  • Forty-eight patients with nasal T/NK-cell lymphoma who received first-line polychemotherapy (n = 44) or chemoradiotherapy (n = 4) were analyzed for expression of active caspase-3 (aC3), granzyme B protease inhibitor 9 (PI9), and Bcl-2 proteins.
  • In view of its constitutive expression by normal NK cells, it is suggested that loss of PI9 expression in tumor cells may reflect some mechanism associated with progression.

  • METHODS: The clinicopathological data of 39 patients with extranodal NK/T cell lymphoma were analyzed.
  • Multivariate analysis showed that Skp2 expression (P <0.001), LDH (P = 0.026) and ECOG PS (P = 0.003) were dependent prognostic factors of extranodal NK/T cell lymphoma.
  • CONCLUSION: High expression of Skp2 is an independent unfavorite adverse prognostic factor of extranodal NK/T cell lymphoma.

  • BACKGROUND: Primary nasal natural killer (NK)/T-cell lymphoma is the most common cellular subtype seen in nasal lymphomas.
  • The purpose of this study was to describe the computed tomography (CT) and magnetic resonance (MR) imaging findings of primary nasal NK/T-cell lymphoma.
  • METHODS: During the period between January 1990 and June 2006, the CT (n=24) and MR (n=6) images of 24 patients with biopsy-proved nasal NK/T-cell lymphoma were reviewed retrospectively.
  • RESULTS: The most common symptoms at presentation were nasal obstruction, nasal discharge, and epistaxis.
  • There was involvement of the unilateral nasal cavity in 16, bilateral nasal cavity including nasal septum in 5 and nasal choana in 3.
  • Sites of extension outside the nasal cavity included tumor extension into paranasal sinuses (n=15), nasopharynx (n=5), nasal labial fold (n=3), oropharynx (n=2), infratemporal fossa (n=2), other subcutaneous soft tissue of the face (n=2) and anterior cranial fossa base (n=1).
  • Bony destruction was demonstrated in 18 cases, involving the sinus bony wall (n=15), nasal turbinate (n=10), lamina papyracea (n=6), orbital floor (n=3), and hard palate (n=2).
  • Regional lymphadenopathy was also detected in 3 patients with nasal NK/T-cell lymphoma.
  • CONCLUSION: The CT and MR appearances of nasal NK/T-cell lymphoma are nonspecific, and the diagnosis requires histologic confirmation.
  • However, the differential diagnosis of nasal NK/T-cell lymphoma should be included if the images present soft tissue of the nasal cavity with bony erosion or destruction; involvement of the orbital cavity, nasopharynx and infratemporal fossa; and subcutaneous or nasolabial fold soft tissue infiltration, especially in Asian populations.

  • AIMS: To investigate the clinicopathological features of six unusual cases of nodal CD56+ and Epstein-Barr virus (EBV)+ T/natural killer (NK)-cell lymphoma, a putative nodal counterpart of nasal NK/T-cell lymphoma (nodal T/NK-cell lymphoma of nasal type) in comparison with nasal NK/T-cell lymphoma with secondary lymph node involvement (n = 24) and peripheral T-cell lymphoma (PTCL) of cytotoxic molecule (CTM)+ and EBV+ type (n = 21).
  • METHODS AND RESULTS: All cases of nodal T/NK-cell lymphoma of nasal type exhibited diffuse infiltration of pleomorphic medium-sized to large tumour cells, reminiscent of those in CTM+ EBV+ PTCL.
  • The tumour cells had a typical phenotype of nasal NK/T-cell lymphoma: CD2+, CD3epsilon+, CD4-, CD5-, CD56+, T-cell intracellular antigen-1+, granzyme B+, perforin+ and EBV+.
  • However, four of six cases demonstrated clonal T-cell receptor gamma-gene rearrangement on polymerase chain reaction analysis, unlike nasal NK/T-cell lymphoma.
  • CONCLUSIONS: Nodal T/NK-cell lymphoma may occupy the grey zone between extranodal nasal-type NK/T-cell lymphoma and nodal CTM+ PTCL in a spectrum of NK to T-cell lymphomas that are EBV+.
  • The close relationship between NK/T-cell lymphomas and cytotoxic T-cell lymphomas was also substantiated.

  • A very rare case of necrotizing sialometaplasia of the parotid gland associated with angiocentric T-cell lymphoma was described.
  • Angiocentric pattern or vascular invasion by the lymphoid cells was observed and the involved parotid gland exhibited squamous metaplasia of the ducts and acini; necrotizing sialometaplasia.
  • Immunohistochemical analysis revealed a cytotoxic T-cell phenotype of the lymphoid cells (CD3+, CD4-, CD5+, CD8+, CD56-, Granzyme B+, TIA-1+, Perforin-) but in situ hybridization showed no relation to Epstein-Barr virus.
  • Although necrotizing sialometaplasia is relatively rare in the parotid gland, angiocentric T-cell lymphoma should be considered for a causative condition of necrotizing sialometaplasia.

  • The biopsy of the nasal mass showed a diffuse infiltrate of medium-sized lymphoid cells, with slightly irregular nuclei and moderate polymorphous.
  • The diagnosis of the case was Extranodal T/NK-cell lymphoma, nasal type.

