  • Male adnexal tumours of Wolffian duct origin are very rare.
  • We report the case of a 56-year-old male who presented with a swelling in the perineum thought to be a haematoma as the result of a complication of a laparoscopic inguinal hernia repair.
  • Histology revealed the lesion to be an adnexal tumour of probable Wolffian duct origin.
  • To our knowledge there have only been two previous cases of male adnexal tumour of Wolffian duct origin reported in the literature.

  • [Title] Is female adnexal tumour of probable wolffian origin a benign lesion? A systematic review of the English literature.
  • AIMS: To establish the prognosis associated with female adnexal tumour of probable wolffian origin (FATWO) and determine the frequency with which it behaves as a truly benign lesion.
  • One patient with a ruptured tumour had recurrent disease and one other had died of disease (p = 0.2278).
  • There was no association between outcome and age (p = 0.6651), size (p = 0.1912), length of survival (p = 0.2351) tumour site, mitotic rate or necrosis (p = 0.5937, 0.4697 and 0.2016, respectively).
  • CONCLUSION: On the basis of these findings, FATWO cannot be regarded as a benign lesion.

  • [Title] [Characteristics of ocular adnexal non-Hodgkin lymphoma].
  • OBJECTIVE: To investigate the clinical manifestation and pathological features of primary ocular adnexal non-Hodgkin lymphoma.
  • METHODS: Data of 18 patients with biopsy-proven ocular adnexal non-Hodgkin lymphoma at Xiangya hospital were reviewed.The disease site, clinical manifestation,imaging and pathological features of the tumor were summarized.
  • RESULTS: All patients had typical presentation of an adnexal mass.Twelve(66.7%) patients had orbital involvement, 3(16.7%) had conjunctival, and 3(16.7%) had lymphoma involving the eyelids.Eight patients were misdiagnosed as "inflammatory pseudotumour" before the operation according to their clinical and imaging examination,another 8 patients were diagnosed as "ocular adnexal tumour with unknown nature" before the operation.
  • CONCLUSION: The typical presentation of ocular adnexal lymphoma is a painless mass.Orbital connective tissue is the most involved anatomical site.
  • The diagnosis of ocular adnexal non-Hodgkin lymphoma is difficult which could easily be misdiagnosed as "inflammatory pseudotumour".

  • A case of adnexal malignancy developing several decades after initial signs of yellow nail syndrome is reported in a 61-year-old Brazilian female admitted for evaluation of peripheral oedema.
  • Physical examination showed yellow thickened nails and lower limb oedema, and a painless huge adnexal mass.
  • Diverse tumours have been described associated with yellow nail syndrome; however, associations can also occur by chance.

  • : 5589 Background: To determine the incidence of adnexal and lymph node (LN) metastases in endometrial stromal sarcoma (ESS).
  • High-grade or undifferentiated tumors and ESS arising in extrauterine sites were excluded.
  • Adnexal metastases were identified in 8 (9.5%) of 84 cases.
  • All adnexal metastases were manifested by gross adnexal tumor and occurred in patients with other gross pelvic extrauterine disease.
  • In patients with gross pelvic extrauterine disease, adnexal metastasis did not affect survival.
  • The rate of adnexal metastasis appears to be negligible in the absence of gross adnexal and extrauterine tumor, but there may be a role for oophorectomy in the hormonal management of ESS.

  • AIMS: To analyse both the clinicopathological and immunohistochemical findings in six cases of uterine tumours resembling ovarian sex cord tumours with a predominant epithelial retiform component resembling the rete ovarii (RUTROSCT) and to compare their immunophenotype with tissues containing retiform structures such as the normal rete ovarii, retiform Wolffian adnexal tumour (RWAT) and ovarian retiform areas of Sertoli-Leydig cell tumours (ORSLCT).
  • The average age of patients was 65 years, and all tumours behaved in a benign fashion.
  • Histologically they had a tubulopapillary or glomeruloid formation; a sex-cord-like or endometrial stromal component was absent or comprised at most 30% of the tumour.
  • CONCLUSIONS: RUTROSCTs have benign behaviour but may be confused with various uterine adenocarcinomas or metastases.

  • The purpose of this study was to prospectively assess the contribution of diffusion-weighted MR imaging (DWI) for characterizing complex adnexal masses.
  • Seventy-seven women (22-87 years old) with complex adnexal masses (30 benign and 47 malignant) underwent MR imaging including DWI before surgery.
  • ADC measurements did not contribute to differentiating benign from malignant adnexal masses.
  • All masses that displayed simultaneously low signal intensity within the solid component on T2-weighted and on b(1,000) diffusion-weighted images were benign.
  • Alternatively, the presence of a solid component with intermediate T2 signal and high b(1,000) signal intensity was associated with a PLR of 4.5 for a malignant adnexal tumour.
  • DWI signal intensity is an accurate tool for predicting benignity of complex adnexal masses.

