  • Intracranial remote metastasis from adenoid cystic carcinoma in salivary gland is quite rare.
  • The authors encountered a case of intrasellar remote metastasis from an adenoid cystic carcinoma of parotid gland origin, presenting with hyponatremia secondary to the syndrome of inappropriate secretion of antidiuretic hormone (SIADH).
  • To our knowledge, this is the first reported case of metastasis from an adenoid cystic carcinoma to intrasellar area.
  • A 78-year-old woman had an adenoid cystic carcinoma in the left parotid gland, which was resected surgically followed by local radiation therapy of 60 Gy.
  • Histopathological examination of the tumor specimens revealed adenoid cystic carcinoma, which had identical histological findings as those of the painful superficial cervical lymph nodes resected in the same operation.
  • Although rare, metastasis from tumors including those of salivary gland origin should be considered in the differential diagnosis of unusual pituitary tumors.

  • BACKGROUND: Adenoid cystic carcinoma comprises approximately 10% of all epithelial salivary neoplasms and most frequently involves the parotid.
  • AIM: The authors report a case of parotid adenoid cystic carcinoma with invasion and spread via the facial nerve with an emphasis on radiologic features.
  • The magnetic resonance imaging (MRI) examination showed a right parotid gland solid mass located in the deep lobe of the parotid gland with mastoid bony involvement.The mass was heterogenously enhanced.
  • Pathologic examination of the resected specimen revealed histological features of adenoid cystic carcinoma with intra neural invasion of the mastoid segment of the facial nerve.
  • CONCLUSION: This study confirms that multiplanar approach of MRI allows rapid detection of adenoid cystic carcinoma.

  • A 14 year-old female presented with complaints of painless enlargement of the right parotid gland and facial asymmetry.
  • Physical examination revealed a firm mass in the region of the parotid gland as well as right facial paralysis.
  • Biopsy obtained from the mass showed an adenoid cystic carcinoma of the parotid gland.

  • Adenoid cystic carcinoma (ACC) constitutes about 4% of salivary epithelial tumors and is the second common malignant epithelial salivary gland tumor involving both the major and minor salivary glands.
  • We report the first case of ACC with high grade transformation involving the parotid.
  • A 54-year-old man with a history of right parotid painful swelling from 1.5 years presented with recent increase in size of the swelling and facial paresis of 2 months duration.
  • The locally invasive salivary neoplasm was composed predominantly of an undifferentiated carcinoma with foci of conventional ACC occupying less than 10% of tumor area.
  • Recognition of occasional clusters of basaloid cells and hyaline globules in association with larger poorly differentiated malignant cell population in aspiration smears can help in cytological diagnosis.

  • We report results of a retrospective study of 12 cases of adenoid cystic carcinoma (ACC) in the parotid gland.
  • Local pain was often observed in ACC among other malignant parotid tumors.
  • Although fine-needle aspiration cytology (FNA) was not effective for preoperative diagnosis, frozen section diagnosis (FS) during surgery showed excellent results.

  • We present the case of a 21-year-old woman with a left renal metastasis from an adenoid cystic carcinoma of the parotid gland that had been surgically treated 7 years previously.
  • During follow-up, the patient developed pulmonary nodules that were metastatic adenoid cystic carcinoma and were surgically excised.

  • PROBLEM: Temporal bone tumours are rare head and neck cancers that may involve both the parotid and the neck.
  • In cases with concomitant temporal bone and parotid tissue involvement, tumour origin determination can be challenging.
  • A tumour with parotid and temporal bone involvement is usually of temporal origin.
  • The tumour may also be a rare parotid neoplasm with intratemporal invasion through the facial nerve, such as adenoid cystic carcinoma.
  • CASE REPORT: We describe a case of parotid adenoid cystic carcinoma invading the temporal bone via the facial nerve, presenting as primary temporal bone tumour with parotid tissue involvement.
  • CONCLUSION: Tumours mimicking temporal bone neoplasms can originate from the parotid tissue and invade through the facial nerve.

  • Adenoid cystic carcinoma of the parotid gland is relatively rare.
  • According to a retrospective study of the medical records and histopathology files of 514 cases of parotid tumors operated at our hospital over a period of 18 years, adenoid cystic carcinoma represented only 2.3% of all parotid gland neoplasms, a total of 12 cases.
  • In our records we retrieved only one documented case of adenoid cystic carcinoma that originated in the parotid gland subsequent to superficial parotidectomy for a benign lesion (pleomorphic adenoma).
  • An even more exceptional presentation of adenoid cystic carcinoma is as a bilobed tumor extending from the post auricular to the temporal and zygomatic region.

  • Examination by fine needle aspiration cytology raised suspicion of mucoepidermoid or adenoid cystic carcinoma of the parotid gland and an excision was recommended.
  • The lateral parotidectomy specimen showed a poorly circumscribed gelatinous tumor measuring 15 mm in diameter within the parotid gland tissue.
  • Retrospective analysis of a skin biopsy from the same anatomic area performed 8 years prior to parotid neoplasm displayed a tumor with identical microscopic appearance and immunohistochemical profile (additionally performed) which was, however, misdiagnosed as a benign lesion.
  • The diagnosis of recurrent primary mucinous carcinoma of the skin infiltrating the parotid gland was established.

  • The expression of neural cell adhesion molecule (NCAM) was analyzed in immunohistochemical preparations from adenoid cystic carcinoma.
  • The goal was to evaluate whether NCAM expression could be used as a biological marker for the perineural invasion of adenoid cystic carcinoma in the head and neck.
  • A high incidence of perineural invasion was found in adenoid cystic carcinoma in the parotid, hard palate, maxillary sinus and oral cavity.
  • All 6 cases displayed positive NCAM staining, and 5 displayed perineural invasion in the primary adenoid cystic carcinoma.
  • In conclusion, NCAM expression can, to a certain extent, be used as a predictor of perineural invasion in adenoid cystic carcinoma.

