  • Nevertheless, ganglioglioma-like foci (GGLF) have not been previously described.
  • Herein, we report seven examples where the primary differential diagnosis was a ganglioglioma with an oligodendroglial component.
  • In contrast to classic ganglioglioma, however, cases lacked eosinophilic granular bodies and CD34-positive tumor cells.
  • Recognition of this pattern will prevent a misdiagnosis of ganglioglioma with its potential for under-treatment.

  • [Title] Malignant and benign ganglioglioma: a pathological and molecular study.
  • The current study of a rare case of a woman harboring a ganglioglioma with areas of malignant transformation addresses two fundamental questions:.
  • (1) Are the ganglioglioma and its malignant component clonal in origin?
  • (2) What are the genetic alterations associated with the initiation and subsequent malignant progression of ganglioglioma?
  • By using the human androgen receptor gene (HUMARA) assay, we found the ganglioglioma and the malignant component to be clonal in origin, suggestive of initial transformation of a single neuroglial precursor cell with subsequent malignant progression.
  • Conventional and array comparative genomic hybridization (approximately 2.5-Mb resolution) analyses found chromosomal losses to be predominant in the benign areas of the ganglioglioma, with gains more prevalent in the malignant component.
  • Regions of chromosomal loss, postulated to harbor genes involved in the initiation of ganglioglioma, included 1p35-36, 2p16-15, 3q13.1-13.3, 3q24-25.3, 6p21.3-21.2, 6q24-25.2, 9p12, Xp11.3-11.22, and Xq22.1-22.3.
  • Direct analysis demonstrated loss of p19 expression and p53 mutation in the malignant areas, highly suggestive of these alterations being involved in the malignant progression of the ganglioglioma.

  • Light microscopic findings were consistent with ganglioglioma.
  • Ganglioglioma is a rare tumor of the central nervous system that typically presents with seizures in children and young adults.
  • Occurrence of this tumor in the cerebellopontine angle is extremely unusual; this rarity is magnified by its occurrence in an adult patient without a history of seizures.
  • Our case illustrates that ganglioglioma should be considered in the differential diagnosis of cerebellopontine angle masses at any age.

  • OBJECTIVES: The utility of apparent diffusion coefficient (ADC) values was evaluated for the differential diagnosis of ganglioglioma.
  • The minADC of ganglioglioma was compared with that of low- or high-grade astrocytomas (astrocytoma, anaplastic astrocytoma and glioblastoma).
  • Tumor cellularity of ganglioglioma was inversely correlated with the minADC value.
  • DISCUSSION: The minADC value reflects in the low tumor cellularity of gangliogliomas and may provide a method for the differential diagnosis of ganglioglioma.

  • The following review of the literature describes the ganglioglioma, an uncommon mixed glioneuronal neoplasm, most often of low-grade histology, with a small, albeit well-documented, malignant potential.
  • An exceedingly rare location of the ganglioglioma is within the lateral ventricle.
  • A systematic literature search revealed only eight reports documenting the occurrence of a ganglioglioma within the lateral ventricle.
  • We describe an illustrative case of an intraventricular ganglioglioma with a prominent cystic component and enhancing mural nodule, which represents the classic radiographic appearance of gangliogliomas described in other locations.
  • Regardless of location within the central nervous system, ganglioglioma should be on the differential diagnosis for any cystic mass with a mural nodule, particularly in the setting of epilepsy.

  • OBJECTIVES: To define the postoperative seizure outcome and its predictors in patients with ganglioglioma-related temporal lobe epilepsy (TLE).
  • PATIENTS AND METHODS: We reviewed the pre- and post-surgical evaluation data of 23 patients with temporal lobe ganglioglioma, who had completed >or=1 year of postoperative follow-up.
  • CONCLUSION: We emphasize that, in patients with temporal lobe ganglioglioma, when the seizures are medically refractory, surgery offers potential for cure of epilepsy in the majority.

  • Ganglioglioma is usually a well differentiated slowly growing mixed neuronal and glial neoplasm corresponding to WHO grade I or II.
  • We report a case of a 25-year-old woman with a family history of neurofibromatosis who presented initially with a World Health Organization grade IV anaplastic ganglioglioma (a mixed ganglion cell multiform tumor glioblastoma).
  • Histologically, two cell populations were noted: a predominant glial component consisting in a multiform glioblastoma and ganglion cells supporting a diagnosis of ganglioglioma.
  • Although other cases of grade III gangliogliomas and twelve cases of grade IV gangliogliomas have been reported, the present case is exceptional in that, to our knowledge, it is the second case of a patient who presented initially with a composite grade IV ganglioglioma and who was clinically followed up to the time of death.
  • This case allows direct comparison between the histological findings in a multiform glioblastoma and a ganglioglioma.