  • Pilomatrix carcinoma is the rare malignant counterpart of pilomatrixoma, a skin adnexal tumour originating from hair matrix cells.
  • Pilomatrix carcinoma can arise as a solitary lesion de novo, or through transformation of a pilomatrixoma.
  • They are rare, slow growing tumours of the skin found in the lower dermis and subcutaneous fat and are predominantly found in the neck and the scalp.
  • Since then, approximately ninety cases of pilomatrix carcinoma have been reported.We report the case of a 41 year old mentally retarded male who had a longstanding lesion in the left neck for approximately fifteen years previously diagnosed as a pilomatrixoma.
  • Here we present the pathological features of this uncommon tumour.

  • BACKGROUND: Mucoepidermoid carcinoma (MEC) of the skin is an uncommon neoplasm with a remarkable resemblance to MEC of the salivary glands.
  • Two MECs metastatic to the skin, histologically identical to primary cutaneous MEC, were included, one of which harboured the CRTC1-MAML2 fusion gene by RT-PCR, verified by interphase FISH and sequencing.
  • CONCLUSIONS: MEC of the skin harbours CRTC1 rearrangements, a molecular finding that reflects morphological similarities between glandular and cutaneous MEC.
  • The absence of oncogenic t(11;19) or MAML2 aberrations in our series, which is the largest reported, may explain the innocuous clinical behaviour of this uncommon adnexal tumour.

  • BACKGROUND: Desmoplastic trichoepithelioma (DTE) is an uncommon form of adnexal tumour that was described for the first time as a separate clinicopathological entity in 1976.
  • We carried out a retrospective histopathological study of a large series of cases of DTE in order to better characterise this tumour and compare it with sclerodermiform basal cell carcinoma (BCC), which is in fact the most common as well as the most complex type of differential diagnosis.
  • In six cases (8.8%), a tumour combining DTE with an intradermal naevus was observed.

  • This review evaluates the importance of laparoscopy in surgery of adnexa and especially of malignant tumours.
  • Certainly, morbidity after laparoscopy is lower, but there is a risk of possibly more restricted staging, rupture of the tumour and port metastases.
  • Analysis showed that the standard treatment of ovarian malignant tumours is laparotomy via a vertical incision, because data on laparoscopy are scarce and prospective, randomized trials for early-stage ovarian cancer are still missing.

  • Tumours of cutaneous sweat glands are uncommon, with a wide histological spectrum, complex classification and many different terms often used to describe the same tumour.
  • In addition to the eccrine and apocrine glands, two other skin sweat glands have recently been described: the apoeccrine and the mammary-like glands of the anogenital area.
  • In this review (the second of two articles on skin adnexal neoplasms), common as well as important benign and malignant lesions of cutaneous sweat glands are described, and a summary for differentiating primary adnexal neoplasms from metastatic carcinoma is outlined, striving to maintain a common and acceptable terminology in this complex subject.

  • Here we test whether activation of SOX9 is a general feature of BCC, or whether it could be used as a biomarker to better define subtypes of these skin tumors.
  • In addition we investigated SOX9 expression in other skin epidermal tumors.
  • METHODS: Tumors sections were stained with hematoxylin & eosin (H&E).
  • Staining was heterogeneous and could be detected among the basaloid cells of the palisading cell layer as well as in the tumour nest.
  • SOX9 expression was detected in all adnexal tumors analyzed and absent in Bowen's disease and Merkel tumor.
  • CONCLUSIONS: SOX9 expression is a general feature of BCC and adnexal skin neoplasms, suggesting a contribution of SOX9 to the pathogenesis of these tumors.

  • PURPOSE: To determine the value of diffusion-weighted imaging (DWI) in the evaluation of adnexal tumors, especially to distinguish between malignant and benign tumors.
  • Fifty-four patients with pelvic tumors (32 malignant and 22 benign) were referred for MRI of the pelvis.
  • The signal of solid and cystic tumor components was evaluated on T2W and b=1000 DWI, and ADC values were obtained.
  • RESULTS: T2W hypointensity or absence of hyperintense signal on b=1000 DWI sequences was suggestive of a benign tumor.
  • ADC values did not appear to discriminate between malignant and benign tumors.
  • Also, the signal intensity of cystic tumor components on DWI sequences was not helpful to distinguish between malignant and benign tumor.
  • CONCLUSION: The evaluation of solid tumor components on DWI appears to be helpful for adnexal tumor characterization.

  • OBJECTIVES: The purpose of the study was to evaluate the helpfulness of sonomorphological index in preoperative diagnosis of ovarian tumours.
  • MATERIAL AND METHOD: 686 patients diagnosed and treated because of adnexal tumours were analyzed.
  • At the admission day the patient underwent an ultrasonographical examination that stated morphology of tumour using the sonomorphological index (SM).
  • During the study, histopathological types of ovarian tumours with the lowest prognostic values were appointed.
  • Among malignant tumours the lowest sensitivity of the test corresponded with: dysgerminoma and folliculoma.
  • Among benign tumours the lowest helpfulness in preoperative diagnosis of ovarian tumours the sonomorphological index demonstrated in following histopathological types: mucinouse cystadenoma and adult teratoma.
  • CONCLUSIONS: In all histopathological types of malignant and benign tumours mentioned above, the use of the sonomorphological index require caution.

