  • The cytology of molluscum contagiosum in an 11-year-old boy, which presented atypically as a solitary nodule over the right cheek, mimicking a skin adnexal tumor is reported here.
  • Fine needle aspiration cytology plays a vital role in establishing the correct diagnosis of clinically unsuspected cases, and hence, the proper management of such lesions.
  • The cytology of molluscum contagiosum is characterized by the presence of numerous large intracytoplasmic basophilic bodies that push the host nucleus to the periphery, giving a signet ring appearance to the superficial epidermal cells.

  • Often the distinction of primary adnexal carcinoma from metastatic adenocarcinoma to skin from breast, lung, and other sites can be a diagnostic dilemma.
  • Current markers purportedly of utility as diagnostic adjuncts include p63 and D2-40; however, their expression has been demonstrated in 11-22% and 5% of metastatic cutaneous metastases, respectively.
  • We performed CK15 and nestin, as well as previously reported stains (such as p63, D2-40, and calretinin) on 113 cases (59 primary adnexal carcinomas and 54 cutaneous metastases).
  • Expressions of p63, CK15, nestin, D2-40, and calretinin were observed in 91, 40, 37, 44, and 14% of primary adnexal carcinoma, respectively, and in 8, 2, 8, 4, and 10% of cutaneous metastases, respectively. p63 appeared to be the most sensitive marker (with a sensitivity of 91%) in detecting primary adnexal carcinomas.
  • Using chi(2) analysis, statistically significant P-values (<0.05) were observed for p63, CK15, nestin, and D2-40 in the distinction of primary adnexal carcinoma versus cutaneous metastases.
  • In logistic regression and stepwise selection for predicting a primary adnexal carcinoma, statistical significance was observed for p63, CK15, and D2-40 (P-values: <0.001, 0.0275, and 0.0298, respectively) but not for nestin (P-value=0.4573).
  • Positive staining with all three markers argues in favor of a primary cutaneous adnexal neoplasm.

  • The conjoint occurrence of follicular, sebaceous, or apocrine differentiations in a cutaneous adnexal neoplasm is a known event, more often encountered in benign neoplasms, whereas reports of cutaneous malignant adnexal tumors with bilineage or trilineage differentiation are few.
  • A new case of a cutaneous malignant adnexal neoplasm with multidirectional differentiation is reported here.
  • A 57-year-old woman presented with a long-standing, slowly growing, asymptomatic solitary tumor the size of a large nut in the coccygeal area, which was surgically excised.
  • Microscopically, the neoplasm was located in the dermis with focal extension into the subcutis.
  • We classified this tumor as a well-differentiated adnexal carcinoma demonstrating combined follicular and apocrine differentiation.
  • It differs from previously published cases of malignant adnexal tumors with multidirectional differentiation and further exemplifies the spectrum of diversity encountered in malignant proliferations with differentiation toward the folliculosebaceous-apocrine unit.

  • A 5-year-old, male Bichon-Frise dog presented with a cutaneous mass in the basal region of the auricle.
  • Histologically, the cutaneous neoplasm was comprised of lobules with solid cellular proliferation separated by thin fibrous septa.
  • Immunohistochemical examinations showed that tumor cells were positive for pan-cytokeratin (CK) (AE1/AE3 and CAM5.2), CK8 and CK18, but negative for pan-CK (KL1), CK7, CK14, CK16 and CK20.
  • Double-labeled immunofluorescence testing indicated that neoplastic cells frequently co-expressed CK and vimentin, suggesting divergent differentiation of tumor cells.
  • Based on these findings, the tumor was diagnosed as canine clear cell adnexal carcinoma.

  • Its proliferating cells seem distinctly programmed to undergo terminal differentiation and death.
  • Trisomy 18 was present in a small subset of cells, suggesting a role in pilomatricoma progression, rather than in tumor initiation.
  • We conclude that trisomy 18 is a consistent feature in pilomatricoma, suggesting that genes carried on this chromosome, such as that for the antiapoptotic oncoprotein BCL2, may have a role in the growth and differentiation of this benign self-limited tumor.