  • This study was undertaken to evaluate the response rate and survival and to document the prognostic factors in patients with T/NK-cell lymphoma who have undergone high-dose therapy (HDT).
  • Eligibility for the study was a mature T/NK-cell lymphoma with initially poor risk (as high or high intermediate risk on age-adjusted International Prognostic Index) or relapsed cases.
  • Twelve patients had unspecified peripheral T-cell lymphomas, 7 anaplastic large-cell lymphomas, 6 nasal T/NK-cell lymphomas, and 3 angioimmunoblastic T-cell lymphomas.
  • Absolute neutrophil count ( > 500/microl) recovered at a median 11 days after autologous stem cell transplantation in 26 patients.
  • Therefore, an initial approach with effective induction and HDT may result in a better outcome in T/NK-cell lymphoma.

  • OBJECTIVES: Nasal natural killer (NK)/T-cell lymphoma is characterized by chemo-resistance, angiodestruction, and aggressive tumor progression.
  • METHODS: Expression levels of major apoptosis-related proteins Bcl-2, Bcl-x, Mcl-1, Bax, and a proliferative marker Ki-67 were analyzed in 11 nasal NK/T-cell lymphoma cases by immunohistochemical methods.
  • Nine cases were of NK-cell lineage and two cases were of T-cell lineage.
  • For comparison, 12 follicular lymphoma (FL) cases and 16 diffuse large B-cell lymphoma (DLBCL) cases were also studied.
  • RESULTS AND CONCLUSIONS: Bax expression was low in all nasal NK-cell lymphoma cases, which constitute the major population of nasal NK/T-cell lymphoma.
  • Bax expression in nasal NK-cell lymphoma was similar to FL and significantly lower compared with DLBCL.
  • Bcl-2 expression was significantly lower in nasal NK/T-cell lymphoma compared with that of FL and DLBCL.
  • Two distinct Mcl-1 expression groups existed for nasal NK/T-cell lymphoma (6.2 +/- 5.2% and 59.1 +/- 12.3%, 95% CI).
  • Ki-67 expression was high in nasal NK/T-cell lymphoma, and worse prognostic groups tended to express higher levels of Ki-67.
  • The results suggest a combination of impaired apoptosis and aggressive proliferation in nasal NK/T-cell lymphoma, and may provide explanations for its poor prognosis.

  • We described a nasal-type NK/T-cell lymphoma in a 9-year-old girl after a history of 2 years earlier of HMB.
  • Pathologic evaluation revealed the typical histologic features of NK/T-cell lymphoma, and the dermal and subcutaneous inflammatory reaction of HMB lesions.

  • Nasal type NK/T-cell lymphoma is a rare type of predominantly extranodal non-Hodgkin lymphoma.
  • Early and correct diagnosis with prompt treatment of NK/T-cell lymphoma is important in view of its potentially aggressive behavior and poor response to treatment with additional sites of tumor developing sometimes weeks or months after initial diagnosis.
  • Unfortunately diagnosis of NK/T-cell lymphomas often proves difficult.
  • Here we present two cases with nasal-type NK/T-cell lymphoma that illustrate that definitive diagnosis is often delayed due to the atypical initial presentation.
  • We will discuss the pitfalls in diagnosing this rare type of neoplasm and review the treatment options.

  • OBJECTIVES: Case report of a male with a rare nasal type NK/T cell lymphoma that presented as an aggressive nasal infection superimposed with squamous and basal cell carcinoma.
  • A review of the diagnosis, management, and prognosis nasal type NK/T cell lymphoma will be presented.
  • METHODS: Review of the literature for cases of nasal type NK/T cell lymphoma, with particular attention to its presentations.
  • RESULTS: Differential diagnosis includes aggressive infection of the nasal skin, carcinoma, and lymphoma.
  • CONCLUSION: No cases in the literature of NK/T cell lymphoma have been reported that presented with an aggressive infection with initial biopsies that revealed squamous cell and basal cell carcinoma, which led to surgical management and a definitive diagnosis of Nasal type NK/T cell lymphoma.

  • OBJECTIVE: To improve the understanding of nasal NK/T-cell lymphoma by analyzing its phenotypic and clinicopathological features.
  • METHOD: Twenty-three cases of nasal NK/T-cell lymphoma diagnosed between 2003 and 2007 in the department of pathology of Guilin Medical College were included in the study.
  • RESULT: 69.9% (16/23) of the nasal NK/T-cell lymphoma occurred in the nasal cavity.
  • 21.74% (5/23) were accompanied by squamous cell carcinoma-like epitheliomatous hyperplasia.
  • CONCLUSION: Nasal NK/T-cell lymphoma is characterized by multiple clinicopathological features.
  • Attention is needed to differentiate the tumor from inflammatory lesions and low grade squamous cell carcinoma.
  • Understanding of various morphological and phenotypic features (i.e. expression of TIA-1, CD56 and CD3, and lack of CD20 and CK) is the key for the diagnosis of nasal NK/T-cell lymphoma.

  • OBJECTIVE: To study the expression and its significance of STAT3, STAT5, Survivin and Ki67 in the Epstein-Barr virus associated nasal NK/T cell lymphoma.
  • METHOD: The expression of STAT3, STAT5, Survivin and Ki67 were detected with immunohistochemistry in 25 cases of nasal NK/T cell lymphoma, and their relationship was analyzed.
  • Nasal cavity tissues from 20 cases of chronic sinusitis were as the control group.
  • RESULT: The positive rates of STAT3 in the lymphoma and in control group were 56% and 10%, respectively (P <0.01).
  • The positive rates of STAT5 in the lymphoma and in control group were 68% and 15%, respectively (P <0.01).
  • The positive rates of Survivin and Ki67 of nasal NK/T cell-lymphoma were 68% and 72%, respectively; but it in the control group was 0% and 20%, respectively, P <0.01.
  • CONCLUSION: The pathway of STAT may play a role in the development of nasal NK/T cell lymphoma.
  • STAT maybe take part in the formation of nasal NK/T cell lymphoma through induction of Survivin and Ki67.

