  • The other globulomaxillary lesion was managed under the erroneous diagnosis of adenomatoid odontogenic tumor and recurred 4 times over an 11-year period.
  • This tumor was found in retrospect to be consistent with an adenoid ameloblastoma with dentinoid.
  • The diagnosis of cystic squamous odontogenic tumor (SOT) occurring as a radicular lesion of an impacted lower third molar was one of exclusion.
  • Of two unsuspected keratocystic odontogenic tumors, one depicted deceptive features of pericoronitis, while the other case has long been in our files with the diagnosis of globulomaxillary SOT.
  • Two cases of primary intraosseous squamous cell carcinoma appeared benign clinically and exhibited unexpected findings; an impacted third molar began to erupt in association with the growth of carcinoma and another periradicular carcinoma showed dentinoid formation.

  • Pseudoangiosarcomatous, or pseudovascular, squamous cell carcinoma of skin is an unusual form of acantholytic (adenoid, pseudoglandular) squamous cell carcinoma that mimics the histolopathologic appearance of angiosarcoma.
  • We report a case of pseudoangiosarcomatous squamous cell carcinoma arising adjacent to decubitus ulcers.
  • To our knowledge, our patient is the first with pseudoangiosarcomatous squamous cell carcinoma of skin developing within decubitus ulcer.

  • [Title] [Pseudoangiosarcomatous squamous cell carcinoma of the penis: a case report with clinicopathological and human papilloma virus analyses].
  • OBJECTIVE: To further understand the clinicopathological, ultrastructural and molecular features of penile pseudoangiosarcomatous squamous cell carcinoma (PASCC), and improve its diagnosis and treatment.
  • CT scanning showed no enlarged lymph nodes in the abdomen and pelvis, and X-ray examination revealed no abnormality in the chest.
  • RESULTS: The diagnosis was established by biopsy.
  • Under the light microsope, the tumor was mainly composed of vessel-like lacunar reticularis spindle cells and a few local squamous cancer cells.
  • Careful examination revealed some focal areas with evident transition from squamous nests to the more acantholytic areas extending towards the pseudoangiosarcomatous spaces.
  • Pathogenetically, it appeared to be the variant of acantholytic squamous cell carcinoma.
  • Immunohistochemically, most tumor cells were strongly positive for keratin (AE1/AE3) and focally positive for EMA, with the typical squamous cells focally positive for 34betaE12 and vimentin.
  • CONCLUSION: PASCC is a specific and extremely rare subtype of penile SCC with dramatic similarity to angiosarcoma under the microscope, with poor prognosis.
  • Its diagnosis depends on histopathological, immunohistochemical and ultrastructural studies.
  • Such a presentation underscores the importance of timely consultation, early diagnosis and prompt treatment.

  • From 2002 to 2008, 86 patients have undergone surgical treatment of malignant upper maxillary tumors at the Maxillo-Facial Surgery Department, Tor Vergata University, Rome.
  • We found in our patients a high percentage of cervical metastasis in T2 squamous cell carcinoma of the maxilla (32.1%).
  • Squamous cell carcinoma of the maxilla extending to the oral cavity (T1-T2) shows a higher laterocervical lymphophily than the superoposterior ones (T3-T4).
  • Considering the large percentage of cervical recurrences in T1-T2 squamous cell carcinoma of the maxilla that spread up in the hard palate mucosa and upper gum and the consequently high morbidity, performing a prophylactic laterocervical emptying in these patients could be advisable.

  • Histopathological classification of the neoplasm were sebaceous carcinoma (4 cases), basal cell carcinoma (3 cases), squamous cell carcinoma (3 cases), adenoid cystic carcinoma (1 case), transitional cell carcinoma (1 case), malignant fibrous histiocytoma (1 case), lymphoma (1 case) and mucormycosis (1 case).
  • CONCLUSION: Majority of exenterations performed were for neglected ocular or adnexal malignant lesions that could have been avoided provided patients reported earlier for treatment.

  • [Title] Aberrant cytokine expression in serum of patients with adenoid cystic carcinoma and squamous cell carcinoma of the head and neck.
  • BACKGROUND: Squamous cell carcinoma (SCC) and adenoid cystic carcinoma (ACC) represent 2 clinically important subtypes of head and neck cancer.
  • Our objective was to characterize and compare cytokine profiles in the systemic circulation of patients with SCC and ACC.
  • METHODS: Multiplex analysis of 10 different cytokines (interleukin [IL]-1beta, IL-2, IL-4, IL-5, IL-6, IL-8, IL-10, granulocyte-macrophage colony-stimulating factor [GM-CSF], interferon [IFN]-gamma, and tumor necrosis factor [TNF]-alpha) in the serum of patients with SCC (n = 20) and ACC (n = 20) and healthy controls (n = 20) was performed using the Luminex fluorescent-bead technology.
  • RESULTS: Patients with SCC as well as patients with ACC showed an altered cytokine profile compared with healthy individuals.
  • In patients with SCC, significantly elevated serum levels of the proinflammatory cytokines, IL-6 and IL-8, were observed.
  • CONCLUSIONS: A similar serum cytokine profile, with the predominance of proinflammatory cytokines, was observed in patients with SCC and ACC.