  • No adult cases have been reported with a specific diagnosis of sialoblastoma.
  • If even focal cribriforming were present, such cases have undoubtedly been diagnosed as adenoid cystic carcinoma.
  • Each case, however, has the primitive histopathology with discrete nests of basaloid tumor cells, associated bilayered ductal structures and the fibromyxoid stroma characteristic for sialoblastoma with its resemblance to fetal salivary gland or salivary gland with arrested development.
  • Sialoblastoma, whether in a child or adult with or without a cribriform growth pattern, appears to have a more favorable prognosis than adenoid cystic carcinoma.
  • Aspects of the histological differential diagnosis of these 2 tumors are discussed.

  • METHODS: Clinical records of 64 patients with adenoid cystic carcinoma of major salivary glands followed up for more than 7 years were reviewed.
  • CONCLUSION: Presence or absence of nerve invasion, histological subtype, clinical stage, and positive or negative surgical margin are the independent factors affecting the prognosis of patients with adenoid cystic carcinoma of salivary glands.

  • Adenoid cystic carcinoma (ACC) accounts for 10-15% of such tumors.
  • DESIGN AND SETTING: Retrospective study at Head and Neck Service, Universidade Estadual de Campinas.
  • Major salivary gland tumor extent was routinely assessed by preoperative ultrasonography.
  • Diagnosis, surgery type, margin type (negative/positive), postoperative radiotherapy and recurrence (presence/absence) were evaluated.
  • RESULTS: There were eleven major salivary gland tumors (52.3%), seven submandibular and four parotid.
  • Ten patients (47.7%) had minor salivary gland ACC (all in palate), while the submandibular was the most frequently affected major one.
  • CONCLUSION: Adenoid cystic carcinoma has locally aggressive behavior.

  • OBJECTIVE: To examine patient characteristics, pathologic features, and treatment outcomes of adenoid cystic carcinoma of the major salivary glands.
  • Of the 29 patients fully staged at the time of diagnosis, 7 (24%) presented at American Joint Committee on Cancer stage I, 9 (31%) at stage II, 4 (14%) at stage III, and 9 (31%) at stage IV.
  • The majority originated in the parotid gland (21, 64%), with the remaining originating in either the submandibular gland (10, 30%) or the sublingual gland (2, 6%).

  • The behavior of adenoid cystic carcinoma (ACC) of the salivary glands has been shown to be unpredictable in terms of local and distant spread and mortality.
  • We retrospectively studied 35 operations in 34 patients who had had a pathologic diagnosis of ACC of the salivary glands and who had been treated over a 20-year period and followed for a minimum of 10 years.
  • The site of origin appeared to be an important factor in survival rates; survival among patients with tumors that had originated in the parotid gland was fairly good, while survival among those with tumors that originated in the minor salivary glands was significantly worse.
  • We recommend radical surgery with complete resection for all patients with ACC of the salivary glands and a careful assessment of the neck in patients with minor salivary gland tumors.

  • Salivary gland tumors usually occur as single lesions.
  • We report a case of an adult male who presented with a mucoepidermoid carcinoma involving the minor salivary glands of the palate at age 57 years, followed by an adenoid cystic carcinoma of the floor of mouth at age 63 years.
  • There are 31 acceptable cases of multiple malignant salivary gland neoplasms reported in the world literature.
  • Multiple malignant tumors of the same histologic type are more common than those of different histologic type.
  • Bilateral acinic cell adenocarcinoma was the most frequent combination of multiple salivary gland malignancy, accounting for 14 cases (10 synchronous and four metachronous).
  • All involved the parotid glands bilaterally with the exception of one case that involved parotid and submandibular gland.
  • Polymorphous low-grade adenocarcinoma accounted for three of the four cases of multiple malignant tumors involving minor salivary glands.
  • Individuals with a history of malignancy are at risk for the development of additional malignant tumors and should receive appropriate clinical follow-up.

  • PURPOSE: To analyze the recent experience of patients with adenoid cystic carcinoma treated with radiation therapy at Memorial Sloan-Kettering Cancer Center.
  • METHODS AND MATERIALS: From 1990 to 2004, a total of 59 patients with a diagnosis of primary adenoid cystic carcinoma of the head and neck received radiation therapy at our institution.
  • The subsite distribution was oral cavity, 28% (n = 17); paranasal sinuses, 22% (n = 13); parotid, 14% (n = 8); submandibular, 14% (n = 8); oropharynx, 10% (n = 6); sublingual, 3% (n = 2); nasopharynx, 3% (n = 2); and other, 5% (n = 3).

  • [Title] Distant metastasis of parotid gland tumors.
  • The incidence of distant metastasis in head and neck cancer and especially in salivary gland cancer is relatively low in comparison to other malignancies.
  • Patients with malignant salivary gland tumors should have an X-ray or CT scan of the chest at their initial assessment to exclude the possibility of distant metastasis.
  • The likelihood of developing distant metastasis is associated with high-grade tumors, such as adenoid cystic carcinoma, salivary duct carcinoma, high-grade mucoepidermoid carcinoma and tumors located in the submandibular gland, posterior tongue and pharyngeal tumors.
  • A lower risk of developing distant metastasis is known for all other histological entities of salivary gland tumors.
  • Nevertheless all patients who have a histologically confirmed malignant salivary gland tumor should have lifelong follow-up.
  • On the basis of a clinical case regarding a patient with metastatic parotid gland cancer we present a review of the literature.