  • The aim of this study was to determine the clinical and endoscopic characteristics of patients with primary gastrointestinal NK-/T-cell lymphoma.
  • PATIENTS AND METHODS: The clinical features of 14 patients with primary gastrointestinal NK-/T-cell lymphoma and the endoscopic findings in 11 of these patients were reviewed.
  • RESULTS: The initial presenting symptoms of primary gastrointestinal NK-/T-cell lymphoma were gastrointestinal bleeding (n = 6, 42%), abdominal pain (n = 4, 29%), and epigastric soreness (n = 4, 29%).
  • CONCLUSIONS: Primary gastrointestinal NK-/T-cell lymphoma was endoscopically characterized by superficial/erosive, ulcerative, or ulceroinfiltrative lesions without fungating mass.

  • A case of primary NK/T-cell lymphoma of the rectum accompanied with ulcerative colitis (UC) in a 73-year-old man is reported.
  • Total colonoscopy performed 4 months after resolution of refractory UC complicated by cytomegalovirus colitis showed a markedly submucosal tumor in the rectum, which was histologically diagnosed as malignant lymphoma.
  • Therefore, based on a diagnosis of localized rectal lymphoma with UC, proctocolectomy was performed.

  • BACKGROUND: The incidence and risk factors of central nervous system (CNS) invasion is still unclear in extranodal natural killer (NK)/T-cell lymphoma, nasal type.
  • PATIENTS AND METHODS: We analyzed 208 patients to study the clinical features and outcomes of CNS disease in extranodal NK/T-cell lymphoma.
  • The clinical variables associated with CNS disease were Ann Arbor stage III/IV (15.87%, P <0.001), regional lymph node involvement (10.41%, P = 0.006), group III/IV of NK/T-cell lymphoma prognostic index (NKPI; 10.20%, P = 0.003), high/high-intermediate international prognostic index (9.30%, P = 0.072) and extra-upper aerodigestive primary sites (9.75%, P = 0.008).
  • In multivariate analysis, NKPI retained the strongest statistical power to predict CNS disease (P = 0.007, relative risk 9.289, 95% confidence interval 1.828-47.212) in extranodal NK/T-cell lymphoma.
  • CONCLUSIONS: Despite extranodal NK/T-cell lymphoma frequently involves paranasal sinus, a routine CNS evaluation and prophylaxis do not seem to be necessary in NKPI group I or II patients due to a very low incidence.

  • A typical NK-cell phenotype is usually present: CD2(+), CD3 epsilon+, CD56(+), cytotoxic molecules+ and Epstein-Barr virus (EBV)+.
  • For localized nasal disease, radiotherapy is important, although chemotherapy is often added.
  • For extra-nasal or disseminated disease, systemic chemotherapy becomes the mainstay and the prognosis is usually poor.

  • Here we describe cytomorphologic, immunocytochemical, and molecular cytochemical features of three cases of NK/T-cell lymphoma of the nasal type diagnosed by nasal brush cytology.
  • Cytomorphologic findings common among the three cases included the presence of several cell types, including nasal cavity epithelial cells, histiocytes, phagocytic histiocytes, and lymphoid cells, within a necrotic background.
  • Suspected lymphoma cells were medium to large lymphoid cells possessing light blue and abundant cytoplasm.
  • We believe these intriguing cytologic findings are indicators of NK/T-cell lymphoma of the nasal type.
  • Immunocytochemical and molecular cytochemical analyses showed staining for natural killer cell antigen CD56 as well as cytotoxic granule-associated proteins granzyme B7 (GrB7) and T-cell-restricted intercellular antigen-1 (TIA-1).
  • Comparison between cytobrush and cotton swab methodology showed that cytobrush resulted in more cell-rich specimens than did cotton swabs, suggesting that nasal brush cytology with cytobrush is most useful in the diagnosis of NK/T-cell lymphoma of the nasal type.

  • OBJECTIVE: To investigate the changes of gene expression profile in nasal NK/T cell lymphoma.
  • METHODS: Total RNA was extracted from the fresh nasal NK/T cell lymphoma tissue and normal lymph node.
  • A total of 365 (8.9%) genes was found to be differentially expressed by a factor of twofold or greater in both of two identical cDNA microarrays, which included oncogenes, tumor supressor genes, cell cycle regulators, apoptotic and antiapoptotic factors, DNA transcription factors, DNA repair and recombination factors, signal transduction genes, protein translation genes, as well as a large number of metabolic genes.
  • CONCLUSION: This study demonstrates that many different kinds of genes are possibly involved in the initiation and progression of nasal NK/T lymphoma. cDNA microarray technique is useful in screening cancer gene expression for nasal NK/T lymphoma.

  • Nasal natural killer (NK)/T-cell lymphoma is a peculiar lymphoma with an unique immunophenotype.
  • Etiologically, the authors previously first demonstrated the presence of Epstein-Barr virus (EBV) genomes and their products in this lymphoma (Lancet 1990; 335).
  • In this study, we determined full-length of LMP1 sequence isolated from 7 patients with nasal NK/T-cell lymphoma using polymerase chain reaction (PCR) method and compared the sequences with those referred to previous reports.
  • In the major HLA-A2 restricted T-cell epitope sequence YLLEMLWRL (codon 125-133), all 7 patients showed amino acid changes at codon 126 (Leu to Phe) and 129 (Met to Ile).
  • Although it is still not clear what the most specific and biologic variation of LMP1 gene in nasal NK/T-cell lymphoma is, the sequence data may be valuable on the study for pathogenesis of nasal NK/T-cell lymphoma and EBV molecular epidemiology.