  • There was significant and widespread nuclear pleomorphism with increased mitotic activity endorsing the diagnosis of a malignant tumor.
  • Seven months after initial surgery, follow-up magnetic resonance imaging showed a mass with a maximum diameter of 7 cm in the region of the right adnexa consistent with a recurrence.

  • AIMS: To describe 13 examples of clear cell dermal duct tumour, a neoplasm previously underrecognized in the literature.
  • METHODS AND RESULTS: Thirteen examples of a neoplasm that we have named clear-cell dermal duct tumour were studied histopathologically and immunohistochemically.
  • In contrast to some other clear cell neoplasms of the skin, which may be associated with diabetes mellitus, none of our cases of clear cell dermal duct tumour developed in a diabetic patient.

  • OBJECTIVE: To evaluate the feasibility and safety of laparoscopic management of adnexal masses > or = 10 cm in size.
  • POPULATION: All women presenting with an adnexal mass > or = 10 cm in diameter were candidates for laparoscopic management.
  • RESULTS: One hundred and eighty-six women underwent laparoscopic evaluation for an adnexal mass of 10 cm or larger in size.
  • A benign pathological condition was found in 86.6% (161/186) of the women, primary ovarian cancer in 16 (8.6%) women, a metastatic tumour of gastrointestinal origin in 1 (0.5%) woman, and a low malignant potential ovarian tumour in 8 (4.3%) women.
  • CONCLUSIONS: The vast majority of large adnexal masses can be safely resected laparoscopically, provided that there is expertise in laparoscopic surgery, immediate access to frozen section diagnosis, and preparation of patient to receive an adequate cancer surgery where indicated.

  • INTRODUCTION: This paper aimed to study the ability of the "ovarian crescent sign" to predict the nature of adnexal masses and to validate the "ovarian crescent" as an ultrasonographical marker for malignancy.
  • METHODS: A prospective study was carried out in 60 consenting women with an undiagnosed adnexal mass, attending the gynaecology service and requiring operative intervention.
  • An ovarian crescent sign at pelvic ultrasonography was considered to be present if normal ovarian tissue was seen adjacent to the tumour area.
  • Histopathological examination report of the tumour obtained at surgery (laparotomy/laparoscopy) was considered as the gold standard.
  • Presence of normal ovarian tissue was identified in 97 percent of the benign masses.
  • CONCLUSION: The ovarian crescent sign as a method in prejudging the adnexal masses was found to have high sensitivity (90.9 percent) and high negative predictive value (97.4 percent).

  • DNA was extracted from 86 lesions including 17 proliferating tricholemmal and trichilemmal tumors, 15 trichoblastomas, 7 trichoadenomas, 4 pilomatricomas, 1 pilomatrical carcinoma, 4 basal cell carcinomas (BCCs) with shadow cells, 2 trichofolliculomas, 3 BCCs with sebaceous differentiation, 9 sebaceous adenomas, 6 sebaceomas, 14 sebaceous carcinomas (both ocular and extraocular forms), 2 gigantic horns, and 2 apocrine mixed tumors with shadow cells and subjected to polymerase chain reaction with newly designed primers encompassing glycogen synthase kinase-3beta phosphorylation sites of the CTNNB1 gene.
  • These included 5 different point mutations, 3 of them identified in 2 different tumors: S23N (cribriform trichoblastoma), D32Y (pilomatricoma and craniopharyngioma), G34R (pilomatrical carcinoma and craniopharyngioma), S37F (2 BCCs with shadow cell differentiation), and G34V (craniopharyngioma).
  • This study broadens the list of cutaneous adnexal tumors harboring CTNNB1 mutations and extends the listing of the mutations occurring in these neoplasms.

  • OBJECTIVE: The risk of metastatic ovarian tumor is significantly higher in case of women with a history of colorectal cancer.
  • DESIGN: The purpose of the study was to evaluate the clinical presentation and histopathology of adnexal tumors in case of female patients with a history of colorectal adenocarcinoma.
  • MATERIAL AND METHODS: A retrospective study on 13 women (each with a history of colorectal carcinoma, operated due to adnexal tumor between 2004 and 2007), has been conducted.
  • Subject characteristics, ultrasound, CT, serum tumor markers levels, histopathology and findings at surgery were analyzed.
  • RESULTS: Time distance between colorectal cancer surgery and ovarian tumor operation - measured in months -was shorter in cases of malignant neoplasms (10.13 +/- 3.98) than in benign tumors (26.2 +/- 19.37).
  • Ultrasound examination showed solid-cystic adnexal tumors in 8 malignant cases, and ovarian cysts in 5 benign conditions.
  • Unilateral adnexectomy only took place in one case of benign tumor and in one case of disseminated neoplasmatic disease.
  • CONCLUSIONS: When evaluating a patient with an adnexal tumor, a history of malignancy strongly suggests a metastatic nature.
  • The use of ultrasound associated with plasma levels of Ca 125, Ca 19-9 and CEA, represents a useful method of preoperative assessment of ovarian tumors.