  • [Title] Morphological pattern of parotid gland tumours.
  • OBJECTIVE: To determine the morphological pattern of parotid tumours.
  • PATIENTS AND METHODS: During this study, 204 patients with parotid tumours were registered.
  • RESULTS: Out of 204 cases, 152 (74.5%) were benign and 52 (25.5%) were malignant.
  • Benign tumours were more common in females whereas malignant tumours were common in males.
  • The mean age of patients was 34 years and 42 years for benign and malignant tumours respectively.
  • The most common malignant tumour was mucoepidermoid carcinoma (60%), followed by adenoid cystic carcinoma.
  • Superficial lobe of parotid gland was the commonest site, 120 benign and all 52 malignant tumours arising from it while 32 benign tumours originated from deep lobe.
  • Parotid swelling for years was main feature of benign tumours, whereas malignant tumours presented with pain, fixation to skin or underlying structure, cervical lymphadenopathy and facial palsy.
  • CONCLUSION: Pleomorphic adenoma was the most common benign tumour and mucoepidermoid carcinoma was most common malignant tumour.

  • BACKGROUND: Parotid gland carcinoma is an infrequent tumor, and series that report on these neoplasms are relatively scarce in the literature.
  • The objective of the current study was to identify prognostic factors in patients with parotid gland carcinoma and to develop a method for defining the probability of recurrence.
  • METHODS: Patients with parotid gland carcinoma who were treated at the authors' institution from January 1981 through December 2004 and who completed treatment constituted the study group.
  • Mucoepidermoid carcinoma was diagnosed in 34.6% of patients, adenoid cystic was diagnosed in 15.7% of patients, adenocarcinoma was diagnosed in 14.3% of patients, and acinic cell carcinoma was diagnosed in 9.4% of patients.

  • CONTEXT: Adenoid cystic carcinoma (ACC) is a rare neoplasm that usually arises from the salivary, lacrimal, or other exocrine glands.
  • AIM: To analyze the presentation and natural history of cases of adenoid cystic tumors of salivary glands in our institution; and to compare with the existing literature.
  • RESULTS: There were 8 cases of major salivary gland tumors (47%), of which 2 were in the submandibular and 6 were involving the parotid.

  • OBJECTIVES: Our aim is to investigate the expression of kit protein (KIT) and epidermal growth factor receptor (EGFR) in parotid carcinomas in order to correlate the expression to histology and prognosis.
  • Further we want to perform mutation analysis of KIT-positive adenoid cystic carcinomas.
  • PATIENTS AND METHODS: Formalin-fixed paraffin-embedded sections from 73 patients with parotid gland carcinomas were used for the study.
  • Twelve KIT-positive adenoid cystic carcinomas were examined for c-kit mutation in codon 816.
  • Ninety-two percentage of the adenoid cystic carcinomas were KIT-positive.
  • None of the adenoid cystic carcinomas had mutations in codon 816 of the c-kit gene.
  • Adenoid cystic carcinomas express KIT, but no mutations in codon 816 of the c-kit gene were identified.

  • PURPOSE: To evaluate the impact of postoperative radiation therapy on the clinical course of patients with carcinoma ex pleomorphic adenoma of the parotid gland.
  • METHODS AND MATERIALS: Between 1960 and 2004, 63 patients were treated with definitive surgery for carcinoma ex pleomorphic adenoma of the parotid gland.
  • Adenocarcinoma (29 patients), salivary duct carcinoma (16 patients), and adenoid cystic carcinoma (9 patients) were the most common malignant subtypes.
  • CONCLUSION: Surgery followed by postoperative radiation should be considered the standard of care for patients with carcinoma ex pleomorphic adenoma.

  • OBJECTIVE: To report our experience of varied presentations and diverse histopathological spectrum of parotid gland malignancies.
  • METHODS: This retrospective analysis incorporated patients with histological evidence of malignant parotid tumors at King Khalid University Hospital, Riyadh, Saudi Arabia over a 20-year period from 1984 through 2004.
  • Nine (28.1%) patients had mucoepidermoid carcinoma, 8 (25%) adenoid cystic carcinoma, 6 (18.7%) adenocarcinoma, not otherwise specified, and 2 (6.2%) were reported to have carcinoma in pleomorphic adenoma.
  • CONCLUSION: Malignant parotid tumors are exceedingly rare, occurring at a relatively earlier age group with male preponderance, and invariably declare at a late clinical stage in our community.

  • We present a case of papillary cystic acinic cell carcinoma with many psammoma bodies and discuss the diagnostic pitfalls with other salivary gland tumors.
  • Physical examination demonstrated a 2 x 2-cm, nontender, rubbery mass inferior to the auricle in the left parotid area.
  • Sonography showed a 2-cm, cystic mass in the left parotid.
  • A preliminary diagnosis of papillary cystic salivary gland neoplasm was made and supeficial parotidectomy performed.
  • A diagnosis of papillary cystic acinic cell carcinoma with many psammoma bodies was made.
  • Aspiration cytology of papillary cystic acinic cell carcinoma with many psammoma bodies can be confused with more common tumors, such as cystic mixed tumor and adenoid cystic carcinoma with cannonballs, low grade mucoepidermoid carcinoma or cystic papillary carcinoma of the thyroid.

  • We present a case of a woman with scalp lesions and a parotid mass.
  • METHODS: Biopsy of a skin nodule demonstrated cylindroma, and fine-needle aspiration of the parotid mass suggested membranous basal cell adenoma, which was confirmed following superficial parotidectomy.
  • RESULTS: Eighteen cases of cylindromatosis with coexistent salivary gland membranous basal cell adenoma were reported.
  • Seventeen cases involved the parotid gland; there are 2 reports of malignant transformation.
  • CONCLUSION: FADC should be considered in anyone with coexistent dermal and salivary gland neoplasms.
  • Membranous basal cell adenoma may be mistaken for adenoid cystic carcinoma on cytologic or histologic examination.
  • Total parotidectomy with facial nerve preservation or superficial parotidectomy with close follow-up is recommended, as multifocality and malignant transformation exist.