  • OBJECTIVE: To quantitatively detect the expression level of ribosome protein S13 (RPS13) in NK/T cell lymphoma.
  • METHODS: The total RNA isolated from the parafin-embedded tissue of NK/T cell lymphoma was reversely transcribed into cDNA.
  • The expression level of RPS13 gene in tumor tissue of 20 cases of NK/T cell lymphoma was dramatically lower than that in normal peripheral blood lymphocyte of healthy controls.
  • CONCLUSION: The RPS13 gene has lower expression level in tumor tissue cells of NK/T cell lymphoma than in normal lymphocyte, indicating that it plays an role in the development of the NK/T cell lymphoma.

  • BACKGROUND: Nasal NK/T cell lymphoma is an aggressive disease and has a poor prognosis.
  • Nasal NK/T cell lymphoma is refractory to conventional chemotherapy and has strong tendency of widespread relapse or dissemination into distant sites.
  • METHODS: We immunohistochemically studied nasal NK/T-cell lymphoma to elucidate the unique characteristics of nasal NK/T-cell lymphoma, such as its higher metastatic tendency and its vast necrosis which leads to destruction of the involved tissues.
  • The expression of P-glycoprotein and MMP-9 was evaluated in the 20 patients with nasal NK/T-cell lymphoma and 25 with nasal non-NK/T-cell lymphoma and the relationship between expression of these proteins and clinical results were analyzed in this report.
  • RESULTS: Overall 5-year survival rates for patients with nasal NK/T cell lymphoma, and nasal non-NK/T cell lymphoma were 51%, and 84%.
  • Distant involvement free 5-year survival rates for patients with nasal NK/T cell lymphoma, and nasal non-NK/T cell lymphoma were 53%, and 79%.
  • Sixteen of the 19 patients with nasal NK/T cell lymphoma expressed MMP-9.
  • In contrast, only 8 of the 22 patients with nasal non-NK/T cell lymphoma expressed MMP-9.
  • CONCLUSION: Positive immunoreactivity for P-glycoprotein was not an independent prognostic factor in nasal NK/T-cell lymphomas, which stresses the importance of exploring other mechanisms of drug resistance.
  • The strong expression of MMP-9 is uniquely characteristic of nasal NK/T cell lymphoma and may contribute to its strong tendency to disseminatate and the extensive necrosis which is always seen.

  • [Title] Comparative analysis of extra-nodal NK/T-cell lymphoma and peripheral T-cell lymphoma: significant differences in clinical characteristics and prognosis.
  • AIM: We aimed to compare the frequencies, clinical characteristics, and prognostic factors of peripheral T-cell lymphoma (PTCL) vs. extra-nodal natural killer (NK)/T-cell lymphoma and to characterize the subtypes of extra-nodal NK/T-cell lymphoma.
  • METHODS: We reviewed 97 consecutive patients with PTCL and extra-nodal NKT lymphoma from 2000 to 2006.
  • During this period, a total of 780 patients with malignant lymphomas were treated in our center.
  • The diagnostic criteria used were based on the WHO classification system of malignant lymphomas.
  • RESULTS: Extra-nodal-NK/T-cell lymphoma and PTCL comprised 5.0% (39/780) and 7.4% (58/780) of all cases.
  • Of the PTCL cases, histology was PTCL-NOS in 25, anaplastic large cell in 11, angioimmunoblastic T cell in 18 and other subtypes in four patients.
  • Compared with PTCL, extra-nodal NK/T-cell lymphoma was associated with a significantly inferior rates of complete remission (33% vs. 53%, P = 0.05) and 3 yr overall survival (29.5% vs. 47.5%, P = 0.003).
  • On multivariate analysis, extra-nodal NK/T-cell histology was independently associated with decreased survival.
  • Further analysis into this subtype showed the nasal variant (n = 25) differed significantly from extra-nasal variant (n = 14) in terms of stage at presentation (stages III/IV, 36% vs. 79%), international prognostic index scores (high intermediate or high IPI scores, 24% vs. 64%), complete remission rates (48% vs. 7%), and median survival (10 months vs. 1 month, P < 0.0001).
  • CONCLUSIONS: Extra-nodal NK/T-cell lymphoma was associated with a poorer prognosis compared with PTCL and is likely to comprise two distinct variants with different clinical behavior and prognosis.

  • OBJECTIVE: To compare the detection rate of Epstein-Barr virus latent membrane protein 1 (LMP1) 30 base pair deletion in extranodal nasal type NK/T-cell lymphoma with that in chronic inflammation of nasopharynx and tonsillitis; and to analyze the prognostic significance of LMP1 deletion in extranodal nasal type NK/T-cell lymphoma.
  • METHODS: Polymerase chain reaction was used to detect the deletion of LMP1 in 55 cases of extranodal nasal type NK/T-cell lymphoma and 19 cases of chronic inflammation of nasopharynx and tonsillitis.
  • RESULTS: In all the 55 extranodal nasal type NK/T-cell lymphoma cases studied, 9 cases contained the wide-type or predominantly wide-type LMP1.
  • In the non-lymphoma control group, 16 cases contained the deleted or predominantly deleted LMP1.
  • However, no statistically significant difference was found in the detection rate of 30 base pair deleted LMP1 between extranodal nasal type NK/T-cell lymphoma and control group (P > 0.05).
  • The prognosis of deleted or predominantly deleted LMP1 in extranodal nasal type NK/T-cell lymphoma was worse.
  • CONCLUSION: Though 30 base pair deletion of Epstein-Barr virus LMP1 may not be an important pathogenetic step in extranodal nasal type NK/T-cell lymphoma, it may play some role in tumor progression.

