  • [Title] Biological role of estrogen receptor beta in salivary gland adenocarcinoma cells.
  • PURPOSE: This study is intended to investigate the biological role of estrogen receptor (ER) nongenomic signaling in salivary gland adenocarcinoma cells that predominantly express ERbeta.
  • EXPERIMENTAL DESIGN: Salivary gland adenocarcinoma cell lines HSG and HSY were used to study the effect of diarylpropionitrile and estrogen on the nongenomic signaling of ERbeta, cytoskeletal remodeling, and cell motility.
  • CONCLUSION: These results clearly showed that ERbeta nongenomic signaling is active in salivary gland cells and has a biological role in migration, presumably via the stimulation of ERK1/2.
  • In future, the findings of this study might have clinical importance as several ERbeta-selective agonists are currently being available, and these could potentially be used for therapeutic targeting of ERbeta-positive salivary tumors.

  • Material collected by fine needle aspiration underwent cytological examination and after the diagnosis of salivary carcinoma the mass was surgically excised and then was processed for histological examination.
  • The aim of this work is to describe the cytopathological features of this carcinoma and to emphasize the usefulness of fine needle aspiration technique, which is an effective, inexpensive and minimally invasive method of diagnosis that can be performed before incisional biopsy or even before surgical excision.

  • We describe a salivary gland adenocarcinoma arising in the lower lip, characterized by ductal structures and cords, 3-5 cell layers in thickness, set in a dense fibrous stroma and also invading nerves, thus mimicking a MAC in both its clinical and its histopathologic appearance.
  • The diagnostic dilemma presented by this lesion is discussed, along with a differential diagnosis and brief review of the literature.

  • [Title] The Involvement of RCAS1 in Creating a Suppressive Tumor Microenvironment in Patients with Salivary Gland Adenocarcinoma.
  • The aim of the present study has been to evaluate the role of RCAS1 in creating the suppressive tumor microenvironment in cases of parotid adenocarcinoma.
  • The tissue samples of salivary gland adenocarcinomas and their stroma and the palatine tonsils which constituted the reference tissue sample group were obtained during routine surgical procedures.
  • A statistically significantly higher RCAS1 immunoreactivity level was found in the adenocarcinoma tissue samples in comparison to that found in the stromal tissue samples.
  • A statistically significantly higher RCAS1 immunoreactivity was also identified in the adenocarcinoma tissue samples derived from patients who had lymph node metastases in comparison to patients without such metastases.
  • The infiltration of CD68-positive cells was significantly stronger in the adenocarcinoma and stromal tissue slides than in the reference group tissue slides; moreover, the infiltration was a good deal more prominent in the stromal tissue than in the adenocarcinoma tissue.
  • Additionally, the infiltration of CD3- and CD25-positive cells was more prominent in the reference tissue slides than in the adenocarcinoma and stromal tissue slides, and was stronger in the adenocarcinoma tissue than in the stromal tissue.
  • Furthermore, the infiltration of Foxp3-positive cells was seen exclusively in the stroma whereas it was not even detected in the adenocarcinoma tissue.
  • The present study demonstrates that RCAS1 expression by both tumor cells and tumor-associated macrophages may participate in creating the immunosuppressive microenvironment in parotid gland adenocarcinoma, thus promoting tumor development as well as metastases.

  • Distinguishing adenoid cystic carcinoma from polymorphous low-grade adenocarcinoma of the salivary glands is important for their management.
  • We studied the expression of several myoepithelial and basal/stem cell markers (smooth muscle actin, calponin, smooth muscle myosin heavy chain, metallothionein, maspin, and p63) by immunohistochemistry in 23 adenoid cystic carcinoma and 24 polymorphous low-grade adenocarcinoma, to identify the most useful marker or combination of markers that may help their diagnoses.
  • We noted diffuse expression of smooth muscle actin in 20 adenoid cystic carcinoma vs one polymorphous low-grade adenocarcinoma (P<0.0001), calponin in 15 adenoid cystic carcinoma vs one polymorphous low-grade adenocarcinoma (P<0.0001), smooth muscle myosin heavy chain in 15 adenoid cystic carcinoma vs one polymorphous low-grade adenocarcinoma (P=0.001), metallothionein in 22 adenoid cystic carcinoma vs eight polymorphous low-grade adenocarcinoma (P<0.001), maspin in 22 adenoid cystic carcinoma vs 14 polymorphous low-grade adenocarcinoma, and p63 in 21 adenoid cystic carcinoma vs 14 polymorphous low-grade adenocarcinoma.
  • In differentiating adenoid cystic carcinoma from polymorphous low-grade adenocarcinoma, smooth muscle actin as a single ancillary test in support of the histological findings, appears to be as efficient as multiple immunohistochemical tests.