  • [Title] [Magnetic resonance imaging for diagnosis of parotid malignant tumors and the pathological basis].
  • OBJECTIVE: To investigate magnetic resonance imaging (MRI) features of parotid malignant tumors and study their pathological basis.
  • METHODS: Forty-seven patients with parotid malignant tumors confirmed by surgery (41 patients) or biopsy (6 patients) were enrolled in this study.
  • Each of the MRI features was analyzed retrospectively and the typical MRI findings of common parotid malignant tumors were summarized.
  • RESULTS: MRI allowed accurate diagnosis of parotid malignant tumors.
  • Four patients with low-grade mucoepidermoid carcinoma showed well-defined tumor margin and were difficult to distinguish from benign tumors.
  • Six patients with high-grade mucoepidermoid carcinoma had obscure margin of the tumor which easily underwent necrosis with liable lymph node involvement.
  • The 8 cases of adenoid cystic carcinoma was characterized by extensive invasion surrounding the parotid gland.
  • Most of 8 cases of malignant pleomorphic adenoma still showed high and heterogeneous signal on T2WI, with irregular shape and poorly defined margin.
  • The 4 cases of acinic cell carcinoma showed either regular or irregular tumor morphology, presenting with high signal intensity on T1WI and T(2)WI.
  • CONCLUSION: MRI is an important modality for the diagnosis of parotid malignant tumors.
  • Most of the common parotid malignant tumors have characteristic MRI and pathological features, which make possible their differential diagnosis.

  • PURPOSE: To determine survival and factors influencing survival for parotid gland cancer.
  • METHODS: Cases of parotid gland malignancy were extracted from the SEER database for 1988 to 1998.
  • Subset analysis was conducted for mucoepidermoid carcinoma according to grade.
  • Mean tumor size at diagnosis was 2.7 cm; 38.0% of patients had extraglandular extension of the tumor, 26.8% of patients had positive nodal disease, and 59.4% of patients received radiation therapy.
  • Tumor histology did predict survival, with squamous cell carcinoma and acinar cell carcinoma exhibiting the poorest and best survivals, respectively.
  • CONCLUSIONS: Survival in parotid gland malignancy is influenced by multiple factors.

  • We set out to explore the immunohistochemical expression of Numb in normal and neoplastic (28 adenoid cystic and 34 mucoepidermoid carcinomas) salivary glands, and correlated the results with the clinico-pathologic features of the neoplasms.
  • In salivary carcinomas, we detected diffuse and intense Numb immunostaining in 5 adenoid cystic and 8 mucoepidermoid carcinomas.
  • By contrast, the majority of adenoid cystic and mucoepidermoid cancers showed only moderate (14 and 5 cases) or focal staining (9 and 21 cases), respectively.
  • The corresponding expression of Numb mRNA was documented in normal parotid gland and adenoid cystic carcinoma.
  • Numb immunoreactivity was inversely correlated with the histological grade and Ki-67 immunoreactivity of both adenoid cystic and mucoepidermoid carcinomas.
  • These data suggest that loss of Numb is implicated in aberrant differentiation programs of salivary gland carcinomas and may serve as a prognostic indicator in patients treated for these neoplasms.

  • BACKGROUND: To describe the cytologic, histologic and immunohistochemical findings of a case of epithelial myoepithelial carcinoma (EMC) arising from a pleomorphic adenoma (PA) of the parotid with both malignant epithelial and myoepithelial components.
  • CASE: A 29-year-old female presented with a 1.5 x 1.5-cm, palpable mass of the left parotid of 7-8 months' duration with recent enlargement and pain.
  • The cytology was reported as salivary gland neoplasm, "suggestive of adenoid cystic carcinoma, less likely pleomorphic adenoma."
  • The final diagnosis was EMC ex PA.
  • CONCLUSION: Although previous reports mention the difficulties in diagnosing EMC and differentiation from the more common salivary gland neoplasms such as PA, we like to emphasize the cytologic confusion that results when the tumors coexist.

  • OBJECTIVE: To evaluate qualitative ultrasound elastography for focal salivary gland masses identified during routine clinical practice.
  • METHODS: Sixty-five parotid or submandibular masses in 61 patients underwent real-time qualitative ultrasound elastography and were scored on colour-scaled elastograms in terms of their stiffness relative to adjacent normal salivary parenchyma from ES 1 (soft) to ES 4 (stiff).
  • This was correlated with diagnosis from aspiration cytology or histology.
  • RESULTS: There were 29 Warthin's tumours (WTs), 23 pleomorphic adenomas (PAs), 2 adenoid cystic carcinomas, 1 adenosquamous carcinoma, 1 nodal metastasis from nasopharyngeal carcinoma, 1 lymphoma (2 deposits), 3 Kuttner tumours and 4 cases of Kimura's disease.
  • Nine, 19, 14 and 17 of the benign masses and 0, 1, 2 and 3 of the malignant masses were ES 1, 2, 3 and 4 respectively.
  • CONCLUSION: These preliminary findings suggest that qualitative real-time ultrasound elastography, although an ancillary technique to conventional ultrasound in the salivary glands, is likely to have a poor ability to discriminate benign lesions (particularly PAs) from malignant disease.

  • The clinical features of 74 patients (39 men, 35 women; mean age, 62 years) with malignant parotid tumors were retrospectively investigated.
  • Tumors located in both lobes of the parotid gland were the most frequent type of tumor (49%).
  • Histopathologically, sixteen tumor types were observed; mucoepidermoid carcinoma was the most common.
  • As for the major histopathologic types, the five-year survival rates were 69% for mucoepidermoid carcinoma, 48% for adenocarcinoma, 71% for adenoid cystic carcinoma, and 100% for acinic cell carcinoma and malignant mixed tumor.
  • A total parotidectomy with total removal of the facial nerve seemed necessary for T3 and T4 cases, especially those with adenocarcinoma or mucoepidermoid carcinoma.
  • Modified neck dissection may be necessary for N0 cases, especially those with adenocarcinoma, adenoid cystic carcinoma or undifferentiated carcinoma.

  • A limited surgical resection combined with (125)I seed implant brachytherapy is therefore considered to be an alternative treatment for local control of malignant parotid gland tumors with preservation of the facial nerve.