  • An unusual tumor of the neck in a 56-year-old female is reported.
  • The tumor was composed of tubules and small cords of epithelial cells dispersed in the fibromyxoid and adipose stroma.
  • At the periphery, the tumor was not encapsulated and its border was intermingled with the subcutaneous fat.
  • Lack of decapitation secretion and the absence of long tubules suggested an eccrine origin; however, in some of the tumor areas, the cells showed brightly eosinophilic copious cytoplasm that may indicate an apocrine differentiation.
  • As an area of chondroid metaplasia was identified, the diagnosis of a mixed tumor was rendered.
  • This unusual type of skin adnexal neoplasm with unique relation of the epithelial component to the surrounding adipose tissue requires differentiation with the primary cutaneous and metastatic carcinomas.

  • AIMS: To describe 13 examples of clear cell dermal duct tumour, a neoplasm previously underrecognized in the literature.
  • METHODS AND RESULTS: Thirteen examples of a neoplasm that we have named clear-cell dermal duct tumour were studied histopathologically and immunohistochemically.
  • In contrast to some other clear cell neoplasms of the skin, which may be associated with diabetes mellitus, none of our cases of clear cell dermal duct tumour developed in a diabetic patient.

  • METHODS: Twenty-six cases of primary cutaneous cribriform apocrine carcinoma were clinically, histopathologically, and immunohistochemically studied.

  • Chondroid syringoma is a rare benign skin adnexal tumor of eccrine/apocrine origin affecting commonly the head and neck region.
  • It used to be previously called as mixed tumor of skin because of the presence of both the epithelial and mesenchymal components.
  • There are only few case reports describing the fine needle aspiration cytologic features of chondroid syringoma for diagnosis.
  • Overlying skin was normal, and the swelling was fixed to the skin but freely mobile over underlying structure.
  • A clinical diagnosis of dermoid cyst was entertained, and the case was subjected to FNAC.
  • On the basis of these cytologic features, a diagnosis of chondroid syringoma was made.
  • We concluded that Chondroid syringoma may be considered as a rare differential diagnosis in the swelling of head and neck region and the diagnosis can be easily confirmed or ruled out by means of fine needle aspiration cytology.

  • OBJECTIVE: Metastatic cutaneous neoplasms may be difficult to differentiate from primary cutaneous neoplasms.
  • Herein, we report an unusual case of metastatic cutaneous carcinoid tumor mimicking an adnexal poroid neoplasm.
  • METHODS: A 53-year-old male man presented with a neoplasm on the vertex of the scalp, clinically resembling a pigmented basal cell carcinoma.
  • After acquiring additional clinical information and the complete excision of the neoplasm, further immunohistochemical stains supported the diagnosis a metastatic carcinoid tumor.
  • CONCLUSION: To our knowledge, this is the first case of metastatic carcinoid tumor reported that has mimicked a poroid neoplasm.

  • Primary mucinous carcinoma of the skin (MCS) is a rare skin appendage tumor and only a few cases of MCS with endocrine differentiation have been reported.
  • Here we report an additional case of primary pure MCS with endocrine differentiation arising in the face.
  • Histopathologically, the tumor was located in the dermis and subcutis, and nests of slight atypical epithelial cells were floating in the mucinous pools.
  • This tumor did not accompany carcinoma in situ or invasive non-mucinous carcinoma and was difficult to differentiate from metastatic mucinous carcinoma of the skin.
  • There was no tumor in the other organs and synaptophysin and chromogranin A were sporadically shown immunohistochemically in the tumor cells; a diagnosis of primary pure MCS with endocrine differentiation was made.
  • Although endocrine differentiation is not uncommon in pure mucinous carcinoma of the breast, there have been only a few reports published on endocrine differentiation in MCS.

  • BACKGROUND: Adnexal carcinomas are rare and diverse cutaneous tumours.
  • Metastatic adnexal carcinomas are very resistant to conventional chemotherapies.
  • Its use in adnexal carcinomas has never been reported.
  • The second patient had a metastatic malignant hair follicle tumour (trichoblastic carcinoma) and achieved a partial remission with sunitinib, and disease stabilized after 10 months.
  • Dynamic contrast-enhanced ultrasound (DCE-US) performed to evaluate tumour vascularization during treatment depicted a dramatic and early decrease in the tumour blood volume.
  • DCE-US using linear raw data may have an early predictive value for tumour response to sunitinib.

