  • Apocrine metaplasia is a very common finding in the female breast after the age of 25.
  • This, however, is only really the case in apocrine sweat glands of the axilla and in the peri-areolar apocrine glands.
  • The apocrine cell does, however, contribute to a number of different breast lesions, some of which are very taxing diagnostically; apocrine variants of both in-situ and invasive cancer are encountered.
  • This review considers the common apocrine metaplastic lesions seen in fibrocystic change as well as apocrine adenoma, apocrine change within sclerosing adenosis, atypical apocrine lesions and apocrine malignancies.

  • The apocrine hidrocystoma is a benign adenomatous cystic proliferation derived from apocrine glands, which frequently occurs in periocular tissues.
  • Although these lesions have been treated successfully with meticulous surgical extirpation and electrosurgery, this report describes the successful treatment of two patients, each with multiple large (>7 mm) periocular apocrine hidrocystomas by either chemical ablation of the cystic epithelium with trichloroacetic acid (TCA) or surgical excision.
  • The treatment of large apocrine hidrocystomas with TCA is an effective and expeditious method of treating these disfiguring and recalcitrant lesions.

  • BACKGROUND: Apocrine cystadenoma (AC) and apocrine hidrocystoma (AH) have been used interchangeably in the literature to designate cystic lesions of apocrine glands.
  • METHODS: We reviewed 21 cases with biopsies of apocrine cystic lesions diagnosed as AH or AC stained by hematoxylin and eosin.
  • CONCLUSIONS: Apocrine cystic lesions with true papillary projections should be referred to as AC rather than AH, to emphasize the proliferative adenomatous growth and depicted by their frequency of cytological atypia and high mitotic activity.

  • [Title] Palpebro-orbital apocrine cystadenoma: immunohistochemical verification of a unique variant with a critical differential diagnosis.
  • PURPOSE: To describe a unique apocrine cystadenoma of the superonasal eyelid and anterior orbit.
  • METHODS: Clinical evaluation with axial and coronal CT; histopathologic and immunohistochemical studies including sections stained with hematoxylin-eosin, periodic acid Schiff, alcian blue, mucicarmine, and Prussian blue for iron; and monoclonal antibodies against cytokeratin-7, epithelial membrane antigen, smooth muscle actin for myoepithelial cells, gross cystic disease fluid protein-15 for apocrine differentiation, and CD-68 and lysozyme for histiocytes.
  • Both the cyst's lining cells and the adenoma expressed gross cystic disease fluid protein-15 indicative of apocrine differentiation.
  • The adenoma displayed inner adlumenal cells that were CK-7 and epithelial membrane antigen positive, and outer myoepithelial cells that were smooth muscle actin positive.
  • CONCLUSION: This unique lesion is the first example in the ophthalmic and dermatopathologic literatures of a solid adenoma encompassed by an apocrine cyst, which more typically features short or blunt papillae.

  • BACKGROUND: The overlap in histopathologic features and immunoprofile of eccrine and apocrine neoplasms confounds basic issues relating to lineage of these entities.
  • RESULTS: CK15 and nestin expression were noted in 39 of 78 (50%) and 36 of 78 (46%) cases in the benign group, respectively (8 cutaneous mixed tumor, 10 hidradenoma papilliferum, 9 apocrine cystadenoma, 11 cylindroma and/or spiradenoma, and 9 poroma/dermal duct tumor).
  • Except 1, both markers were negative in 4 syringocystadenoma papilliferum, 10 hidradenoma, 1 syringofibroadenoma, 10 syringoma, 1 eccrine adenoma, 8 poroma/dermal duct tumor, 5 eccrine hidrocystoma, and 1 apocrine carcinoma.
  • CONCLUSIONS: Given that follicular germinative cells give rise to the folliculosebaceous apocrine unit, expression of CK15 and nestin in the majority of cutaneous mixed tumor, hidradenoma papilliferum, apocrine cystadenoma, and cylindroma/spiradenoma is suggestive of an apocrine origin/differentiation of these neoplasms.