  • BACKGROUND: The parotid node is an uncommon site of metastasis in head and neck cancer.
  • This study was intended to clarify the incidence and indicators of oral cancer metastases to the parotid node.
  • The histologic diagnoses of the primary tumors were squamous cell carcinoma in 239 patients, mucoepidermoid carcinoma in 5, adenoid cystic carcinoma in 4, and miscellaneous others in 5.
  • In all neck dissections, the tail of the parotid gland below the marginal mandibular branch of the facial nerve was resected.
  • The cervical and parotid lymph nodes were isolated from the surgical specimens.
  • RESULTS: From 183 of the 289 neck specimens, we collected 539 parotid nodes: 222 extraglandular and 317 intraglandular.
  • Of the 253 patients, 10 (4.0%) had 19 parotid node metastases, of which 4 were extraglandular and 15 intraglandular.
  • Parotid node involvement occurred in 2.5% of oral squamous cell carcinoma cases.
  • In terms of indicators, the likelihood of metastasis to the parotid nodes increased with the number of cancer-positive cervical nodes.
  • CONCLUSIONS: Metastasis to the parotid nodes should be considered in patients with oral cancer.
  • Resection of the tail of the parotid gland is warranted during the neck dissection.

  • INTRODUCTION: Salivary gland neoplasms represent the diverse group of tumours encountered by the head and neck surgeons.
  • MATERIAL AND METHODS: The study is analysis of 531 patients with parotid gland tumours in patients treated in ENT Department during 1986-2006.
  • The results of histological diagnosis were presented.
  • RESULTS: The frequency of incidence of benign neoplasm was 80%.
  • Malignant tumours constituted 20% of all gland tumours.

  • BACKGROUND: The aim of the study was to investigate the results of treatment of malignant parotid gland tumours at a single centre during a 56 year period, focusing on tumour control and survival.
  • PATIENTS AND METHODS: At Uppsala University Hospital, Sweden, 144 patients (73 male and 71 female) with parotid cancer were treated between 1948 and 2004.
  • The majority of tumour-related deaths appeared in the first 3-5 years after diagnosis.
  • Age, co-morbidity, the presence of lymph node metastases, adenoid cystic carcinoma and extent of disease were important for outcome; gender, however, was not.
  • CONCLUSIONS: Age, nodal engagement, a higher T-stage, adenoid cystic carcinoma histopathology, facial palsy and intercurrent disease worsen the outcome of patients, whereas gender does not.

  • PURPOSE: Many articles have discussed the clinical features of previously untreated parotid cancer, but the clinical characteristics and treatment of recurrent parotid cancer have not yet been fully described.
  • MATERIALS: We retrospectively reviewed 20 patients with recurrent parotid cancer and analyzed the therapeutic strategies and the prognostic factors.
  • In the definitive surgery group, the presence of lymph node metastasis and high-grade malignant histopathology were associated with a poor prognosis (p < 0.01).
  • CONCLUSIONS: The results suggest that aggressive definitive surgery may be justified for the treatment of recurrent parotid cancer.

  • PURPOSE: To analyze changes in parotid gland dose resulting from anatomic changes throughout a course of radiotherapy in a cohort of head-and-neck cancer patients.
  • The process resulted in deformed parotid contours and voxel mappings for both daily and accumulated dose-volume histogram calculations.
  • RESULTS: The daily parotid mean dose of the 10 patients differed from the plan dose by an average of 15%.
  • At the end of the treatment, 3 of the 10 patients were estimated to have received a greater than 10% higher mean parotid dose than in the original plan (range, 13-42%), whereas the remaining 7 patients received doses that differed by less than 10% (range, -6-8%).

  • AIM: We evaluated the feasibility of perfusion CT (CTP) of the parotid gland and attempted to differentiate benign from malignant tumors.
  • MATERIALS AND METHODS: CTP was performed in 17 patients with benign tumors and 10 patients with malignant parotid tumors.
  • Regions of interest were placed through the tumor site and the contralateral healthy parotid tissue.
  • High Pearson correlation coefficients comparing the two readers' visually measured abnormalities were observed (r=0.79-0.86, P=0.001) for all perfusion maps, The MTT and PS values between malignant and benign tumors were not significantly different.
  • The BF and BV values were statistically significant different between the benign and malignant tumors (0.00<P<0.02).
  • Only the BV ratio criterion between malignant and benign neoplasms was statistically significant (P<0.004).
  • CONCLUSIONS: CTP of the parotid gland is feasible and may differentiate malignant from non-malignant lesions by means of absolute BF, BV and BV ratio values.

  • BACKGROUND: magnetic resonance (MR) imaging of parotid gland tumors has been widely reported, although few reports have evaluated the capsule of parotid gland tumors in detail.
  • PURPOSE: to evaluate the diagnostic usefulness of 3.0 T MR imaging with surface coils for detection of the parotid gland tumor capsule, and to clarify the characteristics of the capsules.
  • MATERIAL AND METHODS: seventy-eight patients with parotid gland tumors (63 benign and 15 malignant) were evaluated.
  • It was retrospectively assessed whether each parotid gland tumor was completely surrounded by a capsule.
  • Visual interpretations were compared with histopathological findings to evaluate the diagnostic ability of MR imaging to detect parotid gland tumor capsules.
  • Statistical evaluation was conducted concerning the presence of capsules, capsular irregularity, and the difference in contrast enhancement between benign and malignant tumors, and that between pleomorphic adenomas and Warthin's tumors.
  • Benign tumors had a capsule completely surrounding the tumor significantly more often than malignant tumors (P = 0.009).
  • Concerning capsular irregularity, malignant tumors tended to have more irregular capsules than benign tumors, although there were no significant differences.
  • The capsules of malignant tumors enhanced significantly more strongly than those of benign tumors (P = 0.018).
  • CONCLUSION: 3.0 T MR imaging using surface coils could correctly depict parotid gland tumor capsules in most cases.

  • METHODS AND STUDY DESIGN: The records of 110 patients with primary parotid neoplastic lesions treated at three Italian institutions from 1993 to 2004 were retrospectively reviewed.