  • BACKGROUND: Primary tracheal tumors other than adenoid cystic or squamous cell carcinoma are uncommon and have a heterogeneous histologic appearance.
  • METHODS: A retrospective analysis was performed of uncommon tracheal tumors among 360 primary tracheal tumors seen over 40 years, excluding adenoid cystic and squamous cell carcinoma.
  • RESULTS: Of 90 patients, 34 (38%) had benign tumors and 56 malignant: 11 carcinoid tumors, 14 mucoepidermoid carcinomas, 13 sarcomas, 15 nonsquamous bronchogenic carcinomas, 2 lymphomas, and 1 melanoma.
  • Dyspnea was the most common symptom in benign tumors and hemoptysis in malignant tumors.
  • After resection, survival at 10 years was 94% for benign and 83% for carcinoid tumors, and at 5 years survival was 60% for bronchogenic carcinoma, 100% for mucoepidermoid tumors, and 78% for sarcomas.
  • CONCLUSIONS: Surgical resection of uncommon primary tracheal tumors alleviates airway obstruction, is curative in patients with benign or slow-growing malignant lesions, and prolongs survival in highly malignant lesions.

  • In lacrimal gland epithelial tumors, benign pleomorphic adenomas are seen most commonly with a well defined benign appearance, and a malignant adenoid cystic carcinoma is seen with a typical invasive malignant appearance.
  • However, a malignant myoepithelial carcinoma is seen with a benign looking appearance.
  • Lymphomatous lesions of the lacrimal gland include a broad spectrum ranging from reactive hyperplasia to malignant lymphoma.
  • The developmental cystic lesions found in the lacrimal fossa such as dermoid and epidermoid cysts can be diagnosed when the cyst involves the superior temporal quadrant of the orbit and manifests as a non-enhancing cystic mass and, in case of a lipoma, it is diagnosed as a total fatty mass.

  • BACKGROUND: Antisense (AS) induced down-regulation of uPAR in ACCS adenoid-cyctic carcinoma cells decreased the cellular adhesion and invasion on various extracellular matrices.
  • Additionally, ACCS-AS cells showed an increased EGFR expression and other behavioral similarities to NA-SCC, a typical highly proliferative but less invasive squamous cell carcinoma (SCC) cell line of the head and neck.
  • ACCS, ACCS-AS and NA-SCC cells were used to elucidate the relationships between uPAR down-regulation and EGFR inhibition.
  • RESULTS: Tyrosine kinase inhibitor Gefitinib (IRESSA, ZD 1839) significantly reduced the chemotactic cell migration and adhesion.
  • In addition, anti-proliferative effect of gefitinib in uPAR down-regulated ACCS-AS was significantly higher than parental ACCS, to levels comparable to gefitinib-sensitive NA-SCC cells.
  • Furthermore, time-lapse videography showed that treatment with gefitinib was also associated with cell rounding and loss of pseudopodia, mostly in ACCS-AS rather than parental ACCS cells.
  • There were also evidences of formation and exocytosis of vacuole-like structures in ACCS-AS, as well as NA-SCC, but not in parental ACCS cells.
  • Interestingly, immunocytochemistry showed that the exocytotic vacuoles actually contained de-activated EGFR.

  • The only false positive was a pleomorphic adenoma misdiagnosed as adenoid cystic carcinoma.
  • The overall accuracy in distinguishing benign from malignant lesions was 79.1%, and the sensitivity for salivary neoplasia was 89.4%.
  • CONCLUSION: Our results are consistent with the literature that salivary gland FNA has good sensitivity, specificity and accuracy in the diagnosis of salivary neoplasms.

  • Adenoid cystic carcinoma (AdCC) cell lines (ACCS and ACCT) showed higher migration responses and adhesion to the extracellular matrix (ECM), especially types I and IV collagen, than did the oral squamous cell carcinoma (SCC) lines (NA and TF).
  • Moreover, AdCC cell lines expressed higher surface levels of urokinase-type plasminogen activator receptor (uPAR) than did SCC cell lines.
  • When AdCC cells were plated on collagen, the surface level of uPAR was increased, and numerous focal adhesions consisting of uPAR, vinculin, and paxillin were assembled; whereas collagen-stimulated SCC cell counterparts or AdCC cells plated on other types of ECM, such as fibronectin, failed to assemble such definite focal adhesions.
  • In order to elucidate the association of uPAR with collagen-induced events, an ACCS-AS cell line transfected with a vector expressing antisense uPAR RNA was established and shown to have reduced uPAR (about 10% that of parental ACCS at both the protein and mRNA levels).

