  • BACKGROUND: Adnexal skin tumors share many features in common and differentiate along one line.
  • The aim of this report is to review and classify all adnexal tumors seen in a pathology department over a 16-year period.
  • METHOD: A 16-year retrospective analysis of all adnexal skin tumors seen in a large University Teaching Hospital in Nigeria from January 1991- December 2006.
  • RESULTS: Fifty-two adnexal tumors were seen, accounting for 0.9% of all cutaneous tumors seen within the same period.
  • Only two cases had a clinical diagnosis of adnexal tumor.
  • CONCLUSION: Adnexal skin tumors have distinct histological patterns which differentiates them from other cutaneous tumors.
  • Malignant adnexal tumors are uncommon in our setting.

  • The purpose of this study was to investigate the pattern of p63 expression in cutaneous adnexal neoplasms and to assess its possible value in the differential diagnosis of primary cutaneous neoplasms vs adenocarcinomas metastatic to the skin.
  • Immunohistochemical analysis for p63 was performed on formalin-fixed, paraffin-embedded archival tissue from 20 benign adnexal tumors, 10 malignant adnexal tumors and 14 adenocarcinomas metastatic to the skin.
  • The expression of p63 was evaluated in epidermal cells, skin appendages and metastatic tumor cells. p63 was consistently expressed in the basal and suprabasal cells of epidermis and cutaneous appendages, including the basal/myoepithelial cells of sweat glands.
  • Out of 20 benign adnexal tumors, 13 (65%) showed strong (score 3) p63 expression; the remaining seven (35%) cases had score 2.
  • In contrast, none of the metastatic adenocarcinomas to the skin was positive for p63 (P<0.001).

  • Even though immunohistochemical comparisons of microcystic adnexal carcinoma vs infiltrative basal cell carcinoma and desmoplastic trichoepithelioma exist, they are mostly restricted to the use of a single stain.
  • In this study, we compare the expression of cytokeratin (CK) 15, CK7, CK20, CK903, carcinoembryonic antigen (CEA), CD10, CD15 and BerEP4 in 13 microcystic adnexal carcinoma, eight desmoplastic trichoepithelioma, 10 infiltrative basal cell carcinoma, and eight squamous cell carcinoma of which five exhibited ductal differentiation.
  • We found that the majority of microcystic adnexal carcinoma (92%) and desmoplastic trichoepithelioma (100%) cases expressed CK15 while the infiltrative basal cell carcinoma and squamous cell carcinoma cases were all negative.
  • Forty percent of infiltrative basal cell carcinoma expressed CK7; while only two microcystic adnexal carcinoma cases (15%) and one squamous cell carcinoma with ductal differentiation (12%) expressed CK7 in the remaining three tumor categories.
  • While the neoplastic cells were negative, luminal staining of ductal structures was noted for CK7, CD15 and CEA in some of the microcystic adnexal carcinoma, desmoplastic trichoepithelioma and squamous cell carcinoma with ductal differentiation cases.
  • Sixty percent of infiltrative basal cell carcinoma, 31% of microcystic adnexal carcinoma, and 25% of squamous cell carcinoma express CD10.
  • BerEP4 expression was noted in 38% of microcystic adnexal carcinoma, 57% of desmoplastic trichoepithelioma, 100% of infiltrative basal cell carcinoma, and 38% of squamous cell carcinoma.
  • In conclusion, we found CK15 to be a useful marker in distinguishing microcystic adnexal carcinoma from infiltrative basal cell carcinoma and squamous cell carcinoma with ductal differentiation.
  • Our experience indicates that microcystic adnexal carcinoma and desmoplastic trichoepithelioma have a similar immunohistochemical profile that is, CK15+ and BerEP4+/-; thus, additional studies are needed to separate these two entities.