  • BACKGROUND: Salivary duct carcinoma (SDC) is a rare adenocarcinoma of the salivary glands which usually occurs in the parotid gland (approx.
  • 80%), but is also found in the submandibular gland (approx.
  • 12%) and the minor salivary glands.
  • Due to the low incidence of this tumour-particularly in terms of involvement of the submandibular and minor salivary glands-only limited clinical data is available.
  • PATIENTS/METHOD: In a retrospective investigation, follow-ups of six patients with a SDC of the submandibular gland were analysed and the surgical measures evaluated for their efficiency.
  • RESULTS: At the time of diagnosis, varying tumor stages were present (2xT1, 3xT2, 1xT1).
  • In all patients, the submandibular gland was resected and neck dissection performed, in two cases (33,3%) this was followed by radiotherapy.
  • CONCLUSION: The clinical course of patients with SDC of the submandibular gland exhibits a high (66.6%) rate of local recurrence, crucial for the following clinical course.

  • RESULTS: A definitive diagnosis of a mucinous adenocarcinoma (MAC), a very rare, high-grade malignancy tumor that more frequently appears in the elderly, was made.
  • With the definitive diagnosis of an MAC of the palate, the upper-left premolars were extracted using a box osteotomy, and the surgical wound was reconstructed with a Bichat fat-pad flap.
  • CONCLUSIONS: An MAC of a minor salivary gland may arise as a gingival exophytic lesion with a clinical appearance similar to a pyogenic granuloma.
  • The existence of a light speckled pattern on the surface of the lesion indicates that an incisional biopsy is needed before surgical treatment to make possible a definitive diagnosis.
  • To avoid delays in diagnosis and erroneous clinical approaches to exophytic gingival lesions, periodontists and practitioners should routinely perform a pathologic analysis to confirm the diagnosis when dealing with these kinds of lesions.

  • AIMS: To evaluate cytokeratin (CK) 7/20 expression patterns in salivary gland neoplasia.
  • METHODS AND RESULTS: Formalin-fixed paraffin embedded tissue from 153 salivary gland tumours were evaluated for CK7/20 immunoreactivity.
  • The tumours included pleomorphic adenoma (n = 24), myoepithelioma (n = 9), papillary cystadenoma (n = 3), oncocytoma (n = 2), adenoid cystic carcinoma (n = 22), mucoepidermoid carcinoma (n = 21), polymorphous low-grade adenocarcinoma (n = 21), carcinoma ex-pleomorphic adenoma (n = 11), acinic cell carcinoma (n = 17), epimyoepithelial carcinoma (n = 7), oncocytic carcinoma (n = 3), hyalinizing clear cell carcinoma (n = 1), papillary cystadenocarcinoma (n = 1), salivary duct carcinoma (n = 3), adenocarcinoma (not otherwise specified) (n = 4) and squamous carcinoma (n = 4).
  • All salivary gland neoplasms showed a CK7+/CK20- immunoprofile ranging from 5 to 100%.
  • CONCLUSIONS: Although the CK7/20 immunoprofile is not useful in distinguishing the various types of salivary gland neoplasms or between benign and malignant salivary gland tumours, it may facilitate differentiation of primary salivary gland neoplasia from metastatic tumours and squamous carcinoma, and the diagnosis of metastatic salivary gland tumours.

  • In the present study we analysed the expression of this molecule in salivary gland tumours.
  • Archival formalin-fixed, paraffin-embedded sections of 40 benign and 57 malignant salivary gland tumours were retrieved and retrospectively studied immunohistochemically using a polyclonal C-KIT antibody in an Envision/HRP technique.
  • In addition five samples of chronic submandibular sialadenitis, five normal minor salivary glands and parotid or submandibular gland tissue adjacent to benign tumour were also studied.
  • C-KIT expression was observed in cases of adenoid cystic, acinic cell polymorphous low grade, epithelial-myoepithelial, carcinosarcoma and basal cell adenocarcinomas, as in luminal cells of pleomorphic adenomas, in serous acinar and only in intercalated and a small number of striated ductal cells of inflammatory salivary gland tissue, whereas normal salivary lobules were generally negative except a weak positivity of intercalated cells.
  • Contrary to other reports, this study suggests that, C-KIT protein does not appear to be an exclusively specific marker for benign or malignant salivary gland neoplasms, but may be useful in differential diagnosis of adenoid cystic carcinoma from polymorphous low grade adenocarcinoma.
  • Furthermore its expression in serous acinar cells in sialadenitis and intercalated ductal cells in normal and inflammatory lesions may indicate a possible participation in pathogenesis of both neoplastic and non-neoplastic salivary gland diseases.