  • Apocrine carcinomas represent a rare group of tumors with a potential for destructive local invasion, regional and distant metastases, and are equally common in both sexes.
  • A case of a 79-year-old woman with axillary apocrine carcinoma associated with apocrine adenoma and apocrine gland hyperplasia is presented.
  • The cells contained abundant eosinophilic, finely granular cytoplasm with pleomorphic nuclei and showed apocrine-like decapitation.
  • In one area, the tumor was lobular and composed of tubular structures lined with one layer of uniform cuboidal or columnar eosinophilic cells, indicating a pre-existing apocrine adenoma.
  • Beneath the tumor, in the deep dermis and subcutaneous tissue, hyperplastic apocrine glands were also found.
  • This and previously reported cases suggest that apocrine hyperplasia and apocrine adenoma may represent successive steps in the development of apocrine carcinoma.

  • The main controversies regarding tubular apocrine adenoma and papillary eccrine adenoma are whether they are two distinct entities or are the very same tumor, and if so, which lineage of differentiation (apocrine versus eccrine) it pursues.
  • We report two cases of tubular adenoma with follicular and, in one case, additionally, sebaceous differentiation.
  • The features in both cases indicated apocrine differentiation of the tubular component.
  • The histologic features that both cases had in common included the combination of a tubular adenoma, foci of follicular differentiation, and areas of immature squamous metaplasia.
  • In conclusion, these two cases of cutaneous tubular adenoma with accompanying follicular and sebaceous differentiation give further support to the proposition that the majority of these neoplasms have apocrine differentiation.
  • Rare cases occurring in the sites normally devoid of apocrine glands may represent the eccrine counterpart.

  • We report on an unusual example of a hamartomatous lesion that showed an abnormal mixture of elements of the folliculosebaceous-apocrine unit and arrector pili muscle.
  • Encased within the mesenchymal component were large infundibulocystic structures and plentiful grouped and scattered roundish solid epithelial nodules, duct-like, glandular, and tubular elements.
  • Some of the epithelial structures were reminiscent of those seen in tubular apocrine adenoma; others resembled syringomatous elements.
  • We interpreted this unusual lesion as a hamartoma of the folliculosebaceous-apocrine unit.

(9) Their appearance has similarities to the apocrine metaplasia commonly seen in fibrocystic change of the breast but they are well-circumscribed tumors with closely packed tubular and papillary structures.

  • Feline eyelid hidrocystoma is a rarely reported disease of the apocrine glands of Moll that has been variably interpreted as proliferative/neoplastic lesion or retention cyst.
  • The purpose of this paper was to investigate feline hidrocystoma pathogenesis by means of a histological and immunohistochemical study.
  • Nine paraffin embedded biopsies of eyelid hidrocystoma were retrieved from our archives.
  • Immunostaining was consistent with apocrine gland epithelium (cytokeratin 8/18; 19 and 14 positive).
  • Although it was not possible to draw a definite conclusion concerning hidrocystoma aetiopathogenesis, the relatively high Ki67-proliferative index observed, suggested a proliferative nature of the lesion.

  • PURPOSE: To describe 6 patients with hybrid cysts of the eyelid that demonstrated follicular and apocrine differentiation.
  • METHODS: Clinicopathologic series of 6 cases obtained from review of 1,099 cases of hidrocystoma diagnosed at the New York Eye and Ear Infirmary from 1990 to 2006.
  • Histopathologic evaluation revealed cystic structures, lined by a combination of apocrine, infundibular (epidermoid), and trichilemmal-type epithelia.
  • CONCLUSIONS: Hybrid cysts can show apocrine, trichilemmal, and infundibular differentiation, suggestive of their origin at the junction of keratinizing squamous and glandular epithelia of the hair follicle.

  • We describe a case of apocrine adenoma with simultaneous occurrence of invasive ductal carcinoma in the breast of a 53-year-old woman.
  • Apocrine adenoma affecting the breast is very rare.
  • The lesion is composed of back-to-back ducts and papillary fronds covered with apocrine cells, and it is sharply demarcated from the surrounding breast tissue.
  • Histologic examination of the excised specimen showed that the hypoechogenic nodule represented an apocrine adenoma in proximity to the invasive ductal breast carcinoma.