  • Metastatic adnexal carcinomas are very resistant to conventional chemotherapies.
  • Its use in adnexal carcinomas has never been reported.
  • The second patient had a metastatic malignant hair follicle tumour (trichoblastic carcinoma) and achieved a partial remission with sunitinib, and disease stabilized after 10 months.
  • Dynamic contrast-enhanced ultrasound (DCE-US) performed to evaluate tumour vascularization during treatment depicted a dramatic and early decrease in the tumour blood volume.
  • DCE-US using linear raw data may have an early predictive value for tumour response to sunitinib.
  • Further studies involving larger cohorts of patients are warranted in order to confirm the efficacy of sunitinib in these rare tumours.

  • OBJECTIVE: We recently showed that frozen section diagnosis has an almost perfect specificity for the diagnosis of malignancy in an adnexal mass, whereas the sensitivity was, though good, not perfect.
  • The aim of the present study was to examine whether the accuracy of frozen section diagnosis is affected by the size of the adnexal masses.
  • METHODS: We included women that underwent frozen section diagnosis for adnexal mass surgery.
  • For each patient we recorded serum CA125 level, menopausal status, tumour size, and histologic classification both at frozen section diagnosis and at definite histological examination.
  • We calculated sensitivity and specificity of frozen section diagnoses, both in tumours below and above 10 cm diameter.
  • RESULTS: We included 257 patients, of whom 142 had a benign tumour, 28 had a borderline tumour and 87 had a malignant tumour at definitive histological assessment.
  • In women with a tumour <10 cm, there was only 1 false negative diagnosis in 50 women with a benign frozen section diagnosis, whereas there were 11 false negative diagnoses in 97 women in women with a tumour > or = 10 cm.
  • The corresponding likelihood ratios of a benign diagnosis for presence of malignancy were .15 for tumours > or = 10 cm and 0.03 for tumours <10 cm, respectively.
  • CONCLUSION: The accuracy of frozen section diagnosis is dependent on tumour size.
  • In adnexal masses > or = 10 cm, a benign result of the frozen section diagnosis is less reliable than in women with a tumour <10 in cm.

  • OBJECTIVES: To prospectively assess the diagnostic performance of simple ultrasound rules to predict benignity/malignancy in an adnexal mass and to test the performance of the risk of malignancy index, two logistic regression models, and subjective assessment of ultrasonic findings by an experienced ultrasound examiner in adnexal masses for which the simple rules yield an inconclusive result.
  • DESIGN: Prospective temporal and external validation of simple ultrasound rules to distinguish benign from malignant adnexal masses.
  • The rules comprised five ultrasonic features (including shape, size, solidity, and results of colour Doppler examination) to predict a malignant tumour (M features) and five to predict a benign tumour (B features).
  • If one or more M features were present in the absence of a B feature, the mass was classified as malignant.
  • If one or more B features were present in the absence of an M feature, it was classified as benign.
  • PARTICIPANTS: 1938 women with an adnexal mass examined with ultrasound by the principal investigator at each centre with a standardised research protocol.
  • Reference standard Histological classification of the excised adnexal mass as benign or malignant.
  • RESULTS: Of the 1938 patients with an adnexal mass, 1396 (72%) had benign tumours, 373 (19.2%) had primary invasive tumours, 111 (5.7%) had borderline malignant tumours, and 58 (3%) had metastatic tumours in the ovary.
  • CONCLUSIONS: The use of the simple rules has the potential to improve the management of women with adnexal masses.
  • In adnexal masses for which the rules yielded an inconclusive result, subjective assessment of ultrasonic findings by an experienced ultrasound examiner was the most accurate diagnostic test; the risk of malignancy index and the two regression models were not useful.

  • Adnexal masses are common among peri- and post-menopausal women.
  • Although ovarian cancer is a significant cause of mortality in menopausal women, large population-based studies demonstrate that the majority of adnexal masses are benign.
  • Despite this, the appearance of an adnexal mass is a concern for the patient and an insight exercise for physicians.
  • In most cases, an adnexal enlargement is an incidental finding, generally corresponding to a benign cyst and easily diagnosed by conventional ultrasound.
  • Exceptionally an ovarian tumour may be malignant and should be treated as early as possible.
  • When conventional ultrasound renders complex morphology other diagnostic tools must be used such as: colour Doppler and functional tumour vessel properties, serum CA 125 levels, nuclear magnetic resonance imaging and in some cases laparoscopy.
  • Several new tumour markers are being studied for clinical application, although there are presently no clear recommendations.
  • Adnexal masses with benign morphological and functional properties must be periodically monitored as an alternative to surgery since malignant transformation is exceptional.

  • Most adnexal neoplasms are relatively uncommonly encountered in routine practice, and pathologists can recognise a limited number of frequently encountered tumours.

  • The frozen section and sonographic diagnosis were compared with the final paraffin section diagnosis to determine whether the lesion was felt to be a benign or malignant tumour.
  • Frozen section diagnosis agreed with final paraffin section diagnosis in 137 (77.4%) cases of primary malignant tumours, 201 (90.1%) cases of metastatic disease and 328 (82%) benign cases.
  • Sonographic results matched final pathology of 133 cases of primary malignancy (75.1%) and 192 cases of metastatic disease (86.1%) and 304 benign cases (76%).
  • Our study indicates that preoperative sonography is accurate enough in differentiating adnexal masses to make the routine use of frozen sections unnecessary.