  • Intraoral minor salivary gland carcinomas are rare tumours the incidence, distribution, and prognostic factors of which differ.
  • The aim of this study was to assess the value of bronchoscopy and gastroscopy in the routine staging of minor salivary gland cancers.
  • We retrospectively reviewed the casenotes of 95 patients who had presented with newly diagnosed intraoral minor salivary gland carcinomas.
  • Data were collected about abnormalities detected during endoscopic screening; a biopsy was taken if reasonable and if the resulting diagnosis influenced the planned treatment.
  • For therapeutic, diagnostic, and prognostic reasons bronchoscopy and gastroscopy should be included routinely into staging of intraoral minor salivary gland carcinomas.

  • OBJECTIVE: Basal cell adenocarcinoma of a minor salivary gland is extremely rare.
  • The goal of this report is to increase awareness of this rare disease and to review and discuss the differential diagnosis and important considerations in treatment.
  • METHODS: Case report of a basal cell adenocarcinoma of a hard palate minor salivary gland and review of the literature of basal cell adenocarcinoma.
  • RESULTS: Basal cell adenocarcinomas are slow-growing tumours that most commonly involve the parotid gland and very rarely involve minor salivary glands.
  • Histological diagnosis is important to distinguish this tumour from adenoid cystic carcinoma given the significant difference in disease prognosis.
  • CONCLUSIONS: Diagnosis of these tumours must be made histologically.

  • AIM: To determine whether DNA ploidy by image cytometry is a good diagnostic tool to distinguish benign and malignant salivary gland tumours.
  • METHODS: A total of 62 salivary gland tumours were studied.
  • According to the World Health Organization (WHO) classification, there were 14 mucoepidermoid carcinomas (MEC), 11 adenoid cystic carcinomas (ACC), 10 pleomorphic adenomas (PA), 10 carcinoma ex PA (CEPA), 9 acinic cell carcinomas (ACCa), 3 polymorphous low-grade adenocarcinomas (PLGA), 2 papillary cystadenocarcinomas (PC), 1 myoepithelial carcinoma (MC), 1 undifferentiated carcinoma (UC) and 1 mucinous adenocarcinoma (MA).
  • RESULTS: Fifty-three of 62 salivary gland tumours were uniformly diploid.
  • CONCLUSIONS: The vast majority of salivary gland tumours were diploid.

  • Here, we report a case of ductal adenocarcinoma ex pleomorphic adenoma of the lacrimal gland.
  • A 70-year-old Japanese female received the resection of the recurrent lacrimal gland tumor (second surgery), under the clinical diagnosis of recurrent pleomorphic adenoma, fifteen years after the initial surgical resection.
  • The resected tumor was composed of recurrent pleomorphic adenoma and adenocarcinoma.
  • Adenocarcinoma component showed infiltrative papillo-tubular or microcystic growth of carcinoma cells, which had pleomorphic large nuclei with prominent nucleoli and rich eosinophilic cytoplasm.
  • Accordingly, the diagnosis of ductal adenocarcinoma ex pleomorphic adenoma was made.
  • These data suggests that lacrimal ductal adenocarcinoma appears to have a poor prognosis, similar to salivary duct carcinoma, which is one of the most aggressive salivary carcinoma.
  • And further study is needed to clarify the clinicopathological features of the lacrimal ductal adenocarcinoma.

  • PURPOSE: To establish an MR factor analysis technique for two-dimensional (2D) MR dynamic structures of benign and malignant salivary gland tumors.
  • MATERIALS AND METHODS: Dynamic contrast-enhanced MRI using a surface coil was performed on 36 patients with benign (N = 24) or malignant (N = 12) salivary gland tumors.
  • RESULTS: The MR factor analysis technique allowed the pixel-to-pixel evaluation of the contrast enhancement kinetics of the salivary gland tumors.
  • The 2D distributions of the time-intensity curve (TIC) patterns correlated well with the histological features of the salivary gland tumors and allowed more detailed dynamic structures of the tumors compared with the results obtained by the conventional dynamic study analysis.
  • CONCLUSION: The proposed MR factor analysis would be clinically feasible to diagnose salivary gland tumors and tumor-like lesions.