  • Often the distinction of primary adnexal carcinoma from metastatic adenocarcinoma to skin from breast, lung, and other sites can be a diagnostic dilemma.
  • We performed CK15 and nestin, as well as previously reported stains (such as p63, D2-40, and calretinin) on 113 cases (59 primary adnexal carcinomas and 54 cutaneous metastases).
  • Expressions of p63, CK15, nestin, D2-40, and calretinin were observed in 91, 40, 37, 44, and 14% of primary adnexal carcinoma, respectively, and in 8, 2, 8, 4, and 10% of cutaneous metastases, respectively. p63 appeared to be the most sensitive marker (with a sensitivity of 91%) in detecting primary adnexal carcinomas.
  • Using chi(2) analysis, statistically significant P-values (<0.05) were observed for p63, CK15, nestin, and D2-40 in the distinction of primary adnexal carcinoma versus cutaneous metastases.
  • In logistic regression and stepwise selection for predicting a primary adnexal carcinoma, statistical significance was observed for p63, CK15, and D2-40 (P-values: <0.001, 0.0275, and 0.0298, respectively) but not for nestin (P-value=0.4573).
  • Positive staining with all three markers argues in favor of a primary cutaneous adnexal neoplasm.

  • PURPOSE: To describe a case of adnexal Kaposi's sarcoma successfully treated with intralesional interferon alpha 2b injections.
  • This resulted in a dramatic reduction in tumor burden and lid edema, an improvement in lid position, and corneal healing.
  • CONCLUSION: Ocular adnexal and conjunctival Kaposi's sarcoma was successfully treated with 3 adjuvant intralesional interferon alpha-2b injections leading to a dramatic decrease in tumor mass.

  • BACKGROUND: The distinction of metastatic ovarian carcinoma from other metastatic carcinomas and primary adnexal lesions in the skin is often difficult.
  • METHODS: PAX8 immunohistochemical analysis was performed on 24 cases of metastatic adenocarcinomas to the skin and compared with 7 cases of primary adnexal lesions and also compared with p63 immunohistochemical staining results.
  • Patients with metastatic adenocarcinomas had clinically documented primary malignancies, and patients with primary adnexal carcinomas had no known history of another adenocarcinoma.
  • PAX8 immunohistochemical staining in primary adnexal and other cutaneous metastases was completely negative (0/7 and 0/16, respectively).
  • The p63 expression profile supported the distinction between adnexal and metastatic adenocarcinomas.
  • PAX8 expression is a useful marker that effectively discriminated metastatic ovarian carcinomas from metastatic breast carcinomas and primary adnexal tumors.

  • The expressions of cytokeratin 8 and 18 (CK8 and CK18) in the normal canine skin (2 cases) and cutaneous adnexal tumors (127 cases) were investigated immunohistochemically.
  • In the normal skin, co-expression of CK8/18 was found in the glandular epithelium of apocrine sweat glands, and single CK8-immunoreactivity was detected occasionally in the external root sheath at the isthmus and suprabulbar regions of the hair follicles.
  • In several cases of trichoblastomas (4/28 cases, 14%) and trichoepitheliomas (2/19 cases, 11%), tumor cells forming glandular structures had co-expression of CK8/18.
  • Thus, the combination of CK8- and 18-immunostainings may have the utility to confirm the directions of differentiation in canine cutaneous adnexal tumors providing a reliable hallmark for histopathological diagnoses.

  • Microcystic adnexal carcinoma (MAC) belongs to the spectrum of locally aggressive adnexal carcinomas and most commonly occurs in the head and neck region.
  • Recently it has been proposed that MAC is an apocrine tumor.
  • The authors report the case of a 78-year-old woman in whom a diagnosis of MAC was made when a tumor on the right cheek recurred for the second time.
  • Local recurrences of the tumor occurred, despite histographic surgery because in hematoxylin and eosin stains, small islands of the deceptively benign-appearing small basaloid cells of MAC were not recognized as tumor cells.

  • OBJECTIVE: The purpose of this investigation was to estimate radiation-absorbed dose in orbital tumors from yttrium-90 ibritumomab tiuxetan (Zevalin) radioimmunotherapy of ocular adnexal lymphoma.
  • METHODS: Three patients participating in a prospective research protocol involving treatment of ocular adnexal lymphoma with yttrium-90 Zevalin consented to quantitative radionuclide imaging to estimate tumor radiation-absorbed doses.
  • Each patient received 185 MBq of indium-111 Zevalin, followed by serial planar whole-body scanning, to derive an activity versus time curve for the tumor.
  • Single photon emission computed tomography (SPECT) and computed tomography (CT) imaging, including a calibration source, were performed at 24 h on a SPECT/CT scanner, to obtain a SPECT estimate of the radioactivity (in megabequerels) in the tumor and correct the planar curve, as well as estimate the tumor mass (M) from CT.
  • The curve was then converted to that for yttrium-90 at the prescribed activity, and absorbed dose estimated from the area under the curve multiplied by the Medical Internal Radiation Dose S value (Gy per MBq-h) for a sphere of mass M.
  • RESULTS: A right orbital tumor in one patient was visualized in both the planar and SPECT/CT images, with an estimated absorbed dose of 3.57 Gy.
  • Tumor uptake in the other two patients was not visualized.
  • CONCLUSION: The radiation dose to the orbit and ocular structures during radioimmunotherapy of ocular adnexal lymphoma is well below the threshold for significant radiation-induced ocular toxicity and about 10 times lower than that delivered during external beam radiotherapy.