  • PURPOSE: To report a case of a MALT lymphoma of the eyelid, which recurred in several sites over a time period of 14 years, and where the identical B-cell clone could be demonstrated in most samples using polymerase chain reaction (PCR) and GeneScan analysis.
  • Excisional biopsy of the tumour revealed a low-grade malignant B-cell Non-Hodgkin lymphoma (NHL) of MALT type.
  • Despite localized radiochemotherapy, the patient developed recurrences occurring in the pharynx, in the right orbit, in the skin of the right foot, and in the bone marrow 1, 7, 11 and 14 years, respectively, after establishment of the first diagnosis.
  • CONCLUSIONS: It is generally assumed that ocular adnexal MALT lymphoma is associated with an indolent clinical course.

  • BACKGROUND: Laparoscopy is becoming common for the management of complex benign gynaecological diseases, in which considerable adhesions are expected, thus making the laparoscopic surgery more difficult to perform.
  • AIMS: To evaluate the efficacy and safety of laparoscopic surgery for adnexal tumours in post-hysterectomy women.
  • METHODS: In a retrospective study, we reviewed and analysed 40 post-hysterectomy women who were treated surgically for benign ovarian tumours at Kangbuk Samsung Hospital, from May 1999 to April 2007.
  • CONCLUSIONS: Laparoscopic surgery for adnexal tumours in post-hysterectomy women might be an effective and safe alternative to laparotomic surgery.

  • Little is known about the expression of Bcl-x in cutaneous adnexal structures and benign cutaneous adnexal tumors.
  • METHODS: Tissues from 31 cases of benign cutaneous adnexal tumors (five trichofolliculomas, five trichoepitheliomas, two sebaceous adenomas, five apocrine hidradenomas, five eccrine poromas, five eccrine spiradenomas, and four syringomas) were immunostained for Bcl-x.
  • CONCLUSIONS: The degree of Bcl-x expression in cutaneous adnexal glandular structures appears to be related to their mode of secretion, being strongest in cells with apoptotic degradation of the entire cell (sebocytes).
  • This pattern is recapitulated in the corresponding benign cutaneous adnexal tumors.
  • Bcl-x may be useful in identifying cells with sebaceous differentiation in poorly differentiated adnexal tumors.

  • Malignant mixed müllerian tumour of the fallopian tube is an extremely rare lesion and to date only approximately 50 cases have been reported.
  • The tumour is seldom distinguished preoperatively from other more common lesions or ovarian cancer.
  • We report a case of a 60-year-old woman who presented to our hospital with pelvic pain.
  • Ultrasound and abdominal and pelvic computed tomography showed a left adnexal mass.
  • Grossly the left side of the fallopian tube was dilated and the cut surface revealed a solid mass filling the entire lumen.
  • Histological examinations showed a malignant mixed müllerian tumour.
  • The tumor was an admixture of both carcinomatous and sarcomatous elements.
  • The prognosis of a primary malignancy of the fallopian tube is poor and depends more on staging than on histologic type and grade.

  • The clinical presentation of skin adnexal tumors is nonspecific, and histologically; the differential diagnosis between primary cutaneous adnexal malignant carcinomas and metastatic tumors with a visceral origin can be challenging.
  • However, the tumor cells labeled strongly and diffusely not only for pancytokeratin and cytokeratin (CK7) but also with p63 and CK5/6; carcinoembryonic antigen highlighted the ductal structures.
  • Based on the p63 and CK5/6 positivity, the differential diagnosis also included the possibility of a primary adnexal neoplasm and a complete excision was advised.
  • The reexcision specimen revealed residual infiltrating dermal tumor and an overlying intraepithelial component with marked cytologic atypia and focal duct formation, diagnostic of a primary cutaneous adnexal tumor with ductal differentiation (porocarcinoma).
  • This discrimination is of a paramount importance because a diagnostic error can result in profound implications for patient's assumed prognosis and subsequently applied therapy.

  • We used a retrospective analysis to evaluate the efficacy of a wound retractor used in gasless laparoscopic-assisted surgery by the subcutaneous abdominal wall-lift method for the management of large cystic adnexal tumors.
  • In the department of gynecology at a general hospital, 39 patients with large cystic adnexal tumors with an excised tissue weight including cystic contents exceeding 1000 g were treated by gasless laparoscopic-assisted surgery with a wound retractor from January 2001 through December 2006.
  • Intervention was with a gasless laparoscopic-assisted adnexal surgery with a wound retractor.
  • Histopathologic diagnosis of the excised tumor was mucinous cystadenoma, 18 cases; serous cystadenoma, 8 cases; mucinous cystadenocarcinoma of low-grade malignancy, 5 cases; dermoid cyst, 4 cases; paraovarian cyst, 2 cases; endometriotic cyst, 1 case; and clear cell adenocarcinoma, 1 case.
  • Bilateral adnexal tumors were noted in 3 cases.
  • We determined that gasless laparoscopic-assisted adnexal surgery with a wound retractor is an effective, minimally invasive procedure to treat large cystic adnexal tumors.