  • We encountered an unusual example of acinic cell adenocarcinoma that initially presented in the lung, whereas the primary parotid carcinoma, despite extensive clinical evaluation, only became apparent 1 year after initial diagnosis.
  • The histologic, immunohistochemical, and ultrastructural features of the tumor in the parotid gland and lung were similar.
  • This presentation is unique, showing that peripheral lung tumors of salivary gland type are likely to be metastatic, and careful clinical evaluation is warranted in establishing their primary site of origin.

  • Polymorphous low-grade adenocarcinoma (PLGA) is difficult to diagnose both clinically and histologically due to its indolent presentation, and because of its morphological diversity that includes several microscopic patterns.
  • The aggressive biologic behavior seen in minor salivary glands as compared to major glands is apparently associated histologically to a predominance of the papillary pattern in the former.
  • Biologic behavior of PLGA in the major salivary glands is uncertain, as some cases have developed recurrences and metastases independently of the presence of a papillary pattern.
  • A case of PLGA originated de novo in parotid gland is presented a 60 year-old male, treatment was surgically excised through superficial parotidectomy and to postoperative radiotherapy (46 Gy).
  • Forty-eight months later the patient is alive with no signs of recurrence, as well as a review of the literature, with particular emphasis in its differential diagnosis and biological behavior.

  • METHODS: A retrospective chart review revealed 24 patients with asymptomatic neck masses treated between 1976 and 2001, whose pathology demonstrated heterotopic salivary tissue or neoplasms arising from heterotopic salivary tissue.
  • RESULTS: Nine cases were benign periparotid lymph nodes with heterotopic salivary inclusions, 3 of which had multimodal involvement.
  • Fifteen cases of heterotopic salivary tumors were identified.
  • Malignant tumors included mucoepidermoid (3), acinic cell (2), and adenocarcinoma (1).
  • CONCLUSIONS: Heterotopic salivary tissue in periparotid and upper cervical nodes is a more common occurrence than historically recognized.
  • Tumorigenic changes arise from heterotopic nodal inclusions, and although infrequent, should be considered in the differential diagnosis for isolated neck/periparotid masses and parotid Warthin's tumor.
  • Suggested management, after a thorough clinical exam/needle aspiration biopsy, includes an imaging survey of the parotid gland and neck lymphatics with an appropriate resection to include a simple excision, parotidectomy, neck dissection, and/or irradiation as indicated.

  • BACKGROUND: this study aimed at assessing the utility of silver nucleolar organizer region (AgNOR) staining in categorization and prognostication of salivary gland tumors.
  • METHODS: Twenty-seven malignant salivary gland tumors, 16 benign neoplasms, and 6 chronic sialadenitis lesions were stained using the technique described by Ploton et al. (1986).
  • In addition, no correlation was found between mean AgNOR counts and the clinical course of malignant or benign salivary gland neoplasms.
  • CONCLUSION: Our results suggest that AgNOR staining is useful for distinguishing benign from malignant lesions, but is neither helpful for discriminating between histological types and grades of malignant neoplasms, nor prognostication of malignant salivary gland neoplasms.

  • We have noticed that p63 routinely stains salivary gland oncocytomas but not metastatic RCC.
  • All primary salivary gland tumors were positive for p63, predominately in basal cell-type distribution.
  • While clinical history and morphology usually are adequate, demonstration of p63 staining can definitively exclude metastatic RCC from the differential diagnosis of similar appearing tumors in salivary glands, namely oncocytoma and oncocytic carcinoma, with 100% specificity and sensitivity.

  • The differential diagnosis included polymorphous low-grade adenocarcinoma, basal cell adenoma and adenoid cystic carcinoma.
  • However, on balance, based on the clinical presentation, a diagnosis of malignancy was favoured and appropriate treatment was considered.

  • PURPOSE: This is a retrospective study of the location, diagnosis, sex and age distribution and treatment of minor salivary glands tumours of the upper aero digestive tract in Lagos, Nigeria.
  • RESULTS: These tumours constituted 40.4% of all salivary gland tumours in the series.
  • Patients with adenoid cystic carcinoma were significantly older at presentation than those with pleomorphic adenoma and adenocarcinoma (P < 0.05).

  • Histopathologically, sinonasal adenocarcinomas fall into four categories: the intestinal type, the conventional salivary gland type (eg, adenoid cystic carcinoma, acinic cell carcinoma), the seromucous type, and the low-grade not otherwise specified type.
  • Recently, a new type of sinonasal adenocarcinoma has been described, called tubulopapillary low-grade adenocarcinoma.
  • In this commentary, the histologic features of this type of tumor are compared with those of the other types of sinonasal adenocarcinoma.