  • PATIENTS AND METHODS: We performed 45 FDG PET studies in 31 patients with salivary malignant tumors, using PET (33 studies) and PET/CT (12 studies).
  • Histology consisted of 8 adenocarcinomas, 8 squamous cell carcinomas, 4 adenoid cystic carcinomas, 4 carcinoma ex pleomorphic adenomas, 2 mucoepidermoid carcinomas, 2 poorly differentiated carcinomas, 1 salivary duct carcinoma, 1 lymphoepithelial carcinoma and 1 melanoma.
  • RESULTS: In the initial staging group, all 12 primary lesions (100%) showed positive FDG uptake (5 squamous cell carcinomas, 2 adenocarcinomas, 2 poorly differentiated carcinomas, 1 carcinoma ex pleomorphic adenoma, 1 salivary duct carcinoma, 1 lymphoepithelial carcinoma).
  • CONCLUSION: This study shows that FDG PET has a significant impact on the management of patients with salivary malignant tumors in both the initial staging and restaging.

  • OBJECTIVE: To summarize the surgical treatment strategy of nasal septum malignant tumor.
  • METHOD: Between 1993 October and 2003 February, 16 patients with nasal septum malignant tumor were treated.
  • The pathology of nasal septum malignant tumor in our department were: seven cases with squamous cell carcinoma in (5 cases were primary, 2 cases were secondary), 4 cases with adenoid cystic carcinoma (3 cases were primary, 1 case was secondary), 3 cases with malignant melanoma, 1 case with adenocarcinoma, 1 case with mucoepidermoid carcinoma.
  • After follow up for 1-10 years, 1 case was undetected, 2 cases were dead (1 case with malignant melanoma, 1 case with secondary squamous cell cancer), 3 cases survive with tumor (1 case with squamous cell cancer, 1 case with secondary adenoid cystic cancer, 1 case with malignant melanoma).
  • CONCLUSION: The prognosis of nasal septum malignant tumor is related to operation, radiation and pathological character.
  • The surgical approach to nasal septum malignant tumor is related to the size of the tumor and the age of patient.

  • BACKGROUND: Primary malignant neoplasms of the trachea are very rare and data relating to them are limited.
  • STUDY DESIGN: Retrospective chart review was conducted for all patients found to have a pathologic diagnosis of primary tracheal malignancy.
  • Among these, 34 (45.9%) were squamous cell carcinomas, 19 (25.7%) were adenoid cystic carcinomas, and 21 (28.4%) were of other histologic types.
  • Most patients (77.3%) were former or current smokers, particularly those with squamous cell carcinoma (93.3%).
  • Patients who had adenoid cystic carcinoma and those with cervical primaries had better rates of disease-specific and overall survival than others (p = 0.036 and 0.006 for the former patient group and p = 0.006 and 0.030 for the latter patient group).
  • Although squamous cell carcinoma was the most common pathology in smokers, adenoid cystic carcinoma was more prevalent among nonsmokers.

  • [Title] Pseudoglandular (adenoid, acantholytic) penile squamous cell carcinoma: a clinicopathologic and outcome study of 7 patients.
  • Almost half of penile squamous cell carcinomas (SCCs) are of the usual type but there is a variegated spectrum of morphologically distinctive subtypes.
  • In a pathologic review of 375 uniformly diagnosed and treated patients with penile SCC, we found 7 tumors with predominant pseudoglandular or adenoid features.
  • The aim of the study was to delineate clinicopathologic features and outcome of an unusual variant of penile SCC.
  • Microscopically, tumors were SCC with acantholytic areas ranging from solid nests with early necrosis or empty pseudoluminal spaces lined by 1 layer of squamous cells or cylindrical cells strikingly simulating glands.
  • The differential diagnosis was with gland forming penile tumors (surface adenosquamous, mucoepidermoid, and urethral adenocarcinomas) and the angiosarcomatoid variant of sarcomatoid carcinomas.
  • The other 5 were either alive with no evidence of disease (12 and 21 y after diagnosis) or died from causes other than penile cancer (3, 4, and 7 y after diagnosis).
  • Comparing with usual SCCs, pseudoglandular SCCs were of higher grade (88% vs. 44%), invaded deeper into CC (71% vs. 52%), and showed a higher incidence of regional metastasis (42% vs. 25%) and higher mortality (29% vs. 19%).

  • Pseudoangiosarcomatous squamous cell carcinoma is an unusual but aggressive variant of acantholytic squamous cell carcinoma of the vulva that mimics angiosarcoma on histology.
  • We present a case of a 57-year-old woman with bilateral inguinal metastatic disease at the time of diagnosis, who died 4 months later because of distant metastatic disease to the lungs.

  • [Title] [The study on the en bloc resection of the external auditory canal to treat external auditory canal carcinoma in the early stage].
  • To improve early diagnosis and the effective surgical management of external auditory canal carcinoma.
  • METHOD: Twelve cases of the early stage external auditory canal carcinoma were reviewed retrospectively.
  • Postoperative pathological diagnosis revealed that 6 cases with adenoid cystic carcinomas, 5 cases of squamous cell carcinomas, 1 case of cerumenal adenocarcinoma.
  • Five cases of squamous cell carcinomas, 1 case of cerumenal adenocarcinoma and 3 cases of adenoid cystic carcinomas received radiotherapy postoperatively.
  • All patients were alive free of carcinoma during the follow-up.
  • CONCLUSION: Timely and accurate biopsy is the key point to diagnose the early stage external auditory canal carcinoma.
  • The complete resection with safety margin of external auditory canal carcinoma can improve the effect of surgery.

