  • We present the case of a patient in whom a serous microcystic adenoma, 26 cm in diameter, was found incidentally in the pancreas, the largest reported in the literature thus far.
  • Despite their size, the resection of serous microcystic adenomas, even of that magnitude, is operatively efficient and should always be taken into consideration when examining therapeutic options.

  • Immunohistochemically, the cyst-lining cells were positive for cytokeratin (CK) 7, CK19, MUC1, MUC6, alpha-inhibin, and neuron-specific enolase (NSE), and negative for CK8, CK20, MUC2, and MUC5AC; these immunoprofiles coincide with those of serous microcystic adenoma.

  • CONCLUSION: Although very rare, serous microcystic adenoma might appear within the head of the pancreas and has to be taken into consideration in differential diagnosis of cystic lesions of the head of the pancreas.

  • We followed up a white woman with a giant serous microcystic adenoma over more than 20 years.
  • Finally, the initial biopsy specimen was re-evaluated, using immunohistochemistry, and the final diagnosis of a serous microcystic adenoma was made.
  • CONCLUSION: This unique case demonstrates that the spontaneous course of serous microcystic adenoma of the pancreas may be favourable even with huge tumour size and that immunohistochemistry may prove a valuable tool for differential diagnosis of cystic pancreatic lesions.

  • CASES: We report 2 cases of microcystic adenoma of the pancreas in which an accurate diagnosis could not be rendered on cytology material alone.
  • These case reports demonstrate the difficulty of obtaining diagnostic cells in microcystic adenoma, highlight the use of FNA to increase cellular yield and emphasize the importance of correlating radiologic findings with aspiration of abundant watery fluid from cystic pancreatic masses to make a definitive diagnosis.

  • Serous cystic neoplasms of the pancreas have currently five recognized subtypes: serous microcystic adenoma, serous oligocystic ill-demarcated adenoma, solid serous adenoma, von Hippel-Lindau-associated cystic neoplasm, and serous cystadenocarcinoma.
  • Solid serous adenoma of the pancreas is by far the rarest subtype with only nine cases published thus far.

  • Pancreatic microcystic serous cyst adenomas are rare exocrine tumors composed of small cysts lined by glycogen-rich cells.
  • Four sporadic cases of pancreatic serous cyst adenoma were examined by conventional histological, immunohistochemical, and ultrastructural methods.
  • It is hypothesized that pancreatic serous cyst adenomas might be under the abnormal regulation of the VHL gene, just like VHL disease itself and certain types of renal cell carcinoma, showing the distinctive histology of a rich vascularity intimately related to epithelial lining cells of cysts accompanied by stromal fibroblasts/myofibroblasts and collagenous stroma.

  • [Title] Microcystic stromal tumor of the ovary: report of 16 cases of a hitherto uncharacterized distinctive ovarian neoplasm.
  • We have encountered 16 ovarian neoplasms of probable stromal origin whose most distinctive feature is microcystic change, which is usually conspicuous.
  • On the basis of our extensive experience with ovarian tumors, the neoplasm is unique and warrants separate categorization; we have elected to designate it "microcystic stromal tumor" because of its most striking feature.
  • The microcystic pattern was characterized by small rounded to oval cystic spaces, in areas coalescing to larger irregular channels; intracytoplasmic vacuoles were also frequently present.
  • Microcystic change can be observed in a wide variety of ovarian tumors and the broad potential differential diagnosis is discussed in the text.
  • For tumors which have been well sampled and exhibit (1) a microcystic pattern and regions with lobulated cellular masses with intervening, sometimes hyalinized fibrous stroma, (2) an absence of morphologic features enabling any other specific diagnosis in the sex cord-stromal category, (3) an absence of epithelial elements, and (4) an absence of teratomatous or other germ cell elements, we propose the designation "microcystic stromal tumor."

  • Ductal adenocarcinoma of the lacrimal gland is extremely rare; to our knowledge, only seven de novo cases and one case of ductal adenocarcinoma ex pleomorphic adenoma have been reported in the literature.
  • Here, we report a case of ductal adenocarcinoma ex pleomorphic adenoma of the lacrimal gland.
  • A 70-year-old Japanese female received the resection of the recurrent lacrimal gland tumor (second surgery), under the clinical diagnosis of recurrent pleomorphic adenoma, fifteen years after the initial surgical resection.
  • The resected tumor was composed of recurrent pleomorphic adenoma and adenocarcinoma.
  • Adenocarcinoma component showed infiltrative papillo-tubular or microcystic growth of carcinoma cells, which had pleomorphic large nuclei with prominent nucleoli and rich eosinophilic cytoplasm.
  • Accordingly, the diagnosis of ductal adenocarcinoma ex pleomorphic adenoma was made.

  • Microscopically, the tumors have a lobulated growth pattern and are composed of microcystic and glandular spaces with abundant eosinophilic homogenous or bubbly secretory material positive for periodic acid-Schiff, mucicarmine, MUC1, MUC4, and mammaglobin.
  • This translocation was not found in any conventional salivary AciCC (12 cases), nor in other tumor types including pleomorphic adenoma (1 case) and low-grade cribriform cystadenocarcinoma (1 case), whereas ETV6-NTRK3 gene rearrangements were proven in all 3 tested cases of mammary secretory carcinoma.