  • BACKGROUND: Recent studies have proposed that minichromosome maintenance (Mcm) proteins may be sensitive proliferation markers and may serve as novel biomarkers for prognostication and diagnosis of various pre-malignant and malignant lesions.
  • The aims of this study were to determine the expression of Mcm-2, Ki-67 and geminin in salivary gland (SG) tumours, and to evaluate their usefulness for diagnosis or for prediction of tumour behaviour.
  • CONCLUSIONS: The findings suggest that Mcm-2 may be a sensitive proliferation marker in SG tumours and may be useful for differential diagnosis between PA and CEPA, and ACC and PLGA.

  • [Title] Adenoid cystic carcinoma and polymorphous low-grade adenocarcinoma of minor salivary glands: a comparative immunohistochemical study using the epithelial membrane and carcinoembryonic antibodies.
  • OBJECTIVE: The purpose of this study was to investigate immunohistochemically the expression of epithelial membrane antigen (EMA) and carcinoembryonic antigen (CEA) in adenoid cystic carcinoma (AdCC) and polymorphous low-grade adenocarcinoma (PLGA) in an attempt to assess the ability of these markers to distinguish AdCC from PLGA when the histological features on routine hematoxylin and eosin are equivocal.
  • MATERIALS AND METHODS: Fourteen specimens of AdCC, 10 PLGA, and five normal minor salivary glands fixed in 10% formalin and embedded in paraffin, were retrieved from the files of our department and were retrospectively studied with the streptavidin-biotin complex method using the epithelial membrane and carcinoembryonic antibodies.
  • CONCLUSIONS: The results of this study suggest that the immunostaining of AdCC and PLGA with EMA and CEA could not offer an adjunctive aid in differential diagnosis between these two tumors.

  • The diagnostic utility of fine needle aspiration cytology as initial work up of salivary gland enlargement was assessed in one hundred and eighty-five salivary gland specimens over three years period and corroborated with histopathology, whenever feasible.
  • All smears were evaluated according to cell size, amount of cytoplasm, cytologic atypia and presence of lymphocytes. (a) Variable cytologic appearances of pleomorphic salivary adenoma were observed. (b) Cellular pleomorphic adenoma and adenoid cystic carcinoma showed basaloid cell features. (c) Tumours with intermediate size cells and bland cytology included low grade muco-epidermoid carcinoma and cystic lesions. (d) Warthin's tumour, oncocytoma, salivary duct carcinoma and high grade muco-epidermoid carcinoma revealed large cells and abundant cytoplasm with or without atypia.
  • Malignant tumours included muco-epidermoid carcinoma (n = 5), adenoid cystic carcinoma (n = 3), acinic cell carcinoma (n = 2), adenocarcinoma (n= 2), squamous cell carcinoma (n = 1), undifferentiated carcinoma (n= 4) and malignant lymphoma (n = 1).
  • So it can be concluded that fine needle aspiration cytology can play important role in early diagnosis and subsequent therapeutic planning of salivary gland lesions.

  • The positive rate of the EB virus antibody VCA-IgA was 56.7% in the whole group and only 23.7% in the salivary gland type of NPAC.

  • PURPOSE OF REVIEW: Polymorphous low-grade adenocarcinoma is an entity described in 1984.
  • An increasing number of reports in the pathology literature since then have identified this as the second or the third most common minor salivary gland malignancy.
  • The natural history of polymorphous low-grade adenocarcinoma is distinct from other malignant salivary tumours.
  • RECENT FINDINGS: Polymorphous low-grade adenocarcinoma is a slow-growing tumour that is locally invasive.
  • Diagnosis is established after a biopsy because cytological studies are not helpful.

  • To our knowledge, this is the largest patient series of polymorphous low-grade adenocarcinoma with clinical follow-up in the otolaryngology literature.
  • STUDY DESIGN: We retrospectively identified 19 patients with polymorphous low-grade adenocarcinoma who had adequate clinical follow-up and pathologic specimens available for examination.
  • The most common initial diagnoses were polymorphous low-grade adenocarcinoma, adenoid cystic carcinoma, and pleomorphic adenoma.
  • CONCLUSIONS: Polymorphous low-grade adenocarcinoma is an increasingly recognized malignancy that originates predominantly in the minor salivary gland.

