  • We describe spectral domain Optical coherence tomography findings in a case of circumscribed choroidal hemangioma which was successfully treated with photodynamic therapy.

  • The range of the best-corrected visual acuity was 0.02-0.8, and the thickness of the angioma ranged from 2.5 to 5.4 mm while the base dimension was between 6.5-12.5 mm.

  • The main outcome measures were autofluorescence features of choroidal hemangioma (intrinsic AF) and overlying retinal pigment epithelium (extrinsic AF).
  • RESULTS: There were 27 eyes with circumscribed choroidal hemangioma (CCH) and 7 eyes with diffuse choroidal hemangioma (DCH).

  • PURPOSE: To evaluate efficacy of photodynamic therapy (PDT) with verteporfin in treatment of posterior pole symptomatic circumscribed choroidal hemangiomas.
  • MATERIAL AND METHODS: Four patients (3 men and 1 woman), 15-55 years old (mean age: 29 years) with circumscribed choroidal hemangiomas of the posterior pole, were examined.
  • The diagnosis of choroidal hemangioma was established on the base of clinical examination, A and B--scan ultrasound imaging, Doppler ultrasonography, fluorescein and indocyanine angiography results.
  • RESULTS: In a follow-up period which range from 3 to 14 months (mean 5.7 months) all cases demonstrated clinical features of lesion regression with a reduction of thickness on ultrasonography and choroidal vasculature on angiograms.
  • CONCLUSIONS: Our preliminary results indicate that PDT with verteporfin is a safe and effective method of treatment for circumscribed choroidal hemangiomas, especially in patients with posterior pole location of the tumour.

  • PURPOSE: To evaluate the efficacy and safety of photodynamic therapy (PDT) for macular circumscribed choroidal hemangioma in Chinese patients.
  • METHODS: Twenty-five eyes (25 patients) with macular circumscribed choroidal hemangioma, 18 subfoveal and 7 perifoveal, with vision impairment attributable to subfoveal fluid and retinal detachment underwent visual acuity testing, fundus fluorescein angiography, ultrasonography, and optical coherence tomography (OCT) examinations to evaluate the efficacy and safety of PDT treatment.
  • The mean thickness of the hemangioma before the treatment was 3.2 ± 0.9 mm and decreased to 1.3 ± 1.0 mm post treatment (P < .01), with complete regression of tumor in 7 cases (28%).
  • CONCLUSIONS: PDT with individualized laser parameters for macular circumscribed choroidal hemangioma is effective and safe, leading to improved or stabilized BCVA as a result of tumor shrinkage and the resolution of the subretinal fluid.

  • OBJECTIVE: To assess the clinical effect of combination facula photodynamic therapy (PDT) on patients with circumscribed choroidal hemangioma.
  • Ten cases of circumscribed choroidal hemangioma were diagnosed by ophthalmoscopy, fundus fluorescein angiography and B-scanning ultrasound examination.
  • B-scanning ultrasound examination showed that the hemangioma was undetectable in 2 patients, and in the other 8 patients,the hemangioma had the diameter of 4.2 to 8.3 mm (average 6.4 mm) and the thickness was 0.5 to 2.7 mm (average 1.9 mm).
  • Retina detachment disappeared in 2 patients CONCLUSIONS: Combination facula PDT may lead to the shrinking of choroidal hemangioma, disappearance of subretinal fluid, reattachment of retina and the increase or saving of the visual acuity.
  • It is a effective and safe treatment for the macular circumscribed choroidal hemangioma.

  • RESULTS: A former 29-week, 1446-g premature infant boy was examined multiple times by both a pediatric retinal specialist and pediatric ophthalmologist before a posterior pole circumscribed choroidal hemangioma was noted, initially at postmenstrual age 39 weeks.
  • CONCLUSION: Circumscribed choroidal hemangiomas previously have been thought to be congenital in nature.
  • We present the earliest described case as well as the first de novo emergence of such a tumor.