  • A variety of tumors, including primary malignant tumors, secondary malignant tumors, and benign tumors, can occur in the tracheobronchial tree.
  • Primary malignant tumors commonly originate from the surface epithelium or the salivary glands, whereas most benign tumors arise from the mesenchymal tissue.
  • At computed tomography (CT), primary malignant tumors manifest as a polypoid lesion, a focal sessile lesion, eccentric narrowing of the airway lumen, or circumferential wall thickening.
  • At fluorine 18 fluorodeoxyglucose (FDG) positron emission tomography (PET), most squamous cell carcinomas show high uptake, whereas adenoid cystic carcinoma and mucoepidermoid carcinoma show variable uptake depending on the grade of differentiation.
  • Carcinoid tumors commonly show intense enhancement at contrast material-enhanced CT, which can be helpful in making the diagnosis, and usually have lower uptake at FDG PET than would be expected for a malignant tumor.
  • Secondary malignant tumors occur as a result of either hematogenous metastasis or direct invasion by a malignancy from an adjacent structure.
  • Their CT manifestations are similar to those of primary malignant tumors, with uptake at FDG PET depending primarily on the metabolic activity and degree of differentiation of the primary tumor.
  • At FDG PET, benign tumors usually show little or no uptake and can be differentiated from malignant tumors.
  • Knowledge of the characteristic CT and FDG PET findings of tracheobronchial tumors can aid in diagnosis and treatment planning.

  • Basal cell adenocarcinoma of minor salivary glands is a relatively rare slow-growing tumor with an infiltrating growth pattern.
  • The infiltrating growth pattern and likelihood of vascular and perineural involvement distinguishes basal cell adenocarcinoma from basal cell adenoma.
  • Other diagnostic considerations include adenoid cystic carcinoma and basaloid squamous carcinoma.
  • Basal cell adenocarcinomas show strong immunoreactivity to cytokeratin 7 and variable myoepithelial staining with S100.
  • It is necessary to differentiate basal cell adenocarcinoma from other basaloid cell tumors of the minor salivary glands because of the prognosis and potential differences in treatment, particularly adenoid cystic adenocarcinoma and basaloid squamous carcinoma.
  • Surgical excision with a wide margin to ensure complete removal has been suggested as the primary treatment for basal cell adenocarcinoma.

  • [Title] Malignant salivary gland tumors and cyclo-oxygenase-2: a histopathological and immunohistochemical analysis with implications on histogenesis.
  • The classification system for malignant salivary gland tumors (MST) is largely dependent on its histogenesis.
  • The histogenesis is uncertain but the "bicellular theory of origin" has been accepted by most and states that malignant transformation of reserve cells from either the intercalated or excretory duct are responsible for the development of MST.
  • Strong cox-2 overexpression was noted in all MST of proposed excretory duct origin: salivary duct carcinoma (100%), mucoepidermoid carcinoma (MEC) (92%), and adenocarcinoma nos (AdC nos) (83%).
  • Primary squamous cell carcinoma (PSCC) was the exception.
  • Negative expression was noted in all tumors of proposed intercalated duct origin (adenoid cystic carcinoma, basal cell adenocarcinoma and acinic cell carcinoma) with the exception of one case of polymorphous low grade adenocarcinoma.
  • Strong cox-2 overexpression was noted in the epidermoid cells of MEC, abluminal duct cells surrounding the duct-like structures and ductal cells of AdC nos and salivary duct carcinoma.

  • MATERIAL AND METHODS: Archival formalin-fixed, paraffin-embedded sections of 54 benign and 56 malignant tumours and 24 samples of normal and inflamed salivary gland tissue were studied immunohistochemically using an Envision/horseraddish peroxidase (HRP) technique.
  • RESULTS: In normal and inflamed salivary gland samples, E-cadherin was expressed at the membrane of acinar, myoepithelial and ductal cells located at cell-cell contact points.
  • Furthermore, a weak to moderate loss of expression which was related to tissue tumour subtype was seen in malignant tumours such as: adenoid cystic carcinomas; polymorphous low-grade adenocarcinomas; acinic cell carcinomas; and mucoepidermoid low-grade, epithelial-myoepithelial, lymphoepithelial and squamous low-grade carcinomas.
  • Moderate to extreme loss or alternative cytoplasmic non-functional expression were observed in cases of salivary ductal carcinoma, carcinosarcoma, myoepithelial carcinoma, oncocytic adenocarcinoma, unspecified adenocarcinoma and squamous high-grade carcinomas.

  • METHODS: A retrospective study was conducted on the clinical data of 23 cases lacrimal gland tumor involving the anterior and middle cranial fossae confirmed by radiological examination, including 11 cases of adenoid cystic carcinoma, 6 cases of pleomorphic adenocarcinoma (malignant mixed tumor), 2 cases of adenocarcinoma, 1 case of squamous cell carcinoma, 1 case of ductal carcinoma, 1 case of mucoepidermoid carcinoma, and 1 case of benign mixed tumor, 15 males and 8 females, aged 42.5 (2 - 76), with a case history of 43 months (2 months to 27 years), with the chief complaints of progressive proptosis, disgenesia of the eye ball, and orbit pain, all undergoing transcranial-orbital operation from August 1998 to February. 2006.
  • CONCLUSIONS: Malignant lacrimal gland tumors, mainly adenoid cystic carcinomas, incline to involve the anterior and middle cranial fossae.