  • BACKGROUND: The overlap in histopathologic features and immunoprofile of eccrine and apocrine neoplasms confounds basic issues relating to lineage of these entities.
  • METHODS: We evaluated expression of follicular stem-cell markers, cytokeratin (CK) 15 and nestin, in 78 benign and 23 malignant adnexal neoplasms.
  • RESULTS: CK15 and nestin expression were noted in 39 of 78 (50%) and 36 of 78 (46%) cases in the benign group, respectively (8 cutaneous mixed tumor, 10 hidradenoma papilliferum, 9 apocrine cystadenoma, 11 cylindroma and/or spiradenoma, and 9 poroma/dermal duct tumor).
  • CK15 and nestin expression were noted in 11 of 23 (48%) and 7 of 23 (30%) cases in the malignant group, respectively (6 microcystic adnexal carcinoma, 7 porocarcinoma, and 9 eccrine carcinoma).
  • Except 1, both markers were negative in 4 syringocystadenoma papilliferum, 10 hidradenoma, 1 syringofibroadenoma, 10 syringoma, 1 eccrine adenoma, 8 poroma/dermal duct tumor, 5 eccrine hidrocystoma, and 1 apocrine carcinoma.
  • CONCLUSIONS: Given that follicular germinative cells give rise to the folliculosebaceous apocrine unit, expression of CK15 and nestin in the majority of cutaneous mixed tumor, hidradenoma papilliferum, apocrine cystadenoma, and cylindroma/spiradenoma is suggestive of an apocrine origin/differentiation of these neoplasms.
  • Reinforcing this and a novel finding of our study is the preferential expression of nestin in myoepithelial cells of these lesions.

  • BACKGROUND: Sclerosing cutaneous neoplasms often represent a diagnostic challenge.
  • In this study, we report its application in the differential diagnosis of microcystic adnexal carcinoma, desmoplastic trichoepithelioma and basal cell carcinoma.
  • METHODS: Biopsy samples from 28 sclerosing and infiltrating basal cell carcinomas, 13 microcystic adnexal carcinomas and 16 desmoplastic trichoepitheliomas were examined after immunohistochemical staining with Ber-EP4.
  • RESULTS: Ber-EP4 did not label any of the microcystic adnexal carcinomas, whereas all 28 basal cell carcinomas were Ber-EP4 positive.
  • CONCLUSIONS: Ber-EP4 reliably differentiates microcystic adnexal carcinoma from basal cell carcinoma to the same extent as it distinguishes the latter tumor from squamous cell carcinoma.
  • While it stains the majority of desmoplastic trichoepitheliomas, these tumors still have to be considered in the differential diagnosis with microcystic adnexal carcinoma, when Ber-EP4 is applied.

  • Records of patients with histologically confirmed adnexal skin carcinoma managed at a single department during the period 1995 to 2004 were reviewed.
  • Patient biodata, surgical management, and main outcome measures such as locoregional recurrence, distant metastasis, and disease-free and overall survival were recorded.Eleven patients were identified from record analysis.
  • Nine of 11 patients were alive and free of local and/or regional disease on final follow-up.
  • Two patients died of causes unrelated to the disease.
  • Clinical and histological features, tumor biological behavior, diagnostic difficulties, and recommended management are discussed.In conclusion, adnexal skin tumors should be considered in the differential diagnosis of skin tumors, particularly in view of their more sinister prognosis compared with other nonmelanoma skin tumors.