  • OBJECTIVE: The purpose of this study was to evaluate the treatment outcome of angiocentric T-cell and natural killer (NK)/T-cell lymphoma, nasal type.
  • METHODS: Between February 1989 and March 2001, 53 patients with newly diagnosed angiocentric T-cell and NK/T-cell lymphoma, nasal type involving the head and neck, were treated with radiation therapy (RT).
  • CONCLUSIONS: There was no difference in survival between RT and CRT in angiocentric T-cell and NK/T-cell lymphoma, nasal type.

  • [Title] An autopsy case of aggressive CD30+ extra-nodal NK/T-cell lymphoma initially manifested with granulomatous myositis.
  • This study reports an autopsy case of a 53 year-old male with rapidly progressive extra-nodal NK/T-cell lymphoma accompanied with unusual clinical and pathologic features.
  • He was initially presented with localized swelling and tenderness in the right lower extremity and the biopsy from the calf muscle was interpreted as granulomatous myositis masquerizing lymphoma.
  • The biopsy from erythematous skin lesion of trunk showed infiltration of medium sized atypical lymphoid cells with relatively plump cytoplasm and immunophenotype of CD30+, CD56+/- and surface CD3-, which lead to the diagnosis of CD30+ anaplastic large cell lymphoma.
  • About 2 months later, nasal obstruction was developed and the nasal biopsy was done.
  • After confirmation of EBV infection, he was finally diagnosed as extra-nodal NK/T-cell lymphoma with peculiar immunophenotype of CD3 dim+ and CD30+.
  • At autopsy, disseminated angiocentric lymphoma was found all over the internal organs including the brain.
  • This case emphasizes that extra-nodal NK/T-cell lymphoma should be considered as a cause of granulomatous myositis and can express CD30 positivity and CD3 weak positivity, which are unusual but rarely predominant feature of NK/T-cell lymphoma.

  • To our knowledge, this is the first report of cutaneous involvement of nasal-type NK/T-cell lymphoma developing in a dermatomyositis patient.
  • When skin lesions are resistant to aggressive conventional treatment in dermatomyositis patients, we should consider the possibility of malignancy, especially cutaneous lymphoma.

  • Extranodal NK/T-cell lymphoma (NKTL), nasal type, is a highly aggressive neoplasm and is strongly associated with Epstein-Barr virus (EBV).
  • In this study, we demonstrate that EBV-positive NKTL cell lines, namely, Hank-1, NK-YS, and NK-L, are resistant to Fas-mediated apoptosis induced by anti-Fas antibodies despite high levels of Fas surface expression and no mutation in the Fas gene.
  • Fas stimulation of Hank-1 and NK-YS cells showed little processing of caspase 8, caspase 3, or bid, although the proximal signaling molecules of the death-inducing signaling complex, namely, Fas, Fas-associated protein with a death domain, caspase 8, and bid were present in these cells.
  • Since etoposide-induced apoptosis involved caspase 3 activation in Hank-1 and NK-YS cells, the caspase 3-dependent apoptotic machinery appears to be intact.
  • Interestingly, cotreatment of Hank-1 with cycloheximide, a protein synthesis inhibitor, markedly sensitized cells to Fas-mediated apoptosis along with caspase 8 activation and c-FLIP(L) (cellular FLICE inhibitory protein long form) downregulation.

  • BACKGROUND: Localized extranodal natural killer (NK)/T-cell lymphoma, nasal type, commonly has a low or low-intermediate risk of the international prognostic index (IPI), so the IPI has shown inconsistency in predicting prognosis.
  • Thus, we analyzed Ki-67 expression and proposed a new prognostic model including Ki-67 expression for stage I/II extranodal NK/T-cell lymphoma.
  • PATIENTS AND METHODS: We studied Ki-67 expression and its relationship with prognosis in 50 patients with extranodal NK/T-cell lymphoma.
  • CONCLUSIONS: Ki-67 expression is predictive of prognosis, and our prognostic model may become a useful tool for predicting prognosis in patients with stage I/II extranodal NK/T-cell lymphoma, nasal type.

  • PURPOSE: To determine whether there are any differences in therapeutic response, patterns of systemic recurrence, and prognosis of patients with extranodal natural killer (NK)/T-cell lymphoma, nasal type, by the cyclooxygenase-2 (COX-2) expression.
  • PATIENTS AND METHODS: Thirty-four patients with Ann Arbor Stage I and II extranodal NK/T-cell lymphoma who underwent chemotherapy or radiotherapy, or both, were retrospectively reviewed.
  • These patients were divided into two groups according to their immunohistochemical staining for COX-2 expressions: a COX-2-negative group (n = 10 patients) and a COX-2-positive group (n = 24 patients).
  • Compared with the patients in the COX-2-negative group, those in the COX-2-positive group had a significantly lower 2-year systemic recurrence-free survival rate (100% for the COX-2-negative group vs. 54% for the COX-2-positive group) (p = 0.02) and a decreased 5-year overall survival rate (70% for the COX-2-negative group vs. 32% for the COX-2-positive group) (p = 0.06).
  • CONCLUSION: Cyclooxygenase-2 expression can serve as a predictive factor for poor treatment response, higher systemic recurrence, and unfavorable prognosis in patients with extranodal NK/T-cell lymphoma, nasal type.