  • [Title] Microcystic adnexal carcinoma: an often misdiagnosed, locally aggressive growing skin tumor.
  • Microcystic adnexal carcinoma (MAC) belongs to the spectrum of locally aggressive adnexal carcinomas and most commonly occurs in the head and neck region.
  • Recently it has been proposed that MAC is an apocrine tumor.
  • In 1982, Goldstein and colleagues first reported MAC to be a distinct histologic entity characterized by a combination of keratinous cysts in the upper dermis, islands and strands of small basaloid, benign-appearing keratinocytes or squamous cells in the deeper dermis within a dense desmoplastic stroma, and areas of ductular differentiation.
  • The authors report the case of a 78-year-old woman in whom a diagnosis of MAC was made when a tumor on the right cheek recurred for the second time.
  • Local recurrences of the tumor occurred, despite histographic surgery because in hematoxylin and eosin stains, small islands of the deceptively benign-appearing small basaloid cells of MAC were not recognized as tumor cells.
  • The reported case demonstrates the difficulties in diagnosing MAC and indicates that MAC should always be considered in the differential diagnosis of slowly growing tumors in the head and neck region.

  • CONTEXT: Cutaneous clear cell tumors can pose a diagnostic challenge even to the experienced dermatopathologist; this is partly because of limitations of existing diagnostic categories.
  • OBJECTIVE: To describe a previously unrecognized, distinctive cutaneous adnexal carcinoma capable of an aggressive clinical course.
  • Histologically, adnexal clear cell carcinoma with comedonecrosis was characterized by dermal proliferation of nests of epithelial cells showing distinctive zonal arrangement.
  • The periphery of the tumor nests was formed by squamoid cells merging with centrally located clear cell areas containing foci of comedonecrosis.
  • No ductal, cuticular, or apocrine differentiation was seen.
  • CONCLUSIONS: Adnexal clear cell carcinoma with comedonecrosis appears to be a distinctive adnexal neoplasm that has to be distinguished from more indolent squamous cell and tricholemmal carcinomas.

  • Other cases were diagnosed as epidermal inclusion cyst with the differential diagnosis of well differentiated squamous cell carcinoma and skin appendageal tumor of undetermined origin in one case each.
  • FNAC should be followed by excision biopsy to accurately type the epithelial neoplasm.

  • Histologically, the tumor cells invaded the subcutaneous tissue and mandibular bone.
  • The tumor consisted mainly of squamous and basaloid epithelial nests and cords embedded in a desmoplastic stroma.

  • Desmoplastic trichoepithelioma (DTE), infiltrative/morpheaform basal cell carcinoma (BCC), and microcystic adnexal carcinoma (MAC) are tumors in this category that may be difficult to differentiate, especially when evaluating thin biopsy specimens.
  • An accurate diagnosis has important clinical implications.
  • While DTE is a benign neoplasm with indolent behavior, infiltrative/morpheaform BCC and MAC can be highly aggressive, leading to substantial local destruction and potential metastasis.
  • We present a patient with an unusual tumor demonstrating basaloid cells in the dermis and discuss the diagnostic approach for these lesions, emphasizing the potential role of cytokeratin 20 (CK20) in determining the need for Mohs micrographic surgery.

  • The clinical presentation of skin adnexal tumors is nonspecific, and histologically; the differential diagnosis between primary cutaneous adnexal malignant carcinomas and metastatic tumors with a visceral origin can be challenging.
  • The biopsy showed an intradermal proliferation of malignant epithelioid cells with ductal differentiation, histologically compatible with metastatic breast carcinoma.
  • However, the tumor cells labeled strongly and diffusely not only for pancytokeratin and cytokeratin (CK7) but also with p63 and CK5/6; carcinoembryonic antigen highlighted the ductal structures.
  • Based on the p63 and CK5/6 positivity, the differential diagnosis also included the possibility of a primary adnexal neoplasm and a complete excision was advised.
  • The reexcision specimen revealed residual infiltrating dermal tumor and an overlying intraepithelial component with marked cytologic atypia and focal duct formation, diagnostic of a primary cutaneous adnexal tumor with ductal differentiation (porocarcinoma).
  • Immunohistochemical studies (like p63 and CK5/6) can help to differentiate a primary cutaneous neoplasm from a metastatic lesion.