  • BACKGROUND: The present study is aimed to assess the usefulness of silver nitrate staining of nucleolar organizer regions (NORs) as a quantitative criterion for the diagnosis of selected head and neck tumors.
  • The samples consisted of 21 squamous cell carcinoma (SCC) of larynx, 28 SCC of oral mucosa and 36 samples of most common salivary gland tumors.
  • RESULTS: A significant difference was seen in the number of AgNOR dots between oral and laryngeal SCC with surrounding dysplastic and normal tissues (P < 0.001) and also between mucoepidermoid carcinoma and adenoid cystic carcinoma with pleomorphic adenoma and normal salivary gland tissue (P < 0.001).
  • CONCLUSION: The silver nitrate staining for NORs is a useful method for aiding the diagnosis of malignant and dysplastic mucosal lesions and also malignant and benign salivary gland tumors.

  • BACKGROUND AND OBJECTIVES: The rarity of malignant tumors in the periorbital area makes it difficult to assess the patterns of lymphatic spread and to determine the optimal extent of treatment.
  • The objective of this study was to analyze the patterns of lymphatic metastasis in malignant tumors arising from the periorbital area according to the primary site and to assess the optimal extent of surgical treatment for these tumors.
  • MATERIALS AND METHODS: We retrospectively reviewed the medical records of 8 patients with malignant tumors in the periorbital area from January 1997 to June 2002.
  • The pathological diagnoses were 3 squamous cell carcinomas, 2 adenoid cystic carcinomas, 2 sebaceous carcinomas, and 1 basal cell carcinoma.
  • CONCLUSION: The results of this study suggest that more aggressive management of the parotid area is necessary in malignant tumors arising from the periorbital area, even in the medial half of the periorbital area.

  • INTRODUCTION: Primary tracheal tumors are rare, and composed of various benign and malignant pathologies.
  • The aims of this study were to establish the clinical manifestations and treatment outcomes of primary tracheal tumors and explore the prognostic variables for malignant tracheal tumors.
  • METHODS: From January 1989 through December 2006, 37 patients (14 with benign tumors, 23 with malignant tumors) were treated in a tertiary referral center.
  • Squamous cell carcinomas (n = 11) and adenoid cystic carcinomas (n = 9) comprised most of the malignant tumors.
  • Five-year overall survival rate was 41.1% for squamous cell carcinoma and 45.7% for adenoid cystic carcinoma and no statistically significant difference (p = 0.673) was observed.
  • Among malignant tumors, surgery should be considered as the first choice of treatment, regardless of the histologic type, if the tumors are resectable.

  • PATIENTS: All patients with biopsy-proved malignant neoplasm of the sinonasal region who were treated with endoscopic resection between 1992 and 2007 were included in the study, and their charts were reviewed for demographics, histopathologic findings, treatment details, and outcome.
  • The most common tumor types were esthesioneuroblastoma (17%), sarcoma (15%), adenocarcinoma (14%), melanoma (14%), and squamous cell carcinoma (13%).
  • Other, less common tumors included adenoid cystic carcinoma (7%), neuroendocrine carcinoma (4%), and sinonasal undifferentiated carcinoma (2%).
  • CONCLUSIONS: To the best of our knowledge, this is the largest US series to date of patients with malignant tumors of the sinonasal tract treated with endoscopic resection.

  • [Title] Adenoid cystic carcinoma of the trachea: a long-term problem.
  • Adenoid cystic carcinoma of the trachea, although rare, is the second most common primary tumour of the trachea.
  • It is a slow-growing tumour found in younger patients than the more common squamous cell carcinoma and is relatively resistant to treatment, but metastasizes late in the course of disease and even in unresectable cases can be palliated successfully for many years.
  • Details collected included symptoms before diagnosis, length of time from onset of the first symptom to diagnosis, resection details, survival statistics and accessory procedures tried before and after consideration of resection.
  • The mean time of diagnosis from the onset of symptoms was 16 months.

  • After carcinoma were completely resected and defects were reconstructed by pectoralis major myocutaneous flap of 7 cm x 5 cm-12 cm x 9 cm.
  • Pectoralis major myocutaneous flap is an ideal material in repair of defect caused by resection of recurrent tonsillar carcinoma after radiotherapy.

  • PURPOSE: To determine the efficacy, toxicity and survival of concurrent therapy with vinorelbine and a platinum compound with radiotherapy (RT), followed by consolidation chemotherapy with the same drugs, for locally advanced non small cell lung cancer (NSCLC).
  • PATIENTS AND METHODS: Fifty-seven patients with stage III NSCLC were included in this phase II study: median age 56 years (range 44-71), males / females 49/8, ECOG performance status (PS) 1/2=27/30, stage IIIA/ IIIB 11/46, squamous cell carcinoma 44, adenocarcinoma 7, adenoid cystic carcinoma 1 and large cell carcinoma 5.