  • INTRODUCTION: Ganglioglioma is an uncommon neoplasm of the central nervous system, most frequently seen in the temporal lobe, and usually associated with medically refractory epilepsy in children and young adults.
  • Few reports have considered ganglioglioma-associated epileptogenicity arising in the temporal lobe.
  • The purpose of our study was to define the imaging features of ganglioglioma in the temporal lobe and their relation to the seizure foci revealed by electrocorticograms.
  • MATERIALS AND METHODS: We reviewed 24 patients with pathologically confirmed ganglioglioma in the temporal lobe.
  • CONCLUSION: A tumor presenting isointensity to gray matter on T1-WI without mass effects in the medial temporal lobe in a young patient with temporal lobe epilepsy (TLE) might be the characteristic imaging of temporal lobe ganglioglioma.
  • However, such tumors are not always associated with epileptogenicity, even if a ganglioglioma is found in a patient with TLE.
  • The seizure foci may be contralateral to the ganglioglioma.

  • bcl-XL, bax, bcl-2, and p53 are apoptotic proteins essential to normal neural development.
  • Aberrant expression of these proteins has been observed in several central nervous system neoplasms.
  • Immunoexpression of these markers is studied in 21 patients with focal cortical dysplasia type II (Taylor-type cortical dysplasia; malformations of cortical development) who had undergone lesionectomy for treatment of pharmacoresistant epilepsy.
  • Paraffin immunohistochemistry using standard methodology was performed on representative sections using antibodies to bcl-XL, bax, bcl-2, and p53.
  • Aberrant expression of bcl-XL, bax, bcl-2, and p53 was observed in the majority of cases, with dysmorphic neurons staining positively for bcl-XL, bax, and bcl-2 in 71%, 76%, and 24% of cases, respectively, and balloon cells staining positively for bcl-XL, bax, and bcl-2 in 89%, 78%, and 17% of cases, respectively.
  • Most cases (86%) showed some expression of p53, with the majority showing expression of p53 most prominently in balloon cells.
  • Previous work has shown gangliogliomas and dysembryoplastic neuroepithelial tumors, both dysplasia-associated neoplasms, to demonstrate aberrant expression of apoptotic markers, suggesting a possible common mechanism of development for these 2 processes in patients in whom they coexist.

  • [Title] Expression patterns of glial fibrillary acidic protein (GFAP)-delta in epilepsy-associated lesional pathologies.
  • AIMS: Glial fibrillary acidic protein (GFAP)-delta is a novel isoform that differs in its C-terminal sequence from other GFAP isoforms.
  • Previous studies suggest restriction of expression to the subpial layer, subventricular zone and the subgranular zone astrocytes, with an absence in pathological conditions causing reactive gliosis.
  • GFAP-delta is speculated to have roles in regulation of astrocyte size and motility and a subpopulation of GFAP-delta-positive glia may be multipotent stem cells.
  • The aim of this study was to investigate its expression in common causes of lesion-related refractory epilepsy.
  • METHODS: Hippocampal sclerosis (HS), focal cortical dysplasia (FCD) type IIB, cortical tuberous sclerosis (TSC) lesions, gangliogliomas, grey matter heterotopias and hemimegalencephaly from a wide age range of patients using both surgical and post mortem tissue specimens were studied.
  • RESULTS: GFAP-delta expression was observed in CA4 and CA1 astrocytes in HS with less frequent labelling in the granule cell layer, even where granule cell dispersion was present.
  • No significant labelling was noted in the subiculum in HS cases or in any subfields in non-HS epilepsy cases.
  • Balloon cells in FCDIIB and hemimegalencephaly, giant cells in TSC and the astrocytic component of gangliogliomas showed immunoreactivity, colocalizing with conventional GFAP.
  • No neuronal expression for GFAP-delta was seen in any of the pathologies.
  • Quantitative analysis in 10 FCDIIB and five TSC cases revealed greater numbers of GFAP-delta-positive balloon cells than conventional GFAP.
  • There was no GFAP-delta expression within nodular heterotopia.
  • CONCLUSIONS: GFAP-delta expression patterns in HS overall appears to mirror regional reactive gliosis.
  • It is a useful marker for the demonstration of balloon cells in FCD and TSC, which may be relevant to their abnormal size and localization.
  • The lack of GFAP-delta within heterotopia supports their composition from cells destined for deeper cortical layers.