  • Distinction of primary skin adnexal carcinomas from cutaneous metastasis of adenocarcinomas is challenging.
  • In this study, we evaluated podoplanin immunoreactivity in a series of primary skin adnexal tumors and adenocarcinomas metastatic to skin using a D2-40 antibody.
  • The initial test series were composed of a total of 93 cases including 32 primary skin adnexal carcinomas, 46 benign primary adnexal tumors, and 15 cutaneous metastatic adenocarcinomas.
  • We found that variable D2-40 reactivity was seen in all of the primary cutaneous carcinomas including sebaceous carcinomas (10/10), squamous cell carcinomas (10/10), porocarcinomas (4/4), trichilemmal carcinomas (4/4), skin adnexal carcinomas not otherwise specified (4/4), and in the majority of benign skin adnexal tumors.
  • To confirm the initial findings and to further explore the utility of podoplanin reactivity in the distinction of these tumors, we also examined a test set of 35 unknown cases, including 21 adenocarcinomas metastatic to skin and 14 primary adnexal tumors, in a blinded fashion.
  • Of the 13 D2-40 positive cases, 12 were proven to be primary adnexal tumors.
  • Our results suggest that podoplanin can be a useful tool to distinguish primary skin adnexal carcinomas form adenocarcinomas metastatic to skin with high sensitivity (94.5%) and specificity (97.2%).

  • We now investigate the pattern of p63 expression in metastases from skin adnexal carcinomas and their cognate primaries and evaluate p63 expression in a larger case series of malignant cutaneous adnexal neoplasms.

  • [Title] Cutaneous carcinosarcoma: adnexal vs. epidermal types define high- and low-risk tumors. Results of a meta-analysis.
  • RESULTS: CS occurred in elderly patients (mean of 80 years) on sun-damaged skin, and were keratotic papules of short duration.
  • Epidermal-derived (basal or squamous cell carcinoma epithelial component) CS arose on the sun-damaged skin of the head and neck of elderly males (mean age 72 years) and had a 70% 5-year disease-free survival.
  • In contrast, adnexal CS (spiradenocarcinoma, porocarcinoma, proliferating tricholemmal cystic carcinoma, or matrical carcinoma) occurred in younger patients (mean age 58 years), showed recent growth in a long-standing nodule and had a 25% 5-year disease-free survival.
  • Age less than 65 years, recent growth, long-standing skin tumor, and tumor size greater than 2 cm significantly correlated with poor outcome.
  • CONCLUSIONS: Cutaneous CS is an aggressive skin cancer with high risk for advanced disease.

  • OBJECTIVE: To describe a previously unrecognized, distinctive cutaneous adnexal carcinoma capable of an aggressive clinical course.
  • Histologically, adnexal clear cell carcinoma with comedonecrosis was characterized by dermal proliferation of nests of epithelial cells showing distinctive zonal arrangement.
  • The periphery of the tumor nests was formed by squamoid cells merging with centrally located clear cell areas containing foci of comedonecrosis.
  • CONCLUSIONS: Adnexal clear cell carcinoma with comedonecrosis appears to be a distinctive adnexal neoplasm that has to be distinguished from more indolent squamous cell and tricholemmal carcinomas.

  • The clinical presentation of skin adnexal tumors is nonspecific, and histologically; the differential diagnosis between primary cutaneous adnexal malignant carcinomas and metastatic tumors with a visceral origin can be challenging.
  • However, the tumor cells labeled strongly and diffusely not only for pancytokeratin and cytokeratin (CK7) but also with p63 and CK5/6; carcinoembryonic antigen highlighted the ductal structures.
  • Based on the p63 and CK5/6 positivity, the differential diagnosis also included the possibility of a primary adnexal neoplasm and a complete excision was advised.
  • The reexcision specimen revealed residual infiltrating dermal tumor and an overlying intraepithelial component with marked cytologic atypia and focal duct formation, diagnostic of a primary cutaneous adnexal tumor with ductal differentiation (porocarcinoma).
  • Immunohistochemical studies (like p63 and CK5/6) can help to differentiate a primary cutaneous neoplasm from a metastatic lesion.

