diagnosis.
  • Isolated CT findings of an enlarged (greater than 6 mm) appendix, fat stranding, thickened bowel or non-visualization of the distal appendix were the most common false-positive CT findings.
  • CONCLUSIONS: Isolated CT findings of an appendicolith, an enlarged appendix, or minimal fat stranding are not sufficient signs for the diagnosis of appendicitis.
  • Pathological diagnosis rather than surgical findings should be used as the reference standard of true-positive imaging findings.

  • IL-6, Tumour necrosis factor (TNF), have also been labelled to be responsible for depressed excretory function of liver and may lead to increase in SB level without rise in liver enzymes.
  • AIM: To evaluate hyperbilirubinemia associated in acute inflammation of appendix (acute appendicitis and its complication).
  • Clinically suspected cases were subjected to investigations to confirm the diagnosis.
  • Subsequently these cases were operated and clinical diagnosis was confirmed per-operatively and post operatively by histopathological examination of the specimen.
  • Among 45 cases diagnosed as acute appendicitis clinically (preoperatively), per operatively, 36 cases had inflamed appendix, 3 cases had gangrene, 5 cases had perforation with peritonitis (4 localized and 1 generalized peritonitis) and only a single case was noted to be of normal appendix (Table 4).
  • Firstly, There was Hyperbilirubinemia in 86.6% of the patients of acute inflammation of appendix (i.e. acute appendicitis and its complications).

  • An enlarged fluid-filled appendix (>7 mm in diameter) was considered an abnormal finding.
  • An appendix with a diameter of 6-7 mm was considered an inconclusive finding; in those cases, the presence of periappendiceal inflammation was used for the final diagnosis.
  • Three radiologists retrospectively assessed the visualization of the appendix by using a 5-point scale.
  • In the three patients with inconclusive results, the appendix was not seen in two patients and was borderline enlarged (7 mm in diameter) in the third.
  • CONCLUSION: MR imaging is an excellent modality for use in excluding acute appendicitis in pregnant women who present with acute abdominal pain and in whom a normal appendix is not visualized at US.

  • BACKGROUND: Several studies reported that pathology of the appendix is frequently detected alongside endometriosis, especially with chronic pelvic pain.
  • Only three (3.3%) of the 106 women had abnormal findings on gross inspection during laparoscopic surgery: two women with endometriotic spots on the surface of their appendixes, and one with peri-appendiceal inflammation with severe adhesions.
  • Of the 106 resected appendixes, 37 (34.9%) had histopathologically confirmed pathology including lymphoid hyperplasia in 12 (11.3%), endometriosis in 14 (13.2%), peri-appendicitis and serositis in five (4.7%), carcinoid tumour in three (2.8%), and others in three (2.8%).
  • CONCLUSIONS: In all surgical treatments for ovarian endometrioma, surgeons need to preoperatively inform the patients of the fact that appendiceal pathology including endometriosis is found frequently regardless of concurrent symptoms or gross finding of the appendix.
  • Furthermore, surgeons should take into account the possibility of appendiceal pathology during operation.

  • RESULTS: Of the 233 patients studied, 174 underwent surgery: three had a normal appendix on histological exam.

  • The purpose of this study was to investigate the effects of an elevated hydrostatic pressure of hydrocele on the structural integrity and steroid receptor expression pattern of the appendix testis in children.
  • The tissue sections of appendix testis expressed both androgen and estrogen receptors in all patients in groups A and B, and epithelial destruction was not present.
  • The presence of androgen receptor (two of 11, P < 0.001) and estrogen receptor (four of 11, P = 0.006) was lower and the number of appendix testes with epithelial destruction was higher (eight of 11, P = 0.001) in group C.

  • US findings showed involvement of the cecum, appendix, ascending colon and proximal middle transverse colon, with features resembling gas containing fissures within the colon wall itself.
  • DISCUSSION: A prompt bedside US examination upon development of symptoms allowed an early diagnosis of NEC and identified features consistent with imminent colon wall rupture, shifting the management of this life-threatening complication from medical to surgical.