  • BACKGROUND: Oncocytic carcinoma is a rare tumor of major salivary glands.
  • Histochemical or electron microscopic confirmation of the oncocytic nature of the tumor cell is needed for differential diagnosis.
  • CASE REPORT: A 58-year old man presented with a recurrent mass in the left parotid gland with a prior diagnosis of monomorphic adenoma in the same localization which had been treated by tumor excision in July 2002.
  • CONCLUSION: For an accurate approach in the management of patients, oncocytic adenocarcinoma should be considered in the differential diagnosis of lesions of the parotid gland, most of which are benign.

  • OBJECTIVE: To present our experience in diagnostic errors and pitfalls on aspiration cytology of salivary region in a high volume of cases.
  • STUDY DESIGN: In a retrospective review of cytology files of a head and neck referral center from 1990 to 2005, the false positive and false negative interpretations on fine needle aspiration (FNA) of salivary lesions were retrieved.
  • These records and slides were reviewed to identify cytologic characteristics that contributed to false diagnosis.
  • RESULTS: Of a total of 1,040 salivary FNA samples, 376 cases had a final histologic diagnosis with interpretations of benign or malignant.
  • The most common false negative cases were acinic cell carcinoma, epithelial myoepithelial carcinoma, adenoid cystic carcinoma and basal cell adenocarcinoma.
  • Benign cases with false positive diagnosis were Warthin tumor and pleomorphic adenoma.
  • CONCLUSION: Knowledge of cytologic overlaps and pitfalls on salivary gland FNA, as well as clinical and radiologic features, may help clinicians arrive at the appropriate diagnosis and reduce false interpretations.

  • IHC has been used as an adjunct to distinguish among many salivary gland neoplasms using both qualitative and quantitative methods.
  • The objective of this study was to determine whether a distinctive immunoreactivity staining pattern to GFAP can be consistently observed among three selected minor salivary gland neoplasms and thus serve as a diagnostic adjunctive procedure.

  • BACKGROUND: Primary colloid or mucinous carcinoma of the salivary glands is extremely rare.
  • Only a few cases have been reported that originated in the minor salivary glands. an even more exceptional presentation is as a metastatic tumor in the parotid region subsequent to superficial parotidectomy for pleomorphic adenoma.
  • A superficial parotidectomy removed the tumor completely, and the diagnosis was confirmed.
  • Six months following the surgery, the patient developed an enlarged nodular mass in the ipsilateral parotid region, with fine needle aspiration showing colloid (mucinous) adenocarcinoma, which proved to be a metastatic colloid (mucinous) carcinoma on excisional biopsy.

  • STUDY DESIGN: The usefulness of 5 items for a cytologically definitive diagnosis of BCA was examined.
  • The 5 items in 8 BCA and 22 non-BCA cases (adenoid cystic carcinoma [ADCC], basal cell adenocarcinoma, myoepithelioma, pleomorphic adenoma and polymorphous low-grade adenocarcinoma) that displayed mimicking cytology were examined cytologically.
  • RESULTS: The useful items were < 5.1 microm in mean of epithelial nuclear short diameter; mild atypia on definitive diagnosis; stromal cell cluster combining smooth margin surrounding the epithelial cell cluster or containing the epithelial cell cluster; epithelial clusters surrounded by or adhered to a thick, hyalinized smooth margin without stromal cluster; and closely fastened, tight clusters with denser cytoplasm than ADCC, but an indistinct border, with oval nuclei and no hyaline cells.

  • BACKGROUND: Pleomorphic adenoma (PA) is the most common neoplasm of the major salivary glands.
  • For proper diagnosis of CXPA, a statement of coexistence of pleomorphic adenoma and carcinoma (or carcinoma after prior PA surgery) is needed.
  • In 15 cases, tumor was localized in the parotid and in 4 in the submandibular gland.
  • The most common malignant component in CXPA was adenocarcinoma (9 cases) and undifferentiated carcinoma (6 cases).
  • Clinical picture and histopathological examination determinate the diagnosis of CXPA.
  • The best method of prevention and treatment of CXPA is early and radical removal of all major salivary glands tumors.

  • This study assessed the clinical and histological features and therapeutic efficacy of 25 cases of sublingual gland tumours from 1998 to 2008.
  • 21 cases were malignant sublingual gland tumours (84%) and of these, 18 were adenoid cystic carcinoma (86%).
  • Adenoid cystic carcinoma was mainly of the histological type, and the other histological classifications included mucoepidermoid carcinoma, pleomorphic adenoma, myoepithelioma, oncocytoma and polymorphous low-grade adenocarcinoma.
  • Sublingual gland tumours are rare and most are malignant.
  • For malignant sublingual gland tumours, early diagnosis and aggressive surgical treatment, especially for tumours with nerve involvement, is the key to improving prognosis.
  • For benign sublingual gland tumours, the resection of tumour and sublingual gland is the preferred treatment.