  • Despite localized radiochemotherapy, the patient developed recurrences occurring in the pharynx, in the right orbit, in the skin of the right foot, and in the bone marrow 1, 7, 11 and 14 years, respectively, after establishment of the first diagnosis.
  • CONCLUSIONS: It is generally assumed that ocular adnexal MALT lymphoma is associated with an indolent clinical course.

  • [Title] CD5-positive mucosa-associated lymphoid tissue (MALT) lymphoma of ocular adnexal origin: usefulness of fluorescence in situ hybridization for distinction between mantle cell lymphoma and MALT lymphoma.
  • Herein is described two cases of CD5-positive MALT lymphoma of ocular adnexal origin.
  • The differential diagnosis between CD5-positive MALT lymphoma and mantle cell lymphoma (MCL), notably cyclin D1-negative MCL, was difficult because both cases consisted histologically of small to medium-sized cells with diffuse or vaguely nodular growth pattern, and the neoplastic cells were positive for CD5 and negative for cyclin D1.
  • Interphase fluorescence in situ hybridization (FISH) on paraffin-embedded section using IgH/cyclin D1 (CCND1) probe showed that in both cases there was no molecular evidence of t(11;14), finally leading to the diagnosis of CD5-positive MALT lymphoma.
  • Although the present two patients had no recurrence over 34 months after initial diagnosis, careful observation is needed because the clinicopathological significance of MALT lymphoma with this rare phenotype remains obscure.

  • METHODS: This prospective, multi-center case series included all patients in Australia treated with MMS for MAC, who were monitored by the Skin and Cancer Foundation between 1993 and 2002.

  • PURPOSE: To report the findings on sentinel lymph node biopsy (SLNB) in 30 patients with ocular adnexal (conjunctival or eyelid) melanomas.
  • PARTICIPANTS: Thirty patients with diagnosis of eyelid or conjunctival melanoma.
  • METHODS: All patients with ocular adnexal melanoma who underwent SLNB at The University of Texas MD Anderson Cancer Center between December 2000 and July 2008 are the subject of this report.
  • MAIN OUTCOME MEASURES: Findings on preoperative lymphoscintigraphy, SLNB, histopathologic examination of the primary tumor and sentinel lymph nodes (SNL), and nodal recurrence after SLNB.
  • RESULTS: Tumor sites were as follows: bulbar conjunctiva only, 14 patients; palpebral conjunctiva only, 8 patients; both bulbar and palpebral conjunctiva, 4 patients; and eyelid skin only, 4 patients.
  • CONCLUSIONS: Sentinel lymph node biopsy is effective for identifying nodal micrometastasis in patients with ocular adnexal melanoma and provides important prognostic information.
  • We recommend consideration of SLNB for patients with intermediate-thickness ocular adnexal melanoma and those with ulceration.

  • A 1-cm-diameter, red, raised, cutaneous mass over the dorsal surface of the left third metacarpal of a 6-year-old neutered male yellow Labrador Retriever was aspirated.
  • The cytologic interpretation was malignant neoplasia with histiocytic inflammation.
  • Histologically, the tumor was diagnosed as a basal cell epithelioma.
  • Neoplasms that once were lumped into the broad histologic diagnosis of basal cell tumors have since been split into distinct entities, dependent on evidence of differentiation into epidermis, trichofollicular epithelium, or sweat or sebaceous glands.
  • Although histologic reclassification has resulted in removal of most of these entities from the original basal cell tumor category, a cytologic diagnosis of basal cell tumor continues to be used to represent the large, heterogeneous group of epidermal, trichofollicular, and adnexal skin tumors with basal cell characteristics.
  • The case in this report demonstrates the heterogeneity of neoplasms that may be diagnosed cytologically as basal cell tumors and supports the need for cytologic criteria and nomenclature that better reflect potential variation in tissue differentiation.

  • Skin adnexal type tumors situated in the parenchyma of the breast are very rare.
  • The tumor was situated in the parenchyma of the breast of a 55-year-old female and showed no connection to the overlying skin on ultrasound and radiology investigations, grossly and microscopically.
  • Histologically, the tumor was identical to its cutaneous counterpart and was surrounded by breast tissue.
  • The neoplasm was composed of solid and cystic areas.
  • Several mammary ducts in close proximity to the tumor showed features of columnar cell hyperplasia.
  • This t(11;19) translocation has been reported in approximately 50% of hidradenomas of the skin.