  • PURPOSE: Ethmoidal malignant tumors, for which intensity-modulated radiotherapy is expected to improve outcome, consist of heterogeneous pathological types.
  • Reports about their outcome are influenced by the inclusion of favorable histology, such as adenocarcinoma and adenoid cystic carcinoma.
  • We investigated the long-term treatment outcome of squamous cell carcinoma and undifferentiated carcinoma of the ethmoid sinus.
  • MATERIALS AND METHODS: Between August 1976 and April 2002, 25 patients (20 squamous cell carcinomas and five undifferentiated carcinomas) received radical radiotherapy or preoperative radiotherapy in our institution.
  • CONCLUSION: Ethmoid squamous cell carcinoma or undifferentiated carcinoma was diagnosed at advanced T stages and was treated with radiotherapy; these patients had a poorer outcome than patients with adenocarcinoma or adenoid cystic carcinoma.

  • BACKGROUND: Basaloid squamous cell carcinoma is a high-grade variant of squamous cell carcinoma that most commonly occurs in the upper aerodigestive tract.
  • The authors describe the cytologic features of a histologically confirmed case of basaloid squamous cell carcinoma arising in the floor of the mouth.
  • Extracellular basement membrane-like material was also noted, which led to the erroneous diagnosis of adenoid cystic carcinoma.
  • Histopathology characteristically revealed the presence of squamoid nests between malignant basaloid cells, and the tumor was finally diagnosed as basaloid squamous cell carcinoma.
  • CONCLUSION: Basaloid squamous cell carcinoma should be considered in the differential diagnosis of tumors arising in oral cavity.
  • Cytologic diagnosis may be difficult, but careful search for a second cell population of squamoid cells in addition to small basaloid cells may aid in rendering the correct diagnosis.

  • [Title] Utility of high molecular weight cytokeratins, but not p63, in the differential diagnosis of neuroendocrine and basaloid carcinomas of the head and neck.
  • High-grade neuroendocrine carcinomas of the head and neck overlap significantly in morphology with both basaloid squamous and solid-type adenoid cystic carcinomas.
  • Basaloid squamous and adenoid cystic carcinomas are aggressive variants of their respective tumor types which both have nests of basaloid tumor cells with round nuclei, little cytoplasm, and inconspicuous nucleoli.
  • We performed high molecular weight cytokeratin (CK) and p63 immunohistochemistry on 19 neuroendocrine carcinomas, 18 basaloid squamous carcinomas, and 11 solid-type adenoid cystic carcinomas.
  • All basaloid squamous and adenoid cystic carcinomas were positive for CK 5/6 and 34betaE12.
  • Staining was focal in the neuroendocrine cases when positive, whereas almost all basaloid squamous and adenoid cystic carcinomas showed strong staining.
  • Basaloid squamous carcinomas were diffusely positive, neuroendocrine carcinomas were diffusely positive, but with weak staining, and adenoid cystic carcinomas showed a distinct pattern with staining at the periphery of the cell nests only.

  • It is generally accepted that the histological types of breast cancer are clinically significant because they serve as prognosticators and as the common language for improving the accuracy of clinical diagnosis.
  • It is necessary to diagnose breast cancer at the level of not only histological findings by needle biopsy, but also the histologic type based on diagnostic imaging and cytological diagnosis.
  • Preoperative drug therapy can be planned by making a histological diagnosis based on needle biopsy findings.
  • Preoperative drug therapy is not indicated for noninvasive carcinoma and papillotubular carcinoma (invasive carcinoma with predominant intraductal components).
  • While complete loss of interstitial infiltration can be expected with solid-tubular carcinoma, it cannot be expected with other histological types, such as invasive lobular carcinoma, adenoid cystic carcinoma, or metaplastic carcinoma (squamous-cell carcinoma and spindle-cell carcinoma).
  • On therapeutic response assessment, the clinical and pathological findings generally match for solid-tubular carcinoma but not for scirrhous carcinoma and invasive lobular carcinoma.
  • With mucinous carcinoma, mucus accumulation can remain, even though most cancer cells disappear; as a result, assessment based on tumor diameter changes is difficult.
  • Histological diagnosis is also significant from the viewpoint of drug sensitivity, and it is important to maintain the accuracy of histological diagnosis.

  • This study evaluated the significance and relationship of COX-2 and c-erbB-2 protein expression in untreated uterine cervical neoplasm.
  • METHODS: This study included 332 patients with uterine cervical neoplasm.
  • COX-2 protein expression was noted in 140 cases of uterine cervical neoplasm (44.0%): In 26.7% of the cervical intraepithelial neoplasia III (16/60 cases), 37.9% of the microinvasive squamous cell carcinoma (39/103 cases), 51.6% of the invasive squamous cell carcinoma (64/124 cases), and 76.2% of the adenocarcinomas (16/21 cases) (P < 0.005).
  • By contrast, except for one case of adenoid cystic carcinoma, none of the uterine cervical neoplasm expressed c-erbB-2 protein.
  • CONCLUSION: COX-2 may participate in the progression of cervical squamous cell lesions, while the contribution of c-erbB-2 to cervical carcinogenesis is probably small.