  • Slides were stained with May-Grünwald Giemsa, and a diagnosis of salivary gland carcinoma was made.
  • At surgery, the tumor was found to involve both submandibular salivary glands as well as adjacent lymph nodes and surrounding tissues.
  • The majority of tissues and organs examined histologically, including mandibular and retropharyngeal lymph nodes, soft palate, laryngopharynx and lungs, contained neoplastic cells whose appearance was consistent with adenocarcinoma.
  • Bilateral salivary adenocarcinoma has not previously been reported in cats, and extensive metastases are rare.

  • [Title] Fine needle aspiration of salivary glands: 5-year experience from a single academic center.
  • OBJECTIVE: To investigate 5 years' experience with fine needle aspiration (FNA) of salivary glands at a single academic center.
  • STUDY DESIGN: A total of 191 salivary gland FNAs were performed at Louisiana State University Health Science Center from 2003 to 2007, and all were done on major salivary glands except for 1 case.
  • There were 5 false negatives: 2 adenoid cystic carcinomas, 1 acinic cell carcinoma, 1 polymorphous low grade adenocarcinoma and 1 metastatic basaloid squamous cell carcinoma.
  • The overall accuracy in distinguishing benign from malignant lesions was 79.1%, and the sensitivity for salivary neoplasia was 89.4%.
  • CONCLUSION: Our results are consistent with the literature that salivary gland FNA has good sensitivity, specificity and accuracy in the diagnosis of salivary neoplasms.

  • BACKGROUND: Salivary duct carcinoma is a rare, extremely aggressive malignant tumor, demonstrating invasive growth with early regional and distant metastasis.
  • We describe a case of parotid gland salivary duct carcinoma metastatic to the eyelid and orbit, as confirmed by immunohistochemical analysis.
  • Immunohistochemical analysis for markers like androgen receptors, gross cystic disease fluid protein-15, cytokeratins, HER-2/neu, epithelial membrane antigen, S-100 proteins, progesterone receptors, and estrogen receptors were performed to establish diagnosis.
  • Diagnosis was metastatic paratoid duct carcinoma to the left lower eyelid and left inferior orbit.
  • CONCLUSIONS: Although metastatic SDC of the eyelid and orbit is a rare phenomenon, the possibility of this extremely aggressive entity should be included in the differential diagnosis when dealing with a patient with an eyelid and/or orbital tumor.

  • Primary malignant melanoma of the salivary gland is a rare disease, and only afew cases describing FNAC have been reported.
  • CASE: We present a case of primary malignant melanoma of the right parotid gland.
  • However, histologic examination revealed the diagnosis of malignant melanoma.
  • It was remarkable that 3 months after diagnosis of primary malignant melanoma in the right parotid gland, supraclavicular lymph nodes appeared on the left side.
  • CONCLUSION: The cellular aspirates from primary malignant melanoma of the parotid gland were cytologically mistaken for adenocarcinoma.

  • OBJECTIVE: To summarize clinical features, diagnosis, differential diagnosis, treatment and prognosis of basal cell adenocarcinoma.
  • METHOD: A retrospective study were subjected to one case with basal cell adenocarcinoma of the nasal septum, and the related literature were reviewed.
  • RESULT: Basal cell adenocarcinoma often occurred in the salivary glands and minor salivary glands of salivary palate and other parts.
  • CONCLUSION: Basal cell adenocarcinoma is rare seen in the salivary gland tumors.

  • We describe a case of basal cell adenoma of the parotid gland.
  • We also review the literature and discuss the diagnosis and management of this rare entity.

  • BACKGROUND: Basal cell adenocarcinoma accounts for approximately 1.6% of all salivary gland neoplasms.
  • METHODS: Case records of 6 patients with diagnosis of basal cell adenocarcinoma of the head and neck, who were treated by carbon ion radiotherapy with 64.0 GyE/16 fractions in our institution, were retrospectively reviewed.
  • CONCLUSIONS: Carbon ion radiotherapy should be considered as an appropriate curative approach for treatment of basal cell adenocarcinoma in certain cases, particularly in cases of unresectable disease and postoperative gross residual or recurrent disease.