  • Circumscribed choroidal hemangiomas (CCH) resemble posterior segment lesions such as choroidal melanoma, choroidal metastases, central serous chorioretinopathy, or nonspecific retinal detachment.
  • A circumscribed choroidal hemangioma often appears as a subtle red-orange mass or as a grayish lesion in the posterior choroid.
  • CASE REPORT: A 35-year-old Hispanic man presented for a routine eye examination and was observed to have a choroidal lesion in the posterior pole.
  • CONCLUSIONS: Circumscribed choroidal hemangiomas must be adequately differentiated from other vascular tumors.

  • PURPOSE: To determine the types and frequency of lesions that clinically simulate choroidal or ciliary body melanoma (posterior uveal melanoma; PUM).
  • The most frequent condition was choroidal nevus, accounting for 851 cases (49%) of the pseudomelanomas.
  • This was followed by peripheral exudative hemorrhagic chorioretinopathy (139 cases; 8%), congenital hypertrophy of the retinal pigment epithelium (108 cases; 6%), hemorrhagic detachment of the retina or pigment epithelium (86 cases; 5%), circumscribed choroidal hemangioma (79 cases; 5%) and age-related macular degeneration (76 cases; 4%).
  • Compared with the 1980 report, the rate of pseudomelanomas diagnosed as choroidal nevus increased from 26% to 49%.
  • Suspicious choroidal nevus is still the lesion most difficult to differentiate from PUM.

  • BACKGROUND: Today photodynamic therapy (PDT) with Verteporfin is the standard treatment option for symptomatic choroidal haemangiomas.
  • With the presented new "paint-brush technique" it is possible to treat peripapillary and large choroidal haemangiomas completely without danger for the optic disc and without overtreatment.
  • In total, 13 eyes of 13 patients (6 with peripapillary and 7 with large choroidal haemangiomas) have been treated.
  • CONCLUSION: The use of the PDT "paint-brush technique" allows complete treatment of choroidal haemangiomas under visual control.
  • The technique is a safe and effective amendment in the treatment of symptomatic choroidal haemangiomas.

  • Carcinoma of breast is the most common primary malignancy that accounts for choroidal metastasis in females and carcinoma of lung is the most common cause in males.
  • Other primary neoplasms which can uncommonly metastasize to the choroid are testis, gastrointestinal tract, kidney, thyroid, pancreas, and prostate.
  • We present a case of carcinoma of breast that had visual disturbances and wholebody F18-fluorodeoxyglucose, positron emission tomography-computerized tomography (FDG PET CT) revealed a choroidal lesion in addition to cerebral, pulmonary, and skeletal metastases.
  • Choroidal metastasis from carcinoma of lung has been reported previously on FDG PET.
  • To the best of our knowledge, this is the first case report of carcinoma of breast demonstrating choroid metastasis on F18-FDG PET CT scan.

  • In spite of their efficiency, prostaglandin F2 analogs (latanoprost, travoprost and bimatoprost) should be used with caution in Sturge-Weber-Krabbe syndrome and particularly in cases of proved diffuse choroidal hemangioma.

  • BACKGROUND: The aim of this study was to evaluate what are the most frequent ocular diseases that were suspected for choroidal melanoma ("pseudo melanoma").
  • PATIENTS AND METHODS: The data of all patients who were seen in the University Hospital of Mainz under the suspicion of choroidal melanoma between 1.1.1994 and 1.1.2004 and underwent ultrasound examination, fluorescein angiography or fundus photography, were analysed retrospectively.
  • Among 458 examined eyes a choroidal melanoma was diagnosed in 212 cases.
  • RESULTS: The most frequent ocular diseases suspected for choroidal melanoma were "suspicious nevi" in 31 % of the cases and "disciforme macula degeneration" in 34 %.
  • Less frequently we found hyperplasty of pigment epithelium (5 %), melanocytoma (10 %), choroidal haemangioma (6 %), choroidal detachment (7 %) and retinal macroaneurysma with epi-/subretinal haemorrhages (3 %).
  • In these cases vitrectomy/cataract extraction was performed and we found a choroidal melanoma in 5 eyes, a disciforme macula degeneration in 48 eyes and in 5 eyes a retinal macroaneurysma.