  • Malignant neoplasms of the salivary gland are uncommon entities in which surgical resection of the primary lesion has been accepted as a standard therapeutic option.
  • We treated one patient with recurrent adenoid cystic carcinoma arising from the sublingual gland and one patient with primary adenocarcinoma arising from the parotid gland with transfemoral intraarterial chemotherapy, based on full-dose cisplatin and docetaxel and concurrent external-beam radiotherapy.
  • The concomitant chemoradiotherapy of cisplatin and docetaxel seemed to be a practicable option for patients with recurrent and unresectable salivary gland carcinomas.

  • OBJECTIVE: A retrospective analysis of management and survival of patients treated for temporal bone carcinoma.
  • PATIENTS AND METHODS: Thirty patients underwent treatment for carcinoma of the temporal bone.
  • Twenty-five squamous cell carcinomas, 1 melanoma, 2 basocellular carcinomas and 2 adenoid cystic carcinomas were treated.
  • CONCLUSION: Long-term prognosis of the carcinoma of the external auditory canal mainly depends on the stage and primary treatment.
  • The improved survival (65%) of patients treated de novo compared with those treated with salvage surgery (23%) suggests that early referral and aggressive primary surgical treatment with postoperative radiotherapy offer the greatest chance of cure.

  • [Title] Clinical analysis of salivary gland tumor cases in West China in past 50 years.
  • In our study, 3461 cases of salivary gland tumor treated between 1955 and 2002 at West China Stomatology Hospital of Sichuan University were retrospectively analyzed, and compared with the previous reports.
  • Measures such as age, tumor location, tumor histological type, and the nature of the growth (benign or malignant) were recorded at the same time.
  • The findings are as follows: the average ages of salivary gland tumor patients were 41.38 years for the benign cases and 45.20 for the malignant ones; the male:female ratio was 0.
  • 99:1 in the benign cases and 1.34:1 in the malignant ones; primary tumors were mostly in the parotid gland, palate and submandibular gland in sequence.
  • Pleomorphic adenoma was the most frequent benign tumor followed by Warthin's tumor and basal cell adenoma, whereas mucoepidermoid carcinoma, adenoid cystic carcinoma and adenocarcinoma not otherwise specified were the most frequent malignant tumors.
  • The incidence of salivary gland tumors increased with age.
  • The male:female ratio of malignant tumors was higher than that of benign ones.
  • The parotid gland and palate were the most common locations of salivary gland tumors.

  • [Title] Salivary gland tumors: a single institution experience in India.
  • Between 1991 and 2006, 684 cases of salivary gland tumours were analysed retrospectively, of which 422 (62%) were benign and 262 (38%) malignant.
  • Sixty-one percent of tumours were in the parotid gland, 22% in the minor salivary glands, and 17% in the submandibular glands.
  • The most common benign tumour was pleomorphic adenoma (86%), and the most common malignant tumours were adenoid cystic carcinoma (25%) and mucoepidermoid carcinoma (18%).
  • Among the minor salivary gland tumours, most were seen in the palate (68%).
  • We analyse the incidence and distribution of all types of salivary gland tumours in an Indian series, and provide data for comparison with other epidemiological studies from different geographical sites and races.

  • STUDY DESIGN: The records of the Department of Pathology at Jordan University of Science and Technology were reviewed for patients who were treated for salivary gland tumors from 1991 to 2002.
  • RESULTS: One hundred two true neoplasms (70% benign and 30% malignant) were found.
  • The most frequent benign and malignant neoplasms found were pleomorphic adenoma (54%) and adenoid cystic carcinoma (13%), respectively.
  • The most common major and minor salivary gland sites were the parotid (51%) and palatal glands (20%), respectively.
  • Although most of major gland tumors were adenomas, carcinomas of the minor glands were only slightly less frequent than adenomas.
  • The most frequent malignant parotid tumors were adenoid cystic carcinoma and mucoepidermoid carcinoma.
  • The most frequent minor salivary gland malignant tumors were palatal adenoid cystic carcinoma.

  • OBJECTIVES: The study was performed to evaluate the prognostic relevance of cell proliferation associated with Ki-67/ Mib-1 immunostaining in malignant tumors of the major salivary glands.
  • METHODS: Cell proliferation was evaluated by Mib-1 antibody against Ki-67 antigen in 41 patients with cancer of the parotid or submandibular glands, including 14 acinic cell carcinomas, 12 ductal carcinomas, 7 mucoepidermoid carcinomas, 5 carcinomas ex pleomorphic adenoma, 1 adenoid cystic carcinoma, 1 undifferentiated carcinoma, and 1 polymorphous low-grade adenocarcinoma.
  • CONCLUSIONS: To our knowledge, this is the first study to stratify different risk classes in early T1-T2 or N0 malignant tumors of the major salivary glands that identified aggressive lesions with elevated Ki-67 expression at an initial stage.

  • RESULTS: Out of the 713 epithelial salivary tumors, 356 (49.9%) were classified as benign and 357 (50.1%) as malignant, indicating a benign-to-malignant ratio of 1:1.
  • The majority of both benign (29%) and malignant (39%) tumors occurred in the minor salivary glands.
  • Malignant tumors were slightly more common in minor salivary glands, indicating a benign-to-malignant ratio of 1:0.75.
  • Pleomorphic adenoma was the most frequent tumor, representing 38% of all tumors (274 out of 713), followed by mucoepidermoid carcinoma 21.6% (154 out of 713) and adenoid cystic carcinoma 13.5% (96 out of 713).
  • The commonest benign and malignant tumors were pleomorphic adenoma and mucoepidermoid carcinoma in both major and minor salivary glands.
  • CONCLUSION: According to the findings of this study, equal frequencies of benign and malignant tumors indicate higher occurrences of malignancies, especially in the minor salivary glands, in the Sri Lankan population.