  • PURPOSE: The aim was to evaluate the risk of ovarian tumor malignancy based on logistic regression analysis and to construct a practical tool which might be used at the bedside.
  • MATERIAL AND METHODS: 379 women with adnexal masses were enrolled to the study The results of the two-dimensional (2D) gray-scale ultrasound examination, color and spectral Doppler blood flow measurement, three-dimensional (3D) sonoangiography examination and serum levels of CA-125 measurement were analyzed.
  • 160 out of 379 women with abnormal findings, (42.2%), were diagnosed with a malignant ovarian tumor and 219 (57.8%) were found to have a non-malignant adnexal mass.
  • Furthermore, the nomograms applicable at the bedside for estimation of the probability of malignancy for the examined adnexal tumor were derived.
  • CONCLUSIONS: The results of the study suggest that the combined use of ultrasound parameters and serum level of CA-125 in the prognostic model improves the diagnostic precision in the primary discrimination of benign and malignant ovarian tumors in women.

  • Herein, we describe our initial experience with isobaric (gasless) LESS-assisted extracorporeal cystectomy in management of selected adnexal tumors.
  • Of 17 consecutive adnexal tumors, 15 (88.2%) (dermoid cyst, 10; serous cystadenoma, 2; mucinous cystadenoma, 1; and paraovarian cyst, 2) were successfully managed with isobaric LESS-assisted extracorporeal cystectomy.
  • Two dermoid cysts required intracorporeal LESS cystectomy because of poor mobility of the adnexal tissue.

  • BACKGROUND: The distinction of metastatic ovarian carcinoma from other metastatic carcinomas and primary adnexal lesions in the skin is often difficult.
  • METHODS: PAX8 immunohistochemical analysis was performed on 24 cases of metastatic adenocarcinomas to the skin and compared with 7 cases of primary adnexal lesions and also compared with p63 immunohistochemical staining results.
  • Patients with metastatic adenocarcinomas had clinically documented primary malignancies, and patients with primary adnexal carcinomas had no known history of another adenocarcinoma.
  • PAX8 immunohistochemical staining in primary adnexal and other cutaneous metastases was completely negative (0/7 and 0/16, respectively).
  • The p63 expression profile supported the distinction between adnexal and metastatic adenocarcinomas.
  • PAX8 expression is a useful marker that effectively discriminated metastatic ovarian carcinomas from metastatic breast carcinomas and primary adnexal tumors.

  • OBJECTIVE: To assess the degree of confidence with which expert ultrasound operators make a diagnosis of benign, borderline and invasive malignant ovarian tumors and its effect on diagnostic accuracy and interobserver agreement.
  • METHOD: Digitally stored static two-dimensional B-mode images of representative cases of benign, borderline and invasive malignant ovarian tumors were independently assessed by three expert ultrasound operators who had not performed the original real-time ultrasound examination.
  • The experts classified the tumors as benign, borderline or invasive malignant and they also indicated the degree of confidence with which they made the diagnosis (certain, probable and uncertain).
  • The diagnostic accuracy and interobserver agreement, in differentiating benign, borderline and invasive malignant ovarian tumors, were calculated depending on the level of confidence with which the diagnosis was made.
  • The agreement in both diagnosis and confidence was lowest in cases of borderline ovarian tumors compared to benign and primary invasive lesions.

  • We chose quantitative tumor density and standard deviation on sonographic images as the new diagnostic parameters.
  • Analysis of variance and Scheffé post hoc test were analyzed to compare the mean tumor density of the endometriomas and follicular cysts.
  • The receiver operating characteristic curve was plotted to choose the cutoff value of the endometrioma tumor density with the best sensitivity and specificity.
  • RESULTS: There was a significant difference when the tumor density of the follicular cyst and endometrioma groups was compared using the vaginal, abdominal or endovaginal approach (p < 0.001).
  • With endovaginal ultrasonography examination, the best cutoff value of tumor density is 28 and the sensitivity and specificity are 64% and 100%, respectively.
  • CONCLUSION: The use of quantitative tumor density and standard deviation on sonographic images is a potential new diagnostic tool in the assessment of endometriomas and follicular cysts.

  • PURPOSE: To test the value of serum CA-125 measurements alone or as part of a multimodal strategy to distinguish between malignant and benign ovarian tumors before surgery based on a large prospective multicenter study (International Ovarian Tumor Analysis).
  • PATIENTS AND METHODS: Patients with at least one persistent ovarian mass preoperatively underwent transvaginal ultrasonography using gray scale imaging to assess tumor morphology and color Doppler imaging to obtain indices of blood flow.
  • RESULTS: Data from 809 patients recruited from nine centers were included in the analysis; 567 patients (70%) had benign tumors and 242 (30%) had malignant tumors-of these 152 were primary invasive (62.8%), 52 were borderline malignant (21.5%), and 38 were metastatic (15.7%).
  • Results were very similar when the models were prospectively tested on a group of 345 new patients with adnexal masses of whom 126 had malignant tumors (37%).
  • CONCLUSION: Adding information on CA-125 to clinical information and ultrasound information does not improve discrimination of mathematical models between benign and malignant adnexal masses.