  • Among the 83 suspected malignant posterior segment lesions, the suspected diagnosis included choroidal melanoma (73), choroidal metastasis (6), 'choroidal tumour' (3), and 'Solid retinal detachment' (1).
  • Only 50 of the 73 suspected melanomas were confirmed (68.5%; 95% CI, 57-78%), the oncologist's diagnosis in the remaining 23 being choroidal naevus (10), choroidal metastasis (1), circumscribed choroidal haemangioma (2) and others (10).
  • Only one of six patients with suspected metastases had this condition, the remainder having melanoma (1), lymphoma (1), circumscribed choroidal haemangioma (1), and others (2).
  • The 'choroidal tumours' and 'solid detachments' were found to be chorio-retinal disciform scar (1), varix of vortex vein (1), eccentric CNV (1), and subretinal haemorrhage (1) CONCLUSION: Approximately 30% of patients referred to an ocular oncology service with the diagnosis of choroidal melanoma have an incorrect diagnosis.

  • BACKGROUND: Verteporfin photodynamic therapy (PDT) is approved for the treatment of predominantly classic subfoveal choroidal neovascularization (CNV) due to age-related macular degeneration (AMD), as well as for subfoveal CNV due to pathologic myopia and ocular histoplasmosis syndrome.
  • Verteporfin PDT has also been associated with encouraging treatment outcomes in case studies involving patients with choroidal vascular disorders such as polypoidal choroidal vasculopathy, central serous chorioretinopathy, choroidal haemangioma, angioid streaks, and inflammatory CNV, i.e. conditions currently considered as non-standard indications of verteporfin PDT.
  • Although the anti-vascular endothelial growth factor (VEGF) therapies, ranibizumab and pegaptanib, have been approved for CNV due to AMD, their role in these other choroidal vascular disorders remains to be established.
  • Randomized controlled studies are warranted to confirm the preliminary results of verteporfin PDT as a monotherapy or in combination with anti-VEGF therapies in the treatment of a variety of choroidal vascular conditions.

  • This rare campimetric deficit involves ischemia or infarction of parts of the lateral geniculate body supplied by the distal anterior choroidal artery.
  • CT scan, MRI, MRA and conventional angiography showed a right cerebral arteriovenous malformation supplied partly by the right anterior choroidal artery.
  • CONCLUSIONS: Quadruple sectoranopia involves ischemia or infarction of parts of the lateral geniculate body supplied by the distal anterior choroidal artery.
  • Following radio-induced angioma thrombosis, blood was no longer shunted away from the right lateral geniculate body, whose viable areas resumed their activity.

  • PURPOSE: To evaluate the long-term efficacy and outcome of low-dose proton beam irradiation in the treatment of symptomatic circumscribed choroidal hemangioma.
  • PATIENTS AND METHODS: Retrospective review of 71 patients with symptomatic circumscribed choroidal hemangiomas treated by proton beam irradiation between September 1994 and October 2002 using a total dose of 20 Cobalt Gray Equivalent.
  • CONCLUSION: Proton beam irradiation with a total dose of 20 Cobalt Gray Equivalent appears to be a valid treatment for circumscribed choroidal hemangiomas, inducing definitive retinal reattachment and decreasing tumor thickness.

  • These extended applications include CNV secondary to choroiditis and retinochoroiditis, angioid streaks, central serous chorioretinopathy, retinal angiomatous proliferation, parafoveal telangiectasia or CNV associated with macular dystrophy and idiopathic CNV, as well as diseases without CNV, such as choroidal hemangioma, retinal hamartoma, choroidal melanoma, chronic central serous chorioretinopathy, angiomatous lesions secondary to systemic diseases, rubeosis iridis or neovascular glaucoma.