  • Primary mucinous carcinoma of the skin is an extremely rare adnexal tumor that is thought to originate from eccrine sweat glands.
  • The neoplasm usually arises on the head and neck, with the most commonly involved area being the periorbital region.
  • The tumor is typically a solitary, asymptomatic nodule, cyst, or ulcer that is slow growing with low metastatic potential.
  • Since primary mucinous carcinoma of the skin is such a rare neoplasm (fewer than 130 cases have been reported to date), a complete workup should be conducted to rule out other internal malignancies that may metastasize to the skin.
  • We report a case of primary mucinous carcinoma of the skin, and discuss the clinical presentation, histology, treatment, course, and prognosis.

  • Mucoepidermoid carcinoma (MEC) of the skin is an exceedingly rare but distinctive neoplasm with respect to its histopathologic features.
  • It is similar if not identical in most respects to MEC of the salivary gland, a neoplasm whose prognosis is correlated with the pathologic grade.
  • We report a case of MEC of the skin in a 79-year-old white woman who presented with an axillary mass.
  • Beneath an unremarkable epidermis, a circumscribed, cystic neoplasm, unattached to the surface, was characterized by the presence of vague lobules of low-grade-appearing squamous cells accompanied by mucigenic and clear cells.
  • Inconsistency was encountered in the literature where some confusion existed as to whether MEC is synonymous with adenosquamous carcinoma of the skin.
  • Elsewhere in the body, the latter tumor type is a squamous and gland-forming neoplasm with intermediate- to high-grade features rather than a tumor with mucigenic cells intermingled among intermediate and squamous cells.
  • As in the case of MEC and adenosquamous carcinoma elsewhere in extracutaneous sites, we would propose that a pathologic distinction should be made in the skin for the sake of consistency and for prognostic purposes.
  • Additionally, the immunophenotype of our case is similar to at least two other cases of cutaneous MEC, as well as MEC of the salivary gland, to support the hypothesis that this neoplasm is adnexal rather than epidermal in origin.

  • These neoplasms can derive from preexisting clear cell hidradenomas but more commonly appear de novo, with the molecular events responsible for the pathogenesis currently unknown.
  • Historically, diagnosis has been difficult because of the few cases, inconsistent nomenclature, variable morphology of cells that compose the neoplasm, and confusion with other visceral metastatic tumors.
  • Future studies are required to identify the histopathologic and immunohistochemical features, which may facilitate diagnosis and foster development of molecularly targeted forms of adjuvant therapy.

  • METHODS: Among 70, 000 skin biopsies, 5 were diagnosed as eccrine mixed tumors.
  • No signs of follicular or sebaceous differentiation were seen.
  • The stroma was mucinous and chondroid, calcification ranging from little to extensive with bone formation in 3 examples giving the impression of a chondroid or osseous neoplasm.

  • BACKGROUND: The current classification schemes of adnexal tumours are predominantly based on morphological and immunophenotypical similarities to adult skin structures, whereas a link between the embryology of the skin and the histogenesis of adnexal tumours has been largely neglected.
  • OBJECTIVE: To describe the expression patterns of two proteins with proven relevance for hair follicle homeostasis (SOX9 and β-catenin) during human cutaneous embryogenesis and to compare the findings with their expression in basal cell carcinoma (BCC) and pilomatrixoma.
  • METHODS: Immunohistochemical evaluation with monoclonal antibodies against SOX9 and β-catenin was carried out in embryonic and adult human scalp skin, and BCC and pilomatrixoma samples.
  • CONCLUSIONS:   An appropriate immunophenotyping validated within the conceptual framework of cutaneous developmental biology allows a logical classification of adnexal neoplasms.
  • Expanding this approach further has the potential to revise the current classification schemes so that not only BCC and pilomatrixoma but all adnexal tumours can be categorized logically.