  • [Title] Cytologic features of primary malignant tumors of skin and adnexae.
  • STUDY DESIGN: Cases of primary malignant tumors of skin and adnexae diagnosed on cytology with histopathology confirmation were retrieved from case records of 1998-2005.
  • RESULTS: Thirty primary malignant tumors of skin and adnexae were analyzed.
  • There were 3 basal cell carcinomas and 2 cases each of sebaceous carcinoma, Paget's disease of the breast and lymphoma.
  • There were single cases of eccrine carcinoma, malignant trichilemmal tumor, undifferentiated carcinoma and extramedullary myeloid cell tumor.
  • All the cases of hemopoietic tumors (n=3) and the majority of the cases (4 of 7) of adnexal were seen to arise from head and neck region.
  • CONCLUSION: Cytodiagnosis of primary malignant tumors of skin and adnexae is possible based on morphology and clinical presentation.

  • [Title] "Squamoid eccrine ductal carcinoma": an unusual low-grade case with follicular differentiation. Are these tumors squamoid variants of microcystic adnexal carcinoma?
  • Squamoid eccrine ductal carcinoma is a rare primary cutaneous tumor exhibiting both squamous and adnexal ductal differentiation.
  • We report an additional case of a slow-growing tumor, occurring on the great toe of a 61-year-old woman, which was unusual as it showed follicular differentiation in addition to squamoid and ductal areas.
  • The lesional cells were positive for cytokeratins 7 and 17, carcinoembryonic antigen (which highlighted the ductal structures), and p63 (favoring a primary cutaneous tumor) and showed low levels of staining with Ki-67 and p53, consistent with a low-grade tumor.
  • We postulate that these tumors may be closely related to microcystic adnexal carcinoma and similarly show differentiation along both follicular and ductal lines, likely indicating folliculosebaceous-apocrine, rather than eccrine, origin or differentiation.

  • Endometrioid carcinoma is a common tumor of the female genital tract, mainly affecting the uterus corpus and ovary.
  • The female adnexal tumor of probable wolffian origin (FATWO), although occasionally seen elsewhere, is most typically encountered in the broad ligament.
  • The tumor was confined to the tube and strongly resembled FATWO.
  • Immunohistochemically, the tumor cells were positive for AE1/AE3, epithelial membrane antigen (EMA), and p16, but negative for CD10, inhibin-alpha, calretinin, vimentin, myogenin, and desmin, CD99,CgA, and Syn.
  • We review the previously published cases of endometrioid carcinomas of the fallopian tube resembling FATWO, and discuss the principal differential diagnosis of the tumor in the fallopian tube.

  • Herein, we describe three unusual cases of Bowen's disease with invasive adnexal carcinoma.
  • The plaque lesions possessed clinical features typical of Bowen's disease.
  • In cases 1 and 3, we confirmed the adnexal tumor within tumors of Bowen's disease, the diagnosis of which is eccrine porocarcinoma.
  • The tumor in case 2 was characteristic to trichilemmal carcinoma.
  • Immunohistochemically, the tumor cells of Bowen's disease and the adnexal carcinoma differed in antigenicities.

  • Little is known about the expression of Bcl-x in cutaneous adnexal structures and benign cutaneous adnexal tumors.
  • METHODS: Tissues from 31 cases of benign cutaneous adnexal tumors (five trichofolliculomas, five trichoepitheliomas, two sebaceous adenomas, five apocrine hidradenomas, five eccrine poromas, five eccrine spiradenomas, and four syringomas) were immunostained for Bcl-x.
  • CONCLUSIONS: The degree of Bcl-x expression in cutaneous adnexal glandular structures appears to be related to their mode of secretion, being strongest in cells with apoptotic degradation of the entire cell (sebocytes).
  • This pattern is recapitulated in the corresponding benign cutaneous adnexal tumors.
  • Bcl-x may be useful in identifying cells with sebaceous differentiation in poorly differentiated adnexal tumors.