  • [Title] A retrospective cohort study of nasopharyngeal adenocarcinoma: a rare histological type of nasopharyngeal cancer.
  • OBJECTIVES: The purpose of this study was to investigate the clinical manifestations, Epstein-Barr virus (EBV) serology, and treatment outcome of patients with nasopharyngeal adenocarcinoma (NPAC).
  • Pathologically, 58% of NPAC cases were common type and 42% were salivary gland type.
  • The positive rate for the EBV antibody VCA-IgA in all patients was 53% and only 24% in the salivary gland type of NPAC.
  • CONCLUSIONS: Nasopharyngeal adenocarcinoma is a distinct entity in all types of nasopharyngeal carcinoma and EBV serology has limited value in its diagnosis.

  • PURPOSE: To retrospectively review a long-term, single-institution experience of subjects with submandibular gland malignancies treated with definitive locoregional therapy with an aim to identify clinicopathologic variables that correlate with outcomes.
  • MATERIALS AND METHODS: A comprehensive chart review of 47 patients presenting to the institute from 1993 to 2005 with a histologic diagnosis of submandibular salivary gland cancer was performed to extract demographic data, clinicopathological characteristics, and treatment details.
  • CONCLUSIONS: Submandibular gland cancer is a rare disease with histologic diversity and variable clinical behavior.

  • The aim of the present study was to evaluate the immunoreactivity of vimentin and MT in the salivary gland adenocarcinoma and its stroma in order to observe the phenomenon of stromal remodeling.
  • The tissue samples of salivary gland adenocarcinomas and their stromas and the palatine tonsils which constituted the reference group were obtained during routine surgical procedures.
  • The immunoreactivity of vimentin, metalothionein, CD56, CD57 antigens was evaluated by the immunohistochemistry method in 30 tissue samples of parotid adenocarcinoma.
  • A statistically significantly higher level of MT immunoreactivity was observed in the adenocarcinoma tissue slides than in either the stromal slides or the reference slides while no differences in MT immunoreactivity were detected when the stroma and reference tissue slides were compared.
  • A statistically significantly higher vimentin immunoreactivity level was identified in the tumor microenvironment tissue slides than in the tumor tissue slides, and a statistically significantly higher level of vimentin immunoreactivity was identified in the tumor microenvironment slides than in the slides of the reference tissue, while no differences were identified between the adenocarcinoma tissue slides and the reference slides with respect to vimentin immunoreactivity.
  • A statistically significantly higher number of CD56- and CD57-expressing cells were identified in the reference tissue slides than in either the adenocarcinoma or stromal slides.
  • In conclusion, the stroma of salivary gland adenocarcinoma in this study has been characterized by remodeling.

  • [Title] Cytoplasmic expression of survivin is an independent predictor of poor prognosis in patients with salivary gland cancer.
  • The aim was to assess its expression and prognostic value in salivary gland adenocarcinoma and muco-epidermoid carcinoma.
  • METHODS AND RESULTS: Survivin expression was analysed in 48 patients with parotid gland cancer (21 muco-epidermoid, 27 adenocarcinomas) by means of immunohistochemistry.
  • CONCLUSIONS: The correlation between cytoplasmic survivin expression and survival in salivary gland malignancies might make this an effective tool in patient follow-up, prognosis and targeted therapy in future.

  • PURPOSE: To determine the accuracy, sensitivity, specificity and prediction values of ultrasound as a means of diagnosis of cervico-facial soft tissue swellings.
  • The accuracy of ultrasound in the diagnosis of pleomorphic adenoma was 80% and 100% for both adenocarcinoma and hemangioma.
  • It was also 100% for majority of the cysts and HIV associated salivary gland swellings.
  • The sensitivity of ultrasound in the diagnosis of pleomorphic adenoma was 80% and 100% for adenocarcinoma, hemangioma, all the cysts and HIV associated swellings.
  • Ultrasound was also 100% specific in the diagnosis of monomorphic adenoma and hemangioma.
  • The accuracy of diagnosis is enhanced by the characteristics of the probe and the experience of the sonologist.

  • Malignant lesions included squamous cell carcinoma (8), sinonasal adenocarcinoma (2), salivary gland tumors (4), soft tissue tumors (18), malignant melanoma (8) and hematolymphoid neoplasms (7).
  • Diagnostic agreement between cytology and follow-up biopsy was seen mostly in benign inflammatory and odontogenic lesions and in squamous cell carcinoma and salivary gland malignancies.
  • CONCLUSION: Preoperative sinus washing cytology is a noninvasive and rapid method of diagnosis, with a definite impact on patient surgical planning.
