  • Desmoplastic melanoma (DM) is an uncommon but potentially devastating malignancy that can be cured with early recognition and surgery.
  • We report a rare case of DM occurring in the ankle in an 85-year-old woman.
  • Magnetic resonance imaging of the affected ankle showed a 4.3 x 4.3 x 2.5 cm sized mass which was located in the subcutaneous region, attached to lower leg muscles, the fibula, and the calcaneus.

  • Melanocytic lesions in certain locations (eg, genital, breast, acral) may have histologic and clinical features simulating melanoma.
  • One hundred fifteen melanocytic lesions from the ankle were retrieved from January 1990 to August 2006 from the files of M. D.
  • Anderson Cancer Center and were classified as benign melanocytic nevi (BN; n=17), DN (n=35), melanomas (MM; n=52), and melanocytic nevi of the ankle with atypical features (MNAAF; ie, cases that did not readily fit in any of the previous categories, n=11).
  • This study highlights a group of melanocytic lesions located on the ankle that share histologic features with acral nevi, DN, and melanoma.
  • Pathologists should be aware of this type of lesions to avoid overdiagnosis of melanoma.

  • BACKGROUND: Malignant melanoma is a rare but potentially lethal form of cancer which may arise on the foot.
  • Evidence suggests that due to misdiagnosis and later recognition, foot melanoma has a poorer prognosis than cutaneous melanoma elsewhere.
  • METHODS: A panel of experts representing podiatry and dermatologists with a special interest in skin oncology was assembled to review the literature and clinical evidence to develop a clinical guide for the early recognition of plantar and nail unit melanoma.
  • Based on these features a new acronym "CUBED" for foot melanoma was drafted and incorporated in the guide.

  • BACKGROUND: Malignant melanoma occurs infrequently in Taiwan.
  • METHODS: To improve our understanding of the rare clinical experiences, we retrospectively investigated clinical characteristics, radiological findings, treatment modalities, survival outcomes and prognoses of 11 Taiwanese patients with osseous metastasis of melanoma treated surgically at two national medical centers, National Taiwan University Hospital and National Cheng Kung University Hospital from January 1983 to December 2006.
  • RESULTS: Six patients suffered from acral-lentiginous melanoma.
  • The interval from the initial diagnosis of melanoma to the clinical detection of osseous metastases varied from 0-37.8 months (mean 9.75 months).
  • Metastatic melanoma was invariably fatal; the mean survival time from bone metastases to death was 5.67 months.
  • CONCLUSION: Due to the high morbidity and poor survival of Taiwanese patients with osseous metastases of melanoma, surgical treatment should be directed towards pain relief and the prevention of skeletal debilitation in order to maintain their quality of life.

  • [Title] The reverse sural flap: modifications to improve efficacy in foot and ankle reconstruction.
  • The aim of this work is to evaluate the efficacy of the reverse sural flap in covering defects in the foot and ankle region when certain technical modifications are employed.
  • We provide a retrospective review of 32 consecutive reverse sural flaps for foot and ankle defects, and compare the technique and results with other reports.
  • Four flaps (12.5%) suffered significant flap loss, and 4 patients had delayed healing.Several modifications are suggested to increase the versatility of the sural flap in covering foot and ankle defects, including preserving the mesentery connecting the sural nerve to the deep fascia, inclusion of skin of the upper third of the leg, limiting pedicle width to 2 cm with preservation of a tongue-like skin process all along its length, and generous release of the fascia over the peroneal compartment.

  • AIM: The aim of this study was to analyse indications and results of amputation for intractable extremity melanoma after failure of isolated limb perfusion (ILP).
  • METHODS: Between 1978 and 2001, 451 patients with loco-regional advanced extremity melanoma underwent 505 ILPs.
  • RESULTS: The indications for amputation were uncontrollable pain (n=2), extensive loco-regional tumour progression (n=4), loss of ankle function due to local tumour growth (n=1), and ulcerating and fungating lesions, not responding to other treatments (n=4).
  • Ten patients died of melanoma metastases after a median of 11 months (range 2-110 months).
  • CONCLUSIONS: Major amputation is rarely indicated for intractable extremity melanoma but long-term survival can be achieved in selected patients.

  • We report the case of a 31-year-old male patient presenting with a rapidly growing mass localized to the hallux, which eventually resulted in the diagnosis of an ulcerated amelanotic subungual melanoma.
  • Because of their lack of pigmentation, amelanotic melanoma is frequently misdiagnosed for other more common forms of cutaneous compromise, and a delay in proper treatment because of failure to make the diagnosis can greatly decrease the likelihood of a favorable outcome.