  • BACKGROUND: Mucoepidermoid carcinoma (MEC) of the skin is an uncommon neoplasm with a remarkable resemblance to MEC of the salivary glands.
  • OBJECTIVES: While t(11;19) and rearrangements of the involved loci have been demonstrated in MEC of the salivary gland and other sites, it remains to be determined if morphological similarities in cutaneous MEC are reflected at the molecular level.
  • METHODS: Cases of cutaneous MEC were defined by three histopathological features: (i) cystic dermal nodule with (ii) overlying intact epidermis and (iii) presence of three cell types (squamoid, intermediate, mucinous), and characterized by reverse transcription-polymerase chain reaction (RT-PCR), interphase fluorescent in situ hybridization (FISH) and immunohistochemistry.
  • RESULTS: Eight primary cutaneous MECs were analysed.
  • Two MECs metastatic to the skin, histologically identical to primary cutaneous MEC, were included, one of which harboured the CRTC1-MAML2 fusion gene by RT-PCR, verified by interphase FISH and sequencing.
  • CONCLUSIONS: MEC of the skin harbours CRTC1 rearrangements, a molecular finding that reflects morphological similarities between glandular and cutaneous MEC.
  • The absence of oncogenic t(11;19) or MAML2 aberrations in our series, which is the largest reported, may explain the innocuous clinical behaviour of this uncommon adnexal tumour.

  • Three were basal cell carcinomas with variable staining according to the type of follicular differentiation in each variant.
  • CONCLUSION: Immunohistochemistry for calretinin allows identification of cutaneous adnexal tumors of the hair follicle or a component of the follicle with differentiation towards cells of the outer root sheath.

  • In this study, we investigated the role of the chemokine receptor 4 (CCR4) and its ligand thymus- and activation-regulated chemokine (TARC/CCL17) for the recruitment of T cells in inflamed skin of patients with CLE.
  • We found significant numbers of CCR4+ T lymphocytes in the skin of all patients with CLE.
  • Interestingly, a subset of patients with disseminated scarring skin involvement were characterized by both lesional and circulating CD8+ T cells expressing CCR4.

  • Clear-cell carcinoma of the skin was described by Kuo in 1980 as a cutaneous tumor composed of clear cells that lacked cytoplasmic glycogen or evidence of tricholemmal keratinization.
  • Tricholemmal carcinoma (TC) is conventionally considered to be a neoplasm derived from adnexal keratinocytes with glycogenated clear cells and evidence of outer root sheath or tricholemmal differentiation.
  • The existence of TC has been questioned as it has been argued that without clear immunohistochemical evidence of outer root sheath differentiation, TC cannot be distinguished from clear-cell carcinoma of the skin.
  • Our laboratory has not routinely stained the cases that appear to be carcinomas with clear keratinocytes to determine if glycogen is present and has not made the diagnosis of TC.
  • Selected cases were then stained for immunohistochemical markers (CD34, CK17, and NGFR/p75) that have been used as evidence for tricholemmal differentiation in some studies.
  • Rare cases of SCC-C met the majority of Headington's criteria for TC or showed immunohistochemical evidence of tricholemmal differentiation.
  • Thus, we also conclude that well-differentiated TC is rare and its description in the literature may overstate the case that it is a well-characterized cutaneous neoplasm.

  • BACKGROUND: Apocrine carcinoma of the skin (ACS) is a rare adnexal neoplasm presenting as an indurated slow-growing dermal or subcutaneous plaque that often occurs in the axilla.
  • Histological distinction between ACS and cutaneous metastases of breast carcinoma may be difficult.
  • Histological examination of the skin biopsy showed dermal invasion with atypical cells in an "Indian file" pattern.
  • The pattern of the tumour and immunohistochemical staining suggested a diagnosis of breast carcinoma metastasis.
  • However, the history of a slow-growing tumour and negative initial testing for a primary adenocarcinoma supported the hypothesis of ACS.
  • Diagnosis may be difficult since the pattern of the tumour can be misleading and immunomarkers are not always specific.

  • Trichoblastoma is a rare, benign tumor that differentiates toward the hair germ, the embryonic precursor of a hair follicle.
  • Several cases of trichoblastoma showed multiple differentiation toward more than one type of adnexal structure.
  • A histologic examination indicated that the specimen had features of multiple differentiation.
  • In addition, there were occasional germinative cells in the palisade, and scattered hair papilla-like structures showing follicular differentiation.
  • An immunohistochemical study showed that the neoplasm, or areas in it, stained positive for low molecular cytokeratin, high molecular cytokeratin, EMA, S-100, and GCDFP-15.
  • This is an unusual case of a trichoblastoma with apocrine and sebaceous differentiation.

