  • PURPOSE: To determine the vascular morphology of racemose haemangioma and related functional alterations in arteriovenous (AV) malformation type 3.
  • METHODS: A 17-year-old patient with unilateral racemose haemangioma received a full ophthalmic examination including Snellen visual acuity (VA) and Goldmann visual field.
  • On fluorescein angiography, there was a fast filling of the retinal branches related to the racemose vessels.
  • Doppler ultrasound confirmed a significantly changed haemodynamic flow in the racemose vessels.
  • CONCLUSION: The racemose haemangioma led to a marked visual field defect.
  • Racemose haemangiomas are associated with severe changes in the haemodynamics of the retinal vasculature.

  • OBSERVATION: A boy born at 27 weeks and 6 days of amenorrhea with extensive plane angioma of the right lower limb, right lower part of the back and abdomen.
  • We also noted hypertrophy of this member with venous dilatations.
  • Arterial Doppler ultrasound of the right lower limb showed an aneurysmal varix between the vein and the common femoral artery, confirming a diagnosis of Parkes-Weber syndrome.
  • Vascular malformations are transmitted in autosomal dominant fashion in the majority of infected families but with incomplete penetrance and variable expressivity.
  • In each generation of this family, we noted the presence of hemangiomas or capillary malformations with aggravation in the third generation and onset of Parkes-Weber syndrome.

  • METHODS: This report presents the case of a 30-year-old female with persistent sciatic vessels complicated with an arteriovenous malformation in the right buttock.
  • RESULTS: The arteriovenous malformation was shown to have been extinguished by multidetector computed tomography.

  • We report a histologically proven case of a posterior mediastinal arteriovenous malformation in a 42-year-old man that was incidentally detected by chest radiography during a routine health check.

  • PURPOSE OF REVIEW: This review highlights recent data regarding factors associated with brain arteriovenous malformation hemorrhage and different treatment options.
  • RECENT FINDINGS: More risk factors were identified in association with intracranial hemorrhage, including age at initial diagnosis of arteriovenous malformation, co-existing extranidal aneurysms and genetic factors.
  • Patients with unruptured arteriovenous malformations were found to be more susceptible to worsening in neurological function after microsurgery compared with those presenting with hemorrhagic arteriovenous malformation.
  • Radiosurgery has achieved satisfactory obliteration of deep arteriovenous malformations, but with increased actuarial hemorrhage rates from the first to the fifth year.
  • Although the Intraoperative Hypothermia for Aneurysm Surgery Trial failed to show a significant neurological improvement, the superior efficiency of endovascular cooling has offered optimism in cerebral protection during neurovascular surgeries by shortening the time to achieve hypothermia and rewarming.
  • A multi-center trial (ARUBA) has been proposed to test the hypothesis that, for unruptured brain arteriovenous malformations, there is no difference between interventional and conservative management.
  • Recent studies have also shown the promise of using tetracyclines to decrease the rate of spontaneous arteriovenous malformation rupture.
  • SUMMARY: The recent identification of clinical and genetic factors associated with brain arteriovenous malformation hemorrhage, as well as studies on treatment outcomes, will help risk stratification in management choices.
  • Future studies are needed to identify arteriovenous malformation patients at the greatest risk of spontaneous hemorrhage and to develop specific medical therapies.

  • OBJECTIVES: To report our experience using embolization in managing localized pulmonary arteriovenous malformations in adults.
  • All patients presenting with localized pulmonary arteriovenous malformations treated with embolization were included in the study.
  • Clinical presentation (respiratory symptoms and previous history of paradoxical embolism) and the characteristics of pulmonary arteriovenous malformations (single or multiple, location, diameter of the afferent artery and simple or complex angioarchitecture) before embolization were analyzed.
  • The details of the procedure, including the number of pulmonary arteriovenous malformations embolized, the number of coils used, and the type of intraoperative complications were recorded.
  • Forty-seven procedures were carried out on a total of 99 pulmonary arteriovenous malformations (mean, 2.3 per patient), using 530 coils (12.6 per patient).
  • The pulmonary arteriovenous malformations were located in the lower lobes in 60% of cases and a simple architecture was reported in 81% of cases.
  • The rate of complete occlusion of treated arteriovenous malformations was 92% using computer tomography.
  • CONCLUSION: Embolization is a highly effective and safe technique for treating pulmonary arteriovenous malformations.