  • [Title] Lymph node metastases in malignant tumors of the paranasal sinuses: prognostic value and treatment.
  • OBJECTIVE: To assess the frequency of nodal involvement and its prognostic value in malignant tumors of the paranasal sinuses, particularly in maxillary sinus squamous cell carcinoma.
  • PATIENTS: The medical records of 704 consecutive patients surgically treated for malignant tumors of the paranasal sinuses from January 1968 to March 2003 were reviewed.
  • The highest incidence of node metastases was found in maxillary sinus squamous cell carcinoma, particularly in T2 tumors.
  • CONCLUSIONS: Lymph node metastases are a poor prognostic factor for patients with malignant tumors of the paranasal sinuses.
  • A prophylactic treatment of the neck in patients with N0 tumors (surgery or radiotherapy) might be considered in T2 squamous cell carcinoma of the maxillary sinus and in undifferentiated carcinoma of the ethmoid sinus.

  • [Title] Response to oxaliplatin with cetuximab in minor salivary gland adenoid cystic carcinoma.
  • Esophageal localization of adenoid cystic carcinoma of minor salivary glands is rare; it may occur in the early to mid-sixties' age group and is more frequently encountered in men than women.
  • In the majority of cases, it arises from subepithelial glands of the middle to lower third of the esophagus, a similar distribution as squamous cell carcinoma.
  • We report a case of a patient with metastatic primary esophageal adenoid cystic carcinoma progressing on platinum- and irinotecan-based regimens, who achieved an objective response with oxaliplatin-based chemotherapy in combination with cetuximab.

  • The prognostic significance of microvessel density and proliferative activity of the neoplastic cells, evaluated respectively by CD31 and Ki-67 positivity, and immunohistochemical expression of vascular endothelial growth factor (VEGF) was retrospectively investigated in 105 cases of sinonasal carcinoma (80 surgical specimens and 25 biopsies).
  • The most represented histologic types were intestinal-type adenocarcinoma found in 36 patients (34.3%), squamous cell carcinoma (SCC) in 34 (32.4%), mucinous adenocarcinoma (mainly made up of signet-ring cell patterns) in 15 (14.3%), and adenoid cystic carcinoma in 7 (6.7%).
  • Microvessel density values (in vessels per square millimeter), VEGF, and Ki-67 were not dependent on histologic type but were rather correlated to the histologic grading in SCC.
  • Most of the patients (81.5%) were at an advanced stage (T3-T4) at diagnosis.

  • Medical records of 76 patients with intraoral adenoid cystic carcinoma treated with definitive loco-regional therapy at the institute between 1992 and 2004 were retrospectively reviewed and analyzed.Majority (85.5%) of the patients had advanced stage disease.
  • Intraoral adenoid cystic carcinoma is an uncommon disease with an enigmatic clinical course.
  • Larger prospective studies with mature follow-up are needed to define the optimal treatment of intraoral adenoid cystic carcinoma.

  • [Title] Malignant lacrimal sac tumors.
  • Overall, 55% of lacrimal sac tumors are malignant.
  • Mortality rates for malignant tumors depend on tumor stage and type, with a mean rate of 38%.
  • PATIENTS AND METHODS: A 15-year retrospective review of all malignant lacrimal sac tumor patients was completed in our department.
  • Seven patients were found to have malignant sac tumors.
  • The histological types were as follows: squamous cell carcinoma (n=5), one case of adenoid cystic carcinoma, and one case of malignant lymphoma.
  • CONCLUSIONS: Malignant epithelial lacrimal sac tumors are rare cancers with significant recurrence rates.
  • Correct diagnosis and appropriate therapy require a multidisciplinary management approach.
  • Treatment of these malignant epithelial tumors is first and foremost complete surgical removal with wide excision.

  • RESULTS: Eighty-nine cases of the non-squamous cell malignancy were identified.
  • In this study, we found minor salivary gland tumor in 31 patients, sarcoma in 19 patients, spindle cell carcinoma (SpCC) in 19 patients, undifferentiated carcinoma in 9 patients, lymphoma in 6 patients and melanoma in 3 patients.
  • Adenoid cystic carcinoma exhibited the best survival rate (3 years survival rate was 77%), whereas melanoma and undifferentiated carcinoma exhibited poor survivals (1 year survival was 25% and 33%, respectively and 3 years survival rate is 0%).
  • CONCLUSIONS: Adenoid cystic carcinoma is the most common squamous cell carcinoma (SCC) of the sinonasal track.
  • Survival for the patients with undifferentiated carcinoma and melanoma involving the sinonasal track is poor.

  • We describe the case of an unresectable basaloid squamous cell carcinoma of the trachea treated with concurrent chemoradiotherapy up to a dose of 60 Gy in 33 fractions with weekly paclitaxel and carboplatin.
  • The pathological recognition of basaloid squamous cell carcinoma and its distinction from adenoid cystic carcinoma of the trachea is important for its management.
  • Combining systemic chemotherapy with locoregional radiation is a logical approach to treatment, especially for the basaloid squamous cell carcinoma of the trachea, given its tendency to metastasize early after definitive therapy.