  • OBJECTIVE: To assess the stability and variability of the LMP1 gene and protein in nude-mice transplanted tumor models of extranodal nasal type NK/T-cell lymphoma.
  • METHODS: PCR and Western Blot were performed to detect the full-length LMP1 gene and protein expression in the established extranodal nasal type NK/ T-cell lymphoma transplanted tumor models and the tissues of donor tumors.
  • Then the sequence of full-length LMP1 gene in transplanted tumor models and donor were compared with the sequence of wild-type B95.
  • 8 and deletion-type CAO cell strains.
  • RESULTS: LMP1 gene and protein were detected in all the extranodal nasal type NK/T-cell lymphoma transplanted tumor models, and the full-length LMP1 nucleotide sequence homology with the donor tumor was 99. 76%.
  • It was found in sequencing analysis that LMP1 gene sequence possessed both conservative character and specificity, similar to that of CAO cell strains.
  • CONCLUSION: LMP1 gene and protein existed stably in the extranodal nasal type NK/T-cell lymphoma transplanted tumor models, which implied that LMP1 might relate to the maintenance and development of this lymphoma.
  • The nude-mice transplanted tumor models provided an important experimental platform for further studies on the correlation of LMP1 and extranodal nasal type NK/T-cell lymphoma.

  • Nasal natural killer (NK)/T cell lymphoma is a peculiar lymphoma with a unique immunophenotype.
  • Etiologically, in 1990, the authors first demonstrated the presence of Epstein-Barr virus (EBV) genomes and their products in this lymphoma.
  • Amino acid changes encoding the CTL epitope on the lymphoma cells may result in a reduced CTL response.
  • We focused on two major CTL epitopes SSCSSCPLSK (codon 340 to 349) and FLYALALLLL (codon 356 to 364) of the LMP2A gene and determined the sequence isolated from nasal NK/T cell lymphoma tissues.
  • All isolates from 7 nasal NK/T cell lymphomas showed the same amino acid change from serine to threonine at codon 348 in the CTL epitope SSCSSCPLSK.
  • The predominant threonine substitution of nasal NK/T cell lymphoma patients may represent disease-associated polymorphism rather than a geographic or race-associated polymorphism.
  • The LMP2A strain including threonine substitution at codon 348 may be selected within tumors and play a role for tumor genesis in Japanese patients with nasal NK/T cell lymphoma through reduced immune recognition.

  • The anatomic definition of extranasal NK/T cell lymphoma has been vague resulting in variable definitions of extranasal sites by different groups.
  • We analysed the clinical behavior of 90 NK/T cell lymphoma patients and attempted to elucidate the prognostic factors for risk-based stratification of therapy.
  • We observed no significant difference between "nasal" and "nasal-type" NK/T cell lymphomas in regards to clinical features and survival using the conventional anatomic classification.
  • We suggest the categorisation of the two subtypes of NK/T cell lymphoma as follows: UNKTL (upper aerodigestive tract NK/T cell lymphoma) including all lymphomas confined to nasal cavity, nasopharynx, and the upper aerodigestive tract and EUNKTL (extra-upper aerodigestive tract NK/T cell lymphoma) group to include all sites other than the UNKTL group.

  • PATIENTS AND METHODS: From March 1992 to January 2005, 46 Chinese patients with midline NK/T-cell lymphoma nasal-type were initially treated with CHOP regimen as first-line chemotherapy.
  • CONCLUSION: In this preliminary study, the results indicated that L-asparaginase based regimen might be a promising new salvage chemotherapeutic regimen in CHOP failures and conduce to improve the treatment outcome in midline NK/T-cell lymphoma, nasal-type.

  • Sinonasal natural killer (NK)/T-cell lymphoma (NKTCL) is closely associated with Epstein-Barr virus (EBV) infection and expresses latent membrane protein (LMP)-1 and EB nuclear antigen (EBNA)-1, i.e., latency II of EBV infection.
  • To elucidate the molecular mechanism of angio-invasiveness in NKTCL, expression of cell adhesion molecules and chemokine receptors at mRNA and protein levels was examined using real-time PCR and immunohistochemistry in 17 NKTCL together with 10 diffuse large B-cell lymphoma (DLBL) and 9 non-neoplastic nasal mucosa as controls.

  • OBJECTIVE: To review on the evolution in the understanding of midline destructive lesions of the sinonasal tract in different period, and to describe the clinical features and diagnostic problems on the nasal extranodal NK/T cell lymphoma (NENKTCL).
  • METHOD: To retrospectively study the nasal CT scannings and the results of serum ANCA of 10 patients with typical clinical characteristics.
  • RESULT: All patients presented with the symptoms of the nasal obstruction and the nasal discharge.
  • Nasal endoscopy and CT scan of paranasal sinuses showed destructive sinonasal lesions.
  • Clinically, Nasal lesions in the cases were similar to those from historical nomenclature.
  • CONCLUSION: Outdated terminology referring to midline lesions includes a variety of diseases, and NENKTCL is an independent disease focusing on destructive lesions of face and nose.

  • The objective of this study was to investigate clinical features and treatment outcomes in patients with nasal-type NK/T cell lymphoma.
  • From January 1991 to December 2003, 26 patients diagnosed as nasal-type NK/T cell lymphoma were included in the analysis.
  • The median survival for 26 patients with nasal-type NK/T cell lymphoma was 7.4 months (95% CI, 0.1, 16.9).
  • In conclusion, Nasal-type NK/T cell lymphomas showed a poor response to the conventional anthracycline-based chemotherapy, and thus an investigation for an innovative therapy is urgently needed to improve survival in these patients.