  • BACKGROUND: Teratomas very rarely arise from the appendix.
  • To our knowledge, only one prior case of mature teratoma involving the appendix has been reported in the medical literature.
  • CASE: Our case is the second reported case of mature cystic teratoma involving the appendix, and, to our knowledge, it is the first reported in a female who had two simultaneous teratomas, one arising from the appendix and one arising from the right ovary.
  • CONCLUSION: Although mature cystic teratoma is a rare tumor of the appendix, it should be considered in the differential diagnosis of appendiceal masses.

  • INTRODUCTION: Mucinous adenocarcinoma is a rare neoplasm of the gastrointestinal tract and one of the three major histological subtypes of the primary appendiceal adenocarcinoma.
  • The most common type of presentation is that of acute appendicitis and the diagnosis is usually occurred after appendectomy.
  • The accurate preoperative diagnosis and management of the above condition represents a real challenge when uncommon anatomic anomalies such intestinal malrotation and situs inversus take place.
  • Both preoperative US and CT scan of the abdomen and pelvis declared situs inversus, with a characteristic thickening in its wall, appendix situated in the left lower quadrant of the abdomen.
  • These findings reached to the diagnosis of acute appendicitis with situs inversus and a standard appendicectomy was performed.
  • Pathologic evaluation established primary mucinous adenocarcinoma of the appendix and three months afterwards the patient underwent a subsequent extended left hemicolectomy.
  • CONCLUSION: In conclusion, the occurrence of primary appendiceal mucinous adenocarcinoma along with situs inversus, definitely accounts as a unique clinical case.
  • Even synchronous manifestation of primary mucinous adenocarcinoma of the appendix and situs inversus totalis represents an unusual anatomo-pathological entity, all physicians should be familiar having the knowledge to make an appropriate and accurate diagnosis that will lead to prompt and correct treatment.

  • PATIENTS: Authors present 0.1% incidency of diverticulitis of appendix in the group of 1496 patients with appendicitis.
  • Ultrasonographic examination determined the diagnosis of subacute appendicitis in one case, a perforation of sigmoid diverticle in the second case.
  • In both patients, a correct diagnosis of diverticulitis was determined only through histopathological examination.

  • BACKGROUND: Appendiceal adenocarcinoma is rare with a frequency of 0.08% of all surgically removed appendices.
  • Few cases of appendiceal carcinoma infiltrating the bladder wall for spatial contiguity have been documented.
  • CASE PRESENTATION: A case is reported of a 45-years old woman with mucinous cystadenocarcinoma of the appendix with bladder metastasis.
  • Laparotomy evidenced the presence of a solid mass of the appendix (2,5 cm x 3 cm x 2 cm) extending to the loco-regional lymph nodes.
  • The subsequent pathological examination revealed a mucinous cystadenocarcinoma of the appendix with metastatic cells colonising the anterior bladder wall and several colic lymph nodes.
  • CONCLUSIONS: The rarity of the appendiceal carcinoma invading the urinary bladder and its usual involvement of nearest organs and the posterior bladder wall, led us to describe this case which demonstrates the ability of the appendiceal cancer to metastasize different regions of urinary bladder.

  • OBJECTIVE AND METHODS: A retrospective review of a pathology database in a district general hospital identified patients with serosal appendicitis and a preoperative diagnosis of appendicitis.
  • CONCLUSIONS: No further investigation is necessary in patients who undergo an appendicectomy but in whom the pathological diagnosis is serosal appendicitis.

  • Pathology showed inflammed appendix only in 56%, peri-appendiceal abscess/perforation in 17%, "fibrosed" appendix in 10% and normal appendix in 17%.
  • Postoperative infection (wound infection, fever > three days) was seen in 7/24 (29%) of cases with peri-appendiceal abscess/perforation and 2/119 (1.7%) of the other cases.
  • Those with appendiceal abscess/perforation were treated parenterally for mean and SD of 6.56 +/- 2.35 days, not significantly different from others.