  • Sex steroids have been postulated to influence pathophysiology of human skin through various skin appendages.
  • The presence of sex steroid receptors has been also reported in adnexal tumors but its details still remained unknown.
  • Therefore, in this study, we immunolocalized sex steroid receptor protein (estrogen receptor (ER)alpha, ERbeta, progesterone receptor (PR)A, PRB and androgen receptor (AR)) in 23 cases of non-pathological skin (male: 10, female: 13) and in 50 cases of skin adnexal tumors (male 24, female 26; 38 benign and 12 malignant).
  • ERalpha immunoreactivity was detected exclusively in basal cells of sebaceous glands of non-pathological skin.
  • In sebaceous gland neoplasms, the number of ERalpha positive cases was significantly lower in skin appendage neoplasms than non-pathological skin.
  • There were no significant differences in PRA, PRB and AR immunoreactivity between non-pathological sebaceous gland and its neoplasm.
  • Therefore sex steroids are considered to play important roles in regulation of non-pathological skin appendage function and pathogenesis and/or development of its neoplasm.
  • In addition, the status of the great majority of sex steroid hormone receptors was maintained throughout the process of neoplastic transformation of skin appendages, except for AR and ERalpha in sweat and sebaceous gland neoplasms.

  • BACKGROUND: We aim to examine p63 expression in basal cell carcinomas (BCCs) and to investigate association with their histopathological differentiation subtypes.
  • METHODS: Eighty-four BCCs were classified according to the histopathologic differentiation subtypes.
  • RESULTS: In nontumoral skin, p63 expression was consistently seen in basal/suprabasal cells of epidermis, hair matrix cells, and outer root sheath of the hair follicle.
  • There was no statistically significant difference between p63 expression and histological differentiation subtypes (p > 0.05).
  • CONCLUSIONS: p63 is consistently expressed in epidermal basal/suprabasal and adnexal basal cells.
  • Most BCCs have higher homogeneous p63 expression than nontumoral epidermis, which is not changed according to histological differentiation subtypes.
  • Thus, overexpression of p63 in all histological subtypes may confirm that basaloid progenitor cells are linked tumor-cell lineage and have a role in the tumorigenesis of BCC.

  • BACKGROUND: Differential diagnosis for a nodule in the nipple or subareolar area of woman includes both primary neoplasms of breast as well as those from skin and adnexae.
  • A biopsy revealed an infiltrating adnexal neoplasm with features similar to those seen in syringomas commonly occurring in locations such as upper face and pubis.
  • Microscopic appearance of such a rare benign infiltrating neoplasm of eccrine duct origin occurring in woman's breast should not be misinterpreted as more common infiltrating primary breast carcinoma.
  • CONCLUSION: Infiltrating eccrine syringomatous adenoma should be included in the differential diagnosis of a nipple or subareolar nodule occurring in woman.

  • OBJECTIVE: To analyze the clinical, histopathologic, and immunohistochemical characteristics of skin metastases.
  • PATIENTS: Fifty-one patients (21 men and 30 women) with biopsy-proven skin metastases and correlative clinical data.
  • INTERVENTIONS: Four dermatopathologists reviewed a random mixture of metastases and primary skin tumors.
  • Immunohistochemical studies for 12 markers were performed on the metastases, with skin adnexal tumors as controls.
  • MAIN OUTCOME MEASURES: Clinical characteristics of cutaneous lesions, clinical outcomes, histologic features, and immunohistochemical markers.
  • RESULTS: Eighty-six percent (43 of 50) of the patients had known stage IV cancer, and skin metastasis was the presenting sign in 12% (6 of 50).
  • Metastases from small cell carcinomas and sarcomas were histologically misinterpreted as primary skin tumors.
  • Immunohistochemical analysis using a panel including p63, B72.3, calretinin, and CK5/6 differentiated metastatic carcinoma from primary skin adnexal tumors.
  • CONCLUSIONS: Cutaneous metastases can have variable clinical appearances and can mimic benign skin lesions.

