  • [Title] Ultrasonographic features of benign adenomyoepithelioma of the breast.
  • OBJECTIVE: The purpose of this study was to evaluate the ultrasonographic features of benign adenomyoepithelioma of the breast.
  • MATERIALS AND METHODS: Between 2005 and 2009, five patients had histologically confirmed adenomyoepithelioma of the breast.
  • The pathology revealed benign adenomyoepithelioma in all patients.
  • CONCLUSION: Benign adenomyoepitheliomas appear as solid or complex echoic masses with suspicious malignant ultrasonographic features, which may be associated with adjacent ductectasia.
  • Although adenomyoepithelioma is a rare breast tumor, awareness of its sonographic features will be helpful for the differential diagnosis from other tumors.

  • Indeed, adenomyoepithelioma has metastatic potential; however, lymphatic spread is rare and axillary intervention may be over-treatment for most cases.
  • While the imaging descriptions of intracystic adenomyoepitheliomas are very limited, this tumor should be considered in the differential diagnosis to avoid unnecessarily aggressive treatment.

  • METHODS: The authors conducted a retrospective evaluation of cytologic findings in fine-needle aspiration biopsy (FNAB) material from 12 patients with histologically proven benign AMEs of the breast.
  • None of the patients were diagnosed originally with AME: Two tumors were classified as benign and consistent with fibroadenoma, 6 tumors were atypical, 2 tumors were suspicious for carcinoma, and 2 tumors were positive for malignant cells.

  • Adenomyoepitheliomas are rare breast tumors.
  • We report an unusual case of adenomyoepithelioma associated with invasive ductal carcinoma here.
  • The other tumor corresponded to adenomyoepithelioma consisting of an inner layer of neoplastic cells with basophilic cytoplasm and the outer layer of neoplastic cells with clear cytoplasm.
  • Immunohistochemically, some myofibroblasts were observed in the stroma of both adenomyoepithelioma and invasive ductal carcinoma, but no CD34-positive stromal cells were seen in the stroma of either lesion.
  • The stromal reaction of adenomyoepithelioma resembles that of intraductal papilloma in the previous study.
  • To the best of our knowledge, this is the first case of coexistent adenomyoepithelioma and invasive ductal carcinoma of the breast that were discovered as separate nodules.
  • Clinicians and pathologists should be aware of such an association because they need to distinguish such a case from malignant neoplasms arising in adenomyoepithelioma.
  • Additionally, our preliminary report suggests that the stromal response of adenomyoepithelioma may resemble that of intraductal papilloma.

  • The clinicopathologic spectrum ranges from benign to malignant but there are important differences as compared with those of their salivary counterpart.
  • In the breast, benign adenomyoepithelioma is recognized in addition to malignant one, whereas in the salivary gland a histologically similar tumor is designated as epithelial-myoepithelial carcinoma without a separate benign subgroup.

  • Mammary gland adenomyoepitheliomas are benign complex mammary gland tumors composed of neoplastic cells of epithelial and myoepithelial origins, described in many species (humans, dogs, cats, rats) and rarely in mice.
  • We report here an adenomyoepithelioma in a C57BL/6 female mouse.
  • The tumor displayed characteristics of a benign neoplastic proliferation with a compressive growth pattern, and moderate cellular pleomorphism and mitotic index.
  • This is to our knowledge, the first report of a mammary gland adenomyoepithelioma in a C57BL/6 mouse.

  • Recently, a Mammotome biopsy, or stereotactic vacuum-assisted biopsy has become popular and various benign or borderline lesions are obtained.
  • We report an adenomyoepithelioma of the breast in a 56-year-old woman.
  • In examinations of non-palpable lesions found on mammography and ultrasound, a Mammotome biopsy is useful for making diagnosis, however, and adenomyoepithelioma is rarely found.
  • In diagnosing such a rare disease from the limited information obtained from a needle biopsy, an immunohistochemical study was thus found to be useful for making a differential diagnosis.

  • RESULTS: FNAC defined the tumors as benign (n = 4), suspicious (n = 2) and malignant (n = 3), based on the cytopathological criteria of malignancy.

  • For the remaining two cases, the diagnosis was set up after IHC (one fibromatosis and one spindle cell variant of adenomyoepithelioma).
  • The latter technique differentiate benign from malignant BSCTs and is able in 50% of the cases to set up the definite diagnosis.
  • IHC is of value to define the nature of different benign lesions and is mandatory in the malignant ones for optimal treatment.