  • OBJECTIVE: To find the risk factors of torsion and malignancy for adnexal tumors during pregnancy.
  • PATIENT(S): Patients from 1990 to 2004 with adnexal tumors >or=4 cm during pregnancy.
  • MAIN OUTCOME MEASURE(S): Tumor size, progression, pathology, incidence of malignancy, and torsion.
  • In 174 patients who were followed through pregnancy with known tumor existence, 14.84% +/- 3.05% encountered tumor torsion.
  • Adnexal masses with sizes between 6 and 8 cm had a significantly higher risk of torsion compared with other sizes (22.41% vs. 9.48%; odds ratio 2.8, 95% confidence interval [CI], [1.1, 6.6]).
  • Tumor diameters >or=10 cm at initial diagnosis had a higher risk of malignancy versus smaller sizes (8.77% vs. 0.85%; odds ratio 11.2, 95% CI, [1.3, 97.9]), and tumor growth rates >or=3.5 cm/week also had a significantly higher risk of malignancy versus lower rates (8.33% vs. 0.88%; odds ratio 10.2, 95% CI, [1.0, 101.2]).
  • CONCLUSION(S): Adnexal tumors bearing higher risks for torsion and malignancy should be strongly considered for an aggressive strategy of management during pregnancy.

  • [Title] Microcystic adnexal carcinoma involving a large portion of the face: when is surgery not reasonable?
  • BACKGROUND: We report a case of microcystic adnexal carcinoma (MAC) involving a large portion of the face, one of the largest of any MAC reported thus far in this area, and review the literature regarding the nature of the tumor and available treatments.
  • We also review all of the reported cases of metastases and the possible role of radiation in the etiopathogenesis of this tumor.
  • OBJECTIVE: To review the literature about what is known about therapy for MAC and what options are available to patients who have this disease.
  • CONCLUSION: Mohs surgery should be the treatment of choice for this tumor; however, when extirpation entails sufficiently large morbidity, given the low rate of metastases and mortality, observation is a reasonable alternative.

  • [Title] [The use of 2-dimensional and 3-dimensional color and power-Doppler in the diagnosis of blood flow indices of adnexal tumors].
  • OBJECTIVES: The purpose of this study was to compare 2-dimensional and 3-dimensional color and power-Doppler as measured with blood flow indices for the prediction of malignancy in complex adnexal masses.
  • METHODS: Two hundred eighty women with complex adnexal masses (mean age, 46,3 years (range, 13-83 years) were evaluated by 2-dimensional and 3-dimensional power-Doppler imaging.
  • Complex adnexal mass was defined as the presence of at least one of the following features: solid areas, thick papillary projections, thick septa, or purely solid echogenicity.
  • Tumor volume, sonomorphology and Doppler indices (PI, RI), as well 3-dimensional power-Doppler assessment (VI, FI, VFI) were calculated.
  • However, the highest sensitivity of 3D indices for malignant tumor detection was found for FI (42.9%).
  • CONCLUSION: Three-dimensional power-Doppler imaging has diagnostic potential for the discrimination of benign and malignant complex adnexal masses, but specific, possibly most vascularised areas of each tumor have to be examined in detail to improve not yet satisfactory predictive values of this method.

  • Recently, conflicting data have been published regarding a possible association between Chlamydia psittaci and ocular adnexal MZBCL.
  • In the present study, we analyzed a cohort of ocular adnexal MZBLs from the Netherlands for the presence of C. psittaci DNA.
  • We found no evidence for the presence of C. psittaci DNA in any of the tumor samples studied.
  • Our data do not support a role for C. psittaci in the pathogenesis of ocular adnexal lymphomas in patients from the Netherlands.

  • Here we test whether activation of SOX9 is a general feature of BCC, or whether it could be used as a biomarker to better define subtypes of these skin tumors.
  • In addition we investigated SOX9 expression in other skin epidermal tumors.
  • Staining was heterogeneous and could be detected among the basaloid cells of the palisading cell layer as well as in the tumour nest.
  • SOX9 expression was detected in all adnexal tumors analyzed and absent in Bowen's disease and Merkel tumor.
  • CONCLUSIONS: SOX9 expression is a general feature of BCC and adnexal skin neoplasms, suggesting a contribution of SOX9 to the pathogenesis of these tumors.

  • Microcystic adnexal carcinoma is a rare, locally aggressive cutaneous neoplasm with a high probability of persistence locally but a low probability of metastasis.
  • Histologically, the tumor cells invaded the subcutaneous tissue and mandibular bone.
  • The tumor consisted mainly of squamous and basaloid epithelial nests and cords embedded in a desmoplastic stroma.
  • Our results indicate that microcystic adnexal carcinoma consists of tumor cells capable of both follicular and eccrine differentiation.