  • OBJECTIVE: To investigate the operative techniques and clinical results of sural neurocutaneous vascular flap pedicled on the relatively higher and main perforating branch of peroneal artery in repairing small and medium-sized soft tissue defects in ankle.
  • METHODS: From July 2004 to February 2007, 14 patients (9 males and 5 females, aged 19-53 years) with small and medium-sized soft tissue defects in ankle were treated, including 4 cases of skin necrosis caused by surgery for achilles tendon rupture, 3 soft tissue defects due to car accident, 2 crush injury due to falling heavy objects, 2 chronical infectious ulcer, 2 skin necrosis caused by surgery for calcaneus fracture and 1 melanoma resection in heel.
  • Ranging from 4 cm x 2 cm to 9 cm x 5 cm and combing with exposure of either tendon or bone, the defects were in ankle areas (12 cases) and weight-bearing heel (2 cases).
  • Over a 7-23 month follow-up period, the texture, appearance and color of the flaps in all cases were good, with two-point discrimination of 7-12 mm.The function of ankle obtained satisfactory recovery with normal in-shoe gait.
  • CONCLUSION: With a reliable blood supply, simple operative procedure, sound repair of wound and satisfactory recovery of limb function, the sural neurocutaneous vascular flap pedicled on the relatively higher and main perforating branch of peroneal artery is applicable for the repair of small and medium-sized defects in the ankle and weight-bearing area of heel, especially for patients who have no satisfactory perforating branch in lower position.

  • The reported incidence of melanoma is rapidly rising and second only to lung cancer.
  • Primary melanoma of the lower extremity accounts for approximately 30% of all cases reported, with half of these cases localized to the foot itself.
  • Unfortunately, melanoma can be misdiagnosed and treatment delayed, as they are usually not painful.
  • This is a report of 2 cases of primary nodular melanoma of the foot initially misdiagnosed as a "wart."
  • Given the rapid increase in incidence of melanoma in the general population, one must have a high index of suspicion and low threshold for excisional biopsy of concerning dermatopathology on the foot and lower extremity.

  • Diabetic foot ulceration is a major complication of diabetes and afflicts as many as 15 to 25% of type 1 and 2 diabetes patients during their lifetime.
  • If untreated, diabetic foot ulcers may become infected and require total or partial amputation of the affected limb.
  • Early identification of tissue at risk of ulcerating could enable proper preventive care, thereby reducing the incidence of foot ulceration.
  • Furthermore, noninvasive assessment of tissue viability around already formed ulcers could inform the diabetes caregiver about the severity of the wound and help assess the need for amputation.
  • This article reviews how hyperspectral imaging between 450 and 700 nm can be used to assess the risk of diabetic foot ulcer development and to predict the likelihood of healing noninvasively.
  • Two methods are described to analyze the in vivo hyperspectral measurements.
  • The first method is based on the modified Beer-Lambert law and produces a map of oxyhemoglobin and deoxyhemoglobin concentrations in the dermis of the foot.
  • The second is based on a two-layer optical model of skin and can retrieve not only oxyhemoglobin and deoxyhemoglobin concentrations but also epidermal thickness and melanin concentration along with skin scattering properties.
  • It can detect changes in the diabetic foot and help predict and understand ulceration mechanisms.

  • Average daily exposures exceeded one minimal erythemal dose (MED) at all body sites except the ankle.
  • Mean doses received at the ankle (0.94 MED), back of the hand (1.28 MED) and side of the head (1.14 MED) were 51%, 71% and 63% of those received at the top of the head (1.80 MED), respectively.

  • The phenome is the complete set of phenotypes resulting from genetic variation in populations of an organism.
  • Saturation of a phenome implies the identification and phenotypic description of mutations in all genes in an organism, potentially constrained to those encoding proteins.
  • The human genome is believed to contain 20-25,000 protein coding genes, but only a small fraction of these have documented mutant phenotypes, thus the human phenome is far from complete.
  • In model organisms, genetic saturation entails the identification of multiple mutant alleles of a gene or locus, allowing a consistent description of mutational phenotypes for that gene.
  • Saturation of several model organisms has been attempted, usually by targeting annotated coding genes with insertional transposons (Drosophila melanogaster, Mus musculus) or by sequence directed deletion (Saccharomyces cerevisiae) or using libraries of antisense oligonucleotide probes injected directly into animals (Caenorhabditis elegans, Danio rerio).
  • This paper reviews the general state of the human phenome, and discusses theoretical and practical considerations toward a saturation analysis in humans.