  • BACKGROUND: The external ear is the second most common site for extracranial arteriovenous malformation in the head and neck.
  • METHODS: This retrospective review of 41 patients with auricular arteriovenous malformation was based on medical records, imaging studies, and photographs.
  • Distribution of auricular and extra-auricular arteriovenous malformation was not limited to "watershed" areas between vascular territories (angiosomes).
  • Fifteen patients had embolization only: the arteriovenous malformation worsened and amputation was necessary in six patients; in the remaining nine patients, two improved, four persisted, and three worsened.
  • Of 20 patients who had auricular amputation, 16 (80 percent) were controlled, three (15 percent) improved, and one had unresectable, residual cervicofacial arteriovenous malformation.
  • CONCLUSIONS: The authors recommend periodic evaluation for stage I to II auricular arteriovenous malformation and intervention if there is evolution to stage III.
  • Preoperative embolization and partial or total amputation effectively control auricular and para-auricular arteriovenous malformation.
  • Extensive extra-auricular arteriovenous malformation requires individualized endovascular therapy and resection.

  • A 45-year-old man with recurrent episodes of hematemesis caused by extensive varices in the esophagus and stomach was admitted.
  • Angiography revealed a racemose vascular network at the same site and early appearance of the portal venous system in the arterial phase.
  • With a diagnosis of pancreatic arteriovenous malformation with portal hypertension, he underwent pylorus-preserving pancreaticoduodenectomy, preceded, 2 days earlier, by transcatheter arterial embolization of some of the feeding arteries.
  • The varices observed preoperatively in the esophagus and stomach disappeared, and he has been well for 6 years after the operation.
  • We reviewed 47 cases of pancreatic arteriovenous malformation previously reported in the English-language literature, with a focus on the clinical manifestations, treatment approaches, and etiological relationship with portal hypertension and liver cirrhosis.

  • A role for the Notch signalling pathway in the formation of arteriovenous malformations during development has been suggested.
  • However, whether Notch signalling is involved in brain arteriovenous malformations in humans remains unclear.
  • Here, we performed immunohistochemistry on surgically resected brain arteriovenous malformations and found that, compared with control brain vascular tissue, Notch-1 signalling was activated in smooth muscle and endothelial cells of the lesional tissue.
  • Western blotting showed an activated form of Notch-1 in brain arteriovenous malformations, irrespective of clinical presentation and with or without preoperative embolization, but not in normal cerebral vessels from controls.
  • In addition, the Notch-1 ligands Jagged-1 and Delta-like-4 and the downstream Notch-1 target Hes-1 were increased in abundance and activated in human brain arteriovenous malformations.
  • Our findings suggest that activation of Notch-1 signalling is a phenotypic feature of brain arteriovenous malformations, and that activation of Notch-1 in normal vasculature induces a pro-angiogenic state, which may contribute to the development of vascular malformations.

  • OBJECTIVE: Arteriovenous malformations of the basal ganglia and thalamus are often managed with radiosurgery or observation, without consideration of microsurgery.
  • METHODS: A review of the endovascular, microsurgical, and radiosurgical literature for arteriovenous malformations of the basal ganglia and thalamus was performed, with close attention to surgical approaches, obliteration rates, and procedure-related complications.
  • CONCLUSION: Given an appropriate surgical corridor of access, often afforded by incident hemorrhage, arteriovenous malformations of the basal ganglia and thalamus should be considered for microsurgical extirpation with preoperative embolization.

  • As other malformations, they are present at birth but may remain quiescent for many years; their flow may speed up quickly consequently on an infection, traumatism or modification of the hormonal climate.
  • A "nidus" appears in the heart of the malformation which acts as a real "turbo" on the blood flow; vascular shunts appear creating ulcerations (vascular steal phenomenon); cardiac failure may be observed.
  • Among all vascular anomalies, arteriovenous malformation is the most dangerous in its evolution which may be fatal.