  • RESULTS: The primary tumor origin was mostly stomach (73 cases) and colon (61 cases).
  • Krukenberg tumor (pathologically proven signet ring cell carcinoma) was found in 34 cases: stomach (25), colon (2), appendix (1), and unknown (6).
  • The median survival times according to the primary tumor site showed significant differences (p=0.002) and were as follows: stomach 12 months, colon 17 months.
  • However, age, bilateral tumors, chronology of diagnosis and mass size did not affect survival.

  • BACKGROUND/AIMS: Carcinoid tumors of the appendix are thought to be the most common type of appendiceal neoplasms.
  • Although the vast majority of appendiceal carcinoids behave in a benign fashion, they are considered malignant because they all have the potential for invasion, metastasis and production of physiologically active substances.
  • Following confirmation of the diagnosis, secretion of 5-HLAA (5-Hydroxy-Inndole-Acetic Acid) was measured after a 24-hour collection of urine.
  • Also, patients with tumor size more than 1cm underwent a Somatostatin Receptor Scintigraphy (Octreoscan).
  • RESULTS: The most common site for the tumors was the tip of the appendix (18/22).
  • In sixteen patients the tumor size was less than 1cm, in seven patients the tumor diameter was measured to be 1 to 2cm and in one patient the tumor was 3cm.
  • Most of our patients (16/22) underwent only an appendicectomy, while in the rest of them (in the patients with tumor size between 1-2cm and in the patient with invasion of mesoappendix) a right hemicolectomy was performed.
  • CONCLUSIONS: Carcinoid tumors of the appendix, in most cases, are found incidentally during appendicectomies, especially in young females and usually are less than 1cm in size, which is probably the reason of the absence of metastases in all cases.
  • Histological examination and size of the tumor are important factors that contribute to the selection of the surgical treatment and both must be estimated by the surgeons to make the final choice.

  • The present version of the semi-structured interview SCID-E (French translation by Bouvard et al.) evaluates the 10 PD of the DSM-IV, the depressive personality and the passive-aggressive personality, included in the DSM-IV appendix B.
  • It provides diagnoses for all DSM-IV and/or ICD-10 PD as well as the schizotypic disorder.
  • The DIP-I is usually preceded by a general "scan" interview in order to assess an existing personality disorder corresponding to Axis I of the DSM-IV or the ICD-10.
  • The diagnosis was not revealed to the examiner during the study.
  • Globally, the agreement between diagnosis by self-evaluation and diagnosis by semi-structured interview is not very satisfactory.
  • Finally, a cluster analysis of the results of the three semi-structured interviews put together reveals that five patients show at least one PD diagnosed in the three clusters, two have no diagnosis, six patients have one or several PDs in clusters B and C, three patients have some in clusters A and C, and five patients only have some in cluster C.
  • The administration of semi-structured interviews leads to an average of 1.3 diagnosis for the DIP-Q DSM-IV and 1.6 for the ICD against 1.9 PD for the SCID interview.

  • Primary malignant epithelial tumors of the appendix are uncommon.
  • The most common presentation of appendiceal malignancy is right lower abdominal pain suggestive of acute appendicitis.
  • We present the case of a 48-year-old woman with an appendiceal malignancy who presented with symptoms and signs suggestive of complicated diverticular disease with an enterovaginal fistula.
  • From a review of the literature, this is the first report of an appendiceal malignancy presenting in this manner.

  • RESULTS: The radiation dose to the fetus at 0 and 3 months, respectively, was as follows: renal stone protocol, 0.8-1.2 and 0.4-0.7 cGy; appendix protocol, 1.52-1.68 and 2-4 cGy; and pulmonary embolus protocol, 0.024-0.047 and 0.061-0.066 cGy.