  • [Title] EBV viral load in tumor tissue is an important prognostic indicator for nasal NK/T-cell lymphoma.
  • We retrospectively studied 19 cases of nasal NK/T-cell lymphoma for various potential prognostic factors and performed real-time quantitative polymerase chain reaction for Epstein-Barr virus (EBV) viral load in tumor tissue.
  • Patients with a low EBV viral load (<1 copy per cell) more frequently survived for more than 2 years compared with patients with a high EBV viral load (>/=1 copies/cell) (7/7 vs 3/9; P = .014; Fisher exact test).
  • In contrast, the overall survival of the patients did not correlate with the extent of lesion, age, stage, necrosis, histologic subtypes, CD56 expression, or angiocentric or angiodestructive growth pattern.
  • Our findings suggest that the EBV viral load in tumor tissues is a useful indicator for predicting outcome of nasal NK/T-cell lymphoma.

  • Nasal natural killer (NK)/T-cell lymphoma (NNKTL) is a clinical illness characterized by progressive unrelenting ulceration and necrosis of the nasal cavity and midline facial tissues.
  • Histological features of the lymphoma include angiocentric and polymorphous lymphoreticular infiltrates, called polymorphic reticulosis.
  • Surface antigens and the NK-cell marker, CD56, as well as pan-T antigen CD2, cytoplasmic CD3 (CD3epsilon), and CD45 are expressed in the lymphoma cells.
  • The origin of the lymphoma is thought to be either NK-cell linkage without T-cell receptor (TCR) rearrangement or gammadeltaT-cell linkage with gammadeltaTCR rearrangement.
  • Since the authors of this study first demonstrated the presence of Epstein Barr virus (EBV)-DNA and EBV oncogenic proteins in NNKTL, the lymphoma has been classified as one of the EBV-associated malignancies.
  • The NNKTL cells produce interleukin (IL)-9, IL-10, and interferon-gamma-inducible protein-10 (IP-10), possibly due to EBV-oncogenic proteins in the lymphoma cells, and such cytokines take an important part in the cell proliferation and invasion, acting in an autocrine manner.
  • Clinically, the serum EBV-DNA copy number is useful as a specific tumor marker and a predictive prognostic factor.
  • Even in early clinical stages, the lymphoma shows poor prognosis caused by the rapid progression of the lesion into distinct organs.
  • In this article, the clinical, pathological, and virological characteristics of the lymphoma are reviewed, along with a report of our investigations.

  • [Title] [Extranodal nasal type NK/T-cell lymphoma: the expression of Epstein-Barr virus latent membrane protein 1 and its significance of prognosis].
  • To investigate the expression and prognostic significance of Epstein-Barr virus latent membrane protein 1 in extranodal nasal type NK/T-cell lymphoma in the Chengdu area.
  • METHODS: The expression of latent membrane protein-1 (LMP1) was detected by immunohistochemistry (IHC) and DNA-PCR in 67 cases of extranodal nasal type NK/T-cell lymphoma, and the differences in survival rate between positive and negative expression groups of LMP1-protien and LMP1-DNA were analyzed respectively.
  • CONCLUSION: LMP1 was shown to be closely associated with extranodal nasal type NK/T-cell lymphoma in Chengdu.
  • No relationship was found between the prognosis and the LMP1 expression in extranodal nasal type NK/T-cell lymphoma.

  • Nasal natural killer (NK)/T-cell lymphoma (NKTCL) and chronic active Epstein-Barr virus infection (CAEBV) are relatively frequent, especially in Asia, and are poor in prognosis.
  • Both diseases are proliferative diseases of NK/T cells that show highly complicated karyotypes, suggesting the involvement of chromosomal instability.
  • To evaluate the role of ATR gene alterations in the pathogenesis of NKTCL and CAEBV, the whole coding region of the ATR gene was examined in cell lines derived from NKTCL and CAEBV, as well as tumor samples from patients.
  • Both NKTCL and CAEBV cell lines with ATR alterations showed a delay or abrogation in repair of ionizing radiation-induced DNA double-strand breaks and ultraviolet-induced DNA single-strand breaks, and both exhibited a defect in p53 accumulation.

  • [Title] Mediastinal single nodal relapse of a nasal NK/T cell lymphoma.
  • A nasal NK/T cell lymphoma is a very aggressive form of lymphoma.
  • We describe here a 40-year old man who presented with a nasal obstruction caused by a protruding mass that was identified as a nasal NK/T cell lymphoma.
  • However, PET performed following radiotherapy revealed a single mediastinal lymphadenopathy, with no evidence of residual tumor in the nasal cavity.
  • A biopsy using video-assisted thoracoscopy (VATS) showed the presence of a recurrent NK/T cell lymphoma with an immunophenotype identical to that of the primary nasal lymphoma.
  • These findings indicate that early detection with PET and prompt surgical excision with the use of VATS can lead to successful treatment of a relapsed nasal NK/T cell lymphoma.

  • [Title] Extra nodal NK/T-cell lymphoma nasal type that responded to DeVIC combination chemotherapy.
  • We report a 76-year-old woman with extra nodal NK/T-cell lymphoma nasal type (ENKL).
  • The tumor cells showed angiocentric growth with necrosis.
  • Immunohistologically, the tumor cells from the skin lesion expressed CD2, cytoplasmic CD3, CD56, and T-cell intracellular antigen-1 (TIA-1), but not surface CD3, CD19, and TdT.
  • The gastric tumor cell, also expressed cytoplasmic CD3, CD45RO and CD56.

