  • PURPOSE: The INK4A-ARF locus at chromosome 9p21 is frequently altered in head and neck squamous cell carcinoma (SCC) and encodes two distinct tumor suppressors, p16(INK4A) and p14(ARF).
  • EXPERIMENTAL DESIGN: p14(ARF) protein expression was assessed by immunohistochemistry in a cohort of 140 patients with SCC of the anterior tongue.
  • CONCLUSIONS: These data show that in patients with SCC of the tongue, combined nuclear and nucleolar expression of p14(ARF) protein predicts for improved DFS and OS independent of established prognostic markers.

  • [Title] Distant metastases of a squamous cell carcinoma of the tongue in peripheral skeletal muscles and adjacent soft tissues.
  • A 66-year-old female patient was admitted to our department with a large tumor of the tongue measuring 10 cm in diameter.
  • Histological analysis revealed a low grade squamous cell carcinoma (SCC) of the tongue.
  • Foci of distant metastases were found in the left upper anterior thoracal wall, near the intraabdominal portion of the aorta, near the right iliac crest and in both the right vastus medialis- and adductor magnus muscles.
  • The final diagnosis was a T4N3M1(G3)(C3) SCC of the tongue with multiple distant thoracal, abdominal and intramuscular metastases.

  • [Title] The effect of desalivation on the malignant transformation of the tongue epithelium and associated stromal myofibroblasts in a rat 4-nitroquinoline 1-oxide-induced carcinogenesis model.
  • The aim of our study was to analyse desalivated rat tongue epithelium for histopathological changes, proliferating cell nuclear antigen (PCNA), and epithelium-associated stromal myofibroblasts [SMF; alpha-smooth muscle actin (alphaSMA)] following 0.001% 4-nitroquinoline 1-oxide (4NQO) administration in drinking water.
  • Tongue length was divided into anterior, middle and posterior 'thirds'.
  • The histopathological changes per 'third' were scored as normal epithelium, hyperplasia, dysplasia, carcinoma-in-situ, and superficial and invasive carcinoma.
  • At all time points, the histopathological changes in the anterior and middle thirds were higher in the desalivated than in the salivated group (P < 0.05) but almost identical in the posterior third (P > 0.05).
  • PCNA scores were significantly lower in the desalivated vs. the salivated group at almost all time points and tongue thirds (P < 0.05).
  • The absence of saliva seems to promote malignant transformation of the tongue epithelium in the early stages.

  • METHODS AND MATERIALS: Thirty-one patients with Stage IV oropharyngeal squamous carcinoma enrolled on a Phase II trial were prospectively evaluated by modified barium swallow studies at baseline, and 6, 12, and 24 months post-IMRT treatment.
  • Twenty-one (68%) cases were base of tongue and 10 (32%) were tonsil.
  • RESULTS: Dose-volume constraints (V30 < 65% and V35 < 35% for anterior oral cavity and V55 < 80% and V65 < 30% for high superior pharyngeal constrictors) predictive for objective swallowing dysfunction were identified by univariate and multivariate analyses.

  • [Title] A comparative analysis of anterior versus posterior squamous cell carcinoma of the tongue: a 10-year review.
  • INTRODUCTION: The study sought to compare and contrast squamous cell carcinoma (SCC) of the anterior mobile tongue with SCC of the tongue base, with emphasis on clinical presentation, management and outcome.
  • METHODS: This was a retrospective, comparative analysis of patients treated for SCC of the tongue over a 10-year period.
  • RESULTS: The study included 142 patients, of whom 86 were treated for SCC of the anterior tongue and 56 for tongue base lesions.
  • Patients with carcinoma of the anterior tongue tended to present with a visible lump or ulceration of the tongue, whereas the majority of patients with tongue base SCC presented with pain.
  • Sixty per cent of anterior tongue lesions were early stage (I or II) at initial presentation as compared with 21 per cent of tongue base lesions.
  • CONCLUSION: Patients with anterior tongue lesions had a better prognosis, but this was not statistically significant when adjusted for stage.