  • OBJECTIVE: The aim of this report is to describe 3 cases of salivary gland tumors with intracranial extension associated to an extracerebral mass lesion, and to discuss the frequence, pathology and treatment of these very rare localizations.
  • The primary tumors were an adenocarcinoma and a malignant oncocytoma of the parotid gland and an adenoid cystic carcinoma of the submandibular gland.
  • CONCLUSIONS: The intracranial extension of salivary gland tumors is a very rare event.

  • [Title] Detection of C-KIT (CD117) molecule in benign and malignant salivary gland tumours.
  • In the present study we analysed the expression of this molecule in salivary gland tumours.
  • Archival formalin-fixed, paraffin-embedded sections of 40 benign and 57 malignant salivary gland tumours were retrieved and retrospectively studied immunohistochemically using a polyclonal C-KIT antibody in an Envision/HRP technique.
  • In addition five samples of chronic submandibular sialadenitis, five normal minor salivary glands and parotid or submandibular gland tissue adjacent to benign tumour were also studied.
  • C-KIT expression was observed in cases of adenoid cystic, acinic cell polymorphous low grade, epithelial-myoepithelial, carcinosarcoma and basal cell adenocarcinomas, as in luminal cells of pleomorphic adenomas, in serous acinar and only in intercalated and a small number of striated ductal cells of inflammatory salivary gland tissue, whereas normal salivary lobules were generally negative except a weak positivity of intercalated cells.
  • Contrary to other reports, this study suggests that, C-KIT protein does not appear to be an exclusively specific marker for benign or malignant salivary gland neoplasms, but may be useful in differential diagnosis of adenoid cystic carcinoma from polymorphous low grade adenocarcinoma.
  • Furthermore its expression in serous acinar cells in sialadenitis and intercalated ductal cells in normal and inflammatory lesions may indicate a possible participation in pathogenesis of both neoplastic and non-neoplastic salivary gland diseases.

  • OBJECTIVE: To study the clinical characteristics, diagnosis and treatment strategy of oral and maxillofacial malignancies in multiple primary malignant neoplasms (MPMNs).
  • RESULTS: There were 44 malignant cases in the 21 MPMNs patients.
  • Among the 44 cases, there were 24 cases in alimentary and respiratory tract such as oral, pharynx, esophagus, stomach and lung, and 10 cases in salivary gland, breast and female reproductive system.
  • There were 25 cases malignant neoplasms in oral and maxillofacial region where squamous cell carcinoma was the most common pathologic type, secondly adenoid cystic carcinoma.
  • In oral and maxillofacial region, MPMNs were often found in tongue, parotid and submandibular gland, buccal mucosa and gingival.
  • CONCLUSION: Tongue and salivary gland were the common locations with MPMNs, and they were closely associated with alimentary and respiratory tract.
  • Regular follow-up, early detection, early diagnosis, active and effective treatment can help to improve the survival quality of MPMNs patients.

  • OBJECTIVE: To describe the fine needle aspiration cytology (FNAC) features of various salivary gland lesions and to analyze the respective diagnostic value and pitfalls.
  • METHODS: 113 FNAC specimens of salivary gland lesions were reviewed and correlated with clinical and histopathologic findings.
  • RESULTS: The FNAC diagnostic failure (2); non-neoplastic lesions (12); benign neoplasm (82) and malignant neoplasm (17).
  • Cytologically, the distinction between cellular pleomorphic adenoma, adenoid cystic carcinoma and basal cell adenoma could be difficult due to their overlapping morphologic features.
  • The cytologic patterns of primary lymphoepithelial carcinoma of the parotid were indistinguishable from those of metastatic nasopharyngeal undifferentiated carcinoma.
  • The three inaccurately diagnosed cases of FNAC are, as follows: reactive lymphoid hyperplasia of lymph node mistaken as non-Hodgkin lymphoma, mucoepidermoid carcinoma diagnosed as "scanty atypical cells present" and primary lymphoepithelial carcinoma mistaken as benign lymphoepithelial lesion.
  • On the basis of FNAC, 97.4% (110 /113) were correctly depicted as benign (95/96; 99.0%) or malignant (15/17; 88.2%).
  • Furthermore, 90.3% (102 /113) (specificity = 91.9%; 102/111) were accurately diagnosed, including 91.7% (88/96) benign lesions (specificity = 92.6% ; 88/95) and 82.4% (14/17) malignant tumors (specificity = 87.5%; 14/16).
  • CONCLUSIONS: FNAC is reliable in distinguishing benign and malignant salivary gland lesions.
  • A specific cytologic diagnosis is often possible.
  • On the other hand, due to the pitfalls in cytologic diagnosis of certain salivary gland tumors, tissue biopsy for histologic examination may be necessary.

  • Therefore adjunctive methods are important for preoperative diagnosis.
  • Since 1985, our research group has performed a series of basic and clinical researches on salivary gland tumors including clinicopathologic characteristics and behavior, preoperative adjunctive diagnosis, mechanism, prevention and treatment of distant metastasis of salivary adenoid cystic carcinoma, and histogenesis of salivary gland tumor.
  • These studies have improved our knowledge on the clinical, histopathological, cytological, imaging features, and bio-behavior of different types of salivary gland tumors.
  • We have also suggested related principles of management, modified the operative methods of benign tumor of parotid gland.
  • Our efforts have raised the standards of diagnosis and treatment of salivary gland tumors.

  • Squamous cell (IR, 3.44) and mucoepidermoid (IR, 3.23) carcinomas occurred most frequently among males, whereas mucoepidermoid (IR, 2.67), acinic cell (IR, 1.57), and adenoid cystic (IR, 1.40) carcinomas were most common among females.
  • Mucoepidermoid, acinic cell, and adenoid cystic carcinomas predominated in females through age approximately 50 years; thereafter, IRs of acinic cell and adenoid cystic carcinomas were nearly equal among females and males, whereas IRs of mucoepidermoid carcinoma among males exceeded IRs among females (IRR, 1.57; 95% CI, 1.38-1.78).

