  • [Title] Multiple eruptive clear cell acanthoma.
  • BACKGROUND: Clear cell acanthoma is a rare solitary benign epidermal tumor of unknown etiology.
  • Although solitary lesions are the rule, less than 30 cases of multiple Clear cell acanthoma have been described in the literature to date.
  • MAIN OBSERVATIONS: We report an unusual case of a healthy 74-year-old male with multiple clear cell acanthoma on the lower extremities treated successfully with cryotherapy.
  • CONCLUSIONS: Despite significant progress in treatment of clear cell acanthoma, cryotherapy, based on liquid nitrogen, remains an important alternative in treating multiple clear cell acanthomas.

  • BACKGROUND: Clear cell acanthoma is a rare, benign epidermal tumor with a clinical hybrid appearance and a distinct histological one.
  • OBJECTIVE: To evaluate the frequency that clear cell acanthoma was listed in the clinical differential diagnosis of histologically confirmed specimens.
  • METHODS: A regional dermatopathology lab database was evaluated from January 1998 through March 2008 for histologically diagnosed cases of clear cell acanthoma.
  • RESULTS: During this time period of review, 411 histologically confirmed cases of clear cell acanthoma were identified.
  • CONCLUSION: Clear cell acanthoma was rarely listed amongst the differential diagnosis of biopsy proven specimens.

  • RESULTS: The epidermis above dermatofibromas was acanthotic, simulating seborrheic keratosis in 62.9% of the cases, and areas of clear and pale cells similar to those of clear cell acanthoma were seen in 18.6%.

  • [Title] Cutaneous clear cell squamous cell carcinoma in situ : clinical, histological and immunohistochemical characterization.
  • BACKGROUND: Clear cell squamous cell carcinoma in situ (SCCIS) has not been defined in the literature with respect to its clinical, histological, and immunohistochemical features.
  • METHODS: Nearly 1500 consecutive cases of SCCIS were assessed for percent clear cell change.
  • The clinical features of all SCCIS with >or=10% clear cell change, including age, sex and site distribution, were compared with classical SCCIS using chi-square analysis.
  • PAS special staining and immunohistochemical analysis with 11 cell markers were performed to characterize the clear cell of origin.
  • RESULTS: Eighty SCCIS cases with a spectrum of clear cell change of >or=10% were identified.
  • Six cases with >or=80% clear cells were defined as clear cell SCCIS.
  • Antibodies labeling outer root sheath (ORS) cells also labeled clear cells in the cases and included K8.12 (labeling CK13 and CK16), cellular retinoic acid binding protein II, CAM 5.2 and CK15.
  • CONCLUSION: Clear cell change in SCCIS is part of a spectrum which displays ORS differentiation.

  • BACKGROUND: Granular cell tumors (GCTs) are neoplasms showing nerve sheath differentiation that can arise in the skin but, to our knowledge, have not been associated with significant clear-cell morphology.
  • METHODS: Two patients developed four separate GCTs with distinctive, diffuse clear-cell change, which completely camouflaged the primary differentiation.
  • The morphology, histochemistry and immunohistochemistry of the lesions are described and are compared with the presence and extent of clear-cell change in 14 other cases of GCTs.
  • RESULTS: The index cases were relatively broad proliferations with uniform diffuse clear-cell change and only minimal overlying epidermal hyperplasia.
  • These clear-cell granular tumors were positive for S-100 protein, p75, CD68, NKI/C3 and neuron-specific enolase and were negative for epithelial mucin, periodic acid-Schiff, carcinoembryonic antigen, HMB-45, Melan-A, smooth muscle actin, Leu7, synaptophysin, CD34, factor XIIIa, epithelial membrane antigen and cytokeratin.
  • Three of the fourteen comparison cases were found to have no clear-cell change, eight showed focal clear-cell change and three showed moderate clear-cell change.
  • CONCLUSIONS: The distinctive morphology and the immunohistochemical results are discussed in the context of the differential diagnosis of clear-cell cutaneous tumors.

  • Clinical differential diagnoses included fibrous papule, verruca, basal cell carcinoma, and a variety of other neoplasms.
  • Histologically, dermal aggregates of clear cells with finely granular to vacuolated cytoplasm, and centrally located nuclei, were found.
  • Recognition of this variant of fibrous papule is important to distinguish this benign lesion from other clear cell neoplasms.
