  • We report 6 patients with adenolipomas of the head and neck region: 3 in the parathyroid, 2 in the thyroid, and 1 in the parotid gland.
  • The patients with parathyroid adenolipomas manifested signs and symptoms of hyperparathyroidism, whereas the others complained of a mass in the neck.
  • Computed tomography and magnetic resonance imaging are useful tools for the preoperative diagnosis of adenolipomas of the head and neck.
  • The findings of adipose tissue and benign glandular elements by fine-needle aspiration biopsy should prompt consideration of adenolipoma in the differential diagnosis.

  • OBJECTIVE: To review one institution's experience with parathyroid lipoadenomas and to report the associated clinical characteristics.
  • METHODS: We present a case series of parathyroid lipoadenomas.
  • A parathyroid lipoadenoma was defined as a single adenoma with more than 50% fat on histologic examination in conjunction with primary hyperparathyroidism and resolution of hypercalcemia postoperatively.
  • RESULTS: Five cases of parathyroid lipoadenomas, including 1 oxyphil lipoadenoma, were identified during the period from 1971 to 2001.
  • Parathyroid lipoadenomas were identified in 3 women (60%) and 2 men (40%), and only 1 patient presented with possible hypercalcemia-related symptoms of nephrolithiasis and hip fracture, leading to diagnosis.
  • CONCLUSION: A parathyroid lipoadenoma is a rare cause of primary hyperparathyroidism.

  • Parathyroid lipoadenomas and lipohyperplasias are rare histologic variants with both an increase in stromal fat and parenchyma.
  • We report the most comprehensive single institution series of lipoadenomas and lipohyperplasias to date and review the literature.
  • Eight lipoadenomas and 3 lipohyperplasias (27 y period) were reviewed.
  • Ultrasound localized lipoadenomas in 50% of tested cases whereas sestamibi was successful in 71.4%.
  • Despite increased stromal fat (median: 50%), the weight and the appearance of large, occasionally nodular expansions of parathyroid parenchyma within the fatty stroma distinguished lipoadenomas and lipohyperplasias from normal parathyroid tissue; none of the cases were misclassified as normal on frozen section.
  • Mean weight for lipoadenomas was 1553 mg (range: 173 to 4587 mg), whereas the mean weight for lipohyperplasia glands was 389.1 mg.
  • Lipoadenomas and lipohyperplasias are clinically similar and as histologically diverse as their conventional counterparts.
  • Lipoadenomas are more difficult to localize preoperatively by imaging.

  • Oncocytic lipoadenomas of salivary gland are extremely rare tumors with only two previously reported cases.
  • In this paper, we describe an additional example of oncocytic lipoadenoma showing sebaceous differentiation, a hitherto unreported occurrence.
  • The keratin profile and p63 expression of this oncocytic lipoadenoma suggest the presence of a dual cell population somewhat similar to the dual cell population described in some ultrastructural studies of pure salivary gland oncocytomas and may represent partial basal-cell differentiation.

  • BACKGROUND: Oncocytic lipoadenoma is an uncommon benign salivary gland tumor.
  • Fine needle aspiration cytologic (FNAC) findings of oncocytic lipoadenoma have not yet been documented.
  • However, histopathologic examination showed it to be an oncocytic lipoadenoma, a tumor we were unaware of at the time of cytodiagnosis.
  • CONCLUSION: Both cytopathologists and histopathologists need to be aware of oncocytic lipoadenoma of the salivary gland in order to diagnose it precisely.
