  • Cystic spinal or ancient schwannoma is a rare form of intradural tumour especially in the conus medullaris region.
  • Due to its indolent behavior and benign course, the diagnosis of schwannoma is always a challenge and the imaging findings can be misleading.
  • We concluded that ancient schwannoma of the conus medullaris is a rare benign tumour that can present with minimal non-specific neurological symptom that lead to misdiagnosis.

  • Ancient schwannomas are rare, encapsulated tumours of long duration and are benign in nature.
  • The term "ancient schwannoma" is used to describe a tumour that has undergone such changes, typified by relative loss of Antoni type A tissue, perivascular hyalinisation, calcification, cystic necrosis, haemorrhage and the presence of degenerative nuclei that may be misinterpreted as sarcomatous pleomorphism.
  • We report two cases of ancient schwannoma in the thigh mimicking malignancies.
  • Identifying the fibrous capsule of the mass and a split fat sign using MRI is important for differentiating ancient schwannoma from other malignant tumours.

  • BACKGROUND: The objective of this article is to define the imaging characteristics of ancient schwannoma, which is a rare variant of benign schwannoma with degenerative changes, arising in the female pelvis simulating ovarian tumors.
  • METHODS: Eleven surgically proven ancient schwannomas of the female pelvis were evaluated retrospectively on the basis of CT and MR findings.
  • RESULTS: Typical intra-pelvic schwannoma was a neurologically asymptomatic large mass, which may situate at presacral or lateral pelvic region with the continuity to the nerve or neural foramen.
  • Ancient schwannomas manifested as encapsulated solid masses with random or eccentric cystic areas, or as cystic masses with marginal crescent-shaped or nodular solid components.
  • CONCLUSIONS: Diagnosis of ancient schwannoma before surgical treatment is important and should be made by its characteristic clinical and imaging findings.

  • Surgical excision was undertaken and histology was an ancient schwannoma of the scrotum.
  • CONCLUSION: Schwannoma is a benign encapsulating neoplasm with an overall low incidence, occurring mostly in the head and neck region and seldom in the scrotum.
  • Variations of schwannoma such as cellular, ancient, glandular and epithelioid are observed based on the appearances.
  • Ancient schwannoma exhibits pleomorphism without mitosis as the result of cellular degeneration, which can lead to an erroneous diagnosis of malignancy.

  • BACKGROUND: Ancient schwannomas are degenerate peripheral nerve sheath tumors that very rarely occur in the retroperitoneum.
  • We describe three cases of retroperitoneal ancient schwannomas and discuss the diagnosis and management of these tumors.
  • CASE PRESENTATIONS: Three female patients with retroperitoneal ancient schwannomas were reviewed.
  • Histological examination showed encapsulated tumors that displayed alternating areas of dense cellularity and areas of myxoid matrix consistent with a diagnosis of ancient schwannoma.
  • CONCLUSION: A diagnosis of ancient schwannoma should be entertained for any heterogeneous, well encapsulated mass in the retroperitoneum.

  • RESULTS: After surgical excision the diagnosis was retroperitoneal old schwannoma.
  • The histological changes apparent in the cases of old schwannoma have been interpreted as degenerative, indicating a long evolution, which has motivated its denomination as old.
  • CONCLUSIONS: The old schwannoma is a rare variety of the benign common schwannoma.

  • BACKGROUND: There are four major lesions which may arise in the peripheral nerve, namely neuroma, schwannoma, neurofibroma and malignant peripheral nerve sheath tumor.
  • Among the nonmalignant cases neurofibroma was the commonest (51 cases, 45% of all) closely followed by schwannoma (39 cases, 34% of all).
  • Among the schwannoma cases 4 were diagnosed as ancient schwannoma with presence of bizarre cells with hyperchromatic nuclei.
  • The commonest site involved for both schwannoma and neurofibroma was scalp-face-neck followed by back.
  • The age range for schwannoma was 16 to 75 years whereas the same for the neurofibroma cases was 2 to 82 years.
  • CONCLUSION: The majority of the tumor are benign and the commonest benign tumor was neurofibroma of sporadic type, closely followed by schwannoma.