  • T- or NK-cell lymphomas have rarely been reported in this location.
  • We present hereby a primary laryngeal T/NK-cell lymphoma, nasal-type.
  • The extranodal T/NK-cell lymphoma, nasal-type is a very aggressive subtype of extranodal lymphoma, usually located in the nasal cavity or in nearby sites.
  • The prognosis of extranasal cases of this type of lymphoma is poor, even when they are diagnosed in localized stages.

  • Quantitative real-time polymerase chain reaction (PCR) was utilized to measure serum EBV DNA levels of BamHI W fragment and latent membrane protein 1 (LMP1) in 20 nasal natural killer (NK)/T-cell lymphoma patients.
  • The BamHI Z fragment, Epstein-Barr-replication activator (ZEBRA) expression was detected in a small proportion (0.1-3%) of lymphoma cells from 10 (50%) of the patients.
  • Patients with ZEBRA expression showed significantly higher DNA levels of BamHI W and LMP1 (P = 0.0081, P = 0.004), suggesting that EBV DNA may be caused by EBV replication from lymphoma cells.
  • These data suggest that simultaneous measurement of serum levels of both BamHI W and LMP1 DNAs may be useful for diagnosis, disease monitoring, and prediction of prognosis for nasal NK/T-cell lymphoma patients.

  • BACKGROUND: Lethal midline granulomas (LMG) are very rare angiocentric NK/T-cell lymphomas in association with Epstein-Barr virus.
  • Conjunctival biopsy revealed a highly malignant, CD3 + and BCL2 + extranodal T-cell lymphoma with features of an NK/T-cell origin (CD56 +, TIA + TCR-rearrangement: germline).
  • All lymphoma cells were positive for Epstein-Barr virus RNA.
  • The patient died within a month because of untreatable pancytopenia due to malignant bone marrow infiltration.
  • CONCLUSIONS: LMG is a rare but highly malignant Epstein-Barr virus associated NK/T-cell lymphoma that can occur in healthy, immune competent Caucasians.

  • OBJECTIVE: To investigate the specific genetic alterations in nasal NK/T cell lymphoma (N-NK/T-L) and the significance thereof.
  • METHODS: Restriction landmark genomic scanning (RLGS) was performed on the specimen of a case of N-NK/T-L and the peripheral blood leukocytes of the same case.
  • The RLGS image was analyzed with the Virtual genome scan (VGS) software so as to discover abnormality of T-bet gene.
  • Southern blotting and dot blotting were performed on 3 cases of fresh N-NK/T-L tumor tissues and the genome of the case of peripheral blood lymphocytes.
  • The expression of the T-bet (T-box expressed in T cell) gene in the tissue sections of 20 cases of N-NK/T-L, 17 cases of B cell lymphoma (BCL), 3 cases of normal spleen, and 5 cases of chronic nasopharyngitis was detected by in situ hybridization.
  • RESULTS: RLGS combined with VGS revealed a genomic alteration in the T-bet gene and consequent Southern and dot blotting confirmed this genomic alteration in the N-NK/T-L cells.
  • In situ hybridization showed that the T-bet mRNA expression rate of the N-NK/T-L cells was 90.0% (18/20), significantly higher than that of the BCL (11.8%, 2/17, P < 0.01).
  • CONCLUSION: The amplification and over-expression of the T-bet gene may play a role in the development of N-NK/T-L.
  • The potential value of over-expression of T-bet gene in diagnosis of N-NK/T-L is worth further investigating.

  • The intensity of p53 and p21 expression, the Ann Arbor stage and the size of tumor cell all were positively correlated with PI (Spearman correlation analysis, P<0.05), while no correlation with AI (P>0.05).

  • [Title] Primary NK/T-cell lymphoma of the cauda equina: a case report and literature review.
  • OBJECTIVE: To describe an unusual case of primary lymphoma of the cauda equina and provide a review of the literature of this condition.
  • SUMMARY OF BACKGROUND DATA: Primary lymphoma of the cauda equina is extremely rare, and has been reported in 8 cases previously.
  • This report is the first to describe a case of primary nasal type NK/T-cell lymphoma of the cauda equina.
  • METHODS: We report the case of a 67-year-old man presenting the symptoms of cauda equina syndrome caused by primary lymphoma of the cauda equina.
  • Further investigations including immunohistochemical stains made a diagnosis of primary nasal type NK/T-cell lymphoma of the cauda equina, and the patient received radiotherapy to the lumbosacral area.
  • CONCLUSION: Although primary lymphoma of the cauda equina is extremely rare, the prognosis of this condition is thought to be poor.

  • A novel nude mice model of human extranodal nasal type NK/T-cell lymphoma was established by subcutaneously implanting the sample taken from the patient with secondary extranodal nasal type NK/T-cell lymphoma of the stomach into the right axillary region of a BALB/c (nu/nu) nude mouse.
  • Histologically, the tumor cells were medium to large size and arranged in sheets, with a little mesenchyma, and disseminated almost in all passages of the lymphoma-bearing nude mice.
  • No T-cell receptor gamma gene rearrangement was detected in the transplanted tumors.
  • Furthermore, both human sequencing-tagged sites SY14 and Y chromosome were detected by PCR or fluorescent in situ hybridization, respectively, in the transplanted tumor.
  • The transplanted tumor in this novel nude mice model maintained the essential features of human extranodal nasal type NK/T-cell lymphoma, and it would be an ideal tool in vivo for further research of the tumor.