  • PURPOSE: To report a series of patients with periocular microcystic adnexal carcinoma (MAC) treated with Mohs' micrographic surgery (MMS).
  • METHODS: This series included all patients treated with MMS for periocular MAC, who were monitored by the Skin and Cancer Foundation Australia between 1993 and 2002.
  • The tumor was initially misdiagnosed as basal cell carcinoma in 3 patients and squamous cell carcinoma in 1 patient.
  • The average number of excision levels required for complete removal of the tumor during MMS was 2.2 (range 1-6).
  • Perineural invasion was recorded in one patient.Five-year follow-up data was available for 3 patients, and there was no tumor recurrence in any of these cases.
  • CONCLUSION: The low 5-year recurrence rate of periocular MAC with MMS emphasizes the importance of margin-controlled excision of this tumor.

  • Here we present the second case of this tumor, which, in contrast to the original example, may be classified as a high-grade neoplasm.
  • The tumor was excised, and the patient had no evidence of recurrence or metastasis 6 years after surgery.
  • Microscopically, the neoplasm demonstrated a fenestrated growth pattern with a slightly myxoid matrix in the background.
  • Despite the close association throughout the tumor, the epithelial and the stromal cells were sharply separated, without transition between both elements.
  • This conclusion is reached after analyzing the embryological development of the hair follicle, its normal histology and the morphology of cutaneous adnexal tumors with follicular differentiation.

  • BACKGROUND: Hybrid skin adnexal tumors are common, and spiradenocylindroma is well described.
  • OBJECTIVE: However, malignant transformation in this setting is infrequent, especially resemblance to lymphoepithelioma-like carcinoma of skin, which is not associated with Epstein-Barr virus.
  • METHODS: A 65-year-old female presented with ataxia and a skin nodule composed of a hybrid adnexal tumor (spiradenoma and cylindroma) that transitioned into an undifferentiated carcinoma with attendant lymphocytes and plasma cells.
  • The tumor was Epstein-Barr encoded RNAs (EBER) negative by in situ hybridization.
  • No evidence of neuroendocrine differentiation was seen in the tumor, despite the patient having symptoms of paraneoplastic ataxia that improved after surgery.
  • CONCLUSION: This case highlights the transition of a benign hybrid tumor (spiradenocylindroma) into a spiradenocarcinoma that resembled lymphoepithelioma-like carcinoma of skin.

  • We report an unusual case of aggressive turban tumor syndrome in a 38-year-old woman with nodules covering her scalp and involving her face, neck, chest, and back.
  • To address concerns regarding hygiene of the scalp and cosmetic disfigurement, she underwent total scalp excision with advancement flap and skin graft reconstruction.
  • Histologic examination of the scalp tumors revealed a predominance of spiradenomas, along with cylindromas and trichoepitheliomas, and tumors containing elements of all 3 of these adnexal neoplasms.
  • We review the literature regarding turban tumor syndrome including the genetic basis for this condition, clinical features, pathology, and treatment.

  • BACKGROUND: Eccrine porocarcinoma is a rare malignant tumor of the sweat gland, characterized by a broad spectrum of clinicopathologic presentations.
  • Instead, eccrine porocarcinoma frequently occurs on the lower extremities, trunk and abdomen, but also on the head, resembling various other skin tumors, as illustrated in the patients described herein.
  • All patients were initially diagnosed as having epidermal or melanocytic skin tumors.
  • CONCLUSION: Eccrine porocarcinomas are potentially fatal adnexal malignancies, in which extensive metastatic dissemination may occur.
  • Porocarcinomas are commonly overlooked, or misinterpreted as squamous or basal cell carcinomas as well as other common malignant and even benign skin tumors.
  • Knowledge of the clinical pattern and histologic findings, therefore, is crucial for an early therapeutic intervention, which can reduce the risk of tumor recurrence and serious complications.

  • Clear-cell carcinoma of the skin was described by Kuo in 1980 as a cutaneous tumor composed of clear cells that lacked cytoplasmic glycogen or evidence of tricholemmal keratinization.
  • Tricholemmal carcinoma (TC) is conventionally considered to be a neoplasm derived from adnexal keratinocytes with glycogenated clear cells and evidence of outer root sheath or tricholemmal differentiation.
  • The existence of TC has been questioned as it has been argued that without clear immunohistochemical evidence of outer root sheath differentiation, TC cannot be distinguished from clear-cell carcinoma of the skin.
  • Thus, we also conclude that well-differentiated TC is rare and its description in the literature may overstate the case that it is a well-characterized cutaneous neoplasm.