  • CASE PRESENTATION: We present the case of a 44-year-old Iranian woman with Cacausian ethnicity with a painless anterior neck mass that appeared gradually over three months.
  • She had a history of frequent painful swelling of the anterior part of her neck, which subsided with antibiotic therapy.
  • A computed tomography scan revealed a large, heterogeneous enhancing soft tissue mass with cystic components in the midline of the anterior neck space.
  • This extended from the base of the tongue,(completely separated from its muscles, to the inferior aspect of the thyroid gland and showed the destruction of the hyoid bone and the thyroid cartilage.
  • A fine needle aspiration revealed papillary carcinoma.

  • Median rhomboid glossitis (MRG) is an uncommon benign abnormality of the tongue, most frequently affecting men.
  • It is typically located around the midline of the dorsum of the tongue, anterior to the lingual "V", appearing as a reddish, rhomboid area, depapillated, flat maculate or mamillated and raised by 2 - 5 mm.
  • This paper reports a case of rhomboid glossitis in a 61-year-old man who consulted for a painless raised lesion on the dorsum of the tongue, in left paramedial (not medial) location.
  • Other diagnoses considered but ruled out on the basis of the clinical and histopathological findings were haemangioma, pyogenic granuloma, amyloidosis, granular cell tumour, and squamous cell carcinoma.

  • [Title] Carcinoma of tongue and the buccal mucosa represent different biological subentities of the oral carcinoma.
  • PURPOSE: Clinico-epidemiological studies show that the behaviour of the tongue cancer is different from the cancer originating at other sites of the oral cavity.
  • METHODS: In the present study, we have attempted to see whether any difference existed in the cell cycle regulatory mechanism of these tumours by comparing immunohistochemically the expression of major cell cycle regulatory proteins in 147 buccal and 94 tongue carcinoma (anterior two-third of tongue) prospectively.
  • RESULTS: On comparison of buccal and tongue carcinoma, expression of p16 and p21 showed significant difference.
  • In combined analysis, simultaneous down regulation of p16 and p21 was seen in 47% of tongue cancer cases as against 28% in buccal carcinoma (P=0.004).
  • In univariate analysis, none of the clinico-biological variables studied showed significant association with survival in tongue carcinoma, whereas, some of the clinico-biological variables associated with survival in buccal carcinoma.
  • In multivariate analysis, the expression pattern of p53 and p16 protein influences the DFS whereas cyclin D1 expression showed independent association with the OS in buccal carcinoma.
  • CONCLUSIONS: Thus, tongue and buccal cancers represent different biological subentities, and such differences should be considered in oral cancer management.

  • We describe what is, to the best of our knowledge, the first use of a double bilobed radial forearm free flap in reconstructive surgery of the tongue and floor of the mouth following bilateral tumor resection.
  • Our patient was a 78-year-old man who had experienced tumor recurrence in the anterior floor of the mouth after previous resection and radiotherapy.
  • Eleven weeks postoperatively, the patient could extend his tongue to his hard palate and past his mandibular alveolus anteriorly.
  • Although the use of a single bilobed radial forearm flap is widely used after hemiglossectomy, our double bilobed modification extends this technique to anterior tongue and floor-of-mouth defects.
  • This technique provides adequate bulk while allowing for depth of a ventral sulcus that will minimize tethering of the tongue and reduce oral incompetence.

  • MATERIAL AND METHODS: We used the radial forearm free flap (RFFF), anterior lateral thigh flap (ALTF) or fibular osteocutaneous flap (FOCF) to reconstruct the oral defect after radical resection in 39 patients undergoing total or nearly total glossectomy with laryngeal preservation.
  • The cumulative 4-year survival rates were 63.8% for tongue cancer and 42.9% for base of tongue cancer.