  • Five controlled trials, two comparative studies and 30 case series were found, most often reporting on uveal melanoma, choroidal melanoma and age-related macular degeneration (AMD).
  • Results for uveal melanoma and choroidal melanoma suggest favorable survival, with, however, significant rates of side effects.
  • Results for choroidal hemangioma and AMD did not reveal beneficial effects from proton radiation.

  • B-scan ultrasonography was performed in 21 eyes and diffused occupying lesions in the choroid were found in 8 eyes (38.1%).
  • Eighty percent of the eyes which underwent trabeculectomy developed choroidal detachment after operation.
  • CONCLUSIONS: The clinical features of Sturge-Weber syndrome-associated glaucoma include facial angiomas, open anterior chamber angle and choroidal hemangioma.

  • Filtration surgery may be complicated by expulsive choroidal hemorrhage and massive choroidal effusion, more often in patients with Sturge-Weber syndrome compared with other patients presenting with glaucoma, particularly in eyes with choroidal hemangioma, because the rapid decrease in IOP during surgery may induce the transudation of fluid from a fragile vessel in the choroidal hemangioma, leading to the development of choroidal effusion.
  • Our case illustrates that significant but transient choroidal effusions can still occur after diode laser procedure.

  • BACKGROUND: To assess the role of digital imaging and a new subtraction method for differential diagnosis of choroidal nevus and small choroidal melanoma.
  • METHODS: Of 241 consecutive patients referred to a tertiary referral center for suspected choroidal melanoma, 110 who underwent digital imaging of the ocular fundus were eligible for this study.
  • RESULTS: Comparative use of digital color, red-free and red light imaging had 85.7% (95%CI 42.1-99.6) sensitivity, 99.0% (95%CI 94.7-99.9) specificity and 98.2% (95%CI 93.6-99.8) exact agreement versus reference standard in differentiation of small choroidal melanoma from pseudomelanoma.
  • Direct comparison between use of digital images and the reference standard showed excellent agreement in detecting small choroidal melanoma from suspected choroidal lesions (K 0.847; 95%CI 0.639-1.0).
  • The subtraction method was useful to show growth in four of 94 melanocytic choroidal tumors.
  • The mean annual incidence of choroidal melanoma in Southwest Finland was 0.80 per 100.000 population.
  • The most frequent choroidal pseudomelanomas were choroidal melanotic and amelanotic nevi, disciform lesions, congenital hypertrophy of the retinal pigment epithelium, and circumscribed choroidal hemangioma.
  • CONCLUSIONS: Combined use of digital color, red-free and red light imaging was a suitable adjunct in differentiation of small choroidal melanoma from different pseudomelanomas.
  • The subtraction method may reveal early growth of the melanotic choroidal tumors.

  • The major vascular tumors of the retina include retinal capillary hemangioma, cavernous hemangioma of the retina, retinal vasoproliferative tumor, and racemose hemangiomatosis of the retina or Wyburn-Mason syndrome.
  • Choroidal vascular tumors include circumscribed choroidal hemangioma and diffuse choroidal hemangioma.
  • While classified as benign, visual symptoms secondary to exudative retinal detachment and a variety of other mechanisms are common and are a major source of long-term visual disability.

  • INTRODUCTION: Photodynamic therapy (PDT) is a method of treatment of choroidal neovascularization (CNV) with a diode laser used after intravenously administered verteporfin.
  • RESULTS: Two thirds of patients had CNV due to AMD, while in others it was caused by pathologic myopia, chorioretinitis, angioid streaks, choroidal hemangioma, except for one patient who had idiopathic CNV.
  • Visual acuity was stabile in 60% (9/15) of patients, of whom in 60% (6/10) of patients with AMD, as well as in patients with pathologic myopia, idiopathic CNV and choroidal hemangioma.