  • METHODS: Twenty-seven primary cutaneous SCCs from 25 patients were evaluated for tumor grade using hematoxylin and eosin-stained slides and for percentage and intensity of immunoreactivity for CK7.
  • Of immunoreactive cases, percentage of tumor staining and distribution of immunoreactivity was documented.
  • The remaining immunoreactive tumor was moderately differentiated, with 1+ intensity and 5% staining in an area of microinvasion.
  • This is important to bear in mind when immunohistochemistry is used to distinguish SCC from simulants, such as porocarcinoma, or other adnexal carcinomas with squamous metaplasia.

  • All mammary and extramammary Paget's disease were labeled with CD23.
  • In comparison, pagetoid Bowen's disease, melanoma in situ and sebaceous carcinoma exhibited negative staining.

  • Mohs micrographic surgery provides microscopic evaluation of tumor margins to ensure complete excision of nonmelanoma skin cancers at high risk of recurrence.
  • OBJECTIVE: This purpose of this study is to confirm the histologic accuracy of Mohs excision of facial skin cancers by evaluating an additional layer of tissue with permanent histopathologic sections after Mohs excision.
  • A review of the pathology reports and tissue specimens on these patients provides valuable data on tumor margins and the effectiveness of the Mohs technique for tumor excision.
  • RESULTS: Two excisions of nodular basal cell cancer were determined by the pathologist to have positive tumor involvement on post-Mohs permanent tissue.

  • We report a patient with multiple trichoepitheliomas whose biopsy material also demonstrated a range of other neoplasms with follicular differentiation, including small nodular trichoblastoma, small nodular basal cell carcinoma (BCC), and areas resembling infundibulocystic BCC/basaloid follicular hamartoma.
  • Peripheral blood and tumor tissues of the patient and his 2 daughters, who apparently had a milder phenotype, were studied for alterations in the CYLD and PTCH genes, but mutations or loss of heterozygosity was not found in either gene.
  • The occurrence of multiple follicular neoplasms within a single lesion adds evidence that, although in most cases BCC and trichoblastoma are distinct lesions, the 2 neoplasms do encompass a morphological spectrum of follicular differentiation, which is probably more overtly expressed in syndromic patients.

  • Brooke-Spiegler syndrome is an autosomal dominantly inherited disease with predisposition to cutaneous adnexal neoplasms, most commonly cylindromas and trichoepitheliomas.
  • The histopathological survey revealed a plethora of benign adnexal neoplasms showing apocrine, follicular, and sebaceous differentiation occurring independently and conjointly.
  • Many lesions had hybrid features of two or more neoplasms.
  • By far the most common composite tumor was spiradenocylindroma.

  • We aimed to assess the utility of p63 immunohistochemical staining in distinguishing microcystic adnexal carcinoma (MAC) from sclerosing basal cell carcinoma (SBCC) and desmoplastic trichoepithelioma (DTE).

  • Trichogenic tumors are very rare in genital skin and often cause diagnostic problems because they are mitotically active and they share some histologic features with basal cell carcinomas (BCCs).
  • BCCs showed no trichogenic differentiation and lacked an organized mesenchymal tumor component.
  • The tumor stroma of BCCs was paucicellular, mucinous, or granulation tissue-like with an inflammatory infiltrate.

  • Familial cylindromatosis (Brooke-Spiegler syndrome) is a rare autosomal dominant inherited disease characterized by the development of adnexal tumors, mostly cylindromas, but also trichoepitheliomas and spiradenomas.
  • In 1997 she underwent surgical excision of the entire forehead and scalp with skin grafting.
  • The lesion was excised and histologic and immunohistochemical evaluation revealed a malignant cutaneous biphasic tumor extending into the subcutis, consisting of a major portion with the features of an adnexal carcinoma and of a minor one of atypical spindle cells.
  • Biphasic malignant skin tumors are rare and only a limited number have been described, none in association with the Brooke-Spiegler syndrome.
  • Since the Brooke-Spiegler syndrome is characterized by a germline mutation in the CYLD oncosuppressor gene, a biphasic tumor in this setting may represent a true carcinosarcoma.