  • BACKGROUND: There is an increasing risk of cervical lymph node metastases as tumour thickness increases in patients with anterior tongue squamous cell carcinoma (SCC).
  • The role of elective neck treatment in early anterior tongue cancer in unclear.
  • METHODS: Patients diagnosed with anterior tongue cancer and treated with glossectomy +/- neck dissection were identified.
  • RESULTS: Between 1980 and 2002 99 patients (63 male and 36 female) with anterior tongue SCC were treated at Westmead Hospital, Sydney, and had a documented tumour thickness.
  • CONCLUSIONS: Elective treatment to the ipsilateral neck is not indicated in all patients with anterior tongue cancer.

  • OBJECTIVE: To determine the feasibility of cine-MRI for non-invasive swallowing evaluation after surgery for lingual carcinoma with reconstruction using microvascular free flaps.
  • METHODS: Ten patients with stage IV carcinoma of the mobile tongue and/or tongue base treated by surgical resection and reconstruction with a free flap were evaluated after an average of 4.3 years (range: 1.5-11 years), using cine-MRI in "single-shot fast spin echo" (SSFSE) mode.
  • Tongue base-pharyngeal wall contact was observed in 5 cases.
  • An increased anterior tongue recoil, increased mandibular recoil, increased posterior oropharyngeal wall advancement and an increased laryngeal elevation were observed in 4 cases.
  • CONCLUSIONS: Cine-MRI is a safe, non-invasive technique for the evaluation of the mobility of oral and oropharyngeal structures after free-flap reconstruction of the tongue.

  • This study assesses the correlation of changes in the density of stromal myofibroblasts to propagation of histopathologic alterations and proliferative activity of the epithelium in a rat 4NQO-induced tongue rat carcinogenesis model.Forty-three male Wistar rats were administered 0.001% 4NQO in drinking water for: 0 (n=7, control), 7 (n=4), 8 (n=8), 14 (n=6), 22 (n=9), and 28 (n=9) weeks, after which they were euthanized.
  • Tongue sections were divided equally into anterior, middle and posterior thirds.
  • Each third was given a histopathologic score (hematoxylin and eosin), ranging from normal, hyperplasia/hyperkeratosis, and escalating degrees of dysplasia to carcinoma, and analyzed by the point-counting method for density of epithelium-associated stromal myofibroblasts (alphaSMA) and of proliferating epithelial cells (PCNA).
  • Histopathologic changes significantly increased in severity (carcinoma) with duration of 4NQO administration (p<0.001) in the posterior third of the tongue.
  • The density of stromal myofibroblasts and proliferating epithelial cells was significantly higher in the posterior third of the tongue at 28 weeks compared to the other time points and locations (p<0.001, p=0.01, respectively).
  • Significant correlations were found between occurrence of carcinoma and the increase in density of stromal myofibroblasts (p<0.001) and of proliferating epithelial cells (p=0.001) in the posterior third of the tongue.
  • Increased density of stromal myofibroblasts was distinctively associated with the development of carcinoma but not with pre-malignant changes.
  • Defining the mechanism of evolvement of carcinoma-associated stromal myofibroblasts is expected to further broaden our knowledge on the micro-environmental events occurring during the malignant transformation.

  • BACKGROUND: Resections of oromandibular squamous cell carcinoma involving anterior mandible, floor of the mouth, and the skin, lead to composite oromandibular defects that can be approached in several ways depending on the extension of the bone defect, of the soft tissue and cutaneous resection, the patient's general status, and the prognosis.
  • CONCLUSION: Reconstruction of the anterior mandible is strongly indicated whenever possible.
  • When the defect involves the tongue, the best results are provided by the association of two free flaps.

  • Measurements of IFP in normal tongue and in tongue cancer are lacking.
  • MATERIALS AND METHODS: In the present study, tongue squamous cell carcinoma (SCC) was induced by adding the carcinogen 4-nitroquinoline oxide in drinking water for 19 weeks.
  • RESULTS: In normal tongue, IFP averaged 3.1 +/- 0.3 mmHg.
  • The IFP, both in the tumor (29.1 +/- 2.9 mmHg) and 0.5 cm anterior to it (15.4 +/- 2.1 mmHg) was consistently increased (P < 0.005) with values ranging from 10 to 40 mmHg.
  • CONCLUSIONS: Our data show that IFP is increased in tongue cancer.