  • Female adnexal tumors of probable wolffian origin (FATWO) are rare neoplasms believed to originate from mesonephric (wolffian) remnants.
  • Although most cases follow a benign clinical course, approximately 10% of them either recur or metastasize and are thought to be resistant to chemoradiation therapy.
  • In 2004, imatinib therapy, a tyrosine kinase inhibitor known to be effective against gastrointestinal stromal tumors, was reported to be effective also in a case of KIT-positive FATWO.
  • Herein is reported the case of a 50-year-old Japanese woman with FATWO arising in the right paratubal site.
  • The tumor had typical characteristics of FATWO in both morphology and immunohistochemistry.
  • In addition, the cytological features of this rare tumor are presented, which have not been described previously.

  • BACKGROUND: The current classification schemes of adnexal tumours are predominantly based on morphological and immunophenotypical similarities to adult skin structures, whereas a link between the embryology of the skin and the histogenesis of adnexal tumours has been largely neglected.
  • METHODS: Immunohistochemical evaluation with monoclonal antibodies against SOX9 and β-catenin was carried out in embryonic and adult human scalp skin, and BCC and pilomatrixoma samples.
  • CONCLUSIONS:   An appropriate immunophenotyping validated within the conceptual framework of cutaneous developmental biology allows a logical classification of adnexal neoplasms.
  • Expanding this approach further has the potential to revise the current classification schemes so that not only BCC and pilomatrixoma but all adnexal tumours can be categorized logically.

  • The aim was to compare the accuracy of magnetic resonance imaging (MRI) and intraoperative consultation (IC) for the diagnosis of adnexal masses, with reference to final histology.
  • MRI was performed in 136 women with sonographically indeterminate adnexal masses.
  • All masses were characterized as benign or malignant according to previously published MR imaging and histopathologic criteria.
  • Histology revealed 168 adnexal masses (99 benign, 23 borderline and 46 invasive).
  • Among the 151 adnexal masses studied by both MRI and IC, respective sensitivities, specificities, positive and predictive values of both methods for the diagnosis of malignancy were 89.7% and 84.5%, 91.4% and 100%, 86.7% and 100%, and 93.4% and 91.3%.
  • Sensitivities of MR imaging and frozen section for the diagnosis of serous versus mucinous borderline tumors were 33.3% and 93.3%, and 62.5% and 12.5%, respectively.

  • CONCLUSIONS: The distinctive expression pattern of SOX9 during human cutaneous embryogenesis indicates a key role in skin homeostasis that includes but goes beyond its role in outer root sheath differentiation.

  • In five of seven cases of nodular hidradenoma (clear cell), we could easily observe clear cells between 20% and 50% of tumour cells.
  • The positivity for p63 antibody, nuclear pattern, was observed in six of seven cases, in the major part of tumour cells.
  • Limitation Samples are in small number because these are relatively rare tumours.
  • CONCLUSIONS: The present study suggests eccrine origin for both tumours: cylindroma and clear cell hidradenoma.

  • Histologically, a nodular dermal proliferation composed of poorly differentiated epithelioid cells in nests and focally forming ducts with pseudopapillary architecture comprised the primary tumor.
  • Features of a clear cell hidradenoma were noted focally.
  • Metastatic tumor involved nine of 28 nodes.
  • Interphase fluorescence in situ hybridization (FISH) demonstrated chromosomal amplification of the Her-2/neu locus within the tumor and a nodal metastasis.
  • We believe this to be the first demonstration of Her-2/neu amplification in a malignant skin adnexal tumor.

  • BACKGROUND: Hybrid skin adnexal tumors are common, and spiradenocylindroma is well described.
  • OBJECTIVE: However, malignant transformation in this setting is infrequent, especially resemblance to lymphoepithelioma-like carcinoma of skin, which is not associated with Epstein-Barr virus.
  • METHODS: A 65-year-old female presented with ataxia and a skin nodule composed of a hybrid adnexal tumor (spiradenoma and cylindroma) that transitioned into an undifferentiated carcinoma with attendant lymphocytes and plasma cells.
  • The tumor was Epstein-Barr encoded RNAs (EBER) negative by in situ hybridization.
  • No evidence of neuroendocrine differentiation was seen in the tumor, despite the patient having symptoms of paraneoplastic ataxia that improved after surgery.
  • CONCLUSION: This case highlights the transition of a benign hybrid tumor (spiradenocylindroma) into a spiradenocarcinoma that resembled lymphoepithelioma-like carcinoma of skin.