  • To improve early diagnosis and the effective surgical management of external auditory canal carcinoma.
  • METHOD: Twelve cases of the early stage external auditory canal carcinoma were reviewed retrospectively.
  • Postoperative pathological diagnosis revealed that 6 cases with adenoid cystic carcinomas, 5 cases of squamous cell carcinomas, 1 case of cerumenal adenocarcinoma.
  • No lesion involved in parotid gland and lymphoid of parotid surface.
  • Five cases of squamous cell carcinomas, 1 case of cerumenal adenocarcinoma and 3 cases of adenoid cystic carcinomas received radiotherapy postoperatively.
  • All patients were alive free of carcinoma during the follow-up.
  • One patient had parotid fistula after surgery and recovered by pressure pocketing.
  • CONCLUSION: Timely and accurate biopsy is the key point to diagnose the early stage external auditory canal carcinoma.
  • The complete resection with safety margin of external auditory canal carcinoma can improve the effect of surgery.

  • [Title] Salivary gland neoplasms: an analysis of 74 cases.
  • Between 2006 and 2009, 74 cases of salivary gland neoplasms were analyzed retrospectively, of which 44 (60%) were benign and 30 (40%) malignant.
  • 61 % percent of neoplasms were in the parotid gland, 22% in the minor salivary glands including sublingual salivary glands, and 17% in the submandibular glands.
  • The most common benign neoplasm was pleomorphic adenoma (64%), and the most common malignant neoplasm were adenoid cystic carcinoma (17%) and mucoepidermoid carcinoma (23%).
  • We analyze the incidence and distribution of all types of salivary gland neoplasms in our series, and provide data for comparison with other epidemiological studies from different geographical sites and races.
  • Further epidemiological surveys should be encouraged for better understanding of the disease and to provide early and better treatment of salivary gland neoplasms.

  • CONCLUSION: In this material consisting of various salivary gland carcinomas, stage I, male gender and age were the most powerful predictors of patient outcome.
  • OBJECTIVES: To retrieve the records of all salivary gland cancer (SGC) patients diagnosed in Finland between 1991 and 1996 and to evaluate the incidence, histological type and location of SGC, the treatment given and the outcome.
  • The commonest tumor location was the parotid gland (n = 152; 64%), followed by the minor salivary glands (n =46; 19%), the submandibular gland (n =38; 16%) and the sublingual gland (n = 1; 0.4%).
  • The most frequent histological types of SGC were adenoid cystic carcinoma (n =65; 27%), mucoepidermoid carcinoma (n =45; 19%) and acinic cell carcinoma (n =41; 17%).
  • Of the commonest tumor types, the best 5-year relative survival rate was for patients with acinic cell carcinoma (96%), followed by those with mucoepidermoid (79%) and adenoid cystic carcinoma (74%).

  • Oral malignant tumors are the sixth most common malignancy in the world, and there is a recent trend of rising incidence of these tumors in young patients.
  • The aim of this study was to compare demographic and etiologic data from young and old patients with orofacial malignant tumors.
  • Data were obtained by retrospective analysis of files of patients with malignant orofacial tumors, who were categorized into 2 groups: the young group (< or = 40 years old) an the older group (> 60 years old).
  • Tongue was the most common site for older group (37.5%), whereas that for the younger group was the parotid (29.3%).
  • We found that the pattern of malignant orofacial tumors in the young age group was changed, and these tumors can occur in young patients who report no tobacco or alcohol use.

  • BACKGROUND: Epithelial salivary gland tumors are very uncommon in pediatric age.
  • RESULTS: The major salivary glands were the main site of tumor occurrence (79% of cases arose in parotid glands); 37 patients had benign tumors (pleomorphic adenoma), 15 had malignant tumors (12 mucoepidermoid carcinoma, 9 low grade).
  • All the patients were treated by surgery; local relapses after parotidectomy were 4% and 25%, in benign and malignant tumors, respectively.
  • At the time of the report, all patients with benign tumors were alive, 35(95%) without evidence of disease; only one patient with malignant tumor died of disease.
  • Parotidectomy (total or superficial) is the best choice for achieving good cure rates in both benign and malignant tumors.

  • BACKGROUND: The knowledge of the distribution and pattern of salivary gland tumors in the tertiary care center can provide overview of the disease pattern in the region.
  • METHODS: A retrospective observational study of all the salivary gland tumors treated in the department of Otolaryngology, B. P.
  • RESULTS: Out of total 51 cases, 81% (n=41) were benign and 19% (n=10) malignant tumors.
  • Mean age for benign and malignant tumors were 32.3 and 46.5 years respectively.
  • Parotid tumor outnumbered all other sites comprising 69%, followed by submandiibular 18% and minor glands 13%.
  • Pleomorphic adenoma was the commonest tumor (76%) of all primary salivary gland tumors.
  • Benign to malignant tumor ratio of parotid, submandibular and minor glands were 6:1, 3.5:1 and 1.3:1 respectively.
  • Adenoid cystic carcinoma (40%) and mucoepidermoid carcinoma (30%) were commonest malignant tumors.
  • CONCLUSIONS: The principal site for salivary gland tumors in eastern Nepal population was the parotid and the pleomorphic adenoma outnumbered all other tumors.
  • Most of the cases in both benign and malignant group presented with painless lump often misleading the gravity of disease.

  • BACKGROUND: To report the outcomes and early to long term treatment complications among pediatric patients with major salivary gland malignancies treated at a single institution.
  • Patients less than 19 years of age with a diagnosis of a major salivary gland malignancy were identified at the M. D.
  • The majority of tumors arose in the parotid gland (83%), and the most common pathology was mucoepidermoid carcinoma (46%).
  • Lymphatic metastasis was identified in 37% of patients, nearly all with mucoepidermoid carcinoma.
  • CONCLUSIONS: Survival of pediatric patients with major salivary gland carcinomas is favorable.