  • BACKGROUND: Mohs micrographic surgery (MMS) offers high cure rates and maximum tissue preservation in the treatment of more common cutaneous malignancies, but its effectiveness in rare aggressive tumors is poorly defined.
  • OBJECTIVE: Evaluate the effectiveness of MMS in the treatment of six rare aggressive cutaneous malignancies as seen by Mohs surgeons working at a referral center.
  • METHODS: Retrospective chart review of 26,000 cases treated with MMS at the Geisinger Medical Center Department of Dermatology during a 16-year period with the following diagnoses: poorly differentiated squamous cell carcinoma (PDSCC), dermatofibrosarcoma protuberans (DFSP), microcystic adnexal carcinoma (MAC), extramammary Paget's disease (EMPD), Merkel cell carcinoma (MCC), and sebaceous carcinoma (SEB CA).
  • Patient demographic data, tumor measurements, treatment characteristics, and marginal recurrence rates were compiled and evaluated.
  • RESULTS: The mean numbers of cases identified per year for each tumor type were as follows: PDSCC, 6.19; DFSP, 2.44; MAC, 1.63; and EMPD, 0.63.
  • CONCLUSIONS: Collectively, our data on PDSCC, DFSP, MAC, and EMPD, combined with other studies in the literature, show that MMS is the most effective therapy for these rare aggressive cutaneous malignancies.

  • PURPOSE: Extramammary Paget's disease (EMPD) is a rare intraepithelial neoplasm arising in skin that bears apocrine glands.
  • We analyzed clinical, pathological and immunohistochemical staining patterns in 31 cases of EMPD to assess possible predictors of outcome, including patient age, sex, involved organs, surgical margin status, invasion depth, lymphovascular invasion, adnexal invasion, and immunoreactivity for cytokeratin 7 and 20, carcinoembryonic androgen, androgen receptor, p53, c-erbB-2 (Dako, Carpinteria, California) and Ki67.
  • Dermal invasion depth, tumor margin status, adnexal invasion and lymphovascular invasion were determined from the hematoxylin and eosin stained slides of each case.

  • [Title] Primary cutaneous neuroendocrine cell carcinoma (Merkel cell carcinoma) with prominent microcystic features, mimicking eccrine carcinoma.
  • Although primary cutaneous neuroendocrine cell carcinoma [Merkel cell carcinoma (MCC)] may show divergent features, including microcystic ('tubuloglandular'), squamous, eccrine and rhabdomyoblastic, a diffuse microcystic pattern is exceedingly rare.
  • In this study, we present two cases of MCC with prominent microcystic features, which precluded a definitive diagnosis in the initial punch biopsy.
  • Punch biopsies from both patients revealed an infiltrating neoplasm with prominent microcystic features that mimick tubuloglandular structures, lined by hyperchromatic basaloid cells, which were strongly positive for chromogranin and BerEP4, variably positive for CK7 and CK5/6 and negative for CK20, synaptophysin, S-100, epithelial membrane antigen (EMA), gross cystic disease fluid protein-15 (GCDFP-15), estrogen/progesterone receptors (ER/PR), thyroid transcription factor 1 (TTF1) and carcinoembryonic antigen (CEA).
  • The re-excision specimens showed a multifocal intra-epidermal component in one case and typical finely stippled nuclear chromatin in both cases.
  • Although the lack of CK20 staining is unusual, the histologic characteristics along with the remaining immunohistochemical studies favor the diagnosis of a primary cutaneous neuroendocrine cell carcinoma over the variants of eccrine carcinoma or basal cell carcinoma with neuroendocrine differentiation.
  • Our cases illustrate that prominent microcystic features, mimicking glandular differentiation, may occur in MCC and pose a diagnostic challenge in small biopsy specimens.

  • Although pleomorphic adenomas (PAs) usually can be diagnosed very accurately with fine-needle aspiration biopsy (FNAB), even this common salivary gland neoplasm can be diagnostically challenging and cause pitfalls in cytodiagnosis.
  • In particular, the presence of either cystic changes or squamous, mucinous, or sebaceous metaplasia can lead to a false positive diagnosis of mucoepidermoid carcinoma (MEC).
  • The subsequent parotidectomy specimen revealed an encapsulated cystic PA with mixed appendageal differentiation including areas of squamous, mucinous, sebaceous, and oncocytic metaplasia.

  • Clinically considered a syringocystadenoma papilliferum, the excised lesion contained multiple dermal nodules that exhibited nearly all stages of follicular differentiation.
  • This benign lesion of abortive hair follicles was unassociated with any established genodermatous syndrome or other adnexal neoplasm.