  • BACKGROUND: Adenoid cystic carcinoma of the cervix is a rare and aggressive neoplasm for which there are no standardized treatment protocols.
  • CASE: A 38-year-old G3P3 Asian female diagnosed with a stage Ib adenoid cystic carcinoma of the cervix with intermediate risk histologic features received a type III hysterectomy in July 2004.
  • CONCLUSION: Patients with stage Ib adenoid cystic carcinoma of the cervix, especially those with histologic features associated with a higher risk of recurrence, should receive aggressive local therapy, following the guidelines established for similarly staged patients with squamous cell carcinoma of the cervix.

  • [Title] Extramammary Paget's disease mimicking acantholytic squamous cell carcinoma in situ: a case report.
  • BACKGROUND: Extramammary Paget's disease (EMPD) is an uncommon skin neoplasm characterized by Paget's cells with pale-staining cytoplasm in the epidermis, and cases with distinguished acantholysis but lacking characteristic Paget's cells were rarely reported in the literature.
  • METHODS: An 80-year-old male with lesion on scrotum was screened histologically and immunohistologically for the diagnosis of his tumor.
  • RESULTS: Histological examination showed acanthosis with cellular atypia and focal acantholysis, consistent with acantholytic squamous cell carcinoma in situ (SCCIS).
  • CONCLUSIONS: These findings strongly indicated the diagnosis of EMPD mimicking acantholytic SCCIS.

  • [Title] Mast cell reaction in malignant laryngeal neoplasm.
  • Mast cell hyperplasia was found in many malignant tumors, but the significance of this phenomenon is still unknown.
  • In the literature, there are few data about mast cell reaction in malignant laryngeal neoplasm.
  • For histological diagnosis two sections were prepared for Hematoxylin-Eosin staining and Alcian blue-Safranin histochemistry at pH 0.2 for identifying mast cells.
  • RESULTS: Most of the cases were squamous cell carcinoma G1 - 24.4%, G2 - 56.69%, G3 - 18.11%, and 0.78% adenoid cystic carcinoma.
  • Invasive squamous cell carcinoma mast cell microdensity was 2.19 and 4.66 in microinvasive squamous cell carcinoma.
  • Mast cell microdensity in malignant laryngeal papillomatosis was 9.33 and 46.66 in adenoid cystic carcinoma.
  • In carcinoma-associated mast cell hyperplasia, the large majority of mast cells were Alcian blue positive.
  • CONCLUSIONS: In early stages, the mast cells are numerous (microinvasive squamous cell carcinoma mast cell microdensity 4.66) and rare or even absent in late stages (invasive squamous cell carcinoma mast cell microdensity 2.19).
  • Mast cell microdensity in malignant laryngeal papillomatosis was 9.33 and 46.66 in cystic carcinoma.

  • Of the 18 cases considered diagnostic, there were nine (50%) pleomorphic adenomas (PAs); three (17%) squamous-cell carcinomas (SCC); and one each of oncocytoma, adenocarcinoma, not otherwise specified (NOS), adenoid cystic carcinoma, lipoma, neurofibroma, and non-Hodgkin lymphoma, together comprising the remaining 33%.
  • Four of the six cases deemed nondiagnostic (consisting predominantly of blood) on subsequent tissue follow-up revealed paraganglioma (two cases), SCC (one case), and schwannoma (one case).

  • This paper reviews 22 cases of minor salivary gland carcinoma of the oral cavity or oropharynx which were treated at Kurume University Hospital between 1976 and 2005.
  • Minor salivary gland carcinoma was observed in eight of 362 patients with cancer of the oral cavity (2 per cent), and in 14 of 275 patients with cancer of the oropharynx (5 per cent).
  • The five-year and 10-year survival rates of patients with oropharyngeal minor salivary gland carcinoma were 90 per cent.
  • No statistically significant difference was observed between survival rates for oropharyngeal minor salivary gland carcinoma and for oropharyngeal squamous cell carcinoma (p = 0.06).
  • The five- and 10-year survival rates of patients with oral cavity minor salivary gland carcinoma were 75 and 37 per cent, respectively.
  • No statistically significant difference was observed between survival rates for oral cavity minor salivary gland carcinoma and oral cavity squamous cell carcinoma.Patients' survival results correlated well with the clinical stage of their lesions.
  • Adjuvant therapy is recommended for patients with grade III adenoid cystic carcinoma with perineural infiltration or intravascular infiltration.

  • The authors report a case of basaloid carcinoma involving the anus and rectum of a 57-year-old woman.
  • Microscopically, the tumor showed unusual morphologic features strongly resembling a spiradenocylindroma because it consisted, in most parts, of basaloid cell nodules arranged in a jigsaw-puzzle fashion containing or surrounded by eosinophilic basal membrane material; in addition, there were intratumoral lymphocytes.
  • The overlying squamous epithelium manifested dysplastic changes compatible with in situ squamous carcinoma that gradually became invasive and blended with basaloid cell islands; additionally, there were koilocytes in the squamous epithelium.