  • PURPOSE: Several scoring systems have been developed to distinguish between benign and malignant adnexal tumors.
  • EXPERIMENTAL DESIGN: In phase I of the International Ovarian Tumor Analysis multicenter study, patients with a persistent adnexal mass were examined with transvaginal ultrasound and color Doppler imaging.
  • The outcome measure was the histologic classification of excised tissue as malignant or benign.
  • We used the International Ovarian Tumor Analysis data to test the accuracy of previously published scoring systems.
  • RESULTS: Data from 1,066 patients were included; 800 patients (75%) had benign tumors and 266 patients (25%) had malignant tumors.
  • The morphologic scoring system used by Lerner gave an area under the receiver operating characteristic curve (AUC) of 0.68, whereas the multimodal risk of malignancy index used by Jacobs gave an AUC of 0.88.

  • OBJECTIVE: The objective of this study was to evaluate the usefulness of the intra-adnexal and extra-adnexal computed tomographic (CT) features in identifying adnexal torsion.
  • PATIENTS AND METHODS: We retrospectively analyzed CT examinations of 38 adnexal masses with torsion and 270 without torsion, which has been surgicopathologically confirmed.
  • The first group included the intra-adnexal features dealing with the ovary and uterine tube, whereas the other included the extra-adnexal features dealing with changes of adjacent anatomical structures such as the uterus, gonadal vein, and peritumoral zone.
  • We acquired statistical proportions for the frequencies of these 2 groups of CT features in ovarian tumors with adnexal torsion versus those without adnexal torsion.
  • RESULTS: When there were intra-adnexal CT features of adnexal torsion, peculiar uterine tube thickening was identified in 74% of the lesions, eccentric or concentric wall thickening in 54% (of only cystic lesions), eccentric septal thickening in 50% (of only cystic lesions except mature cystic teratoma), and eccentric or diffuse decreased or poor contrast enhancement of the internal solid component or thickened wall in 50%.
  • In extra-adnexal CT features of adnexal torsion, uterine deviation to the twisted side was identified in 61% of the lesions, peritumoral infiltration in 40%, nonvisualized anatomic continuity of the ipsilateral gonadal vein with the twisted adnexal mass in 71%, and ascites in 13%.
  • There were significant differences in all of the intra-adnexal and extra-adnexal CT findings, except for ascites, between twisted and nontwisted adnexal tumors.
  • CONCLUSIONS: The intra-adnexal and extra-adnexal groups of CT features are valuable in identifying torsion of adnexal mass.

  • Distinction of primary skin adnexal carcinomas from cutaneous metastasis of adenocarcinomas is challenging.
  • In this study, we evaluated podoplanin immunoreactivity in a series of primary skin adnexal tumors and adenocarcinomas metastatic to skin using a D2-40 antibody.
  • The initial test series were composed of a total of 93 cases including 32 primary skin adnexal carcinomas, 46 benign primary adnexal tumors, and 15 cutaneous metastatic adenocarcinomas.
  • We found that variable D2-40 reactivity was seen in all of the primary cutaneous carcinomas including sebaceous carcinomas (10/10), squamous cell carcinomas (10/10), porocarcinomas (4/4), trichilemmal carcinomas (4/4), skin adnexal carcinomas not otherwise specified (4/4), and in the majority of benign skin adnexal tumors.
  • To confirm the initial findings and to further explore the utility of podoplanin reactivity in the distinction of these tumors, we also examined a test set of 35 unknown cases, including 21 adenocarcinomas metastatic to skin and 14 primary adnexal tumors, in a blinded fashion.
  • Of the 13 D2-40 positive cases, 12 were proven to be primary adnexal tumors.
  • Our results suggest that podoplanin can be a useful tool to distinguish primary skin adnexal carcinomas form adenocarcinomas metastatic to skin with high sensitivity (94.5%) and specificity (97.2%).

  • The purpose of this study was to investigate the pattern of p63 expression in cutaneous adnexal neoplasms and to assess its possible value in the differential diagnosis of primary cutaneous neoplasms vs adenocarcinomas metastatic to the skin.
  • Immunohistochemical analysis for p63 was performed on formalin-fixed, paraffin-embedded archival tissue from 20 benign adnexal tumors, 10 malignant adnexal tumors and 14 adenocarcinomas metastatic to the skin.
  • The expression of p63 was evaluated in epidermal cells, skin appendages and metastatic tumor cells. p63 was consistently expressed in the basal and suprabasal cells of epidermis and cutaneous appendages, including the basal/myoepithelial cells of sweat glands.
  • Out of 20 benign adnexal tumors, 13 (65%) showed strong (score 3) p63 expression; the remaining seven (35%) cases had score 2.

  • When the morphologic distinction between basal cell carcinomas (BCCs) and tichoepitheliomas is unclear, it poses a rare diagnostic challenge as the commonly defined histologic criterion is insufficient for differentiating these two neoplasms from each other.
  • Podoplanin (D2-40) is a well-known lymphatic endothelial surface marker that has been postulated to be upregulated in the outer root sheath of hair follicles and cutaneous neoplasms, such as adnexal tumors, squamous cell carcinomas, etc.
  • A total of 49 cutaneous tumors, including 22 cases of trichoepitheliomas and 27 cases of BCC were examined.
  • This data suggests that D2-40 expression could be a useful potential marker to distinguish BCCs from trichoepitheliomas, especially when there is a high index of histologic suspicion for either of these two tumors.