  • RESULTS: Lesions localized in the base of the tongue, pyriform sinus, aryepiglottic folds, and arytenoids were well visualized by both RTV and CTVL.
  • The two techniques were not found effective in identifying lesions of the ventricular bands, ventricular cavities, and the anterior commissure.

  • As concerns the study of the density and complexity of oral lymphatic vessels architecture by means of immunohistochemistry (D2-40, CD31 and Ki-67 antibodies) and fractal analysis, we demonstrated that in normal oral mucosa the geometry of the lymphatic vessels was less complex at the level of the lower lip compared to the anterior part of the oral floor mucosa or the tongue.
  • A comparative analysis between the normal and pathological aspects revealed statistically significant differences between the fractal dimension (FD) of the vessels' outline, especially in the tongue.

  • All cases showing p16 methylation were from the anterior tongue, and 75% of them were young patients.
  • Interestingly, HPV 16 messenger RNA was detected exclusively in HNSCC from the base of tongue lesions and was only found in males.

  • A retrospective analysis of Squamous cell carcinoma of oral cavity with N0 neck from 1998 to 2004 was performed.
  • Seventy two percent of the patients had primary tumors in the anterior two-thirds of the tongue.
  • SND (I-III) is a sound and effective procedure in the management of clinically negative neck in squamous cell carcinoma of the oral cavity.

  • METHODS: With the beagle dog under anesthesia, the anterior wall of the stomach was fixed on the abdominal wall in a similar way to percutaneous endoscopic gastrostomy.
  • If the tongue was manually pulled out forward, observation of the lingual root was possible.
  • CONCLUSION: This procedure is easy and effective for pre-treatment evaluation of the feasibility of endoscopic resection in cases of superficial carcinoma of head and neck.

  • The tumours were predominantly found in the right (43%), anterior (19%) and left (18%) aspects of the lower face.
  • The remainder making up 20% were found in the floor of the mouth, tongue and lips.
  • Seventy eight (45.6%) of the patients presented with lesions that were classified as malignant of which 54 (62%) were diagnosed as squamous cell carcinoma (SCC).
  • Fifty-four (62%) of malignant tumours were squamous cell carcinoma; 58 (93.6%) of the benign odontogenic tumours were classified as ameloblastoma.
  • CONCLUSION: Tumours and tumour-like lesions of the lower face comprising the mandible, tongue and adjacent structures are a diverse group of neoplasm and are seen commonly in practice of Maxillofacial surgery.

  • METHODS: A prospective study of 23 consecutive patients undergoing planned straight midline mandibular osteotomies as part of the ablative surgery for intraoral carcinoma.
  • Twelve patients had primary lesions affecting the anterior two-thirds of the tongue, four at floor of mouth, three at posterior two-thirds of the tongue, two at deep lobe of parotid, one at soft palate and one at tonsil.
  • Various flaps were used for reconstruction : Pectoralis major myocutaneous flap ( n=7 ) , tongue flap ( n=4 ) and radial forearm free vascular flap ( n=2 ) cases.

  • The syndrome also includes mucosal neuromas, typically involving the lips and tongue, intestinal ganglioneuromas and a marfanoid habitus.
  • Multiple endocrine neoplasia type 1 is an autosomal dominant predisposition to tumors of the parathyroid glands (four-gland hyperplasia), anterior pituitary, and pancreatic islet cells; hence, the mnemonic device of the "3 Ps"; multiple cutaneous lesions (angiofibromas and collagenomas) are frequent in patients with multiple endocrine neoplasia type 1.
  • Carney complex may be viewed as a form of multiple endocrine neoplasia because affected patients often have tumors of two or more endocrine glands, including primary pigmented nodular adrenocortical disease (some with Cushing's syndrome), pituitary adenoma, testicular neoplasms, thyroid adenoma or carcinoma, and ovarian cysts.
